I have been flashing custom ROMS for quite some time now and lately I have been liking AOKP Jellybean l Task650 & Ktoonsez. I would post this in the development section but since I don't have 10 post it won't let me.
I decided to back up my phone, transfer everything off the internal SD and do a complete wipe then flash the latest [Rom] AOKP Jellybean l Task650 & Ktoonsez (9/14).
Everything went perfect until I went to transfer my music, videos, etc back to the phone. It recognizes it as a MTA device and all the drivers are installed correctly. It will transfer for about 20-30 seconds then stop and give me an error that the device is either no longer responding or is disconnected. I deleted drivers, rebooted phone and PC etc etc and still same thing. I got home from work tried it on my PC and same thing. I have searched all over and have tried everything mentioned with the same result. I wiped everything again and flashed back to a previous ROM with same result.
Any ideas?

Mtp is buggy as hell

I think I ended up using WiFi transfer pro to copy everything back to my phone after a complete internal and external wipe. Before that I would just take the microsd card out of the phone and transfer files via the computer.

Siyah and dream kernel both support ex tweaks. Which has an option in it to force USB mass storage. Is this not working for anyone else besides me or are people just not searching

I ended up doing the WIFI last night and it worked quite well.
I didn't realize Siyah and dream kernel allowed the change. I will switch to one of those kernels.
Thanks for all the help.
Just flash the fix from mrRobinson AesSedai.

He'll release a new version this weekend but its about Inverted Gapps isntallation problem.

I have a Motorola Atrix on Bell (Canada), and for the past few months it's been running perfectly on a ICS version of Paranoid Android with no issues. I recieved a 32gb SD card for xmas, and I was reinstalling the ROM to move it to internal storage so that I can use my SD Card for music. I decided that since I was reinstalling the ROM I may as well get a new one, so I got the Paranoid Android JB ROM. I installed it to internal storage, with the tweaked body option, and it worked fine. Then I used the option chooser and installed some gapps like the play store, maps, youtube, as well as nova launcher and adobe flash, then rebooted. I first noticed that the wi-fi isn't working. It gets stuck at 'turning on wifi'. Also, none of the gapps were installed, just the launcher. The main issue, is that every couple minutes the phone will shut off. It also takes much longer than usual at the M screen (30-60 sec). It seems to turn off whenever I press the power button to wake it, whenever I plug in or unplug the device, and occasionally just randomly when the device is in use.
The phone is semi-operational right now besides having no Play store and the very frequent shut offs. Any help would be appreciated. I've tried a few different JB ROMs and they all have the shutting off issue, but Paranoid Android seems to be the only one with the Options chooser not working correctly.
Flashed back to the CM7 ROM Neutrino 2.9 . Works perfectly. Still though, is there any reason the JB Roms won't work for me? I know I've tried one in the past and it worked fine, but didn't want to keep is since is was still in Alpha at the time.

All ICS/JB ROMs are in early beta still. They're stable, just lacking hardware acceleration.
Did you do proper wipes and format /system partition before installing? Did you re-downloading the ROM and installing again? Did you try CM10 build from EPinter's? Sounds like your using Aroma installer, try flashing normal zip files for gapps, never had issues.
A lot of people are running CM10 as daily without issues, so retrace your steps to see if you overlooked anything.

i'll tried to full wipe the rom and install again ,but still same problem
I've been a lurker here for nearly two years now. But this time I wanted to make an account.
I installed JellyBam 3.3.0 yesterday which is based on CM10. It's an AMAZING ROM but unfortunately I have to go back to stock since I got the same problem than the OP.
I got the problem "The device has either stopped responding or has been disconnected." While I was transferring my mp3s on my SDCard.
The problem occurs randomly and also occurs if I copy file on internal storage of the phone.
I tested my SDCard. There's no bad sector on it
I changed switch usb cable... the problem still occurs
I deleted the usb drivers and re-install it . Still no luck.
I went back to stock ICS 4.0.1 this morning and file transfert work perfectly.
That's crazy stuff...then again, crazy stuff happens.
Problem
Oddly enough, I am having the same issue on my Galaxy Nexus. On my 5th? phone, but came with IMM76K instead of JRO03O (VZW). Unlocked and rooted and flashed updated image. Never have I seen this happen since I've been using Android (HeroC days). I'm going to reset, relock, and report back.

I'm pretty sure it's a firmware issues and not hardware.
As I mentioned earlier, I switched back to stock on my S2 and everything is perfect now (minus the fact that everything is not smooth as JB)
Yep, its definitely software problem.
Used a few roms on my S2 before, it didnt occur on any other, including last official JB.
On CM10.1 when copying files from PC into internal memory, i get same error - it copies few files, and the denies to copy for the next few ones.
any ideas?

EDIT:
@SystemC - I think I've found the reason, which is actually pretty stupid ,)
The problem was with Odin running in the background ^_^ it was somehow interfering with USB transfer. And I reflashed phone 4 times trying to debug it... hahaha ^_^ what a failure

I had the same problem, found one workaround that worked for me. I always copy my music to my music folder in my sd card. So after I got the occasional files that wouldn't copy, I just copied them straight to the sdcard, not in the Music folder. No error message. Don't know why this is happening, definetly a software thing.
Re:
I have the same problem,CyanogenMod 10.1.3 Stable,in mass storage the speed is about 1-1.5 mbps, when on MTP the speed is good, but after a short time it stops with the error "This device has either stopped responding or been disconnected"!
This is a CyanogenMod problem...hope someone will fix it!
So I flashed the stock JB ROM for the AT&T variant. Now, whenever I try to do pretty much anything I get a popup saying "Unfortunately, the process android.process.media has stopped." I was going to flash back to stock using the stock ROM that's posted in the development forum but the phone also won't go into media sync mode when connected to my computer (it did before flashing).
Any help would be greatly appreciated.

Which ROM specifically? There were some that had this issue (one of the Korean and one of the Canadian (e973 based) that were changed for the AT&T version had this issue. When you restored your apps did you restore data too? Several of the ROM's have this issue when data is included. If you restored data, then reflash the ROM and restore the apps without data.
The SnowJB ROM is the most stable 4.x rom that has all critical features (camera, GPS, etc). If you didn't flash it, you may want to give it a try.
Edit: regarding media sync, do you mean MTP? If you can't connect MTP can you connect PC software or Camera (PTP)? If those don't work then download the ROM via wifi. If nothing else, go to original development and download the CM or CFX (SR 14 or 15) roms. They are much smaller than the 4.1.2 ROM's (just under 200MB), so they'll transfer quickly via wifi (or your data plan and won't kill you plan limits, should you go the data plan root). They JB 4.2.2 and work great, but don't have working camera or GPS, so many people can't use them as DD's. They should allow you to connect MTP so you can copy a different 4.1.2 ROM to you device.

"debloated AT&T" is what I'm using (sorry I didn't make that clear, I've been fighting this all day).
Yes, I do mean MTP. I actually just tried to use PC software and camera with no luck. I tried wiping everything, reflashing the ROM and applying the permission fixes in CWM but the errors still occur.
I'm trying to grab CFX via wifi now. Thanks for the help!

Some other tricks that fixed similar issues for others:
1) use a different USB port
2) use a different USB to micro cable
3) reboot PC/Laptop (standard windows fix is to reboot 3 times, to see if it fixes the prob, before calling their helpless desk). I had to do this when I disconnected the phone without switching to charge only. I seem to recall that it took 2 reboots to finally get everything reset, so Windows would recognize the device was connected.

Thanks for the input. I actually tried that when I reflashed the ROM but it didn't help.

I actually tried three different cables and two different machines in completely separate locations but the problem persisted throughout all. I did try rebooting twice last night. Didn't go for a third.
I finally got CFX on it this morning and the phone was recognized by the computer immediately. I'm going to flash SnowJB onto it to see if the same thing happens.
Thanks for the help!
Okay, so I've tried a ton of different JB ROMs and have come to the conclusion it's not meant for the Fascinate. Between slowdowns, memory issues, missed calls, bluetooth crashes and app incompatibilies it just isn't worth it on our phone.
So, this is how you can join me in moving back to a fully working and FAR more responsive ICS (Gummy ROM). Not only is my phone zippy fast again, all apps work as intended (including Samba Filesharing, Pandora, Snapchat and LBE Privacy Guard!). No more weird errors, no more 'such and such stopped responding', no more missed calls! It's just like a working phone.. because now it is!
I ended up:
- Doing an Odin of CI500_VZW_EH03_GB_CM.tar.md5 and atlas_v2.2.pit (re-partition option checked, battery installed)
- booted it
- back to odin again (battery in)
- odined cwm4_fixed_for_cm7-ODIN.tar
- booted into recovery directly (3 finger after battery pull)
- flashed teamhacksung-ota-eng.BUILD2.zip
- booted it
- rebooted to recovery via power menu
- flashed Gummy_fascinatemtd-Release-2012-06-15-03_38.zip
- booted it
- rebooted to recovery via power menu
- flashed gapps-ics-20120317-signed.zip, Glitch-fascinatemtd-SkunkWorks-20120510.1733.zip and TitaniumBackup.zip
- booted
- restored my apps + data (not system data, just app data)
- profit!
If you don't have any of these files, the ones I haven't linked are likely linked elsewhere in this section, but if you want to find them faster, just search for the exact filenames in google. If anyone has links to the missing ones, please post them below and I'll update this.
DON'T SKIP ANY STEPS! THEY ARE ALL LISTED FOR A REASON. IF YOU HAVE A PROBLEM AND SKIPPED ANY STEPS, I WILL KNOW AND YOU WILL BE IGNORED.
Cheers, and welcome back a working phone!

scroll down to the section about ICS/JB ROMs.
also, i wanted to add some things to the guide.
1. if you don't have the samsung drivers and odin, go to Droidstyle's thread, or browse around the Fascinate subforums for info.
2. transfer the zip files to your phone before you odin stock firmware for convenience
Dude, you just saved my ass. I was effin' around with my GF old Fascinate, which I've used basically as a learning device for my kids, and I thought I'd flash a new 4.2.2 ROM (I was on another that I can't remember). Well, it didn't go well and of course I didn't have a nandroid. All of my nandroids were ICS and earlier and simply restoring wasn't gonna work. Thanks so much for this! Took me a while, but I got it done.
After using gummy rom for a while, I've found I've had some pretty irritating lag issues. I'm now running avatar ics rom and recommend flashing that instead of gummy. Also, avatar rom doesn't need a custom kernel as its plenty fast without the need to overclock.
Otherwise the flash directions are the same as gummy rom
Note, I'm speaking of avatar ics, not avatar jb. Cheers!
Today I tried downloading multiple roms onto my HTC incredible 2. But each Rom I installed either looped on the HTC screen or wouldn't install. I tried Cm 10.0 , 10.1 and 7.2. Also I tried paranoid Rom and that also looped. Only roms that worked were the stock roms I downloaded. But only the stripped version of one stock Rom. When I tried the unstripped version which included rooted apps and tweaks the Rom worked fine with one exeption; the WiFi bugged out, constantly turns itself on and off and didn't recognize my WiFi. I rooted my incredible 2 through a website that had a full tutorial that made me unlock my phone through a key and using the command propmt to flash my phone. I'm not sure what I'm doing wrong. I clear everything including cache and davsik or whatever its called before I install the roms. I use the clockworkmod booter thing (holding power and volume down button). I clear everything then install from SD card. I just want jellybean 4.2.3 or anything above 2.3.4. If you guys need more details about my kernal or anything else just ask. I'm on my phone right now so I don't have all the info off the top of my head otherwise I would have written it down. If you guys could help that would be awesome. Thanks in advance!
i know you cleared everything but have tried formatting /system and /data
