I had my phone rooted and after that i did a factory data reset. The Superuser Permissions icon doesnt appear anymore on the phone's menu. Is my phone still rooted or i have to do it again?

Like tiagobt, it's happening with me:
I flashed Chameleon 3.0.3 and tried to do a factory reset from Android (Settings > Backup and reset > Factory data reset). After reboot, the device got stuck in the Samsung logo screen and I was no longer able to access Recovery Mode.
I fixed the problem by entering Download Mode and flashing Siyah kernel with Heimdall. My phone is working now, but I wonder if this is a bug.
Did anyone else experience this issue?
I've bought a Japanese phone ntt docomo a few weeks ago... I've been change my pattern 3 times(my brother always took my phone without my permission). Now I've forgot the pattern and tried to factory reset through the system recovery.
On system recovery>wipe data/factory reset>confirmed the wipe process>then it's asked for 10 digits password
Now this is my problems:
1. Android debugging off
2. Unrooted
3. Apply update from SD card is unavailable on system recovery
How to bypass pattern lock without root and debugging off?

No.. usually we can easily factory reset through others stock recovery..but mine system recovery(stock) requires a password to confirm the factory reset.

Last night my phone was on charging and started restarting automatically.
I disconnected it from charging and removed the battery.
This morning I noticed some water drops in the case and near to the SIM slots.
I put everything in a rice bag and 10 minutes later, I installed the battery again and tried to turn the phone on.
I used the hard reset option where I get following options:
- reboot system now
- Apply update from ADB
- Apply update from sdcard
- Wipe data/factory reset
- reboot to bootloader
- view recover logs
- wipe cache partition
I proceeded with wipe data and it worked. I then did a restart and found with a interface which has the following in bottom:
Chinese (in blue background)
Erase Misc (in red background)
PCBA Test : Fail
MMI Test: Fail
Sensor Test: Fail
I am not sure what I did at this point, but some options in Chinese came up and I disconnected the battery.
Is there any way to recover this phone or is this dead now?

i'll try to explain the problem.
I use the Galaxy s6 SM-G925T and wanted to reset to factory defaults. The phone is in original state. So no custom rom, not rooted or any other modifications. The Battery has been replaced because the old battery had to be charged every 4 hours. Because the new battery does not lives much longer (maybe 6 hours) i wanted the factory reset.
First using the volume-up home and power button, i've choosen wipe cache / factory reset
Down the bottom of the screen was a text : No Support SINGLE-SKU - Supported API: 3 - dm-verify error
I continued and wiped the data
The system returned "Data wipe complete"
After the phone rebooted the system restarted, only to show the blinking SAMSUNG text.
I left the phone blinking for about 1 hour, nothing happened.
So i loaded the official systemfile : CF-Auto-Root-zerolte-zeroltexx-smg925f.tar
using Odin3_v3.14.1 i was able to flash it onto the phone.
After rebooting now the red android puppet showed up and the cf-auto-root program fnished without errors but the phone still wont start
Is the phone bricked or is it still possible to restore factory defaults and get the phone back to work ?
