For some reason anytime i plug my phone into the computer, when i unplug it, it will not mount the SD Card back to the device and i have to power cycle the device.
I have already tried re-formatting the SD Card as well and still is doing it.
Is anyone else having this problem or maybe know what could be causing it ?
I've had this happen once or twice. Have you tried pulling the battery?
Are you "safely ejecting the device" before unplugging.? The icon that looks like this....

I soft-bricked my phone a while back (Phone (!) PC) and I managed to get it to Eugene froyo from that doesnt brick. This is the only rom I can get it to (When I try to do stock eclair it just does the constant boot menu. The only problem is that the phone doesn't recongize the internal or external sd card, I'm not able to install apps or anything (adb install etc etc) it just makes the phone reboot.
Any help would be appreciated!

What is this SIM icon with the red X?, it only goes away if i remove the SIM and reinsert it again, and of course no cellular connectivity while it's on..
Something wrong with the SIM/Network provider or with the phone itself?..

i don't know how this problem comes, but my problem is that my sd card don't mount when I boot into the system (lolipop), I have to put the card out and then again in, thats the way how its working again. But I don't want to do this every boot

Please Delete! My issue is solved.
So, I found a new thing. It is flashing the image in internal storage itself. My phone was connected to computer, I went into fastboot and then just powered it off, then it showed a charging screen and the TWRP screen flashed. Then device just showed up in my PC. The things it is showing in my phone's storage are:

The name of my SD card changed randomly and I am wondering how that happened, it also doesn't show the size of the card anymore on my PC.
