Here is my problem,
Messed around with Batista's 2.2 rom. Messed around with the provided TW launcher theme settings:
From the main screen, <Menu button> -> Preferences > TwLauncher > Install theme...
now instead of pointing to a downloaded theme.zip I pointed to the actual Batista2.2Rom.zip -> big mistake...
It copied the zip file somewhere and deleted it from my root folder. Now I see my rom whenever I go to TW4 preferences > Pick a theme... I can see my Batista2.2Rom.zip..
Nothing helped, deleting dalvik cache, Wipe data/factory reset... nothing
After 3 rom installations I still see my Rom.zip as a theme.
How can I delete it or do a full wipe of the phone, every setting?
(Using Titanium backup to uninstall Tw Launcher didn't help either)
Recovery mounts and storage format data cache system and sd card
Reboot to download mode and flash stock rom .

I decided to use the option in the SD card boot menu to restore to factory settings/wipe user info. 'Brought my nook back to no-user-info land, but kept root keyboard, a few apps (gallery), but no app launcher. I am stuck in BN nook home and cannot find my app launcher in the search function.
I can't install APKS. So no luck just getting an app launcher. Still unable to get apps from unknown sources. No idea what to do.
Wondering- can I root over a root? I was thinking of using this:

i would not recommend you 1.4.1 better upgrade to 1.4.2 i have 16gb NT aswell and the nt hidden setting was working well. I would recommend you (as i suspect you don't have a backup of your entire system made in cwm) to get cwm (ClockworkMod recovery) if you haven't already, if you don't have it installed internal then get the one you boot from sdcard i posted yesterday the new recovery in development section. Once booted into cwm wipe data/factory reset, that will wipe properly your device and all installed apps with data and settings will be gone. Then flash 1.4.2 you can use albert's or my modified version of 1.4.2 rom.
Better start from scratch in that situation than leave peaces of a bad wipe.
There is another option but i really recommend the one above, you could sideload a launcher using adb that will push the launcher to your device no need to have unknown sources enabled for that. then once is pushed you can search it, open it and start from there.

flash is a term you can say install a rom as well does not matter lol and firmware is is the combination of read-only memory ( what we call ROM) , program code and data stored in it, hmmm think about chips like bios chip that is hardware and have software embedded you can find it in computers, cellphones > smartphones, tablets, remote controls, etc.
Here developers, hackers,etc you name it work in software (code, programming) basing on the hardware... (compatibility stuff) so the device can work properly.
Sometimes it gets complex and they have to figure it out when for example B&N did not release the source of their stock 1.4.0, so there was missing drivers or incomplete i think for volume buttons for example and is why in cyanoboot the volume buttons does not work, because of the lack of drivers.
So depending of the experienced devs they sometimes can patch drivers or work in a different level than "code" for example: binary level that is very complex.
By the other hand Android provided by google has complete source out (standard) so developers can download it and start working on it for their repective android devices, fixing what it needs to be fixed, solving compatibility problems, adding new features, addons, etc. so users like us can flash it in our devices and be .

Do wipe data/factory reset then go to advanced and wipe dalvik cache. Then you're good to flash a rom.
Go to mount & storage them select last option mount USB mass storage so you can copy the rom to your sdcard, then in your PC safely eject your sdcard then in cwm unmount mass storage too. Go back with power button and go to install zip from sdcard > choose .zip file from sdcard> select the rom you want to flash > select yes and the installation will start, once done you will see Installation complete!. Wipe data/factory reset again and then reboot device.

Okay, I have entered the "mount USB mass storage" area where my option is to unmount. I can see my device in my windows explorer now. Exactly where inside the SD do I place the .zip file from the new rom? Also, which one is best?
Also, where can I get a copy of your 1.4.2?
Something strange happened to my S3 after a simple reboot I noticed that some app have been removed so I decided to clean my phone on the way.
I made a full wipe + everything you can think of to clean, delete, format the phone but after I install the rom I can still see my apps in the application manager and I can't get rid of them even if I uninstall them through the application manager the come back after a restart.
I tried 2 different roms but it happens on both of them, both are xxDLIB .
How to fix that?

Try the SuperWipe from "Android Revolution HD" here --> http://forum.xda-developers.com/showthread.php?t=1797334
Be careful, this will erase everything that is not on your external SD-card. Read the instruction first.

Are the apps system apps perchance? A wipe of data WILL wipe all user installed apps, the full wipe script alluded to doesn't do anything that you can't do manually from recovery.

Sure the apps aren't part of the ROM. Depends on what the dev has cooked into it.
Sure you're doing a full wipe vs wiping caches only.
Could also be syncing to your Google account meaning when you launch play store it automatically detects your market links and reinstalls previously installed apps

I think I didn't explained my self right, these apps are "user" apps downloaded from Play store.
things I did before installing the rom:
1. factory reset from setting.
from cwm:

/sdcard
* also I removed the external sd.
but still I have these app "ghosts"
Then i suggest as per previous replies to the same question you are auto downloading from Google play after installing a rom .

I disabled wifi/data and sync when it ask for a wifi connection (at the openning screen of the new rom) so there is no way it somehow sync with play store.

Could possibly be a problem with your CWM installation or any dependencies such as ?busybox
Ok so I installed Samsung 4.1.1 stock rom and reinstalled CWM and afterwards install Omega rom and it worked!!!
Clean Rom Install
Many people don't exactly know what a Data Wipe / Factory reset is. NO it does not delete "all" your data on Internal SD-Card. It Deletes all you user data and settings and stuff, like it says it formats "system". For Example if you Full wipe you loose Whatsapp, but the Whatsapp folder on your internal SD-Card remains untouched, that means you can restore your Conversations with ease. This means you can cause a huge mess on your internal SD-Card over time. This is my way how to do a CLEAN install. Doing this prevents many unwanted issues like Bootloops, FC's and all those other nasty laggy ****.
1. Put new rom on EXTERNAL SD Card (if slot available)
2. Data Wipe / Factory Reset
3. Wipe Cache
4. Wipe Dalvik Cache
5. Format Internal SD card
6. Install Rom and enjoy
Keeping your personal Data on your external SD-Card is a good idea (pics, music, vids , docs ...)
[You can do this even if you don't have an ext. SD-Slot. After "cleaning" your device mount USB-Storage in CWM Recoery and you can connect to pc and pull over your rom. ]
And this is probably one of the best ways to do a clean install without aditional Scripts like superwipe or other "flashy" stuff. (DO NOT USE THOSE SCRIPTS UNLESS THEY ARE IN AN INSTALLATION GUIDE)
One addition to above format/preload as well .

Why every time when i install ANY rom to my galaxy s2 after 2, 3 weeks this rom is crashed? It's start crashing with closing apps "application must be stopped". How to fix it? It's the same in every ROM which i install, it makes me crazy right now...

I do it always and rom's are still going to be bugged for 7-14 days.
I think I saw you in Paranoid Rom thread mate. Did you come from GB? Just format everything and start again and report back. Format external and usb storage. Wipe data/factory reset. Wipe cache and dalvik and stats once or twice if you wish. Remove crap from init.d or voltage settings just to be sure. Then once you are happy flash rom. Follow instructions - fix permissions etc, etc..See how you go mate.
If it is a specific ROM you are having a problem with maybe post in that ROM's thread to get quicker replies.
EDIT: Also don't restore settings - start from scratch. Don't always restore App+data with Titanium just app only for now.
Before i had problems with every ROM after few days. Yes i come from GB without formatting storage. Thanks for professional response. That's what i did actually:
1. Format external and internal storage.
2. Wipe cache, Wipe data & Wipe dalvik cache.
3. Flash Paranoid 2.22 ROM.
4. Again point 2.
5. Titanium backup just necessary apps.
We will see how long everything will be fine.
PS. what about fix permission? When to do it? And how to remove this "crap" from init.d?
Point 4: Not sure about point 4 - just the dalvik cache and then select fix permissions here. I wouldn't wipe cache and data/factory reset all over again at point 4.
Point 5: Let ROM boot (will take a few minutes)
Point 6: Run an app or two then reboot to make sure..
Point 7: Begin restoring apps or play with governors etc and whatever you like
The "crap" I meant was if you had any additional scripts or tweaks that are in init.d folder during startup. So they don't interfere on first startup. Never tried a kernel cleaning script/cwm flashable file coz there have been some useless ones in the past but give that a try (someone else may be able to comment here).
Remove voltage settings if you were using voltage control app in GB. Reset to default in Extweaks if you were just using this. (Not sure if all this is necessary because I do this prior to formatting or factory resets - someone more knowledgeable can help here).
I'm going to be ROM-ing for the first time (well, other than Safestrapping the Bionic for a day or two) and want to make sure I have the right thought in terms of running and upgrading a ROM over the long term. If I'm moving to AndroWook as my daily driver, I want to make sure I have a good procedure in place for quickly and easily upgrading to the latest version of the ROM.
The following is my understanding of this whole process. Is this procedure correct? My main concern is making sure I am doing everything right to be able to quickly update to the latest version of the ROM when it comes out, not spend a day re-configuring. Is there anything I am missing?
BACKGROUND: Partitions
Main Partitions Involved With Wiping
/system = Where the OS lives, along with the apps that are preinstalled
/data = Where user installed programs & their associated data/settings live. This data/settings is the part that gets deleted when you go Settings -> Applications -> pick an application and click "Clear Data". This is what Titanium Backup backs up for user apps (both the "apps" and "data" backed up live here). Also has SMS, contacts, settings, etc... the "system data" in TiBu
/cache = Cache files that help programs to load quickly. Harmless to wipe, as it will regenerate.
/sdcard = Internal sd card area where you keep files and use file manager. some programs also put files here. Not usually wiped.
external sdcard = Physical external sd card, if applicable. Not usually wiped.
Other partitions
/boot = Kernel and ramdisk. Don't wipe unless you fix it before rebooting.
/recovery = Where the recovery (stock or custom) lives.
/misc = Other settings not normally touched.
BACKGROUND: Types of Wiping
Cache = harmless. Cache regenerates itself over time to speed up phone.
Dalvik = harmless. This is simply another cache, this one having to do with Java.
Factory Reset = wipe /data and /cache. internal and external sdcard is NOT wiped
Wipe system = wipes /system (where the os and its preinstalled apps live)
Format data = wipes /sdcard (the internal sdcard)
Format external sd = wipes external sd card
ASSUMPTIONS & NOTES
You are rooted
You have Titanium Backup (TiBu)
For multi-user environment, modify the instructions to account for the need to backup/restore with TiBu for each user. Backup for each user, flash, then restore for each user.
BACKING UP
1. TiBu: Batch Backup all user apps & data (everything) in current ROM
2. TiBu: Batch verify the backup
3. Make a sketch of how your widgets/homescreen is supposed to look
4. If you are doing a total deep clean that wipes sdcard (generally not), here is where we want to back up anything else (sdcard files, etc)
5. File Manager: Backup apk for both TiBu & TiBu Pro to SD card
Alternate: Do nothing now, and we use market in new ROM later​6. Download ROM & put on SD
WIPING & FLASHING
This part subject to change based on ROM maker's recommendations
1. Boot into recovery
2. Wipe and flash:
Option A: Clean flash (typically done, does NOT wipe sdcard):
1. Factory reset
2. Wipe cache
3. Wipe dalvik
4. For a total deep clean (not usually done, DOES wipe sdcard): Wipe system; Format data
5. Flash zip
Option B: Dirty Flash (usually only OK for minor updates):
1. Wipe cache
2. Wipe dalvik
3. Flash zip
4. There is no need to set up the ROM again in this case.
SETTING UP NEW ROM
1. Boot into new ROM
It will take 30-60 minutes to get up to full speed. After that time, it will be full speed after a reboot.
You can tell it to keep backed up to Google (backups up TO Google), but tell it NOT to restore from Google right now (restores FROM Google right now).
2. System > Applications > Unknown Sources
Restore TiBu & TiBu Pro from apk backup
Alternate: get from market
3. TiBu: Batch Restore system data - ONLY THE GREEN ITEMS
Do not restore red items
Do not restore user stuff yet. We are doing system stuff first.
Note that icons will not show correctly yet... nothing to be concerned about.
If the new ROM is vastly different, take care to restore only what is necessary.
4. Reboot
5. TiBu: Batch Restore missing apps with data - ONLY THE WHITE ITEMS
6. Click Market Doctor to verify that everything is hooked back up to the Market OK
7. Fix up homescreen - widgets & wallpaper
One by one, delete empty Widget then add new one.
Also choose the wallpaper you want
8. Check:
Date & time, alarms, ringtone, voicemail number, other settings
Anti-theft is operating properly
9. Market Place > My apps > Update any other apps
10. If over the course of time you find any apps giving problems, use the market to uninstall them and reinstall them to clear out any problems it may have had in restoring from TiBu
I just glanced thru that and this is what I saw quickly.
Option A: clean flash..... I always wipe system as well. This is not a big deal if you don't do it because I format the system in the installer in case people have forgotten.
I also delete cache and dalvik in the installer also in case people forget but I still end up doing them on the wipe page.
The only one really you need to use for a major version update is factory reset. This is just on my rom tho.. I can't speak for how others do it so always best to do the manual wipes also.
Tibu : It is never advised to restore system apps. Even between minor updates in case I have changed a system app.
If there is one that you really need restored then try restoring just the data for that one app and test it properly.
In the installer I do not write all apps to system anyway. There is very little in there that would take more than a few seconds to set back up.

Recently, I have been noticing some issues with it hanging, and doing other odd things, so I just think it is time for a reinstall. I may have installed a bad app or something along the way, because it does happen from time to time. My main question is this: How do I do a complete system wipe? Now I am talking every old file, so there are no remnants of the old information left. I have done the factory wipe and dalvik wipe, along with the cache wipe, prior to installing a ROM, every time I have done a new ROM install.. It always seems to leave files behind though. Old game files will still be there and such. Is there a way to get rid of it all, so it is a complete clean slate for the install?
Now, I use TWRP, but the idea is generally the same when it comes to this procedure, and I have found these instructions online;

Volume up, power button, and home button all pressed together for a couple of seconds.

I would basically need to redo all the programming on the phone, including the IMEI and all the info for the networks and what the phone needs to recognize what network it is allowed to access and such. Is this what it will boil down to? I will basically need to reprogram the behind the scenes info also, or no?

First, If you're currently using TWRP I'd suggest updating to the latest version, if you haven't already. There's a few of the newer ROMS which specifically require it. On the question of wiping, before you get too carried away it might help you to backup a few things. Within TWRP (ver 2.8.1.0) you have the options to backup EFS (anywhere you please, Internal storage, internal SDcard or MicroSD card) by clicking on the Storage: bar just above the swipe icon. Your IMEI >> located within EFS folder. Also, search the TWRP website for more specific instructions. What you posted is CW instructions. Hope it helps!
