Just turned on my phone after the exam today and normally it justs checks the usb storage as usual but now it's stuck on checking the usb storage, been like this for hours and I've tried restating the phone but same thing.
As a result of this I can't access anything. It's just stuck on (preparing usb storage -checking for errors)

Just tried that an hour ago. It's something to do with internal sd card pretty sure.

cwm format emmc or sdcard [depends on version of cwm] remove external sd card to be safe.
Might have to do that unfortunately

Yeah, but now it just says that, whatever I mentioned in the OP.
Keeps doing that and can't access gallery, music, or can't be detected on PC.

Whoa I just got a message saying "damaged usb storage" after rebooting again.
What do

1: format all partitions using cwm
2: try flashing stock rom using odin.
otherwise sorry in my opinion repair job

I dont really know what happened to the phone but for some reason, USB debugging and usb mass storage mounting has been disabled. It works fine in all other aspects except these two.
The first time it happened, it was fresh from a repair for not turning on anymore... That was easily fixed by LG within a day. The tech dude said something about "software upgrading".
The second time it happened, i was installing new apps straight to the SD and it wouldnt finish for like 10 minutes. So i turned it off and on again. Now everything found on my SD card wouldnt load and Settings would force close on me when I try to make it show all the apps on my phone.
I was wondering if this is a problem with apps2sd which I used... Maybe my memory card is too slow (class2) for the phone to install directly to it. Does anyone know what happened?

I used app2sd and my phone rebooted for no reason. Now after uninstalling it works fine. The unmount issue is common to P500. Probably it will be fixed on Gingerbread firmware.
So I'll have to hold off on customizing my phone until Gingerbread? =c
Guess I'll have to stick to phone memory for the apps not movable to the Sd on their own...
Also, I believe my problem goes beyond just the unmounting issue... I'll try to have the phone reformat the card when I get it back (soon I hope... I already sent it to LG)
It wouldnt show me that USB debugging and mass storage mounting is on even though those are checked on the Settings screen. When I connected the USB cable before with those settings on, there would be 2 icons on the notification bar that indicates that the two are working. I dont get those 2 icons now...
I'll try to have FroYo move the apps to SD after installing to phone memory when I get it back. Maybe that will solve the "taking forever to install" issue.
I hope the SD card reformatting by the phone would solve the slow SD card loading issue.
Which would leave the USB debugging and mass storage mounting issue... T__T

Well, I don't know how to start. Sooo, I got my phone back from the service center today (they replaced the motherboard) and now my phone doesn't enter the mass storage mode. When I enable USB Debugging and then enable Mass Storage Mode, USB Debugging gets disabled. If I enable Mass Storage Mode first and then USB Debugging, then MSM gets disabled! Basically, my phone won't go into the MSM. Anyone knows why is this happening?

I would say send it back but there are some options before you do that:
I got about the same issue with the stock sd card. I checked the write speed on that and got about 3.80mb/s. Replaced it with another sd card that had 4.50mb write and I didn't get that issue. I'm not implying that this will solve your problem but you can try it nonetheless. Just make sure that you have a sd card that has more than 4mb write speed. (constant)

Same thing happened to me after I sent it in for repairs... I just sent it back and was able to take it home at the end of the day. The tech guy said something about "updating software" which I took as "reflashing firmware".
But the same incident was caused by an sd card issue later on and the same tech guy said that I needed to reformat my sd card using a standard reformatter. I read around and reformatting via phone is more recommended so thats what I did. No problems since then even with a class 2 card.
I also read something about slow cards causing problems. The incident in my 2nd paragraph was caused by a slow memory card + new application taking forever to install directly to it. When I restarted, no more USB debugging and mass storage. Now I just install to phone memory and move apps to the sd card manually.
So I would suggest:
First try using another/faster memory card.
Next reformatting using the phone.
If both fail, try reflashing your phone with the original firmware.
Last, send it in and wait... (the most horrible option)
I hope you would only get to the first 2 solutions

guys, big issue here
On my wife's phone, she tried to take a picture and all of the sudden she got an error "sd card is being used as mass storage". now she cannot locate any of her files on the internal storage. as a matter of fact she doesn't have sd card. I checked under settings > storage and all I see is usb storage avaiable & used is at 11.50 gb.
also all backups are gone
can anyone help?
That happens when the phone is connected to your PC, if the PC is using it then you can't see it on your phone.
1. Make sure you aren't connected to your PC via USB.
2. Reboot the phone.
3. If still no joy then try a factory reset (there are probably other ways to fix this, but this is the simplest and noob friendliest way)

Hi everyone,
after 1 year expericing with evo 3d everything is ok,
but in the last month some problems are killing me.
1- when i put the usb connection to download image, after i get sd error and cant see any images from gallery. (files are visible with android assistan, visible after rebooting, tried different cables, different sd, the system doesnt sees anymore images of applications, maybe some refresh problem?)
2- when i turn off the screen and connected to wifi or datamobile, both turn off till i turn on the screen, its a very annoying problem...
3- also the music application cant find my mp3 folder with music.
4- when i open gmail at first it says "no data connection", even if i'm connected, and clicking refresh works. (but i have to click everytime!!!,,,,
5- today i got also a strange grey icon of the battery with an ? on it, first time ever seen it.
other miscellanea information:
the phone is all original, i'm using apex launcher instead of sense, there is a lot of free space on sd, the phone never fell on the floor, nor in the water. camera is working great and all other function, no lag in applications,,
i hope some of you great xda master can help me finding a solution,

1 and 3 sound like a bad sd card or card reader in the phone. The rest sound like an OS bug, which a factory reset should hopefully clear up.

i guess is the sd card, il try a factory reset. the new sd card that caused problem is a samsung 16 gb cas 6, i thoght it was good.
i mean wipe the garbage, like all those files left behind from applications uninstalled and no longer used.

Well, normally when you connect the USB cable and choose to connect the USB storage then the SD card becomes unavailable to the phone but the computer can then access it. Both the computer and the phone cannot access the SD card at the same time... I will spare you the technical reasons but you should know that this is normal. After you are done copying files with the computer then you must turn off this USB storage mode so the phone can access the SD card again and then it will see all your music and pictures again (all these are stored on your card by default).
This should all be fixed OK by a reboot of the phone though in any case. Are you sure you rebooted the phone completely? maybe you should try to remove the battery once?
If you've done all that then you can try to repair the SD card by taking it out of the phone and using micro sd to SDHC adapter and inserting it directly into a computer then using windows to run a chkdsk on it.

i've done all you said properly, you know the problem came out because there was a differente behavior than in the past.
i remove securely the phone from pc and then is not accessible,
i didn't tried to chdsk the sdcard i'll try, but it sounded strange to me because its brand new.
i also tried to contact htc , they said " try to reboot, or try to remove the battery and the sd and start the phone again, the problem wasnt gone.

Well if it's just gotten corrupted then you can just reformat the sd card. You'll want to backup all your data first so if you can't do that from the USB connection to the PC then you'll need to insert it directly into a pc. Once you've formatted it you can put your backed up data back on the card and put it back in the phone and after a while it will find your mp3s again. If it's truly a bad sd card then the reformat won't help.
Do you have another sdcard? The one that came with the phone or any other? Try putting some mp3s and photos on it and then put that in your phone. If your phone recognizes the photos and mp3s then maybe your current sdcard is the problem. And can you tell the the format of your sdcard? Is it NTFS or exFAT or FAT32? If it is UHS-1 card there is a good chance that it is in exFAT. To make it work on Evo 3d, convert it to FAT32 .

the sdcard was FAT.
yesterday i made a backup of all files, formatted the newer sd,factory resetted the phone and now everything is good, also the speed of the system is increased, thanks to all!! :victory:

It was very laggy, my screen didn't show when pressing the Power Button or the Home Button, and I thought that resetting to factory might fix it. Well, I was so desperate to make my phone work better that I didn't realize that all my files were on the internal storage, I don't have an SD Card. So, I've lost everything.
Now, I'm trying to recover some photos, but I can't connect my phone as USB mass storage to use a recovery soft on Windows 8. I rooted my phone, installed USB Mass Storage Enabler app, but it shows two drives (H: and I, but I can't access those. It gives me "Please insert a disk into Removable Drive (LETTER"
Please help, I did everything I found on forums, but still nothing. Those files might be lost, but now I have another battle.. Making my phone available in Mass Storage.
And by the way, it still works terrible with 4.3. Hate it so much!

Unfortunately I think USB Mass Storage in the way it is implemented in 4.x for the moment can only share the external SD, the internal one is never visible.
IMHO the only approach to recover "something" is to get an external microSD larger than the internal partition and create a block copy (with dd under adb in CWM/Philz recovery for instance) of the entire "internal SD" partition - that one will be an EXT4 (Linux) partition) and there is a very, very small chance that you might recover some stuff from it (I doubt but who knows).

Such error happens if transfer breaks unexpectedly. Think what file did you send lastly and delete it, it should fix your issue.
If you can't remember or it did not, formatting your external sd card will fix it for sure.
