Looks like I'm late to the party, but here is a .PSD template. You can literally copy and paste your screenshots onto it and then export it. Fast and easy.
{
"lightbox_close": "Close",
"lightbox_next": "Next",
"lightbox_previous": "Previous",
"lightbox_error": "The requested content cannot be loaded. Please try again later.",
"lightbox_start_slideshow": "Start slideshow",
"lightbox_stop_slideshow": "Stop slideshow",
"lightbox_full_screen": "Full screen",
"lightbox_thumbnails": "Thumbnails",
"lightbox_download": "Download",
"lightbox_share": "Share",
"lightbox_zoom": "Zoom",
"lightbox_new_window": "New window",
"lightbox_toggle_sidebar": "Toggle sidebar"
}
Included in the .zip file are two files, one with the AT&T logo, and one without it. The layers are setup like this:
1 Glass Reflection
2 Phone Border
3 Blank Black screen
4 Phone
5 Background
You should paste your screenshot between layers 2 and 3.
If you press ( CTRL + ; ) you can show the Gridlines so that you can line up your screenshot perfectly.
ZIP is attached below or you can download from Dropbox:
No Logo .PSD
With Logo .PSD
Both .PSDs
Nice. I'm interested in a white Galaxy Note II, too.
Would love to have this .. but link takes me to a file removed message .. any chance to get it?

This is something i posted for the EVO 3D but it works on this phon too due to the same resolution.
Hi guys. This Wallpapers are edited by myself and i want to share with you. The Dock is taken from a skin for Sense 4(i don't remember the name of the guy) and it is edited to match perfectly with the Highlighted items in Dock. Also the divider lines for the "All apps" Menu are more separated to match with the selection and the dock was squared(i like it more). All the Wallpapers in qHD(960*540) are in the Zip File, don't download the thumbnails.
After you have applied the Wallpaper go to market and download this skin: https://play.google.com/store/apps/details?id=com.htc.skin.szp_sade&feature=search_result#?t=W251bGwsMSwxLDEsImNvbS5odGMuc2tpbi5zenBfc2FkZSJd
And you should have a Home screen like this:
Pack 1
{
"lightbox_close": "Close",
"lightbox_next": "Next",
"lightbox_previous": "Previous",
"lightbox_error": "The requested content cannot be loaded. Please try again later.",
"lightbox_start_slideshow": "Start slideshow",
"lightbox_stop_slideshow": "Stop slideshow",
"lightbox_full_screen": "Full screen",
"lightbox_thumbnails": "Thumbnails",
"lightbox_download": "Download",
"lightbox_share": "Share",
"lightbox_zoom": "Zoom",
"lightbox_new_window": "New window",
"lightbox_toggle_sidebar": "Toggle sidebar"
}
Pack 2
Pack 3
Here is the Dock Bar. You can take it to make your own Wallpapers. Just download the Bar, select a Wallpaper(540*960) and center the dock in Paint or whatever program you like to use.

This is just a very simple modification of the default theme. I'm sure like me many of you have struggled to adapt to the home/back/terminal buttons jammed way up in the upper right corner. All this is, is the default theme w/ those three buttons moved to the lower left instead where it is much more comfortable and more natural to use. Nothing else has been changed.
Paron my reflection. No way to take screenshots of recoveries on the N10
TWRP version needed: - 2.3.3.0
Install: In your TWRP folder in the root of your "sdcard" make a folder called "theme" and drop the ui.zip into it
Uninstall: Delete the ui.zip

Hi
very simple, small Mod (or maybe Theme) for S5 S-View
invisible shortcats (shortcats still works but only are invisible until is Pressed)
copy apk to system/app (with a file explorer like RootExplorer) an replace with original file. change permission to RW-R-R and then restart.

Hey, guys, we know that some ROMs don’t have the “Delete” button on the screenshot preview like the Sony Xperia series. Hence, here is the guide that can help you add “Delete” button to your screenshot preview for your ZU.
PS: Full Guide can be found here: http://xswxm.com/?p=1023
Recommed:
Officail 108&118 odexed ROM
Tested Deviced & ROM:
C6833 with HK_108_ROM
Steps:
1. Backup your ROM or just backup the files ("/system/priv-app/SystemUI.apk" & "/system/priv-app/SystemUI.odex") will be covered later;
2. Unzip SystemUI_ODEXED.zip and copy the files to "/system/";
3. Set 211 or 644 permission for these two files;
4. Copy them to "/system/priv-app/", just cover the files inside;
5. Restart your cellphone and enjoy!
Download: SystemUI_ODEXED.zip (Notice: Not Flashable)

I tried flashing the zip file from DespairFactor found here: http://forum.xda-developers.com/showthread.php?t=2563757&page=25
My emoji keyboard looks like this: https://app.box.com/s/u2a6c773yg7j3fhqom2v05t6k69ls5ag
And no emojis show up on the screen. They don't have the box either. It is just blank space where they were.
I am using a Nexus 6 on 6.0 and its rooted.
Solution Found
The Problem is fixed now, in case anyone else had the same problem you can flash the zip from Caiosilva96 which can be found here: https://drive.google.com/file/d/0BzL4kz3FPLIJYl9QQ25iRnNlM2c/view
You can't access them on the keyboard until Google updates the keyboard, but what I did was put them in my personal dictionary with "mf" giving me the middle finger and then I also put the skin character in there as "c1" "c2" etc and you can add skin colors to the emoji's. Here is what mine look like on the Nexus 6
