Ok, so I have never had this much of a problem before. I have an S-on, unlocked Rezound.
The problem started when I was trying to move from BAMF Rezound ICS v2.1 Official to ViperRez 1.0.3. I backed up all my data using TI backup to cloud and then did a complete recovery backup in amon ra. Then I flashed ViperRez and went to bootloader and flashed the PH98IMG file. I rebooted the phone and it was working. When I started restoring TI data and apps I made the mistake of restoring data without a few apps and following a reboot, an error would pop up telling me android.phone stopped and shortly thereafter the phone would reboot(cycle).
I took out the battery and tried to get to recovery through bootloader and forgot to delete PH98, accidentally pressed up and it reflashed. I found my sd card reader and removed PH98 from the SDcard and rebooted to bootloader to get to recovery. That is when I could no longer get to recovery. With a power on the phone is stuck on HTC splash, I tried amon ra 3.15 and 3.14 flashing through fastboot cli. I was able to flash and get to CWM and TWRP, but neither lets me mount, so I cannot try a new ROM. HELP!

I cannot get to Amon Ra recovery though. I can get to TWRP 2.1 or 2.2, and CWM 5.8.1.4.

I would probably start over with the same or newer RUU. that will put phone back to stock, with stock recovery. You will have to unlock again, then flash a custom recovery, and custom ROM.

If I can't flash a ROM due to the /system /data and /cache in recovery, am I supposed to flash in fastboot? ie fastboot flash boot boot.img? I'm worried that my internal partitions are having issues. How will this fix the problem? Which RUU should I be using, 3.14.605.5?
I attempted to flash Vigor_Stock_Doedexed_3.14.605.5.zip in TWRP 2.2(I can only get to TWRP or CWM recoveries if I flash them in fastboot not amon ra). It failed. Here was the readout:

I said I would post a follow up with details on how to get out of this situation in case anyone else comes across a similar problem. Basically it comes down to the internal storage needs to be rebuilt, and to do that you must flash an RUU. In my case, I was S-ON and Unlocked, and was worried about re-locking. That was my impass.
Step by step:
1. Re-Lock your device. (Make sure you have your Unlock_code.bin so you can unlock later)
2. Find a good RUU for your device, either GB or ICS depending on the current setup. (check md5)
3. Rename it as PH98IMG.img and place it in the root of your ext SD card.
4. Power on to Bootloader and flash the PH98 file. (To complete the process, it will reboot and continue)
5. When this is done, it should boot into the OS. Remove the PH98IMG file from the SD card by mounting in the OS.
6. Boot to bootloader and flash amon ra. (I used fastboot to flash the recovery.img file)
7. Just for kicks, run a backup in recovery.
8. Root the phone in recovery.
9. Boot the phone and turn on usb debugging.
10. Run S-OFF procedure. (make sure you are using the correct program for your device, GB or ICS)
11. Flash the Hboot version of your desire.(2.27 is current as of this date. Maybe try one of apophis9283's sweet new hex edited versions)
12. Flash recovery again.
13. Put your favorite ROM on your SD card and flash in recovery.
14. Enjoy!
I prefer safety measures, so I would run a system backup at this point again. Then restore your data through Titanium backup cloud or other method of your choice. I was provided a great deal of help and moral support from apophis9283 and I'd really like to thank him. If you have any questions feel free to ask. Most of the methods discussed are elsewhere on the forum. Some of the All-in-One toolkits might also be very helpful. The Roll-up sticky in the development thread should have all the links you need. I've been searching through posts for help. But I might have run into a combination of bad luck and noobness, and I can't perform any suggested recovery procedure that I've found smoothly. If you think my problem can be solve by another answered post please link.
My phone was rooted (from the HTCDev tool) and sim unlocked. This morning I started the Update pushed from HTC (that should be the ICS update???). The update last more than 40 minutes (I wasn't watching) and stuck on the HTC boot screen. I thought it's problem hanged, unplugged the battery (maybe I noobed out here? shouldn've waited 4 hours? or do a hard reset instead?).
Anyway, from that point onward it's hang on boot.
I boot it up the the hboot menu. Go to recovery, tried wiping the cache, tried the factory reset. No luck.

If I remember correctly to update to the official OTA you need to put the stock recovery back on your phone and re-lock it.
I don't really care whether it is official OTA or not. I just want to get my phone back to work. Do you have any link to where I can get the OTA and how to do it?
Does it require turning S-OFF? I'm under the impression it is blocking me to do anything. And because my phone is already in a bad state a lot of the tools is not communicating with the phone right now...
No luck with the OTA. I was able to get CWM onto recovery through fastboot, and also tried flashing King Cobra. But the phone still won't be with King Cobra, I'm still on S-ON. Revolutionary still won't connect.

I had an unlocked EVO3D (htcdev method) and the problem you had happened to me too. The phone stuck at the initial splashscreen and iI had to pull the battery off.
To have my phone back it was enough to use 4ext to restore a saved backup I had
done previously.
If you dont have backups then installing KingCobra (1.5, I have issues with 1.6) should be enough to solve. Copy the rom to the sdcard and then install it via 4EXT.
If you want to go S-OFF i recommend to use the wire-trick method
unlimited.io/juopunutbear-public-beta-0-1/instructions

How do you get 4EXT onto the phone? Is there anyway to flash it to recovery? Or do you already have it flashed before? I was trying to find a 4EXT recovery flash but no luck.
I tried using CWM flashed to the recovery and installed King Cobra, it installed but still doesn't boot up. I also tried flashing boot.img onto boot (by using fastboot) but the phone still doesn't boot up. The firmware (PG86IMG.zip) wouldn't flash tho, so maybe I need either S-OFF or 4EXT to do that.

4Ext was already flashed. However what you did should be ok. Are you sure that the boot.img that you flashed is the correct one for the ROM that you installed?
The boot.img on the KingCobra main page belongs to the 1.6 version.
You can extract the correct one for the 1.5 version from the ROM zip file.
The path in the zip file is Custom/Kernels/Stock/boot.img
I used the 1.5 version with the original radio without problems.
Fixed this by:
1) Getting the official RUU on ICS.
2) Connect USB in HBOOT
3) Use fastboot to relock the bootloader
4) Run RUU on Windows and successfully flashed everything
So seems like S-ON pluse unlocked bootloader means no no to both official rom and custom rom.

There's an issue with the 2.3, try to downgrade to 2.2 and see if that works

Select open command window here
Connect the phone to pc and make sure it says fastboot usb
In the terminal type fastboot devices to make sure

Select recovery
Wipe phone and flash rom

Your S-off, why don't you use the PG75IMG.zip of the twrp 2.2.2. Put it on the root of your external sd card and boot into the bootloader let it install. After it installs, you have to remove the file from your sd card or it will keep on installing everytime you enter the bootloader. Here is the link to download it:

Thanks everyone. I'm up and running again on the EVO LTE. I did use the bootloader method to flash the .zip. I also now think my primary issue was self inflicted - I was using a 32gb micro SD card and I don't think the phone liked that. When I used one of my 8GB cards everything seemed to work like a champ.

Not an RUU, a ROM image, there is no RUU available for the current version of software (which according to your radios & hboot you have already) so now you need to flash a ROM with a custom recovery. You are probably getting the security/signature warning because you are not unlocked, you are relocked... You must be unlocked to flash a non-stock (signed) rom.
Make sure you have fastboot working (part of ADB or mini-ADB) and are unlocked
Download the current TWRP img file for Rezound here and place it in your "fastboot directory"
Download a ROM, for stock try this one by MicroMod777 and place it on your microSD card
Start the phone in hboot, and select fastboot, and connect the USB cable
On your PC, go to your fastboot directory and enter "fastboot boot openrecovery-twrp-2.7.1.1-vigor.img" and a green bar should appear in the display, followed shortly after by the TWRP recovery menu
Navigate to the Install/Flash section, find your rom ZIP file and install it
reboot and you should be good to go
i've got twrp successfully installed, but all i get when i try to flash the rom you recommended i get "error executing updater binary in zip". any thoughts?

Check the MD5 of the ZIP file, is it correct? You are browsing to the ZIP file and selecting it, correct? Is the any other error, something about emmc?
the zip was good and ive tried multiple roms, no emmc error, is there a way to s-off and flash a ruu maybe?

OK... Do you have working fastboot and access to a microSD card reader?

First, remove your SD card and put it in the reader connected to a Windows machine. Open My Computer and right click on the microSD drive, select Format, change File System to FAT32, and Allocation Unit Size to "Default", untick Quick Format and select Format... This will take some time. Then copy the untouched ROM and Gapps ZIP file and the CleanWIPE zip file to root (main) directory of the SD card. Use MD5 Calculator or something similar to verify the MD5 image on the SD card. Reinsert the SD card in the phone.
Make sure you have the TWRP recovery image file (openrecovery-twrp-2.7.1.1-vigor.img) in the same directory as fastboot and adb.
Start the phone into Hboot and select FASTBOOT, it should show FASTBOOT USB in red, then connect your PC and wait for it to connect. Start a CMD session and go to the fastboot directory, enter the command:
fastboot boot openrecovery-twrp-2.7.1.1-vigor.img
This will transfer TWRP to the phone and boot it, note that this is different than starting TWRP on the phone directly, it gives TWRP access to more stuff than normal.
Now select Wipes, Advanced, and tick everything except External Storage or MicroSD Storage and wipe it, then repeat this 3 times (note that Internal Storage may change names to emmc, this is normal). Watch for any errors... If there are none go back to the home menu.
Install/Flash the Cleanwipe ZIP, watch for errors.. When this is complete go back home and Install/Flash the ROM ZIP, if there are no errors go Home and Install Gapps, if still no errors reboot the device and disconnect the USB cable.
There is currently no way to S-Off the device without it being bootable.
i was running vipermaxspc 2.0 rom and wanted to try another (adrenaline 1.5 sprint) so i loaded into twrp with the rom on my sd card. to install it it told me to use swipe wipe so i used that, after getting notified adrenaline was installed successfully i rebooted and ti would not load anything so i hit factory restore feature in the hboot thinking that's what i needed to do. a few things later and now i can not enter TWRP (recovery mode) it flashes for a split second and then automatically reboots, i am able to still reach hboot and fastboot usb. i can lock and unlock the bootloader. i took a micro sd card out of the phone since i was seeing errors

i am now back in the TWRP screens hoping i don't do what i did before and somehow the magic works this time. i guess this is called a Softbrick not a bricked phone. hope this helps others who are newer to these android magical wands.

Hey guys, just ended my contract with Verizon and started using Cricket wireless. Up until this point everything went smoothly, just had to swap out the SIM and not much else. Because I no longer needed/wanted all the bloat and VW apps, I decided to s-off and root; I used Sunshine as I had the stock 4.09.605.5 and no other form of rooting or... soffing?... would work. As of this morning, All of that was accomplished and I was left with a bootloader noted below:

So at this point, I still had stock rom, but applied s-off, had root, and importantly - I think - I was using the built in android encryption. So at this point I flashed TWRP successfully using the WinDroid Universal toolkit and started the process of installing a new rom, specifically the ★ ☆ [ROM][5.0.1] Google Play Edition | Lollipop | v2.00 [02-06-15] rom, but ran into a few problems.
first, I could not backup my current rom at all. I even flashed CWM and tried it there but only got 'Waiting to mount SD Card' timeouts so I switched back to TWRP. On my last try, I received nothing but errors, a selection of them below:

Upon rebooting the phone, it loads the Google Play bootloader animation, but the next screen brings up the 'Encryption unsuccessful' notice with the only option to do a factory reset. When I select that, it reboots into recovery, then starts/fails a Factory reset:

Then recovery closes, the boot animation starts, and then I'm back at the Encryption Unsuccessful messagebox. At this point I do not know how to proceed with fixing this; I have no problem formatting my phone as all my data is backed up elsewhere.

1.) Never use toolkit to do anything. Use fastboot for flashing recovery. 2.) You need stock recovery installed to use fastboot's factory reset option.
I'm going to need some handholding here, bear with me:
1) I can get to the Fastboot page on my phone, and I can plug it in to get FASTBOOT USB, but adb does not see the device. Nevermind, I used adb and not fastboot. Still not sure on which steps I need to take there.
2) How do I go about reinstalling the stock recovery?

Since you're unlocked, use fastboot to flash recovery. Once that's done then flash ROMs in recovery. Doas beaups says and don't use toolkits to flash things.
If you want to start clean from square one, use an ruu to flash the stock ROM, recovery, etc. Then do as I said above: flash recovery in fastboot then flash a ROM in recovery.
So it turns out it had nothing to do with TWRP or the rom. I used the full format option and it worked, then installed the ROM again and everything works.
Mods, feel free to nuke this thread as it's not really helpful in the end.
So the lesson is to make sure your phone is not encrypted before flashing/modifying things,except if you do a full reformat.
