I got this problem after applying the Tmo update. The system sound (the water ripple sound on lock screen, sound when restart/reboot) randomly goes muted. There is a slash across the system icon in settings/volume. There is no way I can turn that on again unless I restart the phone (set all the way up doesnt help since there is a slash indicating that it's muted). And this happened to me 3 times now. After restarting, the slash disappears and the system volume goes to Zero. Pretty sure the update causes this since I didnt have this problem before the update. anyone has the same issue?
P/S: all other sounds work fine, just the system sound messes up and now it works again...randomly.. gosh what happened to my phone (
Im suspecting the app "hide it pro" causes some kinda conflict since it also has the function as audio manager... not really sure...

I am wondering if anyone else is having an issue where after making setting adjustments (such as ring tone, or screen brightness), once being plugged in for charging, there settings are reverted back or simply enough...changed.

Yes! I have noticed this too. I keep setting my System Volume to about 50%, and it just randomly gets reset back to 100%. I can't figure out of it is an app that is changing it or if turning the phone off and back on does it, or what. It's very annoying.
Hopefully we can determine what's causing it, or if it's a bug and we need Samsung/T-Mobile to fix it.
Your volume is problably from Maps. Everytime I used Google Maps my volume went from whatever I had it set to to 100%
I was having problems with everytime I turned my phone on my animations went from none to full. Drove me crazy. I ended up switching my rom from Stock to Vibrant6 and the issue went away. I haven't tried going back to the first Nandroid Backup I made to see if it was a stock problem or if it was something I installed.
I do recall that after I played with my Font it started happening. Of course I'm not sure if that caused it or not.
I have had this problem ever since Clockwork recovery. if I reboot to do a nandroid my fonts go back to default however whatever i have chosen (ie choco) is still highlighted. the system volume changes at the same time too.
i asked over at cyanogenmod and i seemed to be the only one.
i was about to post a thread about this too lol. mine isn't rooted. i had my ringtone on the silent one, but all of a sudden i hear the tmobile jingle. i was confused cause i had it on the silent ring tone. also have this issue: once i get a text my lockscreen stays on, it doesn't turn off or anything.
I have the issue with ring and notification tones getting changed back to default.
It doesn't seem to matter where I store them.

when i turn it on i have sound working good.
but after very quicly, it have no more sound at all (even when i push the sound + or soung - button, i see the sound logo going up and down but no sound)
and if i turn it off and on the sound come back.
do you know what can be the problem ?

Happens to me ALL THE TIME. I just chalk it up to WebOS being completely unfinished. But then again it could be some of the patches I have installed, since I spent all of about 3 minutes without them.
Me too, I read that Making a call in skype will bring sound back.
Apparently it helps to disable system sound, keyboard and vibrate.
Happened to me twice more today. So annoying.
Happens to mine pretty much daily. I don't see a whole lot of *****ing in general about it though, mostly here and Precentral, which leads me to believe it's mostly "modified" touchpads with this problem. I'm thinking it's a patch or kernal problem.
It's most likely a problem with a kernel or patch. I have 3 16GB's, two stock, one modded, and none of them exhibit this behavior. Since all of you have modded yours, I suggest removing patches/kernels and see if the problem persists.

Install the Preware Linux App > Increase TouchPad Volume ( Lite)............
See if it makes a difference
I haven't figured out what causes it for me, but a simple solution to fix it without having to restart is to run a couple commands in xterm..

Finally , I had to disable the email audible notification (left the vibrate only). Now it has been OK for more than 1 week, before it will lose sound in a few hours.

If you install the "Advanced Reset Options" patch, you can use it to restore your sound...
Just hold the power switch until the buttons show up, then tap "Cancel".

Advanced Reset option does not fix the issue for me (unless I select Reset).
Sound fails even if the only App I have opened since a full reboot is the email App.
Maybe one of the installed Logging patches is causing the sound to drop out? Maybe we should enable logging to see if the logs give any indication around what is causing the sound issue?
this issue was happening to me everything my touchpad would go idle for longer than an hour (super annoying) however I gave all the suggestions here a try and finally had luck with robrob777's suggestion! Turning my email notification sounds to vibrate has seemed to fix my issue hasn't happened in 2 days. Just wanted to say thanks robrob777!

Sorry about the format of the link, but forum rules for newbies prvented me from posting it properly. Since it obviously isn't spam, I am hoping I won't be in trouble for it.
It's complicated, and you might find a reboot easier, but perhaps some developer who's into patches will spot it and come up with a more permanent fix, either to fix the problem itself, or at the very least, to have the stop/start fix applied when the volume control button reaches max and is pressed again once or twice.
Update: It just became a little easier to apply the start/stop "fix". XServer, Xterm, and Xecutah just became available on webos-internals (.org). Just use Preware, select Applications, All, and the new stuff will show at the top of the page. Just download Xecutah and the other two packages will download with it if you allow it.
Update 2: Even easier...
After installing the X stuff, start Xecutah and create a shell script in the default directory you land in with Xecutah. I just used cat and typed it in.

Here's what I put in it...

stop pulseaudio
start pulseaudio
Then, fix up permissions...
chmod 070 fixaudio
(It's been a while since I used Unix/Linx, but I'm sure someone will correct my permissions choice if it's not right).
Then, when your sound goes away, start Xecutah and type fixaudio.
Even better... Use Preware to find an app called PulseAudio Restarter. Works well.

I can't say exactly which one it is, because you can't select it for any message/notification type. It even plays the sound when I am in silenced mode.
Any one else with this issue? :/

I think one of the updates added a notification sound when you unplug the charger cable. May be that's what you are hearing

A recent Nokia app update has added a bug that plays a notification sound when unplugging from a charger. It has been reported to the relevant development team but I've got no idea how long it will take to correct it.
As for a notification sound when unlocking the 920, there is a setting for that under Ringtones + Sounds. But it shouldn't play if the phone is in silent mode so it could be related to the other bug which has also been implicated as a possible cause of some apps not working and requiring a hard reset to get the phone working properly again.
I've been living with this problem for quite a while so there is a lot to say. I can't stand it anymore & consider installing custom ROM, but before that I'd like to get the opinion of more experience ones if this might be a software glitch (corrupted ROM) or actually there are some hardware issues (if so I should give it back under warranty first). If there is a simple fix / another brilliant "feature" to switch off I would extremely thankful for your help
Story:
phone Verizon S4 i545 bought directly from the carrier in Aug'14
initially worked like a charm, rooted, living the dream
first update - the infamous "broken one" making phone supper sluggish and freezing all the time; also lost root; had to do full wipe which fixed it.
another "small" update - main change is that now I see unlocked padlock at the boot screen (guess b/c of long gone root) & unable to check the KNOX counter in download mode
my current version is VRUFNK1 and that's when my current problem started.
Symptoms - all I've observed so far
Quite often during making sounds (music, game, alarmclock, even caller) all the sounds dissappear. Totally silent even though phone is not in silent mode nor the app crashed (music soft continues playing the song, call still on but neither I can hear other side nor they can hear me, no sound on recorded video). The reboot doesn't help.
What actually brings the sounds back is system notification or repetitive start-stop the music app (not the song play-pause) until it finally brings the sound back.
If I decide to reboot there is sound on Verizon logo boot screen, but the phone gets back to the muted state after booting to main screen. Furthermore, sometimes after reboot it becomes supper sluggish and freezes for couple seconds whenever any kind of sound is supposed to be played.
I found that this behavior is triggered by a stronger tap (or any other vibration like running) on the upper part of the body.
Some apps seems to be more prone that the others (Endomondo w/ GPS on, game "Deck Heroes")
At the moment of loosing the sound I can a "scratchy" digital noise for a fraction of a second. It sounds familiar to when the mp3 is corrupted.
Phone have never actually fallen, but cannot rule out some generic physical damage inside
Things I've tried & failed to fix the issue:
full wipe (again)
all the samsung rubbish switched off, no motion / gesture control, no smart screen etc
"now google" detection disabled
safe mode
safe mode + all connectivity off
no sd-card (never had one)

I've had the following issue for the past 30 days (at first I thought it was some glitch that caused the UI to crash when phoning-home to Cyanogenmod for updates). It SEEMS to have happened right when I first started screwing around with TitaniumBU... that may be a total coincidence, also...
The phone works just fine but will suddenly start showing things in Landscape instead of Portrait (and vise-versa). After I reboot the phone boots to a black screen with just the status bar at the top (showing the time, wifi, etc...) with absolutely NOTHING ELSE except for what appears to be a "back" arrow at the bottom of the screen (that does nothing when pressed).
WTH?! Please. Because I've tried everything (to include wiping internal and ext-SD). :crying: I have, of course, searched other forums but "Android UI Crashing" returns a ton of unrelated results...

I'd wipe the whole internal storage (including ROM) and reinstall the ROM.
Dude - WAY ahead of ya - already did. To the extent that I accidentally soft-bricked the stupid phone!
We all know, as engineers and troubleshooters, that the first step to solving a problem is duplicating it.... and I cannot seem to consistently duplicate the problem! And the crux of it is that this started at the end of December when I was bored around the holidays and playing with my phone (making several different changes).
Obviously I'm not restoring anything from a previous backup... I've even blown away my backed-up App settings in my google account... Formatted Internal and External... Installed latest OS and GApps...
Gun to my head; if I had to guess I'd say that it has something to do with the "System Profiles" feature... I started using it to disable my Lock Screen whenever it detected my home wifi, and to return to the System Default (with the Lock Screen enabled) whenever it no longer detected my home wifi... Because I swear that the crashed-phone looks the same as the screen where you enter your PIN with the addition of the status bar at the top...
The issue has to do with System Profiles to use wifi-detection for toggling the lock screen. Not sure what it is about this that causes the issue, but that was the cause.
