I'm sure someone has ran into a similar issue.
I have a Sprint HTC EVO 4g lte.
Issue: I'm constantly being booted into my bootloader (Where I can see the mini me droids on skateboards)
My TWRP v2.3.3.0 is still fully functional, but for some aparent reason, after i flash a backup, or a new fresh rom from my SD Card, it completes the install and tells me to reboot.
After rebooting, I land straight into my bootloader with no hesitation.
I initially had Viper4g_2.0.2 rom and tried updating the Kernel for custom tweaks (Undervoltage, Underclocking, etc.), but I ended up here
Your assistance will gladly be appreciated
If I missed anything, please let me know.

Yes, I am. I've been S-On ever sense I rooted my phone (over a year ago). The customer Kernel I tried using with the Viper4g Rom is named Beastmode.LTE RC7 by Zarbos which is S-On friendly if I'm not mistaken. It worked well, until i did some extreme tweaking to the voltages and underclocking.
Do you have any suggestions for a fix?
Or am I S.O.L. ?
I never had any luck with that particular kernel on ViperROM, however, I successfully ran the Liberty kernel from dev Evolutionmods. It's S-on friendly and can be flashed from recovery.
One other thought: You didn't do a system wipe from recovery after restoring your backup, did you? Go into recovery, wipe Dalvik cache, data, factory reset and wipe system, then try reflashing Viper like new. The system wipe will remove your current ROM from your phone.

Otherwise, you may need to reflash your ROM completely to clear the issue. You can also retry flashing the kernel again through recovery.

Thanks a lot for your input. However, I tried flashing two different new roms Viper elite 1.2.1(older version) and Viper4g 2.0.2 (newer version) and they both landed me straight to my bootloader after the installations were done.
I tried reflashing the customer Kernel since its still on my SD card, but neither one of these methods have helped me so far.
I don't even see the HTC Sprint logo animation anymore when my phone boots up, it just takes me straight to my boot loader.
How long have you had your phone, you say?
Edit: Don't try flashing the Zarboz kernel any more, at least until you get the phone up and running.

No. I never wipe after installations. I only wipe prior to flashing roms/updates/backups.

I would say for about a year or a little less and never had a problem until today, when I decided I wanted to expand my horizons with custom kernels (For purposes like; underclocking/overclocking, undervoltage, battery life enhancements, etc.. ).

I tried following these steps and I received successful for wiping Cache & Dalvik-Cache set and failed 'factory reset wipe'. It seems I may I have a problem with Updating partition details (What does this mean)? Ex. Shown below:

No I don't think so, I have received those same errors when clearing cache and my phone did load a ROM and works just fine.

I was having a similar issue today, still not real sure what caused the problem but I was able to find a stock/rooted ROM and loaded that through TWRP and I'm back up andrunning just fine.
I am going to try and download a custom ROM again and load it up to see if I still have problems. I am S-Off but from what I've read S-On shouldn't necessarily cause problems loading a ROM.
I would try the ROM's found HERE and see if they'll load up for you. If they do then you can be sure your phone is working and start figuring out why the customs aren't loading up.
I'm not sure if viper has a s-on installer but I know mean Rom has. Your s-on so you have to flash the kernel separately unless the Rom does it for you. Try mean Rom and I strongly suggest you check out the dirty racoob thresd

As a last resort, you may have to format internal storage. Also, try removing SD. Does your internal mount in TWRP, or external?

I followed these instructions and flashed the jewel_1.22.651.3_rooted deodexed_8-27-2012.zip via TWRP. It did show successful but I'm noticing that i receive the same message regarding mounting after every flash (rom, backup, kernel). Example shown below:

I can not mount my SD card or my internal storage via TWRP. I'm starting to think that my TWRP may be corrupted when it comes to mounting and partitioning SD card and internal storage. (a new recovery?)
Do i need to install/flash a new recovery, or should i try formatting the internal storage by mounting USB storage via TWRP and formatting it via my PC?
I had a similar issue awhile back... Everything would wipe but somehow it seemed my system partition had corrupted... I had to run a pj75img stock zip in order to get anything to install or restore... Phone finally booted and I was back to normal... I was s-off too though..

OK, go here: http://unlimited.io/jewel.htm and follow their instructions through step 11. This will restore your phone back to stock and unlock your phone. You then will have the option to continue their steps and s-off if you want or you can install twrp and continue being s-on. It is important to look at step 9 and read it carefully. If you get this error just follow step 9 to the letter and it should re run the ruu and complete. VERY IMPORTANT, make sure you chose the correct ruu. Only run a ruu that is correct for your current hboot. Make sure you use Dirty Racuns ruu for this install. I think this will fix you up and like I said, at this point you might as well go s-off.

First, you're in the wrong forum. This forum is for the original EVO 4G, not the EVO 4G LTE. You can find the right forum here.
Second, you're S-on, which requires the kernel to be flashed separately from the ROM. Some ROM's include an S-on kernel flasher to make flashing a ROM while S-on easier. Other ROM's require you to flash the kernel using fastboot commands or an app called Flash Image GUI. What ROM are you flashing? It sounds like you did a system wipe in recovery, which would have wiped your OS. Did you bother making a Nandroid backup prior to trying to flash the new ROM?
Check out my guide here. It contains a lot of useful information regarding the different Hboot (bootloader) versions on the EVO LTE, and also has a guide for flashing kernels while S-on. Your best bet is to gain S-off via Dirty Racun or Facepalm

Second of all, all I am attempting to do is to restore my phone by way of TWRP to my stock OS...I just want my phone to boot for now.
In doing so, it will not allow me to even format and displays the message I previously copied.
Do you think I should go ahead and attempt to "facepalm" even though I am getting these errors about not being able to mount my internal / sd card?

Try installing a ROM like MeanBean first. It has a kernel installer for S-on. After you get your phone up and running I would look into getting S-off.
You should PM a mod about getting this thread moved.

Sorry - that's my bad. I've asked for it to be moved to the correct forum
I am having trouble getting it to install.
I think it is a problem with the partitions, so I plan on superwiping it first then installing meanbean.
I will get back and let you know how that works.

Unable to mount '/sdcard'
Okay, so I am still having these issues...
What I have done now is:
I took my sd card out, put it in a friends GS3 and formatted it then put my TWRP file, smw6180_superwipe.zip, and MeanBean-v309-jewel-ltevo.zip
I put it back in and installed the superwipe and it ran optimizing:
data
system
cache
devlog
boot
All of these ran perfectly
When it got to the next step
"Updating partition details...
E:Unable to mount '/sdcard'
E:Unable to mount '/sdcard'
E:Unable to mount '/sdcard'"
was displayed and it stopped...
This same message is displayed when flashing roms
I have been working on this for 3 days now with now success
I have no OS installed and really want to get it back up and running, any help would be greatly appreciated!
What version of twrp are you using?

I'm now updated to v 2.5.0.0 TWRP, but I am still having the same error.
I found a fix for it on here but it is for a One X (it has no micro sd) at this link

This post, although made by a junior user (2 posts), arises my concern.
I wouldn't use that method never know what will happen even though it does list the LTE
You could flash a Rom thru fastboot, I don't like to, also you could do a ruu and re-root it. I've had todo that a few times...
Okay I've tried everything...I rooted my phone using the adb method and went to install a rom. Forgot to wipe after installing the rom and since then, this phone's been on the fritz!
I can boot into bootloader and into TWRP recovery...that's it! Most recently, I've tried to install MeanBeam (suggested since it is more up to date and Sense based), followed other forum suggestions, but nothing! The phone will not boot up. Normally, I'd be able to fix it myself but I guess every geek has their down days! HELP PLEASE!!!

I've read that since I am on S-ON, I have to fastboot the boot.img of these roms in order for them to boot up. The problem with that is: I cannot get this thing to "fastboot usb". I've tried two different cables and two different laptops. It will not connect. I bought this phone online and it did not come with the original cord! Am I completely screwed on this one???

What happens when you install Mean Bean?
You are S-on, so you must install the kernel separately from the ROM. MeanBean has an S-on installer, so you must flash the ROM twice to get it to boot. Follow Mikey's instructions in the OP for his ROM.

I was recently S-ON, but on the latest HBoot and Firmware and I was able to flash roms without any issues. While I didn't flash the Meanbean, I was successfully able to flash the roms listed below without doing anything differen than as I would if I was S-Off. Before flashing, I do the proper wipe (cache, dalvik, system etc) and never had any issues.
I'd try a different sense-based rom. Most of the ones are up to date, like

My backup somehow got erased between the time I started this mess and the first rom I tried to flash. Did a wipe and suddenly it was gone.
You can plug it in to the computer via fastboot usb. Move the boot file from any rom you want to flash to a new folder. Open a command Window and type fastboot reboot boot.img. Then reboot back to recovery and flash rom

If you are using twrp 2.5.0.0 and chose wipe you may have formatted your sdcard

How big is your MeanBean download?
Also, there is no need to copy the ROM to your internal storage. You can flash it from your SD card just fine.

I have read this as well and am attempting to install via ext sd card now.
No go!!!!
To start: dropped my old one, shattered the screen. TEP'd it, and got a new one and decided to unlock/root it.
I used Hassoon's kit to unlock, and install recovery. No problems here.
I failed to make a backup at this point, and i'm kicking myself now.
I flashed CM 10.0 (stable version) and rebooted. this sent me into the CM load screen and never left.
Rebooted into recovery and tried to flash again, still same result.
I tried to RUU it with the 3.15 RUU, (image version 3.16.651.3 -> Update, verify 3.16 -> 3.15)
-Gives error 140 Bootloader version error
ADB does not see the device, but fastboot does
D/l'd Mean bean v284 and tried to flash it from recovery, failed, couldn't mount either internal or external sd card
Tried to RUU again with the 3.15, Error 155 Unknown Error and the phone stays on the HTC black background screen, no update bar, no ! in the corners. Go through the recovery process and when I reboot, it stays on the HTC quietly brilliant white screen. Force reboot to recovery, and the RUU says error recover successful. Continue through, and it fails the same way again.

I'm getting frustrated (well past frustrated) and need some help if anyone has a clue as to what I can do.
First off, you can't RUU backwards. Second, since you're S-on you have to install the kernel separately from the ROM, otherwise the ROM won't boot. Third, since you're on the latest firmware, older versions of CM won't work due to a touch panel driver update in the latest firmware. The screen won't respond to touch input if you don't use a ROM based off software version 3.16 (or on AOSP, you have to use a ROM using the newer 3.4 kernel). You also must use TWRP 2.4.1.0. or newer with the updated touch panel driver.
Download the latest version of MeanBean, as it has a S-on kernel flasher to facilitate ROM flashing. Put it on your external SD card and then wipe Dalvik cache, cache, data and system from recovery. Follow the instructions to flash MeanBean: you have to flash it twice, once to install the ROM, then again to install the kernel. Then reboot once the installation completes. Once you're up and running, make a backup. If you continue to have issues mounting your external or internal storage, back your storage up to your computer, then format them both. You should be good to go then
I recommend gaining S-off as soon as possible. You can use Dirty Racun or the newer, Windows friendly method, Moonshine.

You have to flash the ROM zip once, then again, without any reboots in between flashes.

So, decided to run lagfix earlier on my phone (usually not an issue). After it completed, it rebooted a few seconds later. No biggie I thought... However, now it keeps rebooting at the splash screen (before the boot animation).
I'm able to get into Bootloader and Recovery still.
I tried clearing my cache/dalvik, but when I clear my dalvik cache, it simply reboots and I have no idea why. Did lagfix mess up my partitions somehow?
I tried just reinstalling the kernel, but it still just reboots at splash.
Anybody have any ideas as to what I could try? ADB and Fastboot still recognize my device, so I don't think I've bricked it..

Edit: also reboots from recovery when I try to do a factory reset from recovery. I'm guessing my cache partition got messed up somehow?
Have you tried to RUU? If you cannot get recovery working, that may be your only option.
Before an RUU, try fastboot flashing a recovery image. TWRP or CWM.

I am able to get into both bootloader and recovery.
No, I haven't tried to RUU (not exactly sure which one to download, either. Been out of the loop for that stuff for a while).
You said you get reboot if factory reset from recovery. Sounds like something messed up that partition. Fastboot flash a new recovery IMG to see if that fixes that issue, then you could try factory reset again and it may work.
As for RUU, there is no RUU out yet for the recent OTA. So you would need to use the previous RUU OTA so you can update to recent OTA. There is a thread explaining the OTA RUU somewhere in this forum.
Note: I only had this Rezound for a little under 2 Mos but have had HTC devices before, and going off that prior knowledge. most "unofficial" or "leaked" RUU will not update, from what I remember.

Nott sure if its an issue with the recovery or not.
Looks like it reboots out of Recovery if I try to manually wipe /data or dalvik (only ones I've tried so far).
Doesn't hurt to try the recovery option before RUU.
I do not know if Rezound RUU repartitions the memory, but my other HTC devices did when RUU. So if it is partition error, RUU is your best bet.

Ok! Update that looks promising.
I no longer get reboots when wiping /data or dalvik. What I did to fix this was in Amon Ra recovery, it has an option to format /system, /data, and /cache to EXT4. They were already EXT4, but this menu also has an option to format to EXT3. I formatted all the partitions to EXT3, and then upgraded them all back to EXT4 right away. I went into the wipe menu, and was able to perform a factory reset as well as manually wipe all partitions I needed to!
Going to try restoring my nandroid backup now to see if it works or not.
Edit: Restore success! Had to go through setup again, but all my apps are still there. Just all my settings / data for apps got reset which is unfortunate, but not a huge deal. I have a backup from a couple weeks ago, which should be good enough.

So I am having an interesting issue with my Rezound which has served me well for the past two years. Last night while I was browsing the internet I got the warning that my battery was going to go dead. Ignored it and kept browsing, eventually causing the phone to auto shutdown, fine, I have a spare battery (love removable batteries), threw that in and it started to boot but gets stuck at the white "HTC" loading screen. I am able to get into HBOOT and get the following.

When I try to do a factory reset it will act like it is going to start, showing a phone with a green circle and an arrow, and a loading bar. Then before the loading bar begins to get filled in at all (it is just gray with diagonal lines) the green circle changes to a red triangle and an exclamation point.
If I try to boot into recovery I get the same phone with a red triangle image.
This phone is 100% stock unrooted and untouched in any way save for disabling the stock bloatware (I didnt even root it to uninstall that!). AFAIK there was no OTA update done to it yesterday, and I have been using it every day up until now with no real issues.
Simply just the battery died, and now it wont boot. Any ideas?
I am having the same issue on my wife's Rezound. It died last night and went into the same bootloop. Tried a factory reset to no avail. Phone is stock with the same info as previous poster in HBOOT.
I'm familiar with some android development, but know nothing about HTC phones. If somebody could point me in the right direction it would be greatly appreciated.
So I have tried running the stock RUU, no dice, I was already at the latest version.
I then unlocked my bootloader through HTC dev and tried to flash AmonRA so I can do custom ROMs.
Now when I go into my bootloader and select "Recovery" the white HTC screen pops up with a brown progress bar in the top right which fills up quickly, then the phone turns off and that is it until I remove the battery (wont respond to power again after it turns itself off).
So it would appear flashing AmonRA did not work either. Next I will be trying CWM.
Thanks for the update. Keep us posted if anything works. I just don't have the time to learn and try everything right now.
So I got CWM Recovery flashed successfully, but when I boot into recovery and try to wipe I get format failed and mounting errors.
Could my eMMC have been corrupt? How can I fix it?
EDIT: I also get cant mount and cant open on /cache, /system, /sdcard from inside the CWM recovery

I have not done s-off yet, would that fix my mount issues? I am now running TWRP recovery, that also is unable to mount anything. I appear to be having issues with the /dev/block/mmcblk0p35 and /dev/block/mmcblk0p36 and /dev/block/mmcblk0p29 etc areas. At this point any help would be greatly appreciated.
I have put a good 12 hours into trying to get this thing to boot. I feel like I am close and if I could just get it to somehow re-format the whole /block directory then flash a new rom it would would. Pushing an RUU from fastboot did not work, nor did running it from TWRP. TWRP gave me issues in the aforementioned mmcblk areas, and fastboot just says failed, I would guess for the same reason though.

Where can I find the instructions and files to get S-Off without access to a ROM and running windows? The JPBear instructions state Linux only and says I need to have a stock ROM.
OK, so from my research I will need to use Linux. Do I need the HTC USB drivers or anything or can I just take a fresh install of Ubuntu and run JPBear? Also, since I have unlocked my bootloader and (tried) to flash a rooted stock rom, do I now need to re-lock and try to run the RUU again? I did try that tonight (re-locking then running the stock RUU) but it still gave me the error that HBOOT was older on the version I was trying to flash.
You won't be able to run any RUU with being s on as their isn't one available for the latest OTA. Try wiping and formatting cache data and system with the latest CWM (Touch). Then follow flash instructions of a ROM for s on.

I have personally used the windows version of jpbear on a custom ROM to s-off my Rezound. It is somewhere in this forum. I was in a similar pickle and needed s-off to ruu. Albeit, my phone didn't have mount issues, so I don't know if ruu will fix your specific issue, but if anything will, ruu will fix it.

I believe an RUU would fix it if I can manage to get S-Off. I found an old version of ControlBear 0.2.2 for windows, when I run it it tells me to connect my phone and install drivers if I run from recovery. If I run from fastboot USB it trys to reboot my device into the ROM. So I dont know if JPBear will even work for me if I don't have access to my ROM.

1.) yes that is the guide im referring to...2.) AHA 2.6.3.3 HAS MOUNT ISSUES AND IS POSTED....go to an earlier version like 2.6.3.0...2.6.3.3 is only meant for FLASHING KITKAT ONLY and nothing else.....also why did you even trey to wipe internal sd anyway its not required...if you want the rezounds proper wipe method i can tell you...keep in mind the dinc is a whole different phone using a whole different motherboard....hence the differences....3.) once you have the proper version ill tell you how to get rid of mount issues

OK, I installed 2.6.3.0 and that is running. Still have mount issues but I am guessing that is normal at this point? I didn't try to wipe the internal SD per say, I was trying to wipe the cache and system to do a reinstall after the battery died and I was stuck at the white HTC boot screen.
Basically I am a noob and was just trying anything/everything to get it booting again. I found my old DINC now and am running that for now so I have some more time to work on it. On a side note, CM7.2 GB is still awesome.

so wait you are staying on ice cream sandwhich or lower? ok steps on 2.6.3.0 to get internal back is first go to wipe>advinced wipe and tick internal sd only...then swipe to wipe...youll get an error and thats ok....do the same method again except this time you'll notice it says emmc instead of internal sd....now go to mount option from main screen..then make sure only internal sd is checked and right below all options make sure it says Storage: Internal SDcard (size in mb) if it still says micro sd just tap on that area to change it....tap mount usb and plug into pc...reformat in windows using administrative tools in control panel or if it shows in my computer do it that way.....if you are staying on ICS or lower STAY WITH AMON RA ONLY...twrp and cwm is meant for jb and higher on the rezound....if the above method doesnt work...install amon ra and moun usb that way....you will have to fastboot install the recoveries obviously

I have not decided what ROM I want to run, for now just getting it booting on the rooted OEM rom is fine.
I tried that method and it did not work. It does say unable to mount /emmc, but it also comes up with unable to mount just about everything else. Nothing shows as mounted under mount from the main menu (I have my SD card removed for now) and when I tap Internal SDcard to mount it I get unable to mount /emmc in the console.
I tried AmonRA first, flashing through fastboot usb, and it did not work for me. When I would select recovery from HBOOT the phone would go to the white HTC screen then the screen shuts off but recovery never comes up. The phone just got hot and would not respond to power button so I pulled the battery.
