You may have named it PG05IMG.zip.zip
Remove the second zip

It shouldn't matter. This isn't always the case, but I have experienced it before.
Just rename it to PG05IMG and then try flashing in hboot.

PG05DIG.zip? Or PG05IMG.zip?
Rename it to PG05IMG and take .zip off.
Then go into hboot and see if it finds the update.
Having to take off .zip has been the case for me before... but only once.

Put your sd card in your pc or mount it, which ever is your favorite way to see/load files from your pc. Open the drive and find the folder options. Click view, and check "show common file types" or something to at effect.
I have found doing this as a general setting on your pc to be very helpful. Your pc will still show you the file as a zip even if it is name.zip.zip. Hope this helps!

I'm having a similar issue as well; Hboot will search, find PG05IMG.zip, the progress bar runs through at the top right, but then it just drops back to the hboot menu. I'm currently sitting at Unlocked but S-ON, with root in the Liquid 4.0.4 rom, trying to get back to the stock. Any suggestions?

S-On
Unlocked bootloader
ROM: Liquid 4.0.4 Compilation
End goal: Flashing the rooted OTA release of ICS

Depends on the unlock method you used. You'll probably need to re lock if you used htc dev and then use the allinone tool.

Thanks for your help, I appreciate it. I believe it was the HTC Dev Unlock tool; how would I go about relocking?
Edit: For anyone stumbling upon this, I relocked the device from fastboot using "fastboot oem lock", then when the device rebooted to the bootloader ran the executable from the link below. Problem solved, I'm back at stock gingerbread.

i am currently running an ICS rom and need to return mine for another device since it is defected.
Can i just htcdev LOCK then flash the ruu renamed as PH98IMG.zip to get back to stock?
You can do that cause I think ruu is different than just flashing stock. I had an og evo and I flashed the stock and I still had s-off so best advice make sure there is an s-on utility for ur device then use the ruu to return to stock

That will work fine. You can also relock then run the RUU on a windows PC, but pretty much the same thing will be accomplished.
i flashed back to gb then did waht i stated in the op and all it did was lock me in the bootloader and now my phone is stuck no matter what it do.

I'm in the same place as you. I'm on ics and need return it since its defrctive. Please let me know how you get it to work. Someone toke me I had to flash gb kernel and Rom then relock and flash ruu

I'm no expert but I think (if you made a Nandroid) you can extract the boot.img from the Nandroid then flash it and restore your stock Backup. I would also wipe / factory reset in recovery.
If you don't have a Nandroid backup you can download the RUU or a boot.img from a Rom like Scott's GB ROM and use one of those after you extract the boot.img.
I just did this today, I first flashed back to a gingerbread rom, then i unzipped the ruu and took the boot_signed.img from it, renamed it to boot than flashed it from fastboot. Then I put the ruu zip on my memory card (renamed it to ph98img) and fastboot OEM locked the device. I then rebooted the bootloader and applied the update.

I got it from scottsroms.com, I just downloaded the zip file in the ruu section, as for reverting back to a gingerbread Rom, I'm sure anyone of them would work (scotts clean rom has always done me well). I only say that because once you flash the boot image, your ics rom won't boot, so you can't place the ruu on the memory card, unless you have another way to mount your sd card to the computer (i.e. adapter, or another phone)

I believe all you have to do is pick the rom you want, install it and your good. The majority of them automatically takes you to hboot to flash the kernal, but not all of them, so make sure you read the instructions before you flash.

Your method worked great! I was running Scotts Senseless ICS, used your method and now back on Scotts CleanRom 1.5.5! Thanks man!
Wait I thought even if you relocked the bootloader it said "relocked" or something at the top so Verizon could tell no matter what. That's the only reason i haven't unlocked yet because i just got a new device and want to make sure there are no bugs with it.

I wondered this as well. Most csr's don't seem to be as knowledgeable as the people here on xda. They also don't seem to have a lot of knowledge on particular devices besides the obvious specs. I really don't think anyone is going to boot into fastboot to see it's been relocked, or even know what that means.
I'm gonna gamble with it, I'm tired of the noise coming from the headphone Jack whenever I play music.

I just spoke with a Verizon tech on the phone last night, he told me that simply unlocking the phone would NOT void the warranty considering HTC is allowing it via their website. However he did say that any rooting/roms would void it immediately (if they can detect it of course).
So basically just put it back to stock and you are good?

After some headache, I installed Scotts ROM
[ROM / LEAK] 5/8 - Stock Rooted RUU 3.14.605.5 / Truly De-Odexed! / Root works
I read somewhere that the RUU had to be flashed twice and as the newb I am... I flashed it once.
My HBOOT is 2.11.0000
No idea what my radio is... It doesnt list it on the front of the bootloader menu.
Do I need to reflash? Should I rewipe?
I had 2 random reboots already.
I'm S-ON as well.

I'm chalking up the reboots to heat... I'm outside and was charging myphone while using it... I'm guessing it was over 115-120+ degrees. Plus there might be a non ICS compatible app in my phone, who knows.

No that is not the case. Here is the latest leaked RUU
Rename it to PH98IMG.zip and put it on your the root of your SD card. Make sure you don't rename it to PH98IMG.zip.zip. Windows hides the .zip extension.
Your bootloader needs to locked in fastboot.
Code:
fastboot oem lock
(Do this before you place the PH98IMG.zip on your sd card.)
reboot into hboot, it find the image and then update the bootloader. It will take a long long time.
Then boot back into hboot for it to make another pass, this will update the system to ICS.
Unlock the bootloader again
Install Amon Ra recovery.
Install Scotts rooted stock rom that you have on the phone currently.
Done.
Warning, running the RUU will ERASE your internal SD card, so if you have pictures, video, music you want to keep, move it to your PC before you run the RUU.

I ran the Gingerbread patch he included in the directions....
before rooting, flashing roms, and attempting s-off,
read up as much as you can so you know what to do when you get stuck.
there are some very extensive tutorials that people have spent a long time making as noob-friendly as possible.

I find that I get better signal, but that may be subjective. My battery temperature is noticable lower though.
If you are going to update the firmware, you mifht as well S-Off. Use the GB S-Off, then flash this:

I had to use the USB-MS mode to get the file in there... Now I have the PH98IMG.zip file on the root of my SD card... I'm in the Developer menu, but have no idea how to Flash that Boot.img.

reboot bootloader then once in bootloader press the power button once, phone will do the rest

Hello,
I have tried reading all of the stickies (and list of stickies post) for a method to downgrade hboot 1.58 without success. My ultimate goal is to be able to flash a custom recovery so that I can try the radio fix since my wife's Evo 3D will not complete the hands free activation and she does not get data services. We took the phone to the Sprint store and they said the previous owner flashed SOMETHING to the phone and messed up the radio. Any help would be greatly appreciated.

You need to do the wire trick. Its simple. Search for the guide on here and download the required stuff. Look at the video then follow the guide. After you have s-off then you download the hboot you want and put it on the SD card and flash.
There isn't another method to putting a custom recovery on the phone? I just want to fix this hands free activation failing.

Don't be afraid of trying the wire trick - it really isn't that frightening as long as you follow instructions. I managed to do it on my first go, much to my exhilaration
If the previous owner flashed something onto the phone then does it have some sort of recovery on it?
I succesfully downgrade my bootloader from 1.58 to 1.53 with this tutorial (Methode D):

The wire trick is easy. It needs some attemps but it works great and you can not do lots of stuff wrong. :good:

1.4 revolutionary or 1.04 eng if you ate CDMA (which I think you are judging by your signature).

I referenced that thread in the post I gave him above, but took it one step higher since my post gives a little more detail about the flashing methods and options to delete the PG86IMG.zip file if you flash it that way.

Got it !! And stuck in a bootloop. Have taken card out amd removed the zip. Any ideas what I may have done wrong?

Make sure the file is in the root of the SD card and is named exactly "PG86IMG.zip" with the caps and without the quotes. Sometimes an extra ".zip" gets added when the file is downloaded to Windows and copied over, so it's best to check it with a file manager such as ES File Explorer on the phone.
Also, when you first start the bootloader it may not scan the card until you press the power button once to select fastboot.
Edit: I see you edited your last post and you've figured it out.

I think the extra .zip was it. But Now I'm stuck in bbotloop and can't get out. I took out sd card and removed the zip file.

You probably need to flash a ROM that is compatible with the bootloader you just installed. If you've removed the PG86IMG.zip file from the SD card you should be able to start recovery from the bootloader and flash a ROM. If you are using 4EXT, you will probably need to format cache+dalvik before it will let you do anything else.

ram !!! You are a life saver !!
So what I'm getting from this post is that for me to downgrade from 1.58 to 1.4 I need to do the wire trick..... I was thinking that downgrading would have saved me from doing tht. I also noticed that my WiFi says error and 4g wouldn't come on... Not to mention my Bluetooth is sparatic... Very close to flinging it into toxic waste... Lol

All of those issues sound like a bad kernel flash. If you do the wire trick, flashing kernels is easier too.
I have an HTC Rezound I have had since launch day. I unlocked the device via HTC Dev site, rooted and installed Clean Rom Developers Edition sometime in May 2012. I have been running this rom since. I have decided to sell the phone on ebay and want to return it to stock for the new purchaser. I downloaded what I thought and hoped was the ICS ruu, renamed it to the PH128.zip (or whatever it is actually supposed to be named), put it on the root of the SD card. Then I downloaded Hasoon's all in one tool kit so i could first "relock" my bootloader. I clicked the relock bootloader button while connected via the usb cable and in the bootloader, and it ran the commands and relocked the bootloader. It now says "relocked". I then clicked bootloader, the phone booted into the bootloader and appeared as though it was loading the zip file, but then nothing happens. Now no matter what I do the phone will only boot into the bootloader and nothing else happens.
Where did I go wrong? What is my remedy? Any help would be greatly appreciated. Thank you.
Does it even recognize the ruu file on your sd card? I'm assuming you renamed the file PH98IMG.zip and its on the root of your sd

It recognized the ruu file on the sd card. It began to load, the little loading bar on the right of the screen loaded once, then twice, and then it should say hit volume up to apply the update but it never did. It just went right back to the same hboot screen. same options. I try to go to recovery and it only reboots to the same hboot screen. It wont do anything. I am trying to download the ruu exe file now and see if that will work. Any suggestions?

You're probably using an RUU that has lower main version or lower hboot version than what you currently have on the device.

What software version are you currently on?

sometimes the "zip" file is hidden. try renaming it just PH98IMG

I don't know anything about HTC's but if the file is being loaded but gets stuck then I don't think it's a problem with the name.

I just re downloaded the ruu. Renamed it and left the zip off. It started to load the file asked if I wanted to update I but volume up for yes then it looked like it said updating then rebooted and appears to be scanning the zip file again. What did that mean?

I'm pretty sure you need to re-lock your phone and then flash a RUU. You will be on un-rooted, bone-stock ICS. I recommend that you use a T-Mobile RUU.
To do this you need to have ADB installed on your computer. Open up ADB in command prompt (navigate to the platform tools folder) and then hold shift and right click. You should see an option to open a command Window.
When it's open type in " fastboot oem lock ".
That should lock your bootloader. You can now download a RUU and flash it in the bootloader. Just rename it to "PH85IMG". It should be a zip format. Put it on your external SD card and let it flash in bootloader.
You can also find RUUs here. Chose the one for your native carrier, if your phone is unbranded, I recommend the T-mobile one.

thanks but when i try to run the file in bootloader, it says no images are found, the files are there but doesn't do it, how do i know if my bootloader is unlocked or locked because I can't tell

When it says locked it's locked. When it's unlock it may say unlocked or juopunutbear on the top heading in the bootloader.

I'm sure that whether your bootloader is (re)locked or not, it has to flash the ROM anyway. The problem will come later when you try to boot the ROM cause it will fail.
Be sure to put the .zip in the root of the external SD card and name it PH85IMG

Thanks for the help, I'm back to stock, the only problem I'm having is my phone turns off and reboots randomly, when it restarts, the percentage on the battery goes down a lot, any suggestions?

Try to reflash the RUU and if problem persist then the problem should be in your battery.
And ya check the md5sum of RUU( no need to worry more about md5sum but you may check for surity)

I did flash the RUU and the problem still existed, but it stopped when I put a piece of paper behind the battery, for some reason I think the back case was kinda loose and now since its tight, it hasn't rebooted

I'm trying to do that with an old Amaze I have. I have a Wind version so I downloaded a Wind (Globalive) RUU. Do I still need to rename it to PH85IMG ? Also it came into an exe. file and not a zip, but there is an OTA available that is a zip file. Will a exe file still flash in the bootloader? Thanks in advance

Well it should work exactly the same way. You still have to rename it to PH85IMG since it's device specific not carrier specific.
But if your RUU is an exe I think you have a different type of RUU. For that type, it's a software that updates your phone connected to a USB. For that, you need to connect your amaze to the computer and run the RUU installer. This might be your only option if wind doesn't have any other RUUS.
Another easy way is to go to the development section, and look for the thread by krook which has a collection of RUUS. It's on the 3rd page I think. Download the latest ICS one, rename it to PH85IMG and put it on your external SD (not in any folders) and it will flash in the bootloader.
There is also the Windroid toolkit which just got fixed and updated so that can help assist with the file pushing.

If it's an exe (the one that has an installer and all that Jazz), then no. Just connect your Amaze to the PC and let it do the work.
For the manual updating method, you need a micro sd.
