I just upgraded to Revolution HD 32.0 (coming from 31.0).
Here's what i did.
Boot into recovery.
Clear cache+dalvic-cache.
Install zip from sd-card (select revolution hd 32.0)
Started Aroma installer, left everything pretty much default, just removed some of the samsung junk.
After the install was finished i cleared the cache again and rebooted the device.
It started with the start up logo and after a minute or so "Android upgrading..." with the applications counting.
After that it shows "Finishing boot"
Then nothing, it stays at that for like 5 minutes, then reboots and stays stuck at the samsung logo.
So i decided to boot into recovery, whipe caches and fix permissions and reboot again.
Same thing happened as before.
So i wanted to do a full clean install of the rom, wiping all userdata.
Surprise! Can't get into recovery or download mode anymore.
Took out battery for a few minutes and put it back in.
Still can't access download mode or recovery with vol+menu+pwr...
Does anyone know what's going on, or better, how i can possibly fix this?
I have no idea how to access my device now to make changes...

If you can't get into bootloader with vol down, home and power button then you have a brick, service centre repair required.
Worth trying a USB repair jig to get download before paying for a repair .

Okay about a month ago I root my gf evo 3d , she has hboot 1.5 so I followed the steps on this thread http://forum.xda-developers.com/showthread.php?t=1239821.
Everything has been working fine up until about a week ago , her phone started running hot and the battery would drain really quick with barely any usage. So my plan was to do a factory restart and full wipe and reinstall the rom fresh but when I tried to get into recovery the phone just shuts off , then goes to the boot screen then after the boot screen the phone just restarts itself. I even tried to do a factory reset from the phones menu options and the same things happens. The ways I tried getting into recovery are by using the shutdown menu options (hold power , reboot , recovery) , I've also tried rom manager and last I tried getting to recovery from bootloader and nothing seems to work.
I am completely lost PLEASE! HELP!

You should reflash your recovery to TWRP using fastboot. Then boot into recovery using fastboot. From there you should be able to do a complete wipe and flash a ROM. It is outlined in the thread you linked OP and second post. Feel free to come into the d3rp q+a thread in my sig if you need more help.

As a heads up to some of the inner workings.
All the methods you listed are correct methods of booting into the recovery. ROM Manager selecting boot recovery, booting into the bootloader and selecting recovery and also adb shell, reboot recovery.
The boot order goes: bootloader -> kernel -> recovery. Once you see the HTC screen or whatever custom splash screen is loaded, you know the device is now booting the kernel. At this point, if the device hangs on the splash screen or reboots again to the splash screen, something is wrong with the kernel "recovery" itself.
If you get this rebooting to splash screen or hanging on splash screen two or three times in a row, that should be sufficient to know, you have correctly booted the device into recovery mode and the next step is to reflash the recovery, essentially reloading it.
We can speculate on how/why the recovery became corrupt on your device, but in the end, these things sometimes just happen to no intentional fault of the user.
Hopefully this doesn't happen again, but if it does, hopefully this extra information will help prevent a bit of frustration the next time around!

So you mean, stuck at the splash screen(HTC with white screen) and reboot again and again is because of the recovery destroyed? My phone stuck at the splash screen for about 10 seconds and then reboot again and again. I have tried to flash many recovery images, includeing twrp 1.0.3, twrp 1.1.1, cwm-1.0.4, cwm-1.0.5, cwm-5.0.2, all failed to boot up into recovery:-(

So I've been using Kang's ICS preview for about a week, and I really liked it, except from some bugs such as freezing after not using it for a few hours.
Today when I got back from school, (not used the tab for 7 hours) I tried to turn it on but I couldnt get past the loading screen, so I turned it off by holding the power button for a few secs.
Now I just cant get it to load up normaly, its just goes to the bootscreen, stucks, then its impossible to turn it off.
I tried going in to CWM and wipe data/cache but that freezes too.
Is there any other way to wipe it and install another rom like Overcome?
Mine did this as well. It would hit the Samsung loading screen and never go any further. I basically had to unroot it and take it back to stock via ODIN. Hopefully this works for you.

Follow the directions to get into the Download Mode, then jump down the unrooting part.
After its done with the download it will reboot automatically. Soon as it does this make the tablet go into Recovery Mode. It should go through a script and reboot automaticly. Again don't let it boot normally, make it go into Recovery Mode. I think it took a few times for me and then it eventually went into Recovery. I then did a factory reset and rebooted. Now I have an unrooted Samsung firmware. Back to scratch though, but fine with me! I was getting worried I wouldn't be able to get it back.
Oh...and this process will wipe your internal memory as well.
I rooted my phone without any trouble for months, and its been working just fine, until now! Then, I had to do a factory reset. The normal procedure, SETTINGS> PRIVACY> factory reset. The phone then never got past the boot logo. It starts to load, and just gets stuck at the Samsung logo, and nothing else. there's two lights glowing from the capacitive buttons, and that's all. I'm on 4.1.2 JB stock rom. Later, I tried to enter recovery mode, and did a wipe, and cleaned cache partition also. AND, the phone just sprung to life, the whole first time set up thing. I started filling in stuff and the phone just shut down again, and if I tried to reboot again, it doesn't get past boot logo. But if I wipe n do it, it happens but shuts down immediately Please help!!

Boot into download mode, get a stock rom from Samfirmware, flash it with Odin (How ? Thread stickied near top of Q&A).
My device details: SGS3 T999, rooted, running Jellybean 4.3 on a liquid smooth ROM. Its a year old and has been a solid performer up until now.
My device is stuck in a cycle where it gives a quick vibrate, then SGS3 splash screen with white letters, then black screen, then tiny blue "Recovery booting..." words in upper left of screen, then black screen with couple vibrations, and around and around it goes in the same repeating cycle.
Just before it started I had taken the following actions: Dumped the following 2 files onto my EXTSD card (cm-11-20131217-NIGHTLY-d2tmo.zip, gapps-kk-20131119.zip) from my PC, booted phone into recovery mode using TWRP, did a factory reset wipe (success), cache wipe (success), dalvik cache wipe (success), flashed the NIGHTLY file (failed), flashed the gapps file (failed). At this point I should have sought some support. Fool! I next did a reboot step still in recovery mode. Without the reboot happening a message came up stating Superuser permission had been broken (the best I can remember) and the option to have TWRP repair the permission. I opted "yes" to repair. Then the loop started that I can't escape.
I have pulled the battery multiple times, tried a regular boot up, tried a recovery boot up (up volume+power+home) dropping the home button after a few seconds. Nothing is working. Prior to the above steps I backed up everything on the device. So I have backups to use if it can be done. My best guess is that I failed to update my CyanogenMod or TWRP to the very latest versions and it caused this hang up. Otherwise, I don't know what to do and need any help I can get.
Has anyone had this issue or heard of anyone who has and have any helpful suggestions, tips, ideas, etc..?

I've had a few similar events for a few different reasons.
I would suggest booting into recovery but it seems you have tried that already.
If the device is plugged into a wall or pc, unplugg it, that has seemed to cause and fix boot issues for me before.
Other than that, I dunno, if you can't get to recovery or fastboot, I dunno...
i had my amaze repaired (power IC, bad data cable caused it to not charge or even boot up or go in bootloader). Now within 30 seconds off booting up it used to restart. and i cant get into the recovery( shows logo and powers off when going into recovery from bootloader). i cleared the cache from bootloader and its now stuck at boot animation( not boot logo) and wont boot completely. before clearing cache after logo it used to show 4ext recovery boot screen then screen off, now it just shows logo and screen off. really need help i got this phone fixed after 1 year. im s-off and running any 4.4.4 rom (dont remember which rom, i think pacman rom)
so basically i can get into bootloader and use fastboot commands (no adb, authorized device). cant do anything else on the phone. is there a way to fix the phone?

reflashed recovery for the 4th time and it worked. it said the cache partition is corrupt. formatted it. phone works now. updating apps caused system ui stopped working error. going to flash other roms.

Glad you got your phone working--your persistence paid off.
Be sure to make a backup before flashing other ROM's.
