First time happened was back in about a month ago. One day I noticed some of my pictures were gone ( may be 15 to 20 of them ) I could not find them in any where such as SD card internal/external the strange thing was some pictures still there until today so the defective SD card is ruled out and I was still using the stock firmware 2.3.3 then I also noticed there are other pictures ( almost 100 of them ) not actual pictures they just blank but all with different file names like: My piano, A day in Hollywood etc, etc.
No body has been messing up my phones because it stays with me at all times. The question is who deleted my pictures and put a bunch of files back? Could it be happened remotely like some one using Bluetooth to mess around if the person is happened to be near by? later on I upgraded my phone to 2.3.4 through KIES and hope it will help me to solve the mystery but it did not. By now I know those pictures are long gone...
Two or three days later I upgraded my phone to 2.3.5 until I found the official FW XWKL1 here in XDA, again, upgraded.
So some days later I decided to change song for my ringtone I found out all of them have been replaced by some files with number names. 41203,41204 the list go on...When I use Astro to look for my songs and they are all there ( inside the music folder ) I went back & click one of the number file to see what kind of song and the player pops up a message saying the file format is not supported. It took me almost 30 min to figure out and delete all the number files under MUSIC then I see all my songs back.
My question is has my phone been hacked or infected by viruses/spyware? Or software glitch? I don't have data plan. To go online I use my home wifi all the time.
If some one can help I'll be really appreciated.
Sounds like a problem with the sd card or phone itself.

But most of the pictures and songs are still there that was why I think the problem is not in SD card other than these problems the phone is functioning good in all aspect.
Highly doubt someones in ur phone.

Sounds like the card is corrupted. Just make a backup of everything on the card, and do a full format (not quick format) of it from your computer. That should fix it. I had a similar problem too recently. New files I transferred would get deleted and old deleted files would reappear. Maybe the file table is messed or something, I don't know, but a full format fixed it for me.
Note: Preferably use a card reader, or if you mount it as mass storage from your phone, do it from recovery if you're rooted. Otherwise just mount it as you would normally and proceed.

Hey guys, I've been having a maddening problem with music stored on my external SD card and hopefully someone can help figure this out:
I'm running AOKP, and have all of my music stored on a 16GB external SD card. The problem I've been having is that whenever I open up the phone (generally to switch SIM cards as I'm living overseas and use several different cell companies) the media storage gets all messed up.
I used to be using Google Play Music, and whenever I'd do a SIM swap I could no longer play music. I'd get an error saying "could not play the requested track" on all of my music files.
Figuring GPM just sucks, I switched to WinAmp. Well, the same thing kinda happens. Now, when I do a SIM swap, all of my music tracks get listed twice. One version of the track won't play, the other one will.
Basically what it seems like is that every time I change the SIM card, the media scanner is re-running. Except that it doesn't realize that the files haven't changed, so it re-indexes everything. But this causes music apps to go haywire.
I used to do a data wipe on the Media Storage app, which would clear everything out on Google Play Music (haven't done it yet when using WinAmp). But eventually the problem would crop up again.
To be clear, I recently did a full wipe and installed a fresh version of AOKP. Unfortunately after swapping SIM cards just once the problem reappeared, and TBH I shouldn't have to do a clear data on Media Storage every time I swap the darn SIM card.
I used the phone overseas about 6 months ago when I was still running stock 2.3.6 from AT&T, and never had issues with media storage. Is this an issue using non-Samsung ROMs, or is it an ICS issue in general, or is something else at work here? It's a bit maddening that something as simple as using my phone to play music has become such an ordeal, and I'd like to find a permanent solution to this without having to constantly do data wipes.
Thanks
Anyone? I realize probably the majority of people here are using their i777 on ATT and probably don't open the phone much, but this is a pretty maddening problem to not be able to store my music on the external SD card without the phone going crazy after every time I open it up. I may try going back to a Samsung ROM and see if it's just an AOSP thing, but I'd really prefer to stay on AOKP if possible...

This probably isn't the solution you're looking for, but I'm pretty sure PowerAmp uses its own media scanner, so that should mitigate the problem. See if that helps. As for listing twice, that's usually because there's a symlink somewhere; for me it's usually /emmc and /mnt/emmc, so it's listing twice.
Thanks for the tip, if I decide to stick with AOKP I'll probably give PowerAmp a try I'm actually wondering if the Samsung music player works the same way and has its own media scanner; that would likely explain why I don't remember ever having a music issue when I was running Samsung-based ROMs.
Regarding the symlinks, how would I know if that's the real issue? I do have both /emmc and /mnt/emmc, and I also have /sdcard/external_sd. How could the problem be fixed if that's what's going on?

Just a few points, I'm on AOKP and always stored music on the external uSD card and I never have any problem with any sort of players such Apollo, MortPlayer, VLC, GMP, ..... Am I a lucky one? Could be.
Honestly, I've seen this kind of reports already, in this room, maybe in the AOKP thread, can't remember on which page but you search from there. I'm sure there should be the answer.
I don't know about other TW-based ROMs behavior. If you don't get any feedback from users, you can always install one and try it out and see what happens.
Okay, so after a bit of searching I'm (maybe) convinced that it's a symlink issue. My hope is that the problem was that I still had /sdcard/external_sd from running Samsung-based ROMs, and that the media scanner was getting confused between that and /emmc.
Still have one question, however - there are two versions each of the sdcard and emmc folders:
/emmc
/mnt/emmc
/sdcard
/mnt/sdcard
Should each folder exist twice? If not, which ones shouldn't be there, and are they safe to delete? I've tried figuring out what the default mount points are for AOKP, but some people say /emmc and some say /mnt/emmc.
I've deleted /sdcard/external_sd so I'm going to try putting my card back in and seeing if there are still problems. But in the meantime, knowing if both versions of the emmc and sdcard folders should be there would be great

Yes, that's what a symlink is; both paths are the same thing.
Sent from my Galaxy S2
I understand that both folders link to the same place; the question is, are both of the folders necessary? In other words, if /mnt/emmc is deleted, will the external SD card still be able to be read from /emmc (or vice versa)? Or will it make things go haywire?
My hope is that the duplicate music files I was seeing in WinAmp were because /emmc and /sdcard/external_sd were both on the phone. But if that isn't the case, and even after deleting external_sd I'm still getting duplicates and music playing isn't working properly, then I'd think the only remaining issue is because the phone has both /emmc and /mnt/emmc and the media scanner is seeing them as separate entities.
So yeah, mostly I just want to know if AOKP needs both /emmc and /mnt/emmc to function properly, or if only one of those folders is needed.
I'd be inclined to think they are both required.

Yeah, I'm having the same issue after moving my music over from internal storage to a external card. On rocket player the files would be listed twice just like what op stated and using Google play they would not be able to play in the player even if they are playable from directly opening them in astro. While my phone is the skyrocket i727, the issue is exactly the same on aokp milestone 6. I tried reformatting the card then putting back on the music yet to no avail. I'm hoping someone would know what the problem is.

I am having the same issue...
Finally able to fix this problem. Googled a bit more to find solution. Harder to find than expected but anyways this is how I did it.
Went into
setting --> apps ---> all ---> media storage
And cleared data
After I cleared data of both the music player apps I used, play music and rocket player. Somehow on milestone 6, media scanner is very stubborn and refuses to run many times even with the download of media scanner apps and restarts of the phone. Somehow I managed to get media scanner to run and finally I see my files in play music. I click on a file to ru and it starts playing. Very relieved to say the least.
Media scanner apps used were both SD rescan and rescan media. Don't know which one did the trick.
Hope this helps.
Hello all, I had recently upgraded to a SanDisk Class 4 32GB microSD card and within a few days after I had merged all of my phone's data and my old sd's data, my pictures come up as broken files, and my music shows about 5-8 duplicates of each song but only a select few will play and the others will act like they're playing but no audio plays and the seconds of the song stays at zero. I even restart my phone with the sd out and the media scanner doesn't show that it's running, and I'm pretty sure it's not because when I go back to the music player I still see all of the songs that are on my sd and the pictures still show that they're broken...idk how the files still show in there without the sd card being in there, but I'm afraid I might have done something wrong when I had cut and pasted all of the files from the phone itself onto the sd. I have no real knowledge of android programming or rooting or anything like that so I figure it would help to mention my phone is stock but officially upgraded to 4.0.1.
Any help would be greatly appreciated and could possibly help more users out there!
I think you have to go to settings-apps-all tab-media storage- clear data. I think I remember right. Maybe try it without your card in. Replace SD card and turn on phone and I think it will scan correctly.
Or something like that. Hope it helps
Yeah I've tried all of the available functions from the phone and also mounting & unmounting it. I'm assuming from being a windows user that I might have transferred over some sort of "playlist" that remembers all of the file extensions for each file, and since they're not located in their original place it cant find the file...idk but it really takes a toll on my drive to work!
Hi all...I have been flashing for about a year now and I need help finding a JB Rom that works correctly. I have tried a few different ones, and most won't show my pictures or music even after I followed the guides to fix these issues. So I flashed Miui JB latest Rom and its real nice, but no BLN?? So, which JB Rom is stable for you with working GPS, mms, and shows no issue with music or picture files...AND has working BLN? If I could just find a kernel that would work with the MIUI JB rom to enable BLN I would be golden...alas, I can't find one and Devil won't work. Thanks guys!
I've been on AOKP 9-20 with Devil 1.5.2 with almost no issues. The only problem I was having was a few force-closes while using the camera, although after a few reboots it seems to have gone away. I plan to flash the new AOKP build 3 later on, which people are having great results with. I was on CM10 for a long time before this and everything was working perfectly, but I came back to AOKP mainly because Devil didn't work that great with CM10 and I missed blinking notifications and better performance options.
tl;dr AOKP or CM10 are both great

Hey thanks for responding! You haven't had issues with losing pictures or music files?? They are on the sdcard but no music player can see them...same as pictures, although QuickPic seems able to pull them up...I flashed the latest CM10 nightly, no music no pics...went back to stock and worked up to AOKP nightly, same issue! Got mad and flashed Miui JB and poof everything showed! Weird! I followed some of the guides I found through search for how to fix the issue but nothing worked...no problems with this for you though??
EDIT: I clean flashed AOKP build 3 and NO music or pictures!!! Going to flash the Devil and see if that makes any difference.
Yeah I've have no issues with pictures or music on any rom I've tried, however I'm no longer using the sd card that came with the phone. I had an issue mounting the sd card when I originally started flashing my phone (I had zero issues with the card before that time) and decided right away to buy a better sd card before flashing anything else. I ended up getting a 32GB class 10 which has been flawless. So basically the only thing I could think might be going on might be related to the sd card failing, especially if you're still using the junky one they give you with the phone initially.

I have the same sdcard I don't know, it's all over Google and XDA about this bug...I just feel like the only person on a fascinate that has it! Thanks for giving me some input tho!
If I were in your spot I'd probably try a different sd card if you have one available to try, or backup all contents of your sd card to your PC, format the sd card, and then transfer everything back.

LOL I tried that too. I honestly did everything I could think of or every tweak I found...that's why I was hoping someone could point me in the direction of a JB rom that did not have this issue. It's cool though, I started using Google Music...transferred my music onto my pc and uploaded to Google music site. Then every time I update AOKP, I just will get the app and it will download the music to my cell. Not a perfect solution but it's tolerable! As far as pix, as long as I use Quick Pic they show up just fine. Of course I have to use a third party camera app since the AOKP camera won't work since gallery isn't. Thanks for any help!
I remember there being an issue with rom manager creating a .nomedia 'hidden' file:::: here is from a post of what I did to fix that issue::
Yep I was having this issue too,,, yesterday I had tried clearing the gallery cache and data,,, today I 1st tried clearing the media storage cache and data,,, hoping maybe just possibly that would help ... but it didn't... Also Tried both of my SD Rescan widgets, but neither worked ... so I uninstalled the ROM manager update which took me from 5.0.1.5 and asked me if I wanted:: " to replace this app with the factory version " which I clicked ""OK"" and that put it back to version 5.0.1.1 ,,,
then I cleared :: media storage,,, gallery,,, and Apollo :: cache and data ,,, and rebooted .... wallaha!!! gallery shows properly!!... THANKS everyone ,,,
,,,may have been slightly overkill but I what the heck,, just in case gallery and Apollo had anything hanging around ...
Now you were saying that when you flashed MUIU everything worked? I wonder if you could pull the Camera.apk from that rom and install it on AOKP to see if it'll work
I'm having the exact same issue.

Same issue here, happened a couple of cm10 nightlies ago. Never had a problem before, there were no reports of it happening to others but only a few of my pictures loaded, no music was recognized, neither were my custom ringtones, or videos. ODINed back to stock and it carried back to that... no idea how to fix it... although I think it is an sd card issue, the phone just isn't recognizing there being media on the card or only partially. Namely because if I pull the ringtone from sd and place in the ringtone folder in /system they appear. Tried just about everything, specially after hearing about the Rom Manager issue with .nomedia's being placed were it shouldn't have.
Other things I've noticed is that, an app like Root Explorer can access my sd and recognize the files inside to which I can select and use, but even after opening the picture/song/video they still don't show up in their respective apps. Also note, I have reformatted my sd card and ODINed back to stock to start over with no luck in either. I've since installed Rom Manager with no luck either.

Hmm I wonder? But how would that have any effect on the music part of it? Honestly I can live without the stock camera and gallery, its the fact that all my music is missing is the worst part...
Does MUIU have it's own music player? I never used it, so I don't know. If it does, take the apk from the rom and put it on AOKP to see if your music shows up. I wonder if this is just a glitch that hasn't been addressed simply because not that many people have encountered it. Perhaps this is a big enough issue to be posted in the general Q&A forums? Maybe that way you'll get more views and possibly better answers from people on other devices that may have got this bug.
I believe JB puts something on the sd card so ics and below won't show all of the media. I went back to JB and everything shows up fine.

Yup, that's exactly how mine is...I can see the files on root explorer or if I usb to pc but no matter what app I use to play music it says no music found on sdcard. I've used rescan app, moved onto pc and then formatted sd, blah blah. Nothing works. Oh well, it isn't enough to make me leave AOKP...I just work around it and wait for a fix I tried cm10 nightly, cm10 8/19, and a few others...same issue. Also if I use an mp3 downloader it will complete the download but when I try to play the song, it's NOT found! Crazy. I'm glad I'm not the only one!
using steves sept 27 build of aokp with devil 1.6 is my favorite and daily driver right now. only issue thats a serious problem is if you turn it on while its off and plugged in charging, it has a SOD or completely dies when i lock the screen. simple fix, turn on, as soon as you see samsung logo plug it in. this also fixes having no soft keys if you get that every now and then. no issues with with the gallery, camera, music or anything. i always odin back to GB stock and then go forward with THS build 2 of ics, then JB, then google apps and boot, restart to recovery then flash the kernel. im seriously impressed with this build

It could be a coincidence but it wouldn't hurt to try swapping SD cards if you have another laying around. They're also dirt cheap on amazon now too
Did you try this? Found it doing some google searching
The Internet said:
This trick didn't work for me. So, I did what the system should have done with the .nomedia file present. I moved all of my photos to a hidden folder (with a . before the name) and removed the DCIM folder. Then, I looked for all instances of the .nomedia file (Root Explorer helped) and deleted them. Rebooted the phone. Started Gallery. Waited for 10 minutes. Voila! Gallery showed every little image file that was on /sdcard. To remove the files that I didn't want to see, I copied the .nomedia file from a random system folder and pasted it into all the folders that I didn't want indexed. Strangely though, moving even one of the original /sdcard/DCIM/Camera files from the hidden folder into the current Camera folder took Gallery down. Stranger that this also made the Internal Storage show a blank window when I connected the phone to my laptop. Almost positive that this has to do with JellyBean creating a /storage/sdcard0 folder that's linked to the older /sdcard folder. Looking it up and will post updates if I find any.
Update: The same trick works for Paranoid Android's JellyBean ROMs (even the latest 2.13) for the Galaxy Nexus. Something to do with the DCIM folder and how it is handled by Media Storage. A Linux-geek friend of mine confirms this has to do with the /sdcard0 symlinking. Although, why we need something like that is beyond my limited knowledge of the OS.

Yes a root explorer and look for all those hidden .nomedia files and delete those ,,, and copy over an alternate .nomedia that was not associated with the sd card,,, that was one of them tricks

I am new to Samsung, coming from HTC Desire. And got into bizarre already )
A couple of days ago noticed a strange problem with photos in galery unreadable, appearing to be corrupt (all of them on external sd, internal sd photos are displayed properly). Then today happened that Music app couldn't play music files on external SD.
Read countless of similar issues on the internet, with lots of people blaming on the sd card. However, I found that it is not the case.
Put into a SD card reader and plug into PC, the files on SD card are all ok.
Put back card into phone and investigated further. First I thought maybe it's a problem with SD card reader in the phone. BUT! Noticed files are perfectly visible in ES File Explorer, and even display correctly when using Gallery App to open them from ES file explorer. But if Gallery is opened first, it cannot display files.
I'm sure somebody else happened the same, since the internet is full with such reports regarding Galaxy devices (even S1 and Note alike), but anyone identified the sorce of the problem and eventually solved it? I would be very much interested.

Try to run Rescan Media app from Play store.
Maybe your media scanner service does not work properly and needs to refresh the list of media files on your sd card.
Good luck !
rescan is done automatically every reboot. doesn't help. thanx anyway.
investigated further and found out that the files are not corrupt, but the libraries of Music app and Gallery app created duplicated empty files (in fact 2 fake entries in library for every valid file on the card). Duplicate files don't exist physically on SD card, only in libraries of the two applications.
Already tried formatting cache and dalvik cache, but to no avail...
ICS 4.0.4 Orange Romania (BVLPH) stock ROM.
Later EDIT: This was an easy one. Unmounted SD Card and open Gallery app and Music app. Then re-mount SD Card, both apps will refresh libraries, wait a while for media scan in the background and voila: duplicate files disappear, everything is in god order.
I had a similar problem where both gallery and music files were duplicated, but the duplicates were unreadable.
My solution was Settings -> Applications -> All -> Media Storage -> Clear Data.
This forced a refresh and everything turned normal again.
Slightly annoying that the media scan data is stored under a separate application and not within the data of the gallery or music player applications, but once I discovered this it was an easy fix.
