I flashed one Recovery, then flashed a newer version. When I installed a Rom, it couldn't find my internal SD card. It said it was there, but everything was installed to the external and the cameras wouldn't start because they couldn't find the internal...Very strange. I tried various things (Installing new Recovery, but it wouldn't flash, mounting and unmounting, but got error on sd card mount) and nothing worked. So I figured I would do a factory reset and it just boots to the Recovery...weird...
I'm going to flash an RUU and see if going back to stock helps, but I would appreciate hearing if anyone else has ever had this problem...
I searched and found nothing in the Rezound Thread. XDA offered suggestions but they were all from other devices, mostly Samsung and those folks seemed to assume device failure, and it may be that, but the device seems to be working, it's just the internal that's having issues...
Any thoughts?
EDIT: I flashed the latest RUU in HBOOT and all is back to normal!
If you had just installed the stock recovery it would have allowed you to do what you wanted it to.

I can't access my SD Card at all with this http://forum.xda-developers.com/showthread.php?p=25816870#post25816870 rom installed. I click to mount SD card and nothing happens. In recovery I can access the SD card backups etc
I find it strange. Although I do have to ask, I have been having this weird problem. I have Clean Rom 4.3 Installed (ICS) and I have been getting some random reset's and slow this or that. So I saw 4.5 was out - It literally reset everyime your open something and I mean anything Bam reset. However I installed the above rooted bleh bleh stock os (I do not have s-off so I used the PH img) I made sure I had no other boot images on there. Installed setup got everything ready. I have gotten 3 resets in 11 mins so far so now I backed up to Clean rom 4.3 and I have no idea what to do.
I also did the following:
Flashed back to STOCK o.s.
Then Downloaded the above linked rom, cleared data (everything) installed software patch and rom reset back up and same problem. I have been flasing rom's on this phone for a while now and this is the first time no matter what that I get these crashes and resets. Did I miss something?
You really should s-off that puppy. It is pretty easy, and will get you out of messes.

I assume you're always wiping data and cache.
Have to tried Fix Permissions? I don't know if this rom has it in it's settings, but using others I've corrected some weirdness by wiping the cache and then running fix permissions.
Were you seeing the same thing with the stock rom? Or just using clean rom? Might try a different rom for a bit to see if the issues follow you there.
S-OFF isn't going to help you with this. It's perfectly fine to flash your boot image outside of Aman-Ra. I do it all the time.
Are you wiping everything between flashes?
I recommend using this - http://forum.xda-developers.com/showthread.php?t=1562176&highlight=clean
Flash it first and it wipes the needed partitions clean.
If you're still on the GB firmware, you need to flash the old firmware patch after the rom install also to get SD card access.
If none of the above helps and you're still on the GB firmware, set the phone back to stock using the GB RUU, otherwise you will have to s-off it so you can do the same.
Yep full wipes between flashes. I didn't keep the stock Rom for more than about 20 mins.
I didn't see a way with the debloated stock New leak to fix permissions. But with the 4.5 clean Rom I did that and it made no difference.

Well I was running the leaked sense ics build and it was causing my phone to overheat so I decided I would switch back to cm7 for a while, when I when I tried to reboot to recovery it just booted showed a black screen then rebooted back to the rom. So I tried to reinstall the recovery from both rom manger and the bootloader nothing worked. I then noticed that it was hard to get my sd card out and that it was slow to load upon boot, odd but it was still readable by the system and I could get to my files on it. I treid a few other sd cards with the same results. It was then I decided it would be a smart idea to try and flash a ruu to try to just return to stock (but leave the bootloader unlocked). I ended up flashing PG05IMG_no_hboot_2.11.605.19.zip from [HOW TO]return to stock (gingerbread) from revolutionary or normal hboot(11/2/11) - xda-developers Which then lead to my phone not booting to rom or recovery all I can get to is the (still unlocked at least) bootloader.
Any ideas on where to proceed? I attempted to run a ruu exe so I can restore it from a computer in case the problem with sd is why it is failing to flash properly but the one I tried 2.11.605.0_Radio_0.01.78.0802w_3_NV_8K_1.41_9K_1.6 4_release_210841.exe required the phone to be booted to the rom to confirm the current build before flashing a new one. It may not work but I've had some luck with http://forum.xda-developers.com/showthread.php?t=1703455 in rooting and unrooting, it may be able to see it and do its thing. I know you said you tried different cards but it might be worth looking into reformatting your card in Windows (FAT32) and trying it with a good radio by itself to get it to boot. Then pull or rename the radio zip off the card and then reinstall the ROM or RUU.
I read the wonderful "Don't Panic" sticky, but I think I need a more drastic fix.
SHORT VERSION:
TWRP keeps saying, "Unable to mount /sdcard", and "unable to mount internal storage". I can't boot past the HTC screen, and my attempts to flash a ROM have failed due to those errors.
LONG VERSION:
I tried using TWRP 2.2.1 and Cyanogenmod 10 today, my first time flashing a custom ROM.
My phone (HTC EVO 4G LTE) was already rooted using the plain ol' HTC bootloader unlocking method.
I booted into recovery and performed a backup first. That appeared to go okay, and verified a backup existed on my external SD card.
I booted back into the OS and used ROM Toolbox to download the ROM and gapps thingy, wiped the cache and dalvik's cache. The phone rebooted and started wiping the cache and stuff. I went away for a bit.
When I came back thirty or so minutes later, the phone was all black. No lights at all despite being plugged into the AC adapter.
I came back about thirty minutes after that, and the same thing. I rebooted. The phone went to the HTC screen and then blacked out again. I rebooted into recovery. I figured the ROM didn't get flashed right, so I went to flash it again. I used the file manager in TWRP and saw that the directories /sdcard /sdcard2 and /sd-ext were completely empty, except /sdcard had a TWRP directory, which was empty. I didn't see any of my storage!
I set up ADB on my Mac and tried mounting my sdcard. No dice. I tried using fastboot to flash a ROM. Everytime I tried to do that, I got an error in TWRP, "unable to mount /sdcard", "unable to mount internal storage". I tried using fastboot to flash the .zip on my Mac, and tried using "adb put" to move the .zip to the phone and install from there. Same result.
Any ideas?

this seem to becoming a more frequent problem maybe something TWRP is responsible for i came to this same obstacle when trying to flash a new rom like a week ago. i tried all the nifty moves i could but no luck even went to sprint looking for answers ultimately i was forced to format (with windows) both internal and external SD. end result. yep u got it i lost everything almost 26gigs of music and files i had backed up no where else. so i formatted (heart broken) got over it now i back up files daily on my laptop thats the only way ive come to see out of this one. but check around forums a solution might work for i saw a couple didnt work for me tho.
JB ROMs rewrites the addresses of your SD cards, good chance the internal is corrupted. Boot to recovery and go to mount/mount USB storage, if you have windows then as soon as you plug it in windows will tell you to reformat, if you have Mac or Linux manually reformat the card

Tried reformatting, but that didn't seem to help. On #twrp on freenode, they said it looked from my recovery.log that the internal and sdcard storage is hosed, and the maintainer of TWRP said to RUU back to stock. SIGH. . . .
So, since I don't have Windows at work, going to try these at home tonight:

If you're able to access bootloader, you can recover from this issue(been there myself). You'll end up losing all your ad card stuff but you'll be back up and running. Download the newly released 3.15.651.16 RUU, get into bootloader on phone, set it to fastboot, connect to computer, relock your bootloader (it can be unlocked after ruu app), run the RUU, and you should be back and can reunlock and install custom Roms etc.

I had backed up my sdcard so I didn't really care about not losing it. I think the problem was at least partially caused by having it encrypted.
I ended up flashing to stock and it let me format the sdcard from within the phone - that did it.

This just advice just saved my life. I thought for sure I bricked my phone, and right before memorial day weekend toooo! I did run into issue with the latest Lollipop RUU. I had to resort to using the old Kit Kat RUU I had saved to my computer from before. It worked like a charm.
Trying to install MeanBean...I updated the Firmware to the latest, have the phone unlocked and rooted (it says TAMPERED and UNLOCKED in the recovery screen) and did a factory reset. I have TWRP installed and can still get to the TEAMWIN RECOVERY project interface...
I was installing MeanBean and it just stuck on about 75% and stayed there. I waited for about ten minutes and figured something must have gone wrong so I didn't really have a choice but to shut it down. After starting back up when I tried to install again it said "FAILED" in red letters. I decided to re-download the ROM but now when I mount the SD CARD from within the recovery menu (to transfer the new file) my computer says that I must format the drive before I can use it and won't let me see the contents of the sd-card. I didn't want to do that for fear of bricking...so I don't really know what to do from here. I should add that I don't have a backup made in TWRP (I got the phone yesterday and hadn't installed or done anything to the OS)
Anyone know what should be done at this point?

I'm updating this for anyone else who runs into issues like this: I went ahead and formatted the SD card from within windows, then relocked the phone, reinstalled the RUU and I'm back in a workable mode

I had the exact same problem. I just reformatted the internal storage and that fixed a lot of my issues. I can't however get the RUU to work. Can't seem to find my phone, neither can HTC Sync. Is this just a problem caused by MeanBean? I use to run MeanBean but then switched to CM10 for a few months and was going to switch back. Never had an issue like this before.

In the beginning I wasn't having any issues whatsoever. Now it seems like whenever I want to try a new ROM out i get bent over and have to start from the beginning.
Root the phone as stated in Android Dev
Install GooManager download scriptrecovery, reboot into recovery (TWRP)
The ROMs I have been able to install are: CleanROM, Hyperdrive & GE4.6 MyWay
All other ROMs have been causing me nothing but problems.
I have done MD5 checks on my downloads, used different MicroSD cards but every time I always end up at the same brick wall of having to start from stock again.
When i try to restore a backup, it will not restore or it is corrupted when if i do get it to boot (missing apps, factory mode)
I have a 64GB MicroSD card that i just recently formatted in TWRP
Is there anyone that could shed some light on the above??

You said you tried a different sdcard, try a smaller one, try copying a rom to the internal sdcard and see if it errors out. you could have a corrupt recovery also. Lets look at these things first.

So I copied CleanROM over to a 16GB micro. I had GE 4.6 My Way installed, recovery to TWRP. wipe data, installed CleanROM1.6 without any issues.
However, when the phone boots up it went straight into factory mode. I have had this issue 100 times over the past few weeks and am beginning to think that there are some corrupted or missing files.
The solution I have found for factory mode is to download a terminal emulator, and run the following command:
echo -n ON > /efs/FactoryApp/factorymode
After doing this I reboot the phone and the factory mode popup is gone...

I've had much better luck flashing roms after using This to install OUDHS, only downside is can't access external SD from recovery.

Cwm has given me no problems whatsoever. Maybe download ROM manager and flash the free version to see if that helps

I'm having a similar issue. I've been exclusively flashing hyperdrive, but since RL4, i have not been able to reboot the phone without it being stuck in a soft brick.
I've tried everything from re-installing TWRP and re-flashing the kernal through ODIN. I'm not sure what to do anymore.
Had a device that was sent as a replacement do the same. Hyperdrive and synergy would not boot had to Odin back to start every time. However beans works as well as stock. I sent it back.

everytime you reboot the phone will crap out on you??
try using a different micro SD card. I was using a 64GB and the thing got destroyed being in my phone, copying ROMs, making backups, etc.
right now I'm using 2 16GB cards: 1 for backups and the other for installing ROMs off.
TWRP just came out with 2.6.0.0 within the past couple of days. I don't know if this fixed the issue, but as soon as I noticed this everything has been working.
