I read many post about this error and I tried to do like said here, but I still have the same error.
What I have:
1. Backup path without spaces - /sdcard/clockworkmod/backup/2013-01-05.02.02.02
2. 3 files: boot.img, system.ext4.img and nandroid.md5
3. CWM 5.5.0.4
I tried calculate md5 by TotalCommander, MD5 Calculator and Terminal Emulator. All sums were equals.

open nandroid.md5 with root explorer and delete all the contents in it or copy it to another file, than save empty file and try restore.

are you having the same version of CWM that you initially used to make that backup or has it been changed to another version?

I don't know about it. This backup from another device (device like mine, but firmware newer). Perhaps it were made by MTK Droid Tools.

Nandroid backups can only be restored when the CWM used to make them must be of the same version, if you only require certain data or files from it, you can use Nanadroid Browser (download from google play store) to copy them from your backup

Nope, Nandroid browser can allow you to copy paste specific files , provided you are using the same firmware in your phone.

Instructions
Extract the contents anywhere you like.
Place .img files inside /QuikIMG/images.
Start QuikIMG.
Choose .img file.
Choose desired partition.
Click "Flash"
Wait for it to flash and then you're done.
Known Issues
Shell output doesn't update in real time. Seems to be an issue with fastboot.exe.
File filtering currently not implemented. You can put any file in the "images" folder and have fastboot flash it. Please be careful.
Planned Features
Ability to individually flash an .img file anywhere on your PC.
Configurable images directory.

I already checked half the internet after a solution. Read the 9-step solution to no avail, tried using NV-read-write tool to dump a new IMEI, tried autoprime's solution (failed), I tried GSII_Repair and still nothing.
My actual IMEI show as 004999010640000 and my S/N as 00000000000.

Thanks for the help, but I tried re-flashing ROMs a lot of times already, from 4.0.4 up to 4.1.2, and none fixed it. I guess I'll just send it to a technical assistance and have them fix it.

can't format or write anythings !
scaned with windows defender and it apear this :
I remove it and try format with Hard Disk Low Level Format Tools . But when restart the problem still exist.
I try to format by phone , CMW ,ram reader but ...

Thanks for your reply, i checked the rom space, it shows around 50 MB available, but still the problem is the same.
When ever i try to remove any stock apps like "layer" "Notes" etc. etc. with link2sd or similar apps or try to replace any of files in system, eg. fonts etc. the system reboots immediately. But i saw the file being replaced finally. the only problem is it gets reboot each time i try to do something.
Hope you would be able to help me... If you need any kind of details about my phone in order to solve the issue, please tell me.

In order to remove stocks apps from any phone, first phone must be rooted and you should use " Root uninstaller pro" or Titanium Backup" apps, that will allow you to uninstall the stock app or freeze them, its better to freeze first as some stock apps should not be removed as it may cause other problems in that ROM, once all is well you can uninstall them later. Try it will root uninstaller to see if you have this problem.

My Phone is rooted properly with CF-Root. All root functions are working fine but the only problem is the /system read-write error. I googled for my problem and found a similar problem on HTC one, (tag: Disable system write protection on android) but could not find any article about Samsung devices.
problem still not solved, but reboot issue is not there. Thanks to dhiru1602. I am using his CyanogenMod 11

3) Open editor (Alt+E)
4) Add in editor the following:

Yes, it is very easy to use and reliable application. All actions are done in one click.
It pull .shealth.db file from the phone.
Instead, you can use commands SDB (Smart Development Bridge):

I have rooted note2 phone with root explorer, but cannot find .shealth.db file.
There is no directory: /opt/usr/apps/com.samsung.shealth/data/.shealth.db
I found /Android/data/com.sec.android.app.shealth/files but there is no database file.
Where can I find it on Kitkat 4.4.2?
