My Samsung Galaxy SII had been working fine, no issues at all until two days ago I went to take a picture and the camera won’t work, the camera screen is black and hanged up when I press to take a picture for a while then I just keep getting some error messages like force close, and media server not responding. I've tried to clear the cash and data for the camera application but nothing happened, I wiped the cash from the recovery mode without any success.
I tried other apps using camera some of the not working at all, and some recording some videos with the black screen on but after finishing recording it showed me what recorded but even this happened after long time of waiting for the app to do recording.
I tried to do the test for the camera using the code *#0*# for the back and front cam but get the error message of "activity factory test not responding (wait or Close)" then Camera failed. I searched over the web but most of those who had the same problem mentioned that they did factory reset with no luck.
On which Rom, Android version, Kernel are you? Recently installed gapps? Some day I had problems with the camera after flashing gapps.

go to settings-applications and force stop camera app also go to cached processes and stop camera

I experience a reproducable Bug on CM10.1 and want to know if this is connected to my phone or a general bug others experience as well. I'll try to describe it as detailled as possible and I'm keen to find out if some of you have the same problem.
So here we go.
Used Software / Hardware
CM10.1 nightlies since end of Nov 2011 up to now (01/13/2012).
gapps as of (12/12/2012)
SGS II (I9100)
Symptoms
When recording video, at the end (when I touch the stop button) the screen freezes and doesn't react on input anymore. The recorded file is corrupt. It shows up in Gallery as a grey video and can't be played by MXPlayer or gallery itself.
Sometimes I get the "Gallery not responding", when I chose to quit gallyery, I can get back to the home screen and continue using the phone. However, another try to record a video gives me a "Can't connect to Camera" error message. (note that all english screen text are translated by me since I use german localisation)
When I choose to wait for gallery, the freeze gets worse and as the screen went to standby I couldn't get it back to show anything. (The lower touch keys however where still lid when I touched them).
All the helps now is a reset (holding down power button).
Reproducability
The problem is not reproducable everytime when taking a video. However, I think I can reproduce it in 10 Minutes when trying several shots. Even after a fresh reboot. I have the feeling it fails more on longer video recordings (+1:30min).
What I've tried to get rid of it (without success)
Change resolution from 1080p to 720p or 480p
delete data/cache of gallery app through app settings
switch Geotagging on / off
switch power shutter on/off
wipe data/factory reset (from CWM Recovery (v6.0.2.7)
wipe Dalvik cache (from CWM Recovery)
fix permissions (from CWM Recovery)
Nothing helped, so far.
Logs
I was able to reprocude it with adb logcat running on my desktop. I attached the logs. I'm not sure about traces.txt, though. It might contain younger entries and not those at the time of the original crash.
At camerafreeze.log:8809 there was a big pause in logging and online the light entries were logged. That was shortly after I touched the stop button.
Additionally I post a poll to get a feeling for how many of you CM10.1 nightly users the recording works fine.

PS: other than that, the CM10.1 is really great, especially when it comes to battery life compared to Samsungs stock ROMs!

I'm adding
Deactivate RingDimmer
to that list. The Log showed that RingDimmer is recording Audio and crashing afterwards. Maybe there is a race condition or insufficient locking within the audio recording facilities causing this bug. However, since disabling (not even deinstalling) RingDimmer, I can not reproduce this bug so far. To crosscheck things I reactivated RingDimmer and set it to maximum accuracy (decreasing the pause between it's recordings to test environment noise). With that setting, the bug appeared almost immediately with short videos.
Maybe thats the sucker. I'll keep you updated.

This is a problem that I faced with my HTC one. And was able to solve it too.
All of a sudden, when I start my camera, it is stuck with the first picture it captures. It does not update as I move the camera. But this works fine if I change it to video mode. When I opened the camera, It said "something went wrong" (don't remember exactly)
I had shut down and then switched on my HTC One. Still it was not working.
SOLUTION::
Finally restarting the mobile worked. Yes, RESTARTING is different from shutting down and then starting.
So guys, keep this in mind.

My camera suddenly stopped working :[
I have cm11, and when I try to launch the app, it says "can't connect to the camera".
I tried rebooting, tried to factory restore, tried different apps, still nothing :|
Is there way to check that the camera is connected? maybe for some reason it disconnected
from the motherboard?
Flash stock firmware and factory reset, if still the same then reconnect/replace the camera module. I replaced mine recently, it took me 10 minutes and cost £5.
So after everything else didn't work,
decided to take apart the phone.
Turned out that the camera was disconnected as I thought,
Have no Idea how that happened, the phone did fall few days ago,
not that hard though...
Anyway works now, just the resolution settings for the back cam are the same
as for the front one... max resolution is 1.9mp :\
Will try to flash again...

Had the same issue a couple of times. Try clearing data/cache and see if it helps.
Was happening to me almost once a day. Reboot would fix it temporarily but it would come back.
Contacted support. They told me to go to settings, system, reset, reset all settings (doesn't delete any data or app).
I thought it was silly and just their usual first step... But I tried it and so far haven't had the issue in 4 days.. Crossing fingers!
Maybe some glitch in between firmware updates that got cleaned by the reset? I'm on .127 (Telus, Canada)
Issue with camera app

I am seeing the same issue. Sometimes when I open camera app it says 'Camera failed to connect'. Then this lasts sometimes for seconds, sometimes reopening app multiple times solves this and one time I had to restart phone. Also sometimes the camera app opens up but with no camera shutter button or any settings option. This is very annoying. I am using lawnchair launcher. Phone has also frozen in other apps couple of times like in WhatsApp which is not acceptable with this processor.
The in display fingerprint scanner is also not reliable I hardly use it now as it doesn't work many times.
My software version is .126
Can the camera app issue be a manufacturing defect or a software issue as very few people have posted this issue? If manufacturing defect then I will have to head to service centre immediately.

I haven't had this happen again after I swapped back to the stock launcher... Which is admittedly an absolute ridiculous solution

Yesterday i noticed that sometimes, after i taken a photo, Camera app goes into force close immediately, but luckily photo are saved in Gallery and i can restart Camera app speedily, but after 2 or 3 photo... force close again.
This issue happened with every type of configuration (auto or night mode, IA on or off, with or without zoom, etc. etc.).
The only way to solve is to restart the phone, but after many hours it happen again.
Apart from hard reset, what i can try to do? Previously i deleted data and cache file from Fotocamera app, but i not solve the issue.
Is this also happened to you?
