Okay I'm losing it I'm on CM-11 based ROM and I've reflashed and deleted battery stats anytime I connect a charger it'll go from 1% to 99% immediately. I have to reboot to get a true battery level and then if its 50% it stays 50% until the phone dies or I plug it in and it says 99%.
I've tried battery calibration apps and I'm at a lost.

recalibrate via htc way
turn device off plug into wall outlet and press and hold power vol up and vol down for 2 minutes or until device boots.
if it comes on instantly then continue holding and let device reboot itself so it starts booting a second time. if this dont help try a better rom

Whenever I charge my phone, it doesn't go all the way to 100 percent. It just stops at about 95 percent. What's wrong and how do I fix it?

mine stopped at 99% i just kept charging till it was at 100% and green be sure to charge it 8 hours for first charge ( you dont have to its just a suggestion)
Download this and when you open it click on the Percentage (cant miss it big circle) Phone has to be plugged in to see how much time it has left to be fully charged.
Its really accurate.
GO Power Master

I tried everything but all I get is this... What can i do? It's been 3 weeks since this
is this plugged into a computer?
also which rom/kernel are you using?
*edit: on a custom rom such as pac, it will show this battery icon when off and plugged into a wall/usb port on a computer. it will never show a "filled" icon. try unplugging it and turning it on.

This happens to me. Usually, if you plug it in and out repeatedly it finally works. It's best if you turn it on fully and THEN plug it (unless it's dead completely).
When you say "all I get is this", do you mean that the player does now start when unplugged?
You could try removing the battery, pressing the power button a few times and putting the battery back in.

Removed the battery while the phone was on to show some coworkers and booted the phone back up. Checked the battery and it's much less!
Standby has been awesome on this phone and the battery bass been what's expected for its size.

This might be a glitch based on you removing battery while the phone was on, you might want to reboot your phone, or fully charge or discharge the battery.
