I've been trying to install the most recent PAC Rom nightlies on a stock, rooted phone. I'm using the newest TWRP to flash it. The flash goes through fine, so I wipe cache/dalvik. When I reboot my phone it gets stuck in the PAC rom loading screen.
What should I try?

Did you do a full wipe/factory reset before flashing the rom?
I did. Somehow it decided to work when I tried again. Newest TWRP was messing with the install. Had to try several times for it to work.
I believe there was issues with the latest TWRP recovery. The previous build worked better. Switched over to Phil Cwm and haven't really gone back since.

I recently rooted and installed the CM9 ICS nightly rom using the noob video instructions (This included rooting with Odin, installing CMW recovery 4.0.0.4, then I flashed the ROM cm-9-20120624 Nightly and finally flashed GAPPS-ics).
I over clocked using the CM9 settings and everything worked perfectly for about a week, it was great. Then it just crashed while I was reading my email and began to bootloop. After reading for similar errors I rebooted to recovery and tried wiped the cache and dalvic cache, I wiped data and factory reset several times but nothing worked, always the same bootloop. I re flashed the same ROM and still nothing, after a while the boot screen would simply freeze instead of bootloop.
Then I connected to my PC, and did this to no avail. In fact it simply added to the problem by adding a yellow warning triangle while attempting to boot so I did the Oden thing again like the first time I rooted however I installed CMW version 5.5.0.4 instead thinking it might help but it hasn't.
One last point, when I attempt to wipe data/factory reset the tablet seems to freeze and attempt to reboot rather than say something like "success" as it would during a cache wipe. This maybe normal or it maybe the problem! who knows?
On that point, I would be very grateful if some of you could help get me to the point when my PC recognises the tablet so I can reflash a new ROM.

I once got the same problem. The only thing that helped was to flash back the stock rom via Odin with the pit file. And don't forget to do a hard reset after flashing the rom. I'll just stick to the stock rom until the official update is getting released.

I think the best way is still to do factory reset, go back to the stock ROM first and try custom HC ROM until the official ICS is released.
Installed latest CM10 nightly. Now just sits at boot animation. Attempted to restore. Backups gone. All other ROMS gone for SD card. Can get into recovery, but am lost at that point. Tried clearing cache several times through fastboot. Have no clue how to use adb to sideload. Rate myself as an almost noob. Any help appreciated.

I'm in the same boat, stuck at the CM boot animation. How did you get back to recovery? I hold the power button and the home and back buttons flash and that's it.
Edit: The whole "hold the phone up to a bright light while holding the power button" trick did the job. I was flashing from Android Revolution to CM10.1 and I did perform a factory reset and sideloaded CM through CWM. After another wipe everything worked out fine.
Are you coming from another rom? If so, did you factory reset after flashing ?

Im assuming based on his post he is not coming from another rom and has been flashing CM nightlies regularly and this latest flash was done per routine but is not working for some reason.
I updated the latest 6-23 nightly and its working fine so far, sorry to hear about your issues.

I dirty flashed the latest nightly as well, no issues.
Just bought a used phone off Swappa. It has TWRP and had Cyanogenmod installed.
I wanted to try a TouchWiz ROM so I downloaded one. Booted into recovery, made a backup to external SD Card and then flashed the ROM. It said it flashed
successfully but when rebooted, it just stayed at the Samsung Galaxy S4 Boot
screen.
I tried restoring the backup, but it failed!
I thought I might have had a bad download so I downloaded Cyanogenmod nightly and LiquidSmooth on my laptop, transfered to the SD card, popped it into the S4, boot into recovery, and tried flashing each, and both just boot to the boot screen and freeze.
What are my best options now? I saw some guides for flashing back to stock via Odin, but requires that I know if I am on VRUAME7 or VRUAMDK and I don't know. Could I have been on ME7 and running Cyanogenmod? Doesn't seem possible, no?
Me7 ota doesnt have the ability to flash custom recovery or rom, so no you werent on that beforehand. Whenever you flash a rom you need to do a wipe/factory reset and wipe caches, unless op states you can dirty flash. Doing some reading will only help you out, generally every post outlines the installation process. Boot to recovery do your wipes then try booting again. If your still in a bootloop then you will need to flash mdk via odin then start the rooting and romming process over.

Man, I thought I did everything right.
First, did a backup.
Then full wipe with TWRP full wipe, including cache.
Then flash ROM
I'll have to get familiar with Odin and find the MDK images.
Sometimes that happens. Could have been a bad download then it happens more often then you would think. If theres an md5 hash posted its a good idea to verify the download is good. In the dev section theres no wipe mdk images posted so you wont lose your already installed apps and data.

So I downloaded the factory image and flashed via Odin, but accidentally flashed via bootloader rather than PDA. It wouldn't start - still hung at the boot screen, so I re-flashed via PDA and it is still hanging at the boot screen.
Now I don't know if it is just the same problem as before, or if I messed something up with flashing through bootloader. Can I reflash something via the bootloader section of Odin to restore it?
What version of twrp? And try to go back into recovery and wipe cache and dalvik.
That worked for me on the buggy version of twrp when it wouldn't booy

Not sure what version it was because it's now back to factory recovery. I did go back into it and did a factory wipe and it booted!
I've been having a problem getting any 4.3 ROM to boot after the initial boot. I've seen a few other people with the same problem and I can't figure out a fix. Thought I'd ask the general Q&A.
I've used Beanstalk and Carbon (great ROMs) a few carbon 4.3 nightlies and Beanstalk 4.3.00. I installed their 4.3 builds (after checking sums to verify download) flash gapps, wipe cahce/dalvik and boot up. Set up goes great and rom runs great. but even without installing any other apps, using no backups or anything if I reboot it sticks on samsung screen and can't get past it. Every time, I even rebooted without finishing setup and wouldnt advance past samsung.
I even flashed 4.3 rom and gapps, let it reinstall apps from market then booted to recovery and made a backup. Then could'nt reboot. But if I wipe and restore the backup it will boot fine the 1 time, but again any attempts to reboot stick on samsung screen. Wiping cache/dalvik from recovery does not help, fix permissions from recovery does not help.
Anybody have this problem and find a fix?
Ok, I tried slim bean beta 2, no luck, tried ktoons kernel, no luck. Guess I'm staying with 4.2.2 until there is some movement on this issue.

I have a Note 2 and I was running Sophisticated rom 3.1 just fine. I wanted to try Beans new Alliance 23 Rom. So I booted into TWRP 2.6.3.1 and backed up Sophisticated 3.1 first, then wiped everything and flashed Beans 23. Phone would not get past the boot animation. I tried to use the restore I had just created and TWRP fails. Tried upgrading TWRP to 2.6.3.2, didn't work, now any rom I try to install or recover from I get errors in TWRP saying too many files open and a second one about data/data/com.contacts. I tried Philz CWM recovery, I even tried downgrading back to TWRP 2.6.3.0 and nothing. I can not get a rom to boot past the boot animation. Please help. I have unlimited data and I can't go use an upgrade.

I am using twrp 2.6.3.1. I would reflash the recovery. Then I would wipe everything except your external SD card. I have noticed that if I don't wipe my internal SD for awhile I will start having goofy stuff happen. That usually fixes any issues I have.

Yeah, I tried all that as well. Rehashed that version the new version and the older version then tried Phil's, tried wiping internal, external and still had errors. I had to bite the bullet and Odin back to stock 4.1.2 then jailbreak again and after a day lost I finally got it back up. I'm leaving TWRP 2.5 on it
