SD card may have been removed during boot, after that point the SD slot has not been working..My SD micro cards are fine they work well on other computers. Suggestions welcome. Thanks.
Wipe and flash latest stock firmware, if still the same then hardware fault.

It turns out to be the slot its self. when you have the card pushed & held all the way in it works just fine. Eventually I'll swap out that failed slot (tape is doing the trick for now) but for now I'm thinking about getting the Note 3.

problem is my tbolt won't mount a sd card in bootloader, recovery or the rom. I've tried multiple sd cards, even one that's working fine in my evo 4g. i would like to return the phone to stock if this is a hardware issue so i can take advantage of warranty but as far as i know i have to have an sd card for that. is there a way to return to stock or flash everything with adb while avoiding the sd card?
Is your SD card formatted fat32?

yes all sd cards i've tried (4 total) are formatted fat32 and all are recognized by my evo 4g and my cdma hero. when any of them are put into the tbolt android recognizes their presence but deems them as blank or not using the correct file system. within cwm none of the cards can be mounted. I've also tried placing a PG05IMG.zip containing stock everything on the sdcard to try to flash via the bootloader but it seems like it looks for the zip and doesn't recognize it. is there anything else i should try or is this a lost cause? thanks for the reply btw.
Try a factory reset?

not yet but only because it's not my phone and the owner needs it for work. there's currently no guaranty i can get a rom back on there once i've wiped/factory reset.
Hey guys, so my galaxy s2 has been running on RR 3.0.5 with the latest siyah kernel. A problem I've been having is frequent media corruptions, and to solve that, I thought reformatting the internal storage would help. After doing so with cwm touch 6, it doesn't recognize my sdcard when i use my phone. What can I do to fix this?

do factory reset with internal SD wipe in the phone and not in CWM, and you should be fine.

Just did that, and sadly, it didn't help. The phone still doesn't recognize my external sd card.

Strangely, mounting anything from cwm never seems to work for my phone.

Could be that the SD card is faulty. Have you tried the SD card with another phone? Does your PC detect it?

So I tried swapping my SD card for another 32 gb one. However, it isn't the SD card's problem. It's that my phone simply doesn't detect that there IS a SD card mounted in it.

I remember there was this setting to mount the external SD card from settings > storage, but don't remember which ROM had that.
If that doesnt work/if that option isnt there, your last option is to check with stock ROM as suggested earlier. Maybe it is a hardware fault.

In RR there's no option for mounting external SD, only "unmount shared storage". Will try going back to stock 4.0.3 as my last option.
EDIT: Went back to stock 4.0.3, reformatted usb storage and sd card and flashed back RR 3.0.5. So far so good, no problems
My Rezound just started doing this in the past two days. Stock storage under settings can not see my external SD Card. No apps installed on the phone can see it either. Root explorer is the same. Funny thing is that stock Recovery can see it. I can flash RUUs. I can boot to AmonRa and its file explorer can see it and things flash fine. Anyone ever had this problem? I have factory reset in recovery...reflashed last ota RUU.....still have the same problem. The SD card works fine in my wife's phone.

Does it give you the option to format it from within your rom? Or does it just say unavailable? Do you have another sd card to test?
Sent from my ADR6425LVW using xda app-developers app
Does it with any SD card I put in.....says unavailable and is grayed out?
Sent from my ADR6425LVW using Tapatalk 2
Have you tried doing right after doing an ruu like stock unrooted?
And did you s off? That can corrupt external sd cards but that doesn't explain other sd cards that weren't used during the process.
I hate to jump to conclusions but it sounds like a hardware malfunction. Maybe time for a warranty replacement

I've been S-off for quite some time. I have gotten it to read the card a couple of times today. I was able to restore some apps with titanium. When I plugged the phone in to charge ...I saw a quick popup....." SD Card unexpectedly removed". When it starts and the External Card is in Settings the phone works normally. But when the SD Card disappears all of the Apps that draw from or write to the External SD Card behave Strangely. It is very near stock now. It is frustrating that it works very well when set up excluding the external SD Card. I do have alot of things on the SD Card that I use a lot.....Guess I'll call support
Can you pull your SD Card and plug it into a computer? Copy over everything to a folder then reformat Card and put in phone, then if works copy everything back over. If still doesn't work I would suggest flashing the stock image since it will basically reset everything. Good luck.
I would keep working with people on here, those kinds of issues and support usually equal face palm or headache.
For reason unknown...when I restarted this morning the ext_SD mounted fine this morning and has been working fine since 5am. We'll see if everything makes it thru the day...lol

If it happens again, go into storage and select mount sd card. If it is mounted already, unmount then mount. Happens to me occasionally.

Well finally just started over....Wiped and reflashed phone. Then copied and reformatted SD Card, copied back to card ......everything is fine so far. We'll see how it goes tomorrow.
similar problem
I am having a similar problem. I just installed Neo's RAGE 1.9 rom, inverted Gapps, and flashed the boot.img since I am S-ON. Now I can't get to my ext SD once the phone boots up. Similar to manbat above, I can browse it in AmonRa, and even flash from it there, but after booting up the phone I get the "damaged SD card" message, and I can't access it. I've re-flashed the rom 3 times, and also the boot.img...even re-downloaded the rom, inverted gapps and tried all over again - but nothing changed :/ If anyone can help me out on this it'd be greatly appreciated!
You will have to copy your SD Card to computer. Since your phone is reading the SD Card as damaged you will have to reformat on your card on the computer and then copy your information back to your card. This is what worked for me.

I flashed Beans build 11 with pbj kernel. In that I picked the option to swap internal and external sd cards. Now I cant change wallpapers and some of my apps are telling me "sd card required for this application"
I would prefer to just fix this rather than re-flashing the rom again without the sd card swap.
I have been searching the forums for hours and can normally find a way to resolve any issue, but not this time.
Any help would be great.
Thanks
Just wipe caches and flash. Takes a couple minutes.

Yes flash the ROM with only wiping caches. You won't have to set everything up again and it should get rid of the swap

I assume so because I've never seen them mentioned, but I don't use swap. Did you clean flash the ROM the first time?

Yeah clean flash coming from beans 10.
cant install any rom on my note 3 these are steps i use

Dump your SD card, as it appears fried.
Reflash the rom, get a new card, and put your backups on your internal memory for the time being.

I can't recommend brands, since I haven't bought one in years (still using the 16 GB one I had in my E4GT), but I'd go with the Sammy if you like, since they made the phone. Probably a safe call.

before you get rid of your sd card if you don't mind i have a few question?
1. why if your going custom rom would you do step 2? you had no need to root it if you were doing step 3 and installing a custom recovery/rom.
2. do you have a sd card adapter for micro sd? can you see if the computer sees it....
3. if pc doesn't detect it. can you look under windows disc management ... tell us if you see a unallocated space matching sd size?
4.also what format was sd fat32 or or exfat?

i was under impression you need root to instal custom recovery (bin a long time since messed with android)
yes pc see's and write to it so does phone just after reboot or sometimes taking it out of phone or pc and swamping it to phone or pc it gets damaged (yes i unmount it first)
i formatted it both with note 3 and windows as fat32
I am having the same exact issues, did you ever find any solutions to this?
