So I am a first time rooter, so I'll try my best to use the proper terms and whatnot.
Anyways, I have an unlocked HTC One X+ that I am trying to root. I was using the AIO kit by Hasoon2000. I had my bootloader successfully unlocked, and so I proceeded to continue with rooting. I thought I pushed the SuperSU.zip to the device, because the cmd line said it was successful. So I booted into recovery mode, made a backup, and then when to flash the SuperSU.zip file. However, I could not find it in the file explorer in recovery mode.. so I went to push the file to the phone again, but now it says device not found. The only thing I can think that's happened is my USB debugging got turned off... which is bad. So is SuperSU.zip in my phone somewhere, or am I stuck with a phone that can only go into recovery mode?

I have now similar problem too...
actually I had rooted successfully, but after many failed CM11 installation attempts, I decided to relock the bootloader, in order to update the Firmware. But it didn't work too and I came the way back again, in which I unlocked the bootloader with Unlock Token etc. again. Now the bootloader is unlocked but apparently not rooted!?! I can't push superuser via adb because **Device is not found** and I can only go to TWRP Recovery and bootloader...
COULD SOMEONE HELP??!
PS: when i press the reboot in TWRP, it says that "the phone seems not to be rooted, do you want to install the SuperUser? and when I accept it... it reboots only and nothing does change!
well... im not sure what I did differently but it finally recignized the device!! And now I managed to get everything installed... weird!

so I have the latest software, used the HTC unlock method, was rooted and had the TWRP recovery working just fine, I'm an idiot and flashed the CM recovery through RM and of course it didn't work, so I've tried every which way I can find to re-flash the TWRP and even tried flashing AMON but it will not go into recovery with any of them after it says it successfully wrote the recovery.
TWRP and CM just wait for a minute then reboot normal, AMON reboots then sits at the HTC screen... forever and i have to pull the battery.
I tried uninstalling RM to see if it was F'ing with it, didn't matter.
not sure what to try next to get a working recovery back onto the phone, I'm still rooted just running the stock ROM, I had loaded an alternate ROM successfully last week, just didn't like it. So I know all is 'working' as expected I just F'd up the recovery part now
any help??
afaik if you used the HTC unlock you need to fastboot flash it, or maybe hboot zip it? What have you tried so far?
I've tried using fastboot to flash amon and TWRP, same result
tried doing TWRP as a zip as well
what's weird is each method seems to "work" i get that it's pushing, writing, completed, no errors with any of them but then it just won't go into recovery and if i change to different recoveries I get different reboot behaviors just none of them work.
that's where I'm stuck, I feel like I've tried all the methods it just refuses to actually go into recovery when it's loaded.

I'm thinking at this point I almost need to just write everything over again? use a flashable zip in the bootloader and just have it write everything over again, I was going to do a new rom anyways bored with the stock again.
so I need to find that I guess? re-write everything go back to fresh and start over
1. Make sure u r not flashing CDMA recovery over a GSM device, or vice versa.
2. How did u get ur device rooted in the first place?
3. Last resort, relock ur device and apply the correct RUU, then unlock and root again.

2) I followed a tutuorial where I pushed the recovery and the SU file after doing the HTC official unlock method, worked great at first, I'm the idiot that pushed the CM recovery over top of it and now I can't get a working recovery back on.
3) do I really have to relock first, I've seen the simple tutorial on doing so, I was hoping maybe I could find an all in one flashable ROM that would at least get me back to operational on all partitions and then re-work that one with a new ROM later or something, not possible?
This is of no help to you but - I had the same error. TWRP v1.0.3. I tried everything in the book to get it to work... but got stuck at the HTC splash screen like you. And I tried installing the 1.13 PG86IMG and then my device turned off while installing the PG86IMG (it was fully charged during flashing, I checked). Be careful with what you do.
I was unlocked with AlphaRev's tool, running my own ROM.
mine hung like that if i flashed AMON, but i could still pull the battery and go back into the bootloader to flash at least TRWP using FastBoot so I could get back into the phone

htc unlock method has proven to be a bit shady for the community in that it doesnt allow all the features/functions the revolutionary (and prior unrevoked) unlock method does.
most developers unlocked with revolutionary and haven't performed the htc unlock method so it has been hard to gather technical details on the htc unlock method.
one item we can eliminate in the effort to narrow down your issue: there is no working Amon RA recovery for the EVO 3D. not sure where you obtained Amon RA recovery. TWRP was created to pick up the torch from the Amon RA recovery as he retired from developing his recovery to support emmc devices.
stick to either flashing the twrp or cwm recovery images. there are only a handful of ways to load a custom recovery and only a handful of ways to boot the device into recovery mode.
i'd follow a logical pattern and work your way through the avaiable options. by following the available options one step at a time you should be able to narrow down which methods work and don't work. sorry it has been difficult as this htc unlock method is newer to the community we aren't as familiar with it as we are the revolutionary method.
ways to flash a custom recovery:
1) fastboot - fastboot flash recovery c:\downloads\recovery.img
2) bootloader - PG86IMG.zip file containing recovery.img and android-info.txt
3) flash_image - while in normal android mode (my app Flash Image GUI works well for this)
ways to boot into recovery mode:
1) turn device completely off and unplug USB. vol down + pwr loads bootloader. select recovery from the menu
2) from android mode: adb shell reboot recovery
3) from android mode: use a 3rd party app like quick boot.
this is all off the top of my head so it might not be an *exhaustive* list but it should cover all the common approaches. hope that helps and keep us updated! good luck!

I really appreciate everyone's help here I've been a part of various forums for a REALLY REALLY long time... from the days of Nextel, then I was a Palm Lover for a few years and now Android.
this obviously isn't urgent as my phone is working just fine, I just can't flash a new ROM since I can't get any recoveries working.
unfortunately I'm pretty sure I've tried all the methods listed above, I'm going to probably sit down sometime this weekend and systematically go through each method one more time just to be sure. TWRP worked for me just fine in the beginning, I flashed a ROM more than once, reverted back to my old ROM, made back ups, cleared cache's all the stuff I'd need to do.

and no one knows of a flashable 'rom' that would have all the different partitions included in a zip or something like that? like.... what is in place of recovery in a stock phone? I can always unroot and start over but I did use the HTC unlock method so I hope that even works

Ok so after getting 4.5.141 to load via motos update I attempted to root the phone. Which by all accounts looked like it worked however....
My phone does not appear to still be BL unlocked, and there are no apps in the list under superuser. I downloaded CWM and tried flashing then going into recovery and that fails even through prompted for SU by the apps it says it work etc.
Now I cannot even get into recovery via any method. Via CWM it errors with a message cannot boot 2 error then loads rsd mode. Via power on/vol dwn it just gives the android dude with ! in the triangle.
Ok so how do I fix this? I need to be able to root, unlock the BL (if even possible) and fix my recovery partition. (any command used from fastboot fails I suspect its still a permissions root issue)
Any help is appreciated.
NVM I fixed it.
I re-ran the unlock process what was funny is when I flashed the sbf to unlock the BL the screen immediately showed the "unlocked" in the upper left hand corner. When I went to do the fastboot oem unlock part it said already unlocked. (I tried this earlier and it kept failing I think cause I forgot the sbf.
I then went through the steps to re-root it, then re-flashed the newer CWM and now I am able to get into recovery etc.

I got my HTC One yesterday, and this morning i went to root the phone. I followed the steps in Hansoon's AIO toolkit and successfully unlocked the bootloader then flashed the CWM recovery. Then, when attempting to install superuser, i could not get my phone in to recovery mode. Every single time i either select recovery mode from hboot, or attempt to boot in to recover from my PC via the AIO my phone would boot as normal, and not enter recovery.
I assume that my recovery flash did not work correctly, but i could be wrong. Does anybody have any advice?

I unlocked the bootloader. I need root to get S-off. Except I'm currently on S-on and the only way to install a recovery and get root is to have S-off (correct me if I'm wrong). Any workaround to this that isn't super complicated?
I've checked USB ports, and I can't seem to get my device to be recognized. "fastboot devices" shows up nothing. Currently unlocked, and S-on.
Forgot that unlocking bootloader again would wipe my phone, forgot to make backup, so everything's deleted ;(. Not possible to get s-off anymore?
Any help would be appreciated.

You just need an unlocked bootloader to install a recovery, when you are in fastboot (if you turn the phone on and do adb reboot fastboot, or hold down vol down and power and select fastboot when the HBOOT menu comes up) you can do the command line fastboot flash recovery recovery.img (recovery.img being your recovery you want to load) you can then start the phone up, copy superuser.zip on there somewhere and install with your custom recovery.
All that said, you don't need to do any of that to do 4.4.2 OTA, and in fact it wont install if you have a custom recovery. Are you having a problem that it wont download/install. If your phone is stock other than an unlocked bootloader it will install (trust me i just did it). If you have stock recovery just download the 4.4.2 OTA file from the other thread, then save it to the phone somewhere, the go to recovery in HBOOT, wait 1 second and then hold up vol and power for 5 seconds (may have to do this more than once). Menu comes up, do wipe cache, and then do install from device storage.
After thats done you can follow my above instructions and root it
Sorry if I didn't state it more clearly, but my issue is I can't install a recovery. I can boot into bootloader and fastboot but when I type in "fastboot" devices on my PC nothing comes up.

Have you installed HTC Sync? When you boot into the bootloader then fastboot and then plug in your usb cable does it say "fastboot usb" on screen?

I uninstalled HTC sync so as to not have it interfere with any s-off/etc. programs. I still have HTC drivers installed. Re-install both HTC sync and drivers?

HTC Sync won't interfere with anything. You need that and the drivers installed for it to work, I would start there and see what happens.
I'll re-install it again.
Is there anything I can do to just completely get back to stock everything? Willing to manually update it but I'd like to be able to just go to settings and just update via OTA from there.

As long as you have an active AT&T SIM card in there it will update even with an unlocked bootloader, even if its rooted. As long as you didn't mod anything. I just did the update like this, it worked fine.
I've rooted and unlocked the bootloader before but I've unrooted since then. Is there still a chance I just haven't gotten the update pushed to my device yet?

Thanks. I have that file all ready to go in my folder. Only problem I have now is that I can't get s-off. I've installed recovery + custom kernel but the su binaries are outdated for both superSU and superuser. I've tried to flash a 4.3 version of superSU only to have the same error.
I followed instructions for root after I flashed a super tool I also flashed a custom kernel. After I rebooted is where I started to get the "outdated binaries" prompts.

Ok that changes things. IMO you need to backup your data, install the 4.3 RUU to wipe everything out and make sure you have stock recovery, then you can install the OTA zip from stock recovery, or go ahead and download it direct from AT&T. You can then root and go from there. Now as for S-Off if you have HBOOT 1.55 or newer I don't think you can get it. I can't get it on mine I tried every way possible. You don't need S-Off for this to work

I have unlocked my HTC M8 eye with the help of HTCDev site and installed TWRP from playstore. I restarted the phone and went into Fastboot mode and using adb installed TWRP image file for this phone (v2.8.0.3), before it I have saved SuperSU zip file on my local storage. From TWRP in recovery mode I installed SuperSU everything goes good and its shows Success. When I reboot I shows "Your device does not seems to be rooted? Install SuperSu now?" When I gave install now it shows error msg "Failed to copy su binary in to /system/bin/". I tried re-flashing it and tried to install some other versions of SuperSU but none worked. After this phone goes re-booting and stuck in HTC logo screen till I use shortcut keys to enter recovery mode. In Fastboot mode my laptop was able to detect my phone from command prompt and also my internal storage.
I believe I can get back my phone on working. Please help me, I really need my phone back .
If you can still get into recovery, then you can install a custom rom and go that route. Keep in mind, if you are just HTCdev unlocked and you flash a custom rom, you will have to use fastboot to flash the boot.img.
If that doesn't work, then you should be able to find the RUU for your phone and restore it to stock and redo the custom recovery and root. For future reference, just flash the SuperSU zip. TWRP doesn't normally detect that correctly and trying it's method often makes things worse, as you experienced.
