please help me this !
my Kindle Fire HDX 7", maybe hard bricked after factory reset (wipe all) ?
now, it just show "Kindle Fire" and stop here forever ! yesterday, i still turn on and turn off normally !
But today, after I tried to Factory Cable method, it's quite ! "waiting for device" show so long ! then, I turn off by hold power, and all will be : no signal when i plug in charger ! no turn on by press power button ! no connect to PC, no ADB, in Devices manager on PC, only show "QHSUSB_BULK" ???
I have read a thread on this sites, by long press power button in about 2-3 minutes, and my KF turned on and show "kindle fire" as before !
and whenever i turn off by hold power, my KF will be quite ! and PC only show "QHSUSB_BULK", to turn on, I must long press power in 2-3 minutes as... again and again !
if my KF stuck on "kindle fire", when usb cable is plugged, PC will show "Portable devices - MTP USB Device" in Device manager !?
I tried installing Kindle Fire driver, ADB driver, Android SDK... no effect ! In command prompt, I type "adb devices", it show "list of devices attached" and nothing !
I'm confused and nothing I can do ?

How exactly did you perform the factory reset? What exactly did you wipe, and what did you use to wipe? Which original OS version did you run, did you root, install any recovery tools like Safestrap or TWRP, and did you use any custom ROMs like Nexus or CM?

I installed salestrap and CM10, then factory reset, i don't remember what's i wiped ! maybe all !
I tried pressing power and volume UP, then my KF show System Recovery, but only have 2 lines : Reset kindle and factory reset, i saw "Your Kindle doesn't seem to be able to boot", i choose factory reset again, and everything is stuck on "kindle fire" !
i wonder when i plug Factory Cable and try any methods, then It's get black screen and show QHSUSB_Bulk in device manager ! I reallize that it will be drain battery if i do Factory Cable methods ! that's reason make my kindle get black screen => no battery remain ! I try charge it about 30 minutes, and i can turn on normally !

Do I get this right, you were on 4.5.2, rooted, installed Safestrap v4. Then you probably installed CM11 (there's no CM10 for HDX, right?) into the stock slot, i.e. FireOS is gone?
Then you did a factory reset from Safestrap v4 (this would mean you had some protection to not wipe system)?
But then you entered Amazon recovery by pressing the Power & Vol + Buttons, but without something in the stock slot that probably failed, and probably killed Safestrap, too? So you don't get the Safestrap screen after power on/reset anymore?
If that's the way it is, it seems pretty bricked to me. Dunno if this could be rescued anymore.

Now, my KF only : Stuck on "kindle fire" or "system recovery", nothing else ! No adb, no fastboot !

"Factory Cables" do not work with with HDX devices. This recovery technique can not be used.

With HDX you must know what version you're on, because what you can do depends on that. You must pay attention to the compatibility notes in the threads of recovery tools (e.g. Safestrap or TWRP), and the ROMs you install. Otherwise failure is what you will get.

I used a fastboot cable to attempt to load a factory ROM to a Kindle Fire model p48wvb4 in order to fix some flakiness I had with the device, and now I cannot boot, as the device goes to the fastboot screen whether the cable is connected or not. I got this device used, and was under the impression that it was a 2nd gen HD but maybe it is HDX.
Anyway, I used KF First Aide to attempt to load 10.2.3 on it, and everything appeared to operate fine. However it reboots directly to fastboot screen, and I cannot find a way around this. I've done the power switch reset thing, have redone the KFFA load several times, and nothing helps.
Please help!

I'm currently having the same issue. I've noted that the soldering on the fastboot cable I'm using is loose, so that may be the case, but regardless of if I'm using the cable or not, the device STILL boots to fastboot.

Same here. No matter how I try to reboot the device (long press, etc), it boots to fastboot after briefly flashing a red screen. Trying to reboot through software, as with KFFA, does nothing - no reboot at all.

This morning, I was reading comics on my tablet, an it froze completely. The touchscreen and buttons were not responding so I held the power button to turn it off. I turned it on again, and I had an "Encryption unsuccessful" message, saying that I needed to perform a factory reset. The problem is that I never set up my tablet with encryption...
That being said, this screen pops in when my tablet starts, and there is nothing else I can do but to turn it off. I tried the following :
Reboot
Reboot to safe mode
Launch the recovery mode and launch the Android icon (I don't know what it's here for)
Launch the recovery mode and launch a factory reset
The factory reset starts for less than one second before showing the Android sign with a red alert triangle, and resets the tablet. At this point, nothing worked, I am still stuck with this screen, I don't really see what I can do.
I bought my tablet in the US in 2012, it runs Jelly Beans if I recall correctly (the ASUS version, I never rooted it). I have a Mac running Mountain Lion if it can help. Does anyone know what I can try now ?

No, it is the default bootloader. Everything is standard, I never felt the need to modify the OS of my tablet. Do you have a theory?

Yesterday the device shut down and rebooted in fastboot, than it was completly died. I can't boot it. If I connect with USB cable I get: QHUSB_BULK. The kindle wasn't rooted. The display is black, there isn't Kindle Logo.

Press and hold power button for 20+ seconds to see if you get a response. Make sure you have a full charge (any response to the charge cable?). Could be a hardware problem but more likely a botched upgrade. If the device is still under warranty reach out to Amazon. They are pretty good about replacing faulty devices.
I'm in the same boat as the OP.
I have a botched upgrade and Safestrap seemed to got formatted.
Stuck in BULK mode using a Fastboot cable with no ADB access. I can also get to the grey logo screen that prompts for a Factory Reset or Reboot but nothing happens there either.
Am I SOL?

I downgraded from 4.5.3 to 3.2.4 because I wanted to root the device.
The installation was successful and my kindle rebooted just fine....I started to download some apps from store....I left the device for 10 minutes closed not turned off and when I came back to open it not to turn on the screen was black and none of the buttons where responding...I tried every combination of buttons but didn't work...My pc recognize it as QHSUSB_BULK...I found the correct drivers and now I can connect it to PC but only in QHSUSB_BULK mode...
It's not rooted,
doesn't have safestrap installed
it doesn't reboot in stock recovery and
It doesn't have warranty cause is outdated

I was following this steps: http://forum.xda-developers.com/fire-hd/general/root-developer-preview-lollipop-os-5-t3172408 to root and installing gapps. After i use Flashify to put the Ariel TWRP image, after this i reboot the tablet, but now its keep bootloop in the amazon screen. If i connect the kindle fire in my PC, it cant recognize the device, because the kindle keep booting over and over again in the amazon screen. Its just show me in the device manage (in the little time the pc recognize the device) with the name MT65xx or something like that, then its closes again because it keep booting. If i hold Volume Up + Power, it show me the "Fire System Recovery" with 2 options, Reboot or Reset to Factory Defaults. If i reset to factory defaults, start with the Kindle Fire screen and saying: "Deleting" then 10 seconds later reboot again, and the Bootloop keep going.

I got to the point where the login page should appear but there was only the FireOS background (no login page) blinking randomly without being able to make a touch input, only the hardware button work (like shutdown).
I then thought ok, I will make a factory reset with ADB and start all over. I ran "adb reboot recovery" and got to the HDX recovery screen and made a factory reset. Afterwards I rebooted but the problem still remained with the "login page" (blinking screen). Unfortunately my root and ADB got lost with this procedure and now I don't know how to get further.
How do I get to the factory recovery screen to try a factory reset again? I tried all types of boot ups (power + vol up or down; power + vol up and down) but none of them worked. As far as I have read a Fastboot cable with the Fire HDX will not help out, too.
I only have access to the file system (Android File Manager) but nothing more, at least what I know. Is there a way to upload a .bin Firmware-File and let the HDX execute it during boot-up? Or how can I make a factory reset? Is there a special code I do not know?

Unfortunately, You are in FireOS/HDX hell. Borked FireOS w/no adb access. Not much you can do. Even if you were able to access stock recovery a "factory reset" only erases the cache and data partitions. That's not where the problem resides. There is no mechanism on HDX devices to reset or reload FireOS. A huge oversight by Amazon that was addressed in 5th gen devices. But that doesn't help here ...
A bit more history. For several years this condition shelved/landfilled many a 3rd gen HDX. Eventually 'bulk mode' was discovered that permitted reloading the bootloader and/or rewriting the system partition in preparation for installation of a custom ROM. While it may be possible to place your device in 'bulk' mode there is no path forward from there. No one has extracted a replacement/vulnerable bootloader for Saturn not to mention the other items that would be needed for recovery. 4th gen devices are fairly rare with virtually no XDA support and development.
You can try to swipe down from the blank blinking screen with the goal of accessing settings. That might provide a path to re-enabling ADB. Beyond that you probably own an expensive paperweight.
