If you disabled apps you have to re-inable them if you uninstalled apps that were not system apps your fine

Yes it is I used the same recovery and it worked fine how did you flash this and what do you see when you reboot to recovery

Ok that's stock recovery when you see that symbol hold down power button and press both volume buttons it will boot into recovery

cant get the OTA to install.
when i flashed TWRP i chose system to be read-only
all disabled system apps are now enabled.
did a full unroot in SuperSU
when installing the OTA, in mid of progress bar it shows the same exclamation mark as in the recovery.
but no text, so i cant know what error its getting.
any ideas?

Something is still not right somewhere you can try again and when it goes to the exclamation symbol hold down power and then press both volume buttons and at the bottom in yellow it will tell you why it errors out

Wish I could find the thread so I could link to it, however.....
If you took a TWRP before you mounted and modified system you are able to use fastboot in download mode to flash the system.emmc.win 'fastboot flash system system.emmc.win' after that you can restore the TWRP. All that is left to do at this point is to flash the recovery that matches the version of your firmware an you will be able to take OTA.

I think TWRP will prevent the OTA from installing. I had the same exact setup as you and what I tried was to flash a stock recovery.img and then take the OTA. That didn't work for me, I got a red triangle in recovery when it tried to update. I haven't had a chance to figure out what other options I have besides just flashing one of the custom ROMs. Would like to find a way to update without wiping everything but I think that's what I'll end up doing, just trying to avoid it.
You just need to flash the stock recovery back and then take the ota. Afterwhich you can flash twrp back

Go here, there's both odex and deodexed versions you can download and then flash through recovery.

i did it last night and can confirm that it does. just need to make sure that your /system partition is stock as well. (at least that's what i did)
Figured mine out so thought I'd post back. Learned somewhere that if you hold Vol Up and then hit Power when you get the Red Triangle with Exclamation Point it will show you a log of what happened. Mine was failing due to a custom boot animation (live dmesg lite) - so I uninstalled that and the OTA installed fine.

I have looked through numerous threads to find an answer to my problem but cannot seem to find it anywhere. I have tried obtaining the new update both OTA and installing on my SD card and am getting the same problem all the time. My phone will reboot into recovery ( I have CWM) and the screen with say "Install this untrusted package?" I have to tap the volume down button to "install untrusted zip" and when I hit the power button I get the same error that it failed to verify the whole file signature. D: Error in /cache/fumo/OTApkg.zip
I am rooted completely stock with s-on.
Does anyone have any idea how to get this to work?

You have to relock the bootloader and be on stock recovery to accept the ota.

Yeah just relock the bootloader (hasoon2000 all in one is easy if you dont know the script), and name the file ph98img.zip, then boot to bootloader and accept the update.

I want to add you MIGHT need to do the update twice once you do what we both mentioned. I had it once require me to flash the ph98img file twice before it booted into htc stock ics, maybe because I was on one of the new JB roms

Only way i know of getting stock recovery is to flash the RUU but this will wipe the phone.
back up the rom you are on now.
back up internal partition
lock bootloader
flash RUU in hboot
this will get you 100% out of the box stock
accept the OTA update.

First you need to delete your win32 folder and profit.

I was able to get it to stop rebooting and failing by renaming /system/app/HTCdm.apk,
(after seeing that was what was doing the download). I then deleted the OTA zip file,
and rebooted. Since then it has not tried to reprocess the OTA.,
My feeling is that shouldn't have tried in the first place. But this workaround seems to work.
So I was trying to root/rom flash my wife's X+ to the Viper rom. I unlocked the bootloader from HTC, downloaded the all-in-one app, but it only had CWM on it, so I flashed CWM, pushed SU and installed SU with CWM, everything was fine. \
Loaded Viper rom on to internal memory, rebooted into CWM, installed Viper rom with full wipe. Phone rebooted, got Viper start up screen, booted to Viper lock screen, but when I unlock it, it shows a white screen with HTC on it, and wont let me get into anything. If I hold the home button, it says set up stopped responding, wait or close. I then reboot and all I get is a black screen with the battery, wrong time, and no signal at the top and then it resets it's self after about 2-3 mins. I cant get into the memory card or debugging before it reboots. What can I do to get the phone back functioning?

If you've updated the phone to the new firmware ( AT&T/Telus) the only roms you can flash are the one with new base.

I assume you wiped data if you got the setup screen?
As @garmig said, if you updated the firmware and are on the latest HBOOT, you won't be able to flash ViperXL+ until I update it to the latest build. Did you take the OTA update?

yes, i wipped all data, cache, etc.
if that was the update that came out about a week or so ago, then yes i remember her saying something about an update from ATT.. How can I get another rom on there to flash if i cant get into ADB and debugging mode? ive tried the side load in CWM but i cant get it to connect, and the phone just sits there waiting.
I've always flashed galaxys and such, so im use to being able to remove the memory card and put what ever i needed on with a card reader. I just got my HTC One and now she wanted her OneX+ flashed for more options. Guess I didnt read close enough about the update.. and of course she is mad because she doesnt have a working phone at the moment, lol, GO ME!!!

You can use the RUU from here keeping in mind that it will wipe your phones internal memory COMPLETELY!
or you could flash this rooted and deodexed version from here
Both are the new build 1.19.502.1

First thing's first, what hboot version are you on? you can see that in the booloader screen. Mine is 1.32.0000 which is the previous version.
Anyways, ADB works in TWRP. I'm running 2.4.4.0 and I noticed it can take up to 30 seconds for it to start. You may want to start with that version: http://techerrata.com/browse/twrp2/evitareul
Once in TWRP, you can either use "adb push" or "adb sideload":
ADB Push:
Run "adb devices" until your serial # shows up
run "adb push c:\somefolder\rom.zip /sdcard/" to push the rom zip to your phone's sdcard. It can take up to 10 mins to transfer it this way. You can use one of these: http://forum.xda-developers.com/showthread.php?t=2235248
ADB Sideload:
While in TWRP, select Advanced then ADB Sideload
Type "adb sideload c:\somefolder\rom.zip"
Once flashed, reboot to bootloader and flash the boot.img then reboot your phone.

Yes, the first is an .exe and can be run from Windows. However, you may need to flash stock recovery( which can be found here ) first.
The second file can be flashed like any other rom as well as the boot.img

you have no idea..... lol kinda glad im at work so i dont have to hear about it, but kinda wish i was home so i could fix it faster, lol
i should be home in about 3 or 4 hours though, and i will try a few things and let you guys know the status. I appreciate all the help so far, hope i can get it back up and running asap!
before going back to stock, i would flash a different rom mate. latest viper as a known issue where it leaves you stuck at that very point I'm sure while its stuck on the htc screen you should be able to connect it to your pc and drag a new rom across, if not flash TWRP 2.4.4.0 and "sideload" or "push" your rom. Trust me theres no need to go back to stock.

hboot is 1.40.0000 so yeah, its updated... i will try the earlier version of TWRP, as the 2.5 didnt work for me either

If you install the international 1.0.7 from a full wipe you get stuck at that very point.
I'll follow up with m0n then see if he knows anything about it.
However, I'm pretty sure we're talking about the AT&T variant of the HOX+

I can do the first OTA update. The second one downloads and attempts to install but about 1/3 of the way into the install, I get the red triangle. I cannot boot into recovery. It just gives me a red triangle. The phone works fine and I still get the 4.2.2 update/install nag but it won't install.
I'm stuck. Any help would be appreciated.
Reloaded TWRP. Guess I will stay on 3.06 for a while longer.
Using fastboot commands on your computer type fastboot clear cache and see if that helps your recovery issue.

The red triangle screen, is stock recovery.
If you hold volume up and power, you'll get a menu.
In that menu, you would choose to install from cache.
But due to it not doing it automatically, I'd say it's gonna fail to complete the update anyways.
You must be on a full odex stock Rom for the update to take.
If you removed any system apps, it will fail...
My stock Odex full JB Rom and stock recovery from same thread is all that's needed to successfully take the OTA.
I also posted the kit Kat update in stock form, fwiw.

In that menu, you would choose to install from cache.
But due to it not doing it automatically, I'd say it's gonna fail to complete the update anyways.
You must be on a full odex stock Rom for the update to take.
If you removed any system apps, it will fail...

Thanks for the suggestion. That didn't work but I used a method to unlock a long time ago where I had to change my CID. As the roms progressed from the 2.0x, things with recovery and root were spotty at best.
Funny how not wanting to reset my phone long ago lead to me resetting it 4 times trying to get this update to work.
I got the OTA, but when the device reboots, it doesn't update. Then it wants to download the OTA again.
I'm s-off, unlocked bootloader, stock recovery. I'm thinking I need to relock the bootloader.

If you have frozen any apps, have xposed installed, or altered anything in system folder it will not update when your phone is in recovery and fails update look at the yellow writing at the bottom of recovery and it should have the the reason it failed

I unfroze all apps, uninstalled xPosed.
It reboots to recovery (stock) and starts, but then quits to recovery after a minute.
I'm not sure what the scoop is.
I might just run the RUU...
What error message did it say at the bottom of recovery when it stopped

I didn't see an error. I'll see if I can get it to download again (I keep having to set my clock ahead more and more days...) Almost doesn't seem worth it.

Here's the ota download it and put on your ext-sd card that way you don't have to keep moving forward the clock

Thanks for the file. I think the problem is with either my stock recovery, or with some system file that's been modified.

Check out my thread you can probably flash twrp recovery and use my backup so you don't have to go through the long process of backing up your entire internal memory and flashing the ruu

Before I RUU the whole thing, I tried the OTA zip package after re-flashing another stock recovery.
I got further, and actually have an error that makes sense:
Package expects build fingerprint of htc/cingular_us/htc_m8:4.4.4/KTU84P.......; this device has htc/cingular_us/htc_m8:4.4.2KOT49H.....installation aborted.
I'm guessing I have the wrong firmware or recovery, but I was running 4.4.4.
Any ideas before I ruu?

Download 4.4.4 firmware from link below and place it in ext-sd reboot into hboot it will ask you to press power to update firmware just press power wait until it's finished then reboot and try updating ota again

Didn't work.
I'm thinking that I may have either a bad OTA zip file (I didn't grab yours) or there's something really odd about what I've done to the device (not sure what - I disabled xPosed, unfroze all the disabled apps).
I'm probably going to grab the OTA one more time (It started downloading after a TWRP restore).
If that doesn't work, I'm going to probably download the RUU and restore using Titanium backups.
