OTA update not updating - AT&T HTC One (M9)

I have a refurbished HTC One M9 (att) that I've had for about 9 months or so. I've never bothered to root it, much less try to do any custom roms or anything. Last night it decided to implement an OTA update overnight while I had it plugged into my computer for charging and I woke up to find this screen:
Android system recovery <3e>
LRX22G release-keys
Volume up/down to move highlight;
enter button to select.
reboot system now
apply update from ADB
wipe data/factory reset
wipe cache partition
reboot to bootloader
power down
view recovery logs
apply from phone storage
apply from sd card

E:failed to map file
Installation aborted.
I selected the reboot and the phone rebooted normally and then gave me a message that update failed and to wait for the next update. Thinking maybe the failure might have had something to do with being connected to my computer, I made it download the update again and try installation without the computer connection but I simply got the same screen as above again.
Can anyone tell me what this issue is here? Is there a solution and perhaps what that solution is? Is this update even worth bothering with? I haven't really been following along so I don't even know what's in this update.
Thanks,
tbmguy

This has happened 2 me before. But enough free space on the internal memory. I forgot the exact number but it was a little more than half

works like a charm everytime.
no need for button combos
Nonsense.
Okay, check this out.
1. With the phone unplugged.
2. hold all three buttons down
3. Let it cycle through vibrant, Blank, Vibrant, blank.
4. after the third or fourth time, plug in the USB (Keep in mind your hands are still on the 3 buttons, you haven't let go)
5. Now, when the little hourglass starts to spin, your hands are still on the 3 buttons.
6. as soon as the hourglass disappears, you want to let go of the POWER button ONLY.
7. This will likely get you back at the hourglass/battery screen.
8. That's okay, Back on the power button again. Let go of the power button as soon as the screen goes black again.
I promise you, you can get into download mode.
I've only owned my first android phone, for 1 month. Prior to this I had Blackberries for 4.5 years. But, I've gone through.
Stock, Bionix 1.8, Bionix 1.9, Back to stock, Eugene's Froyo, Back to Stock. Bionix 1.9, Fusion 1.0, Fusion 1.1, Fusion 1.2 *(I've flashed more than 20 times in the 29-30 days i've had this phone). I've got a lot to learn, but i'm confident that the method outlined above will get you to D/L Mode.

Not to be mean but didn't the warning/instructions for Obsidian say NO lagfix? Please be more careful and read before flashing anything next time.
And please post in the right forum too.

Both you guys make it seem harder to get into download mode than it is.
Here do this.
Have the latest Odin open on your PC. I have Odin3 v1.7 which can be found here http://forum.xda-developers.com/showthread.php?t=821625
Load the correct PIT and PDA files
Then take the battery out your phone
Plug in the usb cable into the PC
Then plug in your phone to the usb cable
hold volume up and down buttons/DO NOT HOLD DOWN THE POWER BUTTON
then insert your battery while still holding down both volume buttons
You should get into download mode right away.
If that doesn't work then do this:
Plug in the usb and press and hold vol up and down and power.
When the screen goes black let go of power while continuing to hold vol up and down.
Should bring you back to download mode
I know this 2nd method is pretty much what a4 moda said but you don't have to wait the couple times. Just when it goes black the first time after the Vibrant screen you can let go of power and it will go into download mode.
Wasn't it already posted NOT to try to use Voodoo on 2.2 roms.
Well do this, go into a T-mobile store, tell them that you voided your warranty by rooting your phone, flashing a leaked rom, then tried to flash a kernel and now your phone won't boot up. After you tell them all that; ask if they can exchange it for a new one
power the phone off
plug the usb cable in
wait till the battery icon appears
press and hold both vol and power button
when the screen flashes, release the power button
continue to hold both vol buttons
that works every single time for me.
how has no one suggested doing it through ADB?
"adb reboot download"
if you have ADB/SDK and proper Samsung drivers installed, this command will get you right into download mode

I'm on the same boat. I was on AOSP 1.1 and went back to my AOSP 1.0 backup. CWR does not start. I was stuck on the same screen for a while but after an hour or so, I figured it out. I can get you restarted but recovery doesn't work for me as well.
Plug your phone in (charger or comp, it doesn't matter).
Remove battery.
The screen is going to remain on for 10 sec or so, wait til it black outs.
Reinsert battery.
You'll be looking for the battery icon with the green status bar.
As soon as you see that, power the phone back on. It should restart.
Only problem is, I can't get into recovery so I can reflash another ROM or Clockwork. (Sadly, I'm on a Mac). You should be able to ODIN and etc...
Let me know if it works...

I've tried getting using adb to get into recovery as well, but it takes me to the old recovery and when I do the reinstall packages to get to clockwork I get the same message. Can anyone help?
I had that same issue when I tried out the i9000 voodoo for froyo. I had to use Odin. Just start up Odin, power down phone, pull battery, plug cable into computer, replace battery but dont power on, then hold volume up and volume down and plug in cable to phone. You should boot right into download mode, and assuming you have the correct Samsung USB drivers, Odin should detect your phone. Then just load in the stock JI6 eclair or stock JK2 froyo .pit and .tar files to their respective places and click start. Should work for you.

why would you have to re-ODIN just to get recovery to work..
I too am having recovery issues. Can only flash to stock recovery with trying to reboot via ROM Manager
If you do decide to Odin...here are some "surefire" methods...let me know if it doesn't work for you.
Here is a couple of methods to get into download mode. Once you get to
download mode, follow the directions in the second part of my post...
First Method
1) Pull the battery, sim card, and sd card.
2) Reinsert battery but do not press any buttons.
3) Plug into usb that is connected to your computer. If something shows up
on the screen (charging logo) then you should be golden.
4) Hold the vol up, vol down, and power until the screen comes on,
immediately release the power button but not the other two. Hopefully it
will boot into download mode and than you can use Odin to get back to stock.
Second Method
1) Another method that seems to work for some people is to pull the battery,
sim, sd card.
2) Reinsert battery and hold both volume buttons but do not press power
button.
3) Then plug in usb. The phone should boot into download.
Third Method
1) You will need the samsung ADB drivers installed for your phone and ADB.
Samsung Drivers for 32 bit Windows: http://files.androidspin.com/downlo...=SAMSUNG_USB_Driver_for_Mobile_Phones_x86.exe
Samsung Drivers for 64 bit Windows: http://files.androidspin.com/downlo...ung/vibrant/&file=usb_drivers_GalaxyS_x64.zip
2) Remove sim card and sd-card. Remove the battery.
3) Hold buttons vol up, vol down, and power.
4) While holding down those 3 buttons put the battery back in and keep
holding all 3 buttons until the vibrant disappears and reappears. Once it
reappears, release all 3 buttons.
5) Now connect your phone to your pc via USB cable.
6) Open a cmd prompt and navigate to the folder with ADB and type "ADB
REBOOT DOWNLOAD" caps are not necessary.
7) Your phone will reboot into the download
How to get back to stock once in Download Mode:
Download the files in the linked thread and follow the directions there or
the ones in my post below.
Odin and Files Needed Available Here: http://forum.xda-developers.com/showthread.php?t=734475
1) Open Odin and verify that one of the Com ports shows a device connected.
If it doesn't try using a different usb port.
2) Load the .pit and pda files. It should be pretty obvious how to do that
once you are in Odin.
3) Make sure that the option to partition is unchecked. The other two
options should be checked.
4) Hit start and wait.
5) Your phone will restart when Odin is done and you will be good to go.
Thanks for the replies guys I will give Odin a shot

[Q] Can't get to CWM or stuck in APX mode.

I'm not able to get to CWM.
I was switching ROMs and now I seem to be stuck in APX mode.
I'm able to use NVFLASH as outlined here viewsonic-gtablet-for-dummies.webs.com/nvflash, using the 1.2 branch with CWM preinstalled. After the process is finished, I get the stock rom and everything appears to be working but if I power off and try to turn it back on using power and volume + (I'm sure it is '+') I just get a black screen. If I plug the gtablet into my pc I hear the familiar usb connected sound and can run nvflash again, but if I power off it won't reboot.
I searched around this forum for possible solutions, but everything I've tried is not working for me.
Am I missing something?
I've been successfully switching between ROMs in the past.

So it's going into APX when you do Power & Vol +. What happens when you do Power & Vol -?
I am seeing the exact same issue after trying to revert to stock mod w/ CWM... any ideas?

Would you like to check out this fix ?
Dave
Thanks Dave, I was able to get further with your link (which is promising), but it didn't quite fix my issue. Everything goes great through 2.7, then on 2.8 when flashing with part6 (the 1st time), it stops at the "Android system recovery <3e>" screen, with the following at the bottom:
E:Can't mount /dev/block/mmcblk2
(No such file or directory)
E:Can't mount SDCARD2:recovery/command
Finding update package...
Opening update package...
Installing update...
Installing Tap 'n Tap build 4349
bootloader_file[0]="/tmp/bootloader.bin", partition_name[0]="EBT"At this point if I reboot, it gets stuck at the screen with the screwdriver and wrench crossed in the middle. The only resolution is to hold the power button... then back to APX mode. *sigh*
From the posts that I could find, the first mounting errors seem benign, but I looked at the recovery.log, and here is what I see at the bottom:
Installing Tap 'n Tap build 4349
wrote recovery partition from /tmp/recovery.img
wrote boot partition from /tmp/boot.img
mtd: not erasing bad block at 0x0b940000

script result was [0.100000]
bootloader_file[0]="/tmp/bootloader.bin", partition_name[0]="EBT"
It would appear that I have some bad blocks
I'm not quite sure what to make of the message about no /data partition (while trying to install angry birds, of all things)
Any suggestions for the next course of action?
Any thoughts on where I go from here? I'm thinking it might be time to flash with the stock ROM, no CWM, and send it back to Viewsonic. From the log, it looks like I've got some bad memory blocks.
Any other ideas before I ship it back and hope they fix it under warranty?

Need help, searched and cannot figure out partionss and restore cache logs S8+

I can't get rid of the below, no matter how many wipes, or cache partition clears, and the recovery logs list just grow and grows.
My phone was never rooted by me, until I found my cell restarted one day for no reason.
There was no power issue, OTA update, it was very odd.
Can get into CRAZY details, so, anyway.
I get the below, was able to get rid of the bottom two lines once, how? But trying what I thought I did to get rid of them, brought them back again.
#Reboot Recovery Cause is [[BootChecker]RebootRecoveryWithKey]#
No Support SINGLE-SKU
File-Based OTA
Supported API: 3
E:unknown volume for path [/odm]
E:unknown volume for path [/vendor]
E:unknown volume for path [/odm]
E:unknown volume for path [/vendor]
# MANUAL MODE v1.0.0#
Flashed using odin and sammob firmware, and still have the above, and the crazy recovery log list (forg that is just growing, up to 9 now.
Ideally? Would love to have the memory totally wiped, if possible, to the last binary that is not needed and wiped.
Add a fresh stock flash, so I can put my new sim and phone number, as soon as possible.
Just the hacking got so bad, that I turned off my phone last month, new years eve.

any hope for Fonepad K019 ?

My Fonepad K019, got it from ebay months or years ago and used it daily, is not starting anymore without me doing something like root or flash.
I use it for information and news, too. So I need it really.
It powers on with Asus symbol on display and then comes
for 2sec “installing system update...”
then “error!” for a few minutes?, then starting again. Same again. And again.
I have not tried to update or root or flash !! I put it on factory mode a few weeks ago and then installed apps I need, apks from not google play, too (banking, packstation, writing).
I get with power on/off and volume high into a menu, sometimes:
"Android system recovery 3e
LRX22C.WW_fonepad-12.14.3.18-20150916 release keys
Volume up/down to move highlight;
enter button to select.
reboot system now
apply update from ADB
wipe date/factory reset
wipe cache partition
reboot to bootloader
power down
view recovery logs
Finding update package...
Opening update package...
E:failed to mount /sdcard (No such file or directory)
E:failed to map file
SD installation aborted.
E:Installation failed with status 1"
I have done “wipe cache/factory reset” for ten times
and “wipe cache partition” for ten times, too.
Its still the same.
No I dont know what I can do now to make Fonepad work again?
Where is this update, which the phone is talking about, coming from? From Google play, some kind virus? I havent done this!!
Is there any hope for my Fonepad?
