I spent a lot of time on this and finally reaching out for help.
None of my apps are able to access storage (internal sd card) for writing, even after granting permission manually or when prompted.
Setup: flashed with TWRP 2.7.1: aicp_klimtwifi_n-12.1-NIGHTLY_20171121.zip (which is Android 7.1) and open_gapps-arm-7.1-stock-20171123.zip. also used CFautoroot to gain root access (successfully).
But all downloads from Chrome fail because of unknown errors. Trying to create a folder inside Download in File Manager results in an error saying this requires elevated access. All these apps have storage permissions.
I can install new apps from play store. In the terminal as root, I can see that everything in /storage/sdcard0 is owned by root/sdcard_rw and has drwxrwx--x access bits.
Trying to fix file system permission in TRWP resulted in a nasty boot loop and I had to retrace my steps.
Not quite sure how to proceed. I'm not worried about data loss. For instance, I would happily use TRWP to reformat internal storage (if there is such an option), but I'm getting worried of digging a deeper and deeper hole for myself.

I have successfully configured my phone to install apps on the sd-card before. But because of some problems, I have to do a restore, and my latest back up file was before I partitioned my card and do the conversion stuffs. Now, I noticed that my free phone storage is filling up fast when installing apps. But using Ap2sd, I can see that the phone can detect my partition(304mb free out of 457mb to be exact). Do I have to re-run the conversion of extensions again? I'm pretty hesitant to do so until someone confirms that its needed since I did a nand restore.
Another thing - my sd card keeps on going to R/O privilege. I have to always use Root Explorer to change its permission state. Is this a common bug or what? Thanks
It's probably a corrupted a2sd script. Try another version of it and make sure that it is supported by your ROM.

Ok, so I went to TWRP and flashed a .zip (Dual_Core_Mod_Ultimate_v2.1) from sdcard - I wiped cache/dalvik before I installed the .zip
After a reboot, sdcard2 worked fine, but the zip didn't seem to stick - so I repeated the same exact process.
Now, sdcard2 is blank. In Root Explorer, sdcard2 is showing the same memory usage as sdcard. And under Settings - Storage, it shows Storage card memory as unavailable, and when I click "Mount storage card" it does nothing.
In TWRP (It's 2.1.8 - the one that came with RegawMOD) I went to Mount and checked Storage card, and was able to see the files on it (sdcard2)
So, why can I see it in TWRP but not in Root Explorer?
I'm rooted, HTC Dev Unlocked, on Stock rom (1.3.651.1) HBoot v1.12
Someone please help me/tell me the steps so I can access my ext_sd again through Root Explorer! I need help! - zmag
Edit: When I boot into TWRP, click Mount, there's a checkbox on Cache and internal - storage card is unchecked. If I check it to mount it, I can see the files on my ext_sd; but ONLY in TWRP. Again, it used to work perfectly fine, but now I can't get it to mount when Android Stock rom is running. Root Explorer shows sdcard2 in the directory list, but when I open it, it's blank. And it shows the same GB used/free as sdcard (sdcard being the internal memory)
And for what it's worth, the Dual_Core_Mod_Ultimate_v2.1.zip WAS working fine, now it doesn't seem to be sticking anymore.
And I was hoping if I left the checkbox marked for the external sd in TWRP and rebooted, it would fix it - but it's not.
I've tried searching the forums already, and came up desperate to solve this. And I was gonna buy a 64 gb sd card today too! WTH Thanks in advance if ANYONE has experienced this! - zmag
Edit 2: I just installed the latest version of TWRP via GooManager to see if it would help, and it didn't. The only difference I noticed is when I rebooted into TWRP, the external sd box was already checked, unlike in v2.1.8, where I had to check it myself under the Mount option. I'm really hopeful someone can help me figure out what's wrong here... - zmag

Edit: NOT SOLVED!
Here's how I THOUGHT fixed this. I noticed that in TWRP, I could see the files via it's Advanced - File Manager under sdcard2, but NOT ext_sd...
So, I decided to try "Fix Permissions" as a last resort, and voila! It worked! After reboot, Superuser asked for access again to Root Explorer (and a few other apps) which I obviously granted. But thankfully, all is well now.

Edit 2: Well it seems that the same exact thing is happened. In TWRP, sdcard2 shows the files, but ext_sd is nothing. And, Root Explorer is blank in both, but RE shows the same date/time/GB space in sdcard & sdcard2
WHY does this keep happening? PLEASE HELP!

Ok, I flashed Dual_Core_Mod_Back_to_Stock_v2.1.zip - seems all is well.
Lesson here: DON'T INSTALL DUAL_CORE_MOD_ULTIMATE_V2.1.ZIP... EVER AGAIN!

I've reset my factory settings countless of times just to root my phone because I can't install towelroot becuase INSUFFICIENT STORAGE AVAILABLE. And everytime i successfully root my phone, iI'll have to reformat again just to download apps like root browser, catlog, lucky patcher, etc.
I've tried lucky patcher, deleted odex files and cleared the dakliv cache, which worked for a while.
Using catlog, i'd see this error for every app that failed to download:
08-31 12:19:26.613 W/ActivityManager(745): No content provider found for permission revoke: file:///data/data/com.android.vending/cache/gzipped/com.twitter.android-1279829649.apk.
So i used root browser to find the file but when I reach thr gzipped folder, i don't see anything inside. I'd want to delete it but I can't.
I even spent money on SD maid but still, can't find the file.
I also looked at my /data/app-lib/ but i don't see the residual files of the apps i want to download. So i can't delete.
Help please!!

If you are certain that you have enough storage on the internal data partition as well as cache partition, the most likely cause are existing files that fail to be overwritten or incorrect permissions.
SD Maids CorpseFinder (if rooted) can help in some cases (activate extra Corpse Filter in settings).
This is also an issue sometimes creates by LuckyPatcher.

I've even paid for SD Maids pro version and unfortunately, it still doesn't work.
I'll try swapping my internal storage with my external SD but I'll need a card reader. Will update soon. I hope to be able to find a solution while I'm still on vacation.

I am no stranger to Android hacking so I downloaded newest ROM for my Doogee DG280 (mt6582).
I flashed it, then backed up using SPFlashTool, processed using MTKDroidTools to mod boot and recovery
and from this new CWM I installed SuperSU update from ZIP file.
So far so good, BUT:
Once any application requests root (i.e. calls "su"), it loses the rights to READ from sdcard (actual or emulated, doesn't matter).
I am emphasising READ because common problem on Kitkat is that the apps cannot write on SD card, which is easily
fixed by the SDFix app (tried it - no help).
So basically, all my apps can now read/write sdcard, but once an app requests root access, from that moment it can only write
to the SD, but not read from it. This creates lots of ****ty situations - for example, I download a file to an SD card and want to
copy it to /system/media/audio using Total Commander. So I do this, Total Commander says it needs to remount the partition.
I click Yes, it requests the root, remounts the partition (success) to rw and then tries to copy the file from SD but because it now
has root, it has lost the rights to actually read the file I want to copy.
Funny thing is, when I connect USB debugging and use ADB, the ADB is rooted as well and it CAN read from the sd card.
So there is a workaround for me now to use Android terminal, connect to the adbd on localhost and execute all the commands
I need but that is VERY nasty and of course it only works for manual operations.
Many root apps use sdcard to store data, for example Lucky Patcher stores downloaded patches on SD card. But with this bug
it cannot.
Anyone encountered this problem? Any solutions?
Try flashing this zip in recovery. Remember to take a backup before flashing. Did wonders for my phone running on MTK6582 chipset on KK ROM.

Unfortunately, didn't help. I examined the package and it does the same as SDFix app (which I already tried). But all these apps focus on giving write access to SD cards for ordinary apps.
I have a different problem - my apps actualy CAN read/write SD card right to the moment when they request root (call "su" binary). After that, they lose read access to the card.
Have you tried installing Busybox?
Maybe this helps: use SuperSU, disable the "mount namespace separation" option in SuperSU's settings. Reboot.
I've downloaded the lastest official ROM, and installed it within TWRP.
Everything seemed to be okay after rebooting into the system.
While I found i was not able to download the wallpaper from web browseror to take pictures due to lack of storage.
I'm not able to upload pictures here, so will try my best to describe the situations:
1. after opening camera, it pops out "Turn off camera? /n Insert an SD card before using the camera. /n Cancel or Turn off"
2. when i try to save something from web browser, it says "couln't save due to limited storage space."
3. opening the file manager, I do find internal storage shown at the bottom, indicating 50.20 GB/total 64.00 GB. But opening it, only two folders appear, namely "Android" and "TWRP".
I wondered if it was caused by some read-only issue to the system drive, but I did allow TWRP to modify the system partition upon booting the rec.img.
I've tried a few rounds, the problem is still there. and my yota 3 is kind of a brick to me now.
Help me, please.
My yota3+ also lost the ability to write to the internal memory (possibly after I've inserted a sd card and decline formatting it). Searching for a solution only come up with a very old thread on HTC which involved commands on a rooted phone like msdos_fcsk and restorecon. I couldn't try these commands as my phone is not rooted. You can try these if your phone is rooted.
