Ok, so i keep getting this problem with my hd2, i tried flashing different radios & roms but no help.
its hard explaining the problem so here it is what happens:
sometimes the phone dont sleep, if i press the power button it just goes to lock, but the screen wont go off, while dis happens when i try changing the brightness, the "on battery brightness" dont change the brightness, but the "on external power brightness" changes the brightness, but the phone is not connected to anything.
when i finally connect my hd2 to my laptop or to charge and take da cable out den its perfect, it works normally
please help
the phone thinks it is connected to an external power source. similar sounding to the "stuck in car kit mode" bug. probably a faulty usb port.
had to send mine back for a new mainboard.
any other symptoms? loudspeaker auto activating when you make/take a call? phone pad screen auto rotating?

No the loudspeaker don't turn on automatically, no rotation problems and yes im using the original cable.

I experience a very weird problem with my HD2.
When I want to connect my HD2 to pc then my pc (and other pc's) doesn't recognize my phone, nothing happens. Also my phone isn't giving me the option between "Activesync" "Disk drive" or "Internet sharing". (Altough the option to ask it on connection is turned on under settings). So not an active sync problem I guess. The usb doesn't seems dead because it still charges, on my HD2 the power sign comes up when I connect the usb to pc and the led is on. When I pull it out then the led goes off and the power sign dissapears. But no "data" connection is possible anymore.
Is something broken in my registry? I tried soft resetting several times.
No hard reset yet but I am waiting to do this, maybe someone here knows the answer.
I started experience the problem after I used Tomtom last week ( i used it alot already) . I experienced a problem that my HD2 was stuck in "Carkit mode" thus when someone called me he flipped the screen and putted speaker on. Even when I set it off under settings it. Navipanel was also turned on. I downloaded the hotfix from HTC for Navipanel and now that problem is gone. My phone is again in normal mode when I call or receive calls. BUT the usb data connection doesn't seems to be working anymore .
Someone here knows the magic answer ?
Can you post the Radio version and ROM version you are using?
ROM version : 1.66.405.2 (76641) WWE
Radio version : 2.06.51.07
Have you ever changed the ROM on your device or is all still origional as you got it from the store?
If origional:
1) bring it back for repair, since its not your fault
2) or install HSPL change your radio and rom
I would just send it for repair when that happens. Specially when its in origional state.
I had this problem once but at the moment i connected the phone through a USB Hub
No it is the official upgrade rom from htc site.
Phone is acting weird also.
Everytime someone is calling it puts the speaker on .

Hmm did a hard reset and the problem persists.
No usb data connection.

Although I completely agree that your unit is probably on the fritz, take into account that the MicroUSB cable can be the problem as well when it comes to connecting to your pc. I bought some Ebay cables and my phone wouldn't connect properly with those. Charging was no problem.

The problems started suddenly and I also tried another usb cable who was also working fine before.
I managed to get connection yesterday just once after pushing it in the phone and moving it a bit. So I guess the usb input is broken or the motherboard. Guess it is the motherboard because of the other weird problems (when want to call someone the screen turns vertically and puts the speaker on), also when i got called. And this after a fresh hard reset.
Hope they will replace the unit without a hassle.
Hi guys. I'm totally new to the android/smart phone world btw. Anyways, I recently got my Samsung Galaxy S2... first thing I noticed, my touch screen wasn't working. I popped the battery in and out and then it started working as it should.
My phone was working fine this morning, then all of a sudden out of no where, the touch screen stopped working again. I've tried turning on/off the phone multiple times. I've taken out the battery and even my SIM card nothing... i'm worried.
Has this happened to anyone else? Is there a fix? Do i have to go through Samsung? I hope not. Incase anyone asks, I did not root my phone.
Any help/feedback would be appreciated.
When the touchscreen stops working... were you charging the phone at that time?
The first time i turned on my phone, was actually by accident and it was charging.
However, just earlier now, it was not. it was just sitting on my computer desk. The home button, lock button and touch capacitive buttons work.... but i can't get past the lock screen
you could try a factory reset to see if its software related, but it sounds like hardware. I would definitely Return the phone .
I guess I can't do a factory reset, since I can't get into any of the menus... man this sucks. I spent $900 *gulp*
To do a factory reset,
1. Turn the POWER OFF, pull the battery out and put it back again.
2. Simultaneously press and hold the Volume Up, Home and Power button
3. If the Samsung Symbol appears, release the Power button
4. A Boot Menu will then show, Use the volume up/down to choose wipe data/factory reset.

oo that's interesting... I tried it. and it didn't get to a boot menu. maybe I was doing it wrong. I tried a few different ways, but no success. It turns out I have warranty with the store. They're going to replace it for me Thanks for the feedback guys. Awesome forums.
Something similar happened to me yesterday. When I had the in-box USB cable attached to a BoxWave VersaCharger (combined AC/DC charger), and the VersaCharger was plugged into 120V AC, the touchscreen would stop working. As soon as I unplugged the USB cable from the SGS2, or plugged the VersaCharger into a 12V DC lighter socket, the touchscreen would work fine.
I've got the same problem as the OP... I had my phone charging overnight and this morning when I first went to use the phone I could not unlock the screen. I could wake the screen via the home button, and see I had a missed call etc but it just wasn't responding to touch. BTW I can't remember if when I first tried to unlock the phone if it was charging or not but really this shouldn't matter.... oh and btw, I'm not rooted or anything either! Naturally I rebooted the phone but still the same problem....so I tried a battery pull and still the same problem. Just now I did a factory reset but the touch screen still isn't working! I guess this is a hardware problem so I'm going to get it fixed under warranty by Samsung! Gutted that I will likely be without my phone for a week or 2!
EDIT, UPDATE...
Well, I was just on my way to the repair shop Samsung warranty had told me to take it too, and I took my sim card out to put in my old Hero (the only thing I hadn't tried!) and I couldn't believe it but the touch screen started working fine again! Weird, but that is now my advice if anyone else has a problem with their touch screen not working - oh and I took the sim out whilst the phone was still turned on. Crisis averted!
UPDATE 2
Sadly this has happened about 4 times to me now and I'm just waiting for the inevitable next time. Each time doing a factory reset then taking the sim card out seems to bring the screen back to life (though last time not immediately) but I shouldn't have to put up with this. Problem is, I don't want to send off a seemingly perfectly working phone to get repaired as I fear they will struggle to notice the problem, never mind fix it! I'm hoping a new firmware comes out soon, in the hope that it may resolve my issues...

I have had this happen to me twice now (Touchscreen not responding).
The first time I had just installed a game and thought it must have been related to that. Pulled the battery and no joy, left it for a while and it came back to life...
Second time I was surfing the web and it stopped responding, pulled the battery and left it for maybe a minute but no joy .... pulled the battery again and left it for probably 20min and it came back to life. When it came back up I noticed the livewallpaper had been turned off and my widgets were gone from the homescreen.
What I did notice when it wasn't working was it had "safe mode" displayed at the bottom left of the screen

As of this morning, I have exactly the same issue as the OP. Screen doesn't react to touch, with the exception of the capacitive buttons. I found a dude on YouTube that had the same issue: youtu.be/0oIdllWiV4I
The last thing I did with the phone before it started misbehaving was to answer a call. The phone was on the charger when the call came in - I can't recall whether I unplugged it before answering, or after, or at all during the call. Not sure it has any relevance anyway, only mention it because I've seen reports of people's screens no longer reacting to touch when the phone is plugged in to a charger (apparently it even states in the manual that this may happen).
Anyway, whilst all my contacts, SMS, photos etc are backed up to the cloud, the same can't be said for my app data. Before I lose hours and hours of progress in various games, or all my settings in numerous other apps, etc, can anyone think of any way to get access to the memory of the phone, bearing in mind the following:
I can't connect to Kies to pull a back-up because bluetooth is currently enabled on the phone and when one connects the USB cable, it says "Activating USB mode will deactivate Bluetooth. Continue?" but I can't click the Yes button
I can't connect in USB mass storage mode because I can't navigate to the settings menu entry to enable it
I can't turn Bluetooth off in order to get Kies working, becuase I can't get to the settings menu or pull down the notification bar to reach the power control.
I can't use Voice input to control the device (on the off chance there was a "bluetooth off" voice command) because (presumably with the most recent firmware I flashed via Kies a few weeks back) for some reason Vlingo is no longer the app on the double-home button action. Instead it calls Samsung Voice and wants to walk me through a first use tutorial, but I can't click the next/skip buttons
I installed via Android Market push the app belonging to www_remoteaccessphone_com as it promises to be able to remotely switch bluetooth off, but the website says I haven't installed the app yet, presumably because alrhough I was able to install it without touching the phone, it probably needs to be run at least once to set it up, and I obviously can't do that
I can't start Kies Air to try and get my data that way (starting it would require using the touch screen)
Kies via bluetooth isn't supported on Android
Needless to say, the data I'm trying to retrieve is on the internal memory, not the SD card. As you can see I've been trying all sorts of creative things, but I'm fairly sure I'm screwed. Any bright ideas I haven't thought of?

flash it with CF-root, boot into CWM recovery, do a nandroid backup after that, use CWM recovery again to mount USB storage, connect to your PC and pull everything off.

Thanks for the quick advice. I guess I'll need to use my girlfriend's phone to download CF-root and then transfer the SD card to my device. Quick question before I proceed though - is it possible to remove all traces of such tampering, since I've already made out with Samsung that I'm sending it in for repair, and I'm not sure they will honour the warranty if they see it has been rooted etc.

get a USB jig off ebay or amazon to reset the flash counter. should not cost more than 5 or 6 euros, shipped.
anyways you need to flash CF-root through ODIN if you are coming from stock. instructions are all in chainfire's cf-root thread.
Nandroid Restore
So I successfully rooted, used CWM to do a Nandroid backup, mounted the sd_card and saved the contents (including the nandroid backup) to my PC, reflashed the stock rom & kernel, usb-jigged it to get the counter back to zero/original and sent the phone in for repair. My efforts paid off as they repaired it without question under warranty. I just received the phone back and today set about restoring my backup:
Copied the sd_card contents I'd saved to my HDD back onto the phone
Re-roooted it with Cf-mod.
Used CWM to restore the backup I took before sending it in
It restored boot/system/data/cache successfully
selected reboot now in CWM
The result is that it is still missing all my apps. It only seems to have the "data" like my camera shots and SMS messages, system settings like whether bluetooth is on or off, the volume level, Wifi networks etc. So something was restored. But there is no Google Account configured, the lock screen picture is the default not the one I had beforehand, and as I said, all the apps - the main reason I did this whole thing as my personal data is all backed up to the cloud on a regular basis anyway - have not been restored. Am I confused? I thought a nandroid backup took a complete snapshot of everything, apps included.
Finding out the list of Apps I had in order to reinstall them is not an issue - I have a list of everything I had installed saved on AppBrain. But if I reinstall the apps, all the settings (or progress in the games) within those freshly installed apps will be back to square one, not where I left them, right? Or should I reinstall the apps and then restore the data partition over the top again?
Are you guys using any type of screen protectors, cause if you are, that may be the issue s their have been reports that the screen becomes unresponsive with certain screen protectors when connected to a charger.
However, if you are not using a screen protector, then I would suggest getting the unit replaced as you should not have to do anything, root or otherwise, to get the touchscreen working on a touchscreen phone

Nope, no screen protector or any kind of case for that matter. Anyway, that isn't my issue anymore. The touch-sensitive unit was defective, the phone was repaired under warranty. My issue now is getting my apps & settings back. I have meanwhile read elsewhere that Nandroid doesn't back up the apps - pretty deceptive "backs up everything!" "it is like time travel" etc comments around here then.
Anyway, I reinstalled all the apps I had previously, and thought by reapplying the data partition from the nandroid backup it might then bring back the settings for the reinstalled apps. Instead all it did was remove all those apps again - now I am really confused. How come the restore DOESN'T bring back the apps I used to have, but DOES remove the ones I subsequently install. Does Nandroid touch the apps or doesn't it?
Touch screen not working
So i am having a very peculiar problem related to the GT-I9100 Touch Screen.
I bought the phone from Saudi Arabia and I live in Bangladesh at the moment. So for now I don't have warranty.
I bought it on 4th of this month. It had 2.3.3. So I updated it to 2.3.5. I did not use the phone much before that so I don't know if this problem was there before.
So here is the problem.
First time I noticed it when i pulled the phone from my pocket and could not unlock the pattern. First I thought I forgot the pattern. After sometime I noticed that i could not touch the buttons. I was dragging my finger over the points but there was no connecting line. I restarted the set. But it still did not work. After sometime I took the battery out. And after that it started normally and unlocked. And the touchscreen is totally fine.
I tested it using *#0*#. And every single point in the screen registers touch.
After that this happened couple of times. First couple of days I thought it was a pattern unlock problem. But i tried PIN and Password even using no unlock. It happened in every case.
So after couple of days it totally stopped happening.
But from yesterday it is happening again. And today it happened 3 times.
So today as I had no unlock protocol i tried tapping Home twice. As this brings on the voice command menu. And for the first time i was in the phone without touching the screen.And first i noticed the Menu and Back Soft keys works.And the touchscreen still does not work. And i noticed some thing weird. Always some thing in the screen is Selected. In Menu the + Icon is selected. In first screen the Map icon is selected. Also in Recent Apps the first App is selected. So after a lot of reboots it is back to normal again. Now i see something interesting. In the Battery Usage Graph i can see some really sharp drops. 3 times. Looks like when this happens phones charge takes a nose dive.
So here is the Gist.
Info related the Problem:
1. Usually happens when the phone is my pant pocket for sometime. It does not happen if it is sitting on the table.
2. Touchscreen gets totally disabled.
3. I have no idea why this happens.The exact reason behind it.
4. Battery Charge takes a nose dive when this happens. See the attached images.
5. I still don't know how to bring it back. Sometimes a simple reboot works. Sometimes I have to pull the battery. And sometimes it works on its own.
6. Every other function of the phone works. Like music playback using my UE 200vi music keys using PowerAMP, Siri(Android version), Call can be received using headset.

So this is the thing. My biggest problem right now is, I don't know why it happens and I don't know how to bring it back from this situation. If I knew I could make sure I don't get it again or I would just fix it every-time. As when this happens I cant make calls or anything which requires the touchscreen.
So guys help me if you can by giving ideas. As i will not go back to KSA in 6 months. So buying a $550 phone unlocked was a total waste. As i cant use it here.

since this evening, my Galaxy S2 (unbranded, unrooted, with stock firmware) was connecting fine with my PC in the "kies mode". I could use kies flawlessy, and I could regularly see my phone in my windows 7 (x86) explorer, and use explorer to read and write files from and to its memory.
Then, this evening, I got a notification, from kies, that an update (of kies) was available (2.0.2.11071.182.2), and I gave my consent to the updater. It did all its dirty job but after the installation of the new version, despite the phone connects via USB, every time I try to access the phone's memory from the PC, the connection drops. It restores backafer one second or two, but then again, if I try to access the phone's memory, it drops agains. The problem appears indifferently when accessing phone's memory from kies, and when doing it from windows' explorer, and this makes me suspect of the drivers, that the new version of kies brought along.
The phone connects correctly to the PC in the direct USB mode, and I can access its memory with no problem, and this also suggests that the problem should be the drivers.
So I uninstalled everything (new kies and drivers), and rebooted the PC, then I dowloaded what I believe is the previous version of kies: 2.0.1.11053.99.3, and launched its installation. After the end of the process, I rebooted the PC, just to be safe. According to the list of the installed software in my PC (in "Control Panel > Uninstall a program"), the drivers have rolled back, as expected, to version 1.3.2410.0 (before they were 1.4.something).
At the first connection of the phone to the PC after the reboot, I got notification from windows that the device installed correctly and it is ready to use (all the tree drivers are indeed reported as installed correctly)...
The point is that all this did not solve the problem at all, and USB connection still behaves exactly as badly as before.
I tried again uninstalling and reinstalling everything, also adding a manual clean up of the windows' registry from any entry related to "samsung", checking that the phone was not set in "USB debugging" mode, and disabling the antivirus, but despite everything appears to be fine, the phone regularly keeps on refusing to maintain USB connection alive everytime I try to access its memory.
What drives me mad is the fact that until this evening, everything was working flawlessly!
What else can I try????
I've just made a test on another PC running windows XP, with latest Kies 2.0.2.11071_128 installed. It's my office PC, and although I'm not 100% sure, I seem to remember that, until yesterday, my phone was connecting fine with it, and with that version of kies. But now, all of a sudden, I have the same problem also with this PC...
So, likely, it is a problem of the phone... the MTP application restarts everytime I try to access the phone from the PC, so... what can I try on the phone, to see if I can fix this annoying issue? I DON'T WANT TO FACTORY RESET MY PHONE, PLEASE!!!!
If any of you has any suggestion, I would be more than grateful...

I had the same problem the other day and I could not find a way to fix it, in the end I did a factory reset from Settings/Privacy and its ok now, just wiping cache did not fix it, factory reset was needed.

So... it appears that there's nothing else to do, is there? Buhahhaa!!! I want to cry!!!! I haven't made a backup and resetting the phone will be really painful... I'm already starting to feel the pain in my ass!!!
By chance, do you happen to know if I will have to buy again the apps I have already bought from the market? I have always kept both "Backup personal data" and "Automatic restore" ticked in "Settings>Privacy"... Could this be of any help?

You can always make a backup now before the reset and you will not have to buy paid apps again.
Yes but how can I backup my phone if kies will loose the connection with the phone as soon as it tries to access its memory?
Oh wait a moment, Perhaps I could use Kies air? Deos it allow to backup the phone? 'cause I can't test it now in my office, there's no wifi here...
From what I can tell, its the amount of the power that is supplied to the USB port itself that is the issue. I have the same problem when I put my i9100 on certain computers. Same thing happens to my WD MyPassport. Certain computers it works fine, and some it will power it for a bit, but once the device starts to draw too much current, it just disconnects.
This is why I try to do all my modding and flashing on a laptop, since, for me, they seem to be the most reliable in keeping devices powered up properly.
This is certainly a good point, but in my particular case, USB connection used to work up until yesterday with all the PCs I have (2 laptops and a desktop PC), whereas since yesterday, it works with none (with and without using the same USB ports). In addition if I use straight USB connection (hence not through the MTP app used by kies), it works fine.
Anyway what you have just mentioned is actually what scares me, because it would mean a hardware fault. But I'm crossing my fingers, hopefully it's (just) a software related issue and a factory reset will solve it. After all, straight connection works, so it would be really surprising if it were a hardware fault.
I will try resetting everything when I get home. Another "nice" busy evening to account to Samsung!
I guess if all samsung customers were honestly paid for every hour spent mending/fixing the issues of samsung products, the company would declare bankrupt in a matter of a few days!

I spent 8 hours backing up and restoring everything included all the settings and personalisations. I went to bed at 3 am, eating only a quick sandwich in front of the bloody phone.

I hate when something in a device screws up all of a sudden and noboby knows why. It could happen again any time, and again I would have to spend hours mending a stupid phone instead of enjoying my life.
By the way, does anybody knows exactly what data is automatically backed up and restored on google servers by these functions?

I always had both tiched, and I noticed that almost all the applications downloaded from the market that were installed on the phone, were automatically re-downloaded and restored on the phone at first boot after reset, but I'm not too shure I have understood what else was automatically restored.

This is a simple solution try it and it will work for you the next time you face it most of the time it is a hardware issue i have been facing the same issue and nothing helped neither the factory reset nor the *#7284# procedure.
Take a pointy object like a pin or a paper and then clean your usb slot of the phone properly cuz most of the times usb pin points gets connected with each other due to the atmosphere and metal reaction. After that it will work like a charm for you. Before commenting anything negative i would appreciate if those who are facing this issue for a long time try this and then comment
Had the same problem, solved it by upgrading the phone software.
Setting > About Phone > Software update.
No need to afraid losing files. Hope that helps.
i faced the same problem on the GalaxyS sometime back. i did factory reset too. I got the same problem again. but i remembered it working on my LG TV so i did not go for factory reset. I went for re-install of samsung's USB driver for Windows. now it is working fine.
I fully agree that Samsung needs to be more responsible and responsive in resolving the such problems without causing data losses.

You could have saved a lot of hassle by installing a custom Rom and Kernel. I hate MTP mode! Mass storage mode is much better, I never had any problems with that....
Workaround - Power Management
This may be fixed, for now. However, in the future, there is a small workaround that you can use so that you're not required to wipe your phone every time this happens.
Here's what I do:
Since the device's connection is severed after the use of a certain amount of power, we can control the power management of the port that the device is connected to.
On WinXP:
Enter "Control Panel" > "System"
Click on "Hardware"
Click on "Device Manager"
Find the "Universal Serial Bus Controllers" section
Click the "+" next to that, and find the "USB Root Hub" that the device is connected to.
Right-click on the Hub, then select "Properties"
Click on the "Power Management" tab
Then, UNcheck the box that says "Allow the computer to turn off this device to save power."
This will prevent the connection from resetting. It's a quick workaround that works pretty well for me.
However, I ought to note that the changes should be undone after you're done with syncing the device, ESPECIALLY if you're on a laptop. Otherwise, you could risk a lot of battery/power usage within a short time, even if there is no device connected to that port.

Please don't bail out because of that comment, i have been having this issue for a month and AT&T almost sold me another phone telling me it was the USB PORT.
I changed my phone setup dialing *#7284# and changed the UART to : PDA from Modem and same for USB (to PDA) modes. This helped get connected as before it would not even recognize the phone (computer would not do so).
I was am not sure if it is not the battery or charging circuitry, as others also reported, but my charging circuit is working fine and my battery charges 100% and last all day as normal. I can connect to the USB port and see my phone for a short while but it will suddenly disconnect.
But same as before it suddenly disconnects and will reconnect cycling continuously. I also believe it is the charge circuit because when it is charging, i hear the phone constanlty beeping from turning the schreen on/off. Same as when it is plugged into the usb port on computer.
Am tempted to buy a battery, but is can run the phone on the USB port without a battery in the phone. Problem continues without a battery...disconnecting and connecting to the computer.

I was plagued with this same problem, until I tried the above fix, and it works brilliantly! No more disconnects, great stuff.
It's pretty old topic but maybe someone has same issue and doesn't want to loose data. I had similar problem (couldn't connect to device, also couldn't install/uninstall any stuff because of no-space-available error).
I managed to download all pictures/documents via adb - worked pretty well. A sample command to retrieve all camera pictures:
adb pull /mnt/sdcard/DCIM
Currently on stock, non-rooted rom. Just restored from my smart switch backup but something was changing my ringer mode (from sound to vibrate) at random times. So, I downloaded NiceCatch in order to identify what process is doing it. When it occured, I checked NiceCatch and it says it was the AS.AudioService.onSetStreamVolume process that set ringer mode to vibrate then back to sound. Sometimes it sticks to vibrate only and I often miss calls. Any ideas how to stop this? I've already checked Bixby Routines, DND, Easy Mute, and SmartThings. They're all turned off and their not scheduled to turn on. This was not happening before the smart switch backup (I've reset the phone because I was having touch issues, turns out it was just the screen protector.)
Try clearing the system cache.
SmartSwitch can transfer the problems along to the new load. Don't use it if you can't ID the issue.
Never trust SmartSwitch as a stand alone backup for critical data as it can fail you miserably.
Done clearing the cache partition in recovery. I've also disabled all DND permissions from all apps in the settings. Let's see if it still happens. I'll post the screenshot here again from nicecatch if it still happens. If this still keeps happening, I guess I have no choice but to start from scratch. What a pain.
Yeah, I've learned not to trust smart switch ever again. Quite sad, in my opinion, it was actually better than google's backup and restore. Can you recommend some backup and restore apps? Best one I've used is Nandroid backup from TWRP. But it is only for phones with custom recovery though.

Helium but it's sort of a pain. Never did a restore with it.
I just make redundant backups and load 'em the hard way. I used SmartSwitch to transfer homepage settings on my N10+'s although I accidentally included apps. I'm using Android 9 on one and the newest is running on 10.
SmartSwitch worked well... who knows?

As the name suggests, this is part of the Android Audio Service and it is a perfectly legitimate part of the OS. Unfortunately, the cause is going to be nigh on impossible to find out without a stack trace as the service is being called by something else and NiceCatch won't tell you that. onSetStreamVolume is a function that is called when something is adjusting the volume, be it an app or some hardware interaction. An audio/video app or another part of the OS can make this call. Hardware could be the volume buttons, inserting or removing headphones (or a faulty headphone socket thats mimicking insertion or removal) or a third party device connected to the phone such as Bluetooth headphones or a smart watch. Try disconnecting any Bluetooth headphones and smart watches and see if it happens without them connected. Disable any related apps for those devices too and see if it still occurs (update them if available). Since you have NiceCatch you must have Good Lock and that offers you SoundAssistant. If that's installed, try disabling and updating it and seeing what happens. Disable/update any audio/video apps and see if it occurs. My best guess is that because used Smart Switch, you may have restored an app thats no longer compatible thats making erroneous calls to the function. Thats all I can think of right now. It seems you have a lot of trial and error to do!

3rd party apps can alter hidden user settings on stock phones that do not revert back even if you uninstall the app. This is rare but it happens. Saw a Google book reader app do this. Had to reinstall it, change the display setting, then finally uninstall it for good. With some patience and persistence many times you can track a hidden settings issue down and avoid a reload.
I prefer a doing clean load especially if malware caused the need for a reset. A Android load can lasts for years if you load it right and leave it be.
It's worth doing it right the first time...

Smart Switch isn't as smart as its title suggests. For about 80% of the apps I have installed, it only backs up the APK for the app but not the app data itself. Yes it will install all my apps on mass but I still have to set each app up individually afterwards. That makes it a useless solution. I might as well install them all fresh from the Play Store so that they are up to date too. There are some third party apps that get data backed up along with all the Samsung apps but I find virtually all games reset unless the game itself saves a cloud backup. I've never had any apps be faulty after a restore but I've only done it twice personally and I've heard plenty of horror stories from others to know its bad.
As for OP, it could be a strange setting on a smartwatch that triggers it. I can silent my phone from my watch and if it developed a bug then it could cause a situation like OPs. I'm still betting on it being a dodgy Smart Switch restore but it will take days for OP to pinpoint what actually caused it.

Ahh okay. This will be tough to trace then. I wonder if this can be traced via system tracing in developer's option. It did include system tracing for audio. I don't know what program does it need though in order to read the logs.

Also, I did have my screen replaced recently. I did it myself, it was a reglassed screen from China (Screen with frame). Works perfectly fine though. I tested all the functions on Samsung's testing menu thing (*#0#*). All functions seem to be working fine. Except for the Grip Sensor. It doesn't seem to work correctly. It used to say "Release" when the phone was not being held and "Working" when the phone was held. Not really sure what this Grip sensor does. While being inside the phone, I also replaced the thermal pad with Thermal Grizzly's Kryonaut (Non conductive thermal paste). Though I doubt this would cause this to happen. It improved thermals indeed compared to stock.
Anyways, after the screen replacement (before the backup and restore in smart switch), I had no issues with the ringer mode. It only started occuring after the restore on smart switch.
As for the bluetooth, I use the Xiaomi Mi Band 6. Including its Mi Fit app. I tried uninstalling the app to see if the issue still occurs. It still does. So I guess it wasn't due to this app.

Yes, I have disabled several apps to test if the issue was gone. I also tried updating them to their newest version. Unfortunately, it still occurs from time to time. I also have SoundAssistant installed. But even though I cleared its data and cache, the problems still continues. I guess it wasn't due to this either.

Oh okay. But upon backup and restore, I never updated the phone though (November 2021 Security Patch). So I doubt there is some compatibility problems. Maybe smart switch is just buggy. I also did some research about this issue and it turns out, it also occurs to other samsung models (S21). This seems to be a bug from smart switch that samsung hasn't fixed yet. I guess if all else fails, I have no choice but to completely start from scratch. It'll surely be a pain though because I use a lot of apps.

I've used system tracing but not for the audio component so I'm not exactly sure what its output would be. The system traces can be viewed through Android Studio and can be very technical. If your used to debugging kernel logs you will be at home as they can be just as cryptic!
One wild thing that I'm thinking now is that the headphone socket got damaged when you replaced your screen and its triggering as if you plugged in your headphones. I'm not sure how that would result in your phone going in to vibrate but it would certainly trigger activity within the Audio Service as it contains code to check the volume limiter which is specifically for headphones. Its a possibility to consider but a wild one at that.
The grip sensor is a sensor that notifies anything that needs to know when your holding your phone. Its one of the things Samsung Health uses to guess when your asleep or not. I dont know how Sammy actually do it but I'm guessing its going to be from slight current changes on the edge of the digitizer as you grip your phone but thats a wild guess too! As for the test, the release message is a message to tell you that you need to release it not to say your currently holding it. Its a bit strange to us because you think it would say gripped or ungripped or something but its messages for a human or robotic tester, not for us.

Unfortunately Smart Switch is just not reliable as a backup solution. Use it to backup you content and settings but don't rely on it for backing up your apps. Headphones are completely working though. Both left and right, including the volume up, down, play/pause controls on the earphones. Headphone detection is working fine. If there was a problem with the headphone jack, there would be no sound coming out from my phone speakers or atleast it would be intermittent. But thats not what really happens on my phone.

The grip sensor is located on the frame of the phone. It has a touch sensor chip located on the charging board module. It makes contact with contacts on the frame. I first thought it was for the wifi/cellular antennas, but then I noticed that antenna cable traces on the charging board PCB led to a signal amplifier chip which then leads to contact pins connecting to the speaker/wireless charging coil plastic piece (which probably serves as the antenna). My grip sensor is probably not working because the pin was not making contact with the frame part. Probably 'cause of corossion or some dirt. Maybe I need to scratch up the part where the pin connector touches the frame. I'll try to get this fixed soon if I ever need to open up the phone again. I kinda already sealed it up with a new water resistant seal. Its gonna be a pain to remove and replace again just for the grip sensor.

Yeah. I decided I'm gonna start again from scratch. Instead of doing all the trouble finding out whats causing this problem. It went bonkers again last night and early morning. I give up. I decided to reset the phone and reinstall all of the apps. Its going to take a while for sure. I would still back up my current data using smart switch though. Just in case it still occurs even after a full factory reset.
Be aware the issue may return if you don't find the root cause. It's likely a 3rd party app or a settings issue. It's always better to find the root cause. Malware is also a possibility in which case a reload will kill it.
You can use ApkExport to make installable app copies in the future, no need for Playstore then on reloads. Available on Playstore.

Already done factory resetting the phone. And freshly installed all the same apps from the play store (also individually set them up). I also only used smart switch to restore my contacts and messages. Did not use it to restore settings and apps. All good so far. No more annoying ringer mode switching. So that confirms it then, it was indeed a bug in smart switch.

Lol, SmartSwitch is a Pandora's box of trouble.
I would also use the backup feature in Contacts to create an export backup file. This is important to have.
Its back again. Huhu.
Seems to occur less frequently now though. Tried doing setting up some automation on sound assistant to see if it was the one thats changing the ringer mode. But its process can clearly be seen by Nicecatch. This is really starting to bug me. I've already done a factory reset and reinstalled all my apps fresh from the play store.
I've tried plugging a headset with volume control and tried changing the ringer volume over there. It still says audioservice but the process is different. Its the checkforRingerModeChange process. Not the onSetStreamVolume.
Update: I've decided to open the phone again and fix the grip sensor. Grip sensor probe is marked Red. It connects to the lower portion of the aluminum frame. Those "antenna bands" on the aluminum frame are not actually antenna bands. They are plastic molded spacers to separate a piece of aluminum (sky blue) from the side aluminum frame. The lower piece of aluminum is then connected to the probes of the grip sensor IC (located on charging board and as well as on the top of the motherboard). There are also two grip sensors. One at the top and one at the bottom. But on the test menu, only the test for the bottom one is present. (Newer models have test for top and bottom grip sensors). When I touch the bottom probe, grip sensor test becomes "working". Therefore there is no problem on grip sensor circuits. Problem is on brand new frame I installed, I had to scratch the part where the probe makes contact on the frame and viola, grip sensor works now (I also scratched the top grip sensor probe contact). Yellow marked ones are wifi/cellular/gps antennas. They're present as well at the top.
I wonder if the grip sensor was the one triggering AudioService.onSetStreamVolume. I'll just wait and see if it still occurs after this.
2nd update: Its still here.
3rd update: Updated to March 2022 update. Issue completely gone.
