Ok I've had this problem for the last couple days. I will be in the middle of listening to music through Winamp over my bluetooth headset.
All of the sudden the bluetooth headset stops playing music and the music starts playing out of the speaker. However, it is not the same music...it is a completely different song. If i use the volume rocker it shows it adjusting bluetooth volume but does not change the random speaker music volume.
If I go into settings>Applications>Running Services and Force Stop all Music programs the music still does not stop playing out of the speaker.
If I turn the bluetooth headset off and then use the volume rocker it shows adjusting ringtone volume, but the random speaker music will be adjusted.
Also, while I was not even using bluetooth the other day this same thing happened. It seems to only happen when the screen is turned off.
I am really trying to figure out what is causing this but I cannot figure it out.
I have frozen my "music 2.3.4."; "Music 3.0.1" ; and "Phantom Music Control Widget" and the problem persists.
I don't think it is related but this did not start happening until I installed the Faux123 1.9 kernel. I may also try to revert back to 1.7 to see if the issue persists.
I am still trying to figure this out and will probably try another music program other than winamp to see if the problem still happens.
Any insight would be much appreciated.

I had something similar happening a while back. What it turned out to be was that the widget for another music program was still active somehow after deleting it. Freezing the program didn't stop things. I eventually realized what was happening, dropped another widget on my desktop again, made sure all music was stopped, then deleted it again.
Also check your settings in all music programs for bluetooth and playback. I have found that having multiple music programs installed would really cause a sh*tmess sometimes. Better to uninstall everything but stock music player, then only install the one you wish to use, then freeze the stock player after turning off all "auto" functions in it.

So I recently got a 2011 Subaru Impreza and it came with built in bluetooth and music controls on the steering wheel.
The first time I pair my thunderbolt with the car everything works fantastically.
I can hear my music and I can control songs with the steering wheel buttons.
After I leave the car or turn off/on the bluetooth, the music will still play through the speakers, but I lose the steering wheel controls.
The only way I have found to fix this is to completely un-pair and re-pair the device.
Has anyone else had this issue, and does anyone have any idea how I can fix it?
I only have 1 media player installed (winamp, and well the default music app) so I don't think its a problem with handing off bluetooth control (again, winamp plays the music fine, the steering wheel controls just stop working).
I have tried this with Songbird and PowerAmp (uninstalling the others while the other is installed) and the issue still persists.
I am on OMFGB 1.30 nightlies 7-20 with imoseons most recent lean kernel
Never used OMFGB specifically but I've definitely had times I had to un-pair/pair bluetooth housephones.
In your case though, first question I would ask is if your device is still playing music @ the time the connection is broken. If so, try to stop playing and close the music app before leaving the car or turning off bluetooth. Perhaps it's screwing up because of this sync loss while the app is running. Just a guess for you to check out.

I am thinking it might be an OMFGB issue.
I don't recall if it happened on bamf 1.8 or not.

I recently purchased some Nokia BH-505 bluetooth headphones. I like them and I primarily use Pandora to listen to music. I'm having a periodic issue where the headphones sort of disconnect and quit playing music. This only happens when I choose to turn the screen off. If I keep the screen on, I never have any disconnect issue. Pandora is happily still streaming music though, and if I power off the headphones, the phone/pandora realizes that it's no longer connected and starts playing through the speaker of the phone (that's why I say "sort of" disconnecting). If I power the headphones on again, they connect, pandora pauses, then I can resume play and then a short time after the condition happens again (if I turn the screen off). I imagine it's some sort of memory related thing because it seems if I make sure I have no programs running in the background things seem to work as desired. I haven't found any sort of setting to tweak, and neither Pandora or any bluetooth like program is in an autoclose list.
Anyone have any ideas on any workarounds to this issue?

Before custom ROMS were built on ICS, when I was listening to streaming music in my car (via bluetooth), the phone would automatically stop playing when the car turned off. Same thing if I'm listening to music and I pull the headphones out, whatever was playing gets paused.
Now, when I turn my car off, the music doesn't stop playing, it just starts playing through the phone's speaker.
So my question is, is it the ROM that makes a player pause on disconnect or is it the actual APP that decides to either stop or keep playing through the speaker.
I know PowerAMP has options to auto play and auto stop on disconnect, but other apps like youtube, Audiostation (for my synology NAS) and a few others don't have settings.

I think the fact that some apps have the option indicates that at very least, the app does have some control over it. I don't know if the ROM/kernel have some default setting built in for apps that don't define it.
It seems to be a feature missing in ICS. I'm hoping it shows up by the final update.
I use Tasker to accomplish the same thing. The whole task for media is when I plug headphones in, it launches a menu with media apps (music, netflix, etc) and turns the media volume up to 6. When disconnected, it mutes media volume, pauses media, and returns to the homescreen.
I'll take a look at that in the interim, all I really need is it just to pause on BT disconnect like it use to. The is already a separate volume setting for BT vs the headphone jack. Thanks for the tip.
That's interesting, I didn't know tasker had something to pause media. I'll definitely give that a try.
I believe that was an AOSP tweak..my Inc running CM7 would pause on bluetooth disconnect.

So I got the Kate version of this phone for a couple of months and I really like it. But today, it stopped playing sound from the back speaker when using media apps like Spotify and Youtube. If I plug in my headphones, it works just fine though.
Here is the fun part: If I start a song on spotify, go into settings>sound and change the volume for the notification with the slider, the sound of the notification is played from the speaker as the volume is adjusted. When the sound has played, it resumes playing the sound from spotify. But when the song ends (and another one begins) or I skip to the next song, the sound is gone again. As I am running custom rom(Aosp Extended), I'd thought it was just some weird bug. I did a complete reflash of another rom, Pure Nexus, but I still have the same problem. What could be causing this!? I have never experienced this with any other phone. At first I thought the speaker was broken, but it does seem to work. I still get notification and ringtone sound if I get a text or somebody calls me.

Does anyone noticed that the AI makes phone to act unexpected?
For example my bt earbuds, speaker gets connected/disconnected for no reason sometimes .. for example I go to smoke at work, bt speaker stays at workplace, coming back, connecting my phone to speaker, starting to work and suddenly after some time music just stops. I see I'm connected to a speaker in Bluetooth settings, but once I start music again, music is playing thru the phone speaker instead. And I have to reconnect to a speaker again.. and it continues like that till I reboot my phone..
If I plug in fabric earphones which comes with phone, sometimes automatically music is started, sometimes after couple minutes, sometimes never.
Call recording apps/and overall all apps are killed without a reason even excluded from all battery optimations/savers.
Also never knew that I have so many different songs after trying different music player, have anyone noticed that also?
In my case I have about 200 songs in my library, but with default music player, it seemed that I have like 20songs only.. always listening on shuffle mode.
