Ok so I got this phone the other day. It was rooted but I dont know about a custom ROM. It has Clockwork v3.1.0.2 on it and OS ver 1.12.605.6.
I did a factory wipe on the phone, obviously a mistake, and now it's locked on the HTC boot screen. It doesnt reboot over and over, it's simply locked. I tried to install PG05IMG_Mecha_VERIZON_WWE_1.12.605.6_Radio_1.16.00.0223r_NV_8k_1.41_9k_1.64_release_174685_signed.zip on it by renaming it PG05IMG but it comes up no image error. I checked and the SD card is mounted. Any ideas on what I can do to recover?
Try the 1.13.605.7 RUU (MR1-first OTA) or the 1.70.605.0 RUU.(MR2-second OTA) Making sure you download the .zip not .exe version. Rename PG05IMG.zip like you did before. Download to you computer first and transfer to the sdcard or at least make sure you are downloading over WiFi if you use your phone.

Still came up as no or wrong image. It see's the file in the recovery menu but says it's (bad). Im not sure why it's not loading. Unless its that I loaded it using a SD card that was in an eris to get the zip on the card...

Because those are meant to be flashed from bootloader not recovery.

using the PG05IMG.zip file you want
You dont need to rename rooted rom zips. You only rename radio zips (or certain files that devs tell you to) to the PG50IMG.zip format.
In recovery flash the rom zips with whatever name it has (do not flash .EXE files as stated above)
In Bootloader you flash the radios or PG50IMG.zip files
Thank you very much for the answers. I believe you do have to rename the RUU .zips and flash like a radio as they are signed ROMS, as opposed to rooted. I did try to flash in bootloader first, but when that failed I rebooted in recovery to see if the phone was reading the file by accessing the load from ZIP. It did see the file but failed to load in bootloader.
I also double checked to see if the SD card was mounted, which it was. Im at a loss.....this thing should load. Could the bootloader be damaged or corrupt??

I just rooted using a guide for the revolutionary method. It says the .exe is supposed to ask if you want to install clockwork after it obtains s-off & it never asked me. I do have s-off but no recovery and no superuser as well. I searched this problem and found answers. I found a zip & renamed it to PG86IMG.zip on the root of my sd or whatever & tried to install it but i don't think it worked. I've tried to go into recovery & all I get is an image of a phone & a red triangle. I have a clockwork img now but I have no idea how to install it & i tried to go to my sdk but all my laptop says it has is sdk manager & it won't let me enter any commands. I've been reading for hours & I'm stuck. Any help would be appreciated if anyone knows & thanks
Download this file and rename it PG86IMG.zip and place on the root of your sdcard. Its twrp recovery, which I prefer to cwm

Now turn off phone and boot into the hboot and wait for it to check for update files and follow instructions to install. I think its up for yes and down for no, and wait for it to install. Then reboot and delete the file from your SD card. You should be able to boot into recovery now. Then flash this fixed su found in the twrp recovery thread.

I renamed it and everything & put it on the root of my sd but when I get into hboot & it loads it says no image
Make sure you don't have the file named as PG86IMG.zip.zip
If you have file extensions turned off in Windows you may have done that by accident. If you have file extensions hidden just name is PG86IMG without the .zip part.
You're also probably trying to flash a recovery on an HBOOT v1.5 phone; I've had no luck flashing it with the aformentioned .zip method try using fastboot to flash the recovery:

fastboot flash recovery [whatever the name of the file is].img

This was actually the issue. I put the .zip in there when I really just should've put PG86IMG so it wouldn't have .zip twice. Thanks for the help guys

Wiped my ROM from my phone and was trying to flash SuperLTE. However, it wouldn't flash because I had an outdated version of TWRP. So I downloaded updated version of TWRP and flashed via bootloader. This means that the PJ75IMG.zip file is in my phone's storage and I get prompted for it each time I boot to the bootloader. However, I do not want to flash it again so I say "No" when it asks me to flash the file. After I say no it then prompts me to reboot the phone. Since there is no ROM on the phone it automatically goes into the bootloader. It detects the PJ75IMG.zip file again, asks to flash, I say no, it prompts to reboot again. And I do that over and over and over again. I can't get into recovery because I cannot enter the bootloader section that allows me to select recovery. I also cannot remove the PJ75IMG.zip files from my phone as there is no opportunity to mount/unmount storage in order for me to remove them.
Any advice?
Edit: I did a search for this issue but didn't see anything that matched exactly what I was dealing with. Thus the reason I created this thread.
Edit 2: Nevermind! I got it working. I opened the back and removed the SD card. I am not sure why it worked since I had the PJ75IMG.zip was on both my SD card as well as my internal storage. Regardless, after removing my SD card I was able to get into my bootloader and select recovery. From there I am now able to flash a ROM. Mods can either delete this thread or leave up for anyone else that has this issue. Thanks!
AFAIK the bootloader only reads from the ext sd
edit.... so I somehow miraculously now have it working again.... installed amon-ra recovery, then unsuccessfully flashed Neo, but was able to get Ecliptic Rez to work... can't seem to delete this thread though...

So I was running the most recent RUU re-locked, S-on.
I went through the unlock process successfully, then got TWRP on my phone (not even sure how this worked... was using the Rezound all-in-one tool, it didn't seem to work properly the first time, but was able to pull up TWRP through Recover later on) then used TWRP to flash SuperSU and was rooted. Then feeling cocky I went ahead and did a clean wipe (note: I was very dumb and never setup a Restore... I thought, oh everything is going great, I'll just flash real quick, no reason to create a backup.... lesson learned haha) and tried to flash Ecliptic Rez. The first time it didn't work... it almost instantly said it was "installed" but didn't work. I tried to Flash it again, this time it looked good, took a while to install, everything looked great. I rebooted and the EclipticRez splash screen came up.... but then it stayed up, and it's still up. I can go back to recovery/hboot/whatever.
If it matters I pretty much flashed Ecliptic with all the options as default (except I chose S-on)...
I also tried just now to flash NeoMax2.0 but this is stuck at the splash screen as well. I tried to flash this rom first by having it as PM98IMG.zip on my root, but it didn't seem to install properly (it went through an initial phase on hboot for about a minute then went back to the hboot screen. So I then flashed it using TWRP, it said it was "injected successfully" (or something like that) but then when I rebooted it went to the splash screen and is stuck there...
If it matters too, my phone is "unlocked" and "tampered" on hboot.
My questions are...
Where should I go from here?
I really don't wanna mess with S-OFF..
I'm trying to learn more about amon-ra recovery but it's gonna take a while for me to digest all that info.
Could it be a TWRP issue in some way?
Any help/ideas would be greatly appreciated, thanks!
For future reference, since you are s-on (as am I) you need to flash the kernel after the ROM.
Some ROMs you have to pull the kernel out and do it in fastboot, or some, like Ecliptic, will put a PH98IMG on your sd card and put you in hboot after the ROM flashes. Then you accept the update if it doesn't start automatically and it will flash the kernel and then you should be able to boot.
You shouldn't need to rename the ROM to PH98IMG, just select flash zip in recovery and navigate to it.

That's dead on! Thank you. That was the difference I noticed. I'm not sure why, but amon-ra after flashing EclipticRez sent me back to hboot and there I realized that I was supposed to do another step, and at that point I thought, hey this might work this time lol
Out of curiosity.... so you say some ROMs have you "pull the kernel out" - what exactly does that mean? Would I have to download (or perhaps just located the kernel in the SD card) and change that to PH98IMG and put it on SD Root so it loads that update in hboot? Thanks
Since I download everything to my pc then transfer to my phone, what I do is just look in the ROM zip file in explorer and then copy the boot.img out of it into my fastboot folder. Then I just flash it from there using:
fastboot flash boot boot.img
There's probably a way to get it out using Root Explorer or ES File Explorer, but I don't know how you would put it into a PH98IMG file on the phone.

Ok cool, so you'd flash your ROM first using the command prompt, then after the ROM has finished installing and you're back in hboot you would then flash the boot.img (aka Kernel right?)?
Flash the ROM in recovery - on the phone, not fastboot. I just download to my pc and store everything there then copy what I need to my phone when I want. So when I flash a ROM I copy it to my sd card to use in my phone.
Then you may or may not have to flash the kernel in fastboot. Only need to use fastboot if the ROM doesn't make a PH98IMG file for you and put it on your sd card. If it does make one I believe it will put you in bootloader and ask you if you want to update and you just select yes with no need to use pc.
i downloaded [ROM][11.18.2012] Newts OnE XxX RLS 6.0.2 *Global*[Sense 4.1], pulled the boot.img out of the zip and put it in my folder with fastboot, put them on my sd card (although i may not have put the boot.img on my phone), flashed cleanwipe.zip (or w/e it's called), flashed rom, tried to flash boot.img from my computer but after putting in the fastboot flash boot boot.img command it just sat there saying "waiting for device." i couldn't get back to the bootloader from clockword mod recovery so i pulled/replaced the battery and power+vol down, bootloader came up but before i could do anything i get this:

if i go up it says it updates, and even if i decline it doesn't go back to bootloader, it just tells me to hit power to resart the phone. so i restart, and get stuck in a bootloop of the new mikroms loading animation. i can't seem to get to the bootloader to load recovery. i'm not sure what i did wrong, and i hope someone can please help?
You have something on your SD Card that the bootloader is trying to load, you're gonna need to put the SD Card in a reader and delete it first or boot the phone up with the SD Card out so you can at least fastboot the kernel in.

i'm not sure how to do that. i thought i knew how to use fastboot from my computer but i can't even remember where i learned how to do it. but you were right, i pulled out the sd card and it went to bootloader. i just don't know what to do from here, i guess i need a ph98img.zip to put on my internal storage?
1.You need to delete the zip on the external SD card.
2.Wipe your phone(like you normally would when flashing a rom )
3.flash the ROM
4.flash s-on stock zip
5.flash the flashable boot image zip you can use to flash the boot.IMG for the ROM.
Head over to the mikmik you can find everything you need their under newts section.

Put the boot.img on your PC, boot to bootloader/fastboot, run in the command prompt - "fastboot flash boot boot.img" without the quotes. If boot.img is not in the same folder as fastboot.exe, add to path to it, ex. fastboot flash boot c:\path\to\boot.img.

Your fine, did you just wipe everything or re-did the ruu?
If your did the ruu than you need to get your recovery back in your phone
If you just wiped everything in recovery than you need to download the ROM you want to use than install the zip in recovery but don't boot into it yet. Pull the battery and boot into bootloader and go into fastboot. Than on your PC take the boot.img file out of the ROM zip you just installed and flash it over ADB.
Fastboot flash boot boot.img

You need to have a locked bootloader when flashing the ruu,
After downloading the file, rename it to "PH98IMG.zip" (no quotes)
Place the renamed file onto the root of the SD Card
Than boot into bootloader and it should load it up and install it, make sure to just leave it be. The phone might turn off than back on by itself once during the installation and that's normal, it'll continue installing and it will let you know when its finished

Add a rom zip file to your external sd card via your computer. And install it via yiur custom recovery. If for some reason you don't have a custom recovery, use Odin to go back to stock.
