Troubleshoot that a couple of ways. First, go into the market, and download rootchecker by joeykrim. That should tell you if you somehow lost root. If you did, root again, I actually prefer photon torpedo method found in these forums.
If you still have root, go the market and download superuser. If you are showing that you still have root, and that superuser is installed, then you should be good, if not, then I would re-root with photon anyway.

I too lost root when I went back to 154....got the OTA to 198_6 then I re-rooted with AIO...all is good now

[Q] updated while rooted

I have a Samsung Fascinate, through Verizon, that I rooted a while back using Dirkk's EZ root v0.2 (Sebastien Krahmers Exploit). I made a mistake a couple of days ago, and took the OTA while rooted. Now, I still have the superuser app, but nothing (setcpu, titanium back-up, etc...) can gain root access. I have tried to reroot the phone, but this does nothing. Does anyone have any advice for me? I have no idea what to do from here. Thanks in advance.
Try super one click: http://forum.xda-developers.com/showthread.php?t=803682
That always works for me.
Thank you very much for the timely reply. I ran the SuperOneClick. It says that I am successfully rooted, but I still don't have root access through Titanium Back-up, SetSPU, Superuser, etc. Any other ideas? Am I doing something wrong (other than updating while still rooted)?

Just saying...have you rebooted yet?

I have rebooted several times now. I ran the unroot, and rerooted a couple of times now. Still no luck. I really do appreciate the advice so far, but nothing seems to be working. When I unroot, it does remove superuser, when I reroot, superuser is back, but still, nothing has root access. I reboot after every change.

No root access. Need help

Ok I used the directions posted on this site to sbf back to froyo which I did I then did bootstrap and gingerbreak. I got the error message at the end. I do have the super user app and I did download barnacle and root check. Both say that I do not have root access. I purchased the tbh app and have it on my phone but not sure which app to use inside the tbh app. Im currently running gb but its not the ota update. This is on a Dx2
Should I sbf back to froyo and start over or will the blackhat app save me from doing that. Just trying to get this thing rooted and appreciate the help.

[Q] Having Unusual problems with ISC root. Help needed.

I have problems with rooting or unrooting my Maxxed Droid Razr.
I had leak ICS rooted. I downgraded to GB(rooted) and sideloaded official .211. Tried to restore root with OTA RootKeeper. My phone showed SU icon and asked permission to run OTA Rootkeeper I did allow the permission everything looked good. I tried to check the root root checker but it does not show that my phone is rooted.
I tried DroidRazrUtility 1.6 to root it again but it fails all the time saying that files already exists. Then I tried to unroot but it said that my phone does not have root.
I tried to get back to GB but it did not work. Just stuck on boot loop. Sideloaded the ICS .211 and SU icon was gone and no root at all. I did couple master resets with deleting all the internal and sd storage. Still no luck.
Now I can not root my phone. Any ideas how to get the root back.
Appreciation "Thank you" will be given to anyone who tries to help.

Download unstuck ics 211. Flash the .85 kernel then root using matts utility 1.6 then use unstuck again to flash the .211 kernel back to the phone. There's a tutorial for it online in better detail

Superuser installed but I have no root access?

Let me start off by saying I'm a total noob. I have rooted my device before, but that was so long ago I forgot everything. Anyway I don't remember how my tablet got this way because I haven't used it in a while, but I have superuser installed yet I have no root access. I opened up the Voodoo Rootkeeper and it says super user app is installed, device is rooted, but root permission hasn't been granted.
So I tried to root again on top of it hoping it would work but now I just have another superuser app. Somebody please help me get root access.

Use motochopper to root it properly.

Thats what I used this last time when I tried to root it. It installed the second superuser app and when I opened it, it said that I needed to update binary or something like that. But I still had no root access after using motochopper.

I had the same issue after the last OTA. Root was lost and OTA rootkeeper didn't work.
This worked for me: http://forum.xda-developers.com/showthread.php?t=2038464
After successfully unlocking and installing TWRP and root, I went on to install this (but you can stop here if you don't want a different ROM): http://forum.xda-developers.com/showthread.php?t=1674827
I'm delighted by this ROM. Much better than stock.

I did a lot of searching, and tried several methods, but I couldn't find a solution which worked. Is your Prime still under warranty? If not, there's no reason not to unlock.

No its not. I just have no reason to unlock. I dont care about custom ROMs and I would like to be able to return it to stock if I want to later.

[Q] Rooting NC5, Towelroot Fails.

As the title states, I'm trying to root my phone using towelroot, yet everytime that it runs all it does is reboot my phone which according to the splash screen for towelroot, says that it's unsuccessful. How do I go about correcting this?

Have you downloaded SuperSU yet? I had the same issue until I did. I think I remember that was the solution.
Here are the complete instructions for rooting with Towelroot, not much to it.

Install SuperSU and busybox onto your phone before you try towel root, these will install but not set up. make sure you have usb debugging turned on, then try it again. SuperSu will bond with towel root and then ask to turn off knox...knox is trying to block it. You need SuperSu and then busybox running. if you get it rooted you'll be able to update SuperSu and set up busy box...maybe rom tool box, lucky patcher blackmart alpha, and rom manager after you get rooted too.
I had an instance where towel root said it worked and apps were saying I still needed full root access, so I used kingo android root to finish over it.
you can also try kingo android root (your decision and you own risk) it worked for me most times without any crap installed. even the chinese version rooted my phone but it puts its own program and SU, but if you already have SuperSU and busybox installed, the SuoerSu will uninstall those other SU apps and still have root. (your decision and you own risk).
Idk what those programs are that the post above me refer to, but all I did is download selinux mode changer and set to permissive, then use towelroot. See if that helps any

@ryant35: Thanks for the insight, I had it downloaded, and I was following that guide - it didn't mention installing SuperSU until the device is rooted ( and SafeStrap installed )
@Neontc: SELinux Mode changer depends upon the device already being rooted from what I read on the download page. I'll give it a shot though.
If anyone knows how to fool TowelRoot into thinking that it's online, I'd be very happy to hear that news. I'm going to be out to sea without internet for my phone for about a month, and I'm still actively trying to root my phone...
Thanks again for the quick responses, and I'll try to update when I can as far as what's happened and how I fixxed it!
Maybe I am wrong more than likely I am but I was under the impression that busybox was included somehow with the towel root install. I went with the towel root first then came back with the su after the root was installed after I checked to make sure the root took using root checker from the play store. Jesse
