Am I missing something? I can't figure out how to enter the bootloader from a powered-off state with the typical "hold volume down button while pressing power button" method. I have been able to enter the bootloader using random button-mashing combinations, but have not figured out how to reliably enter bootloader. What's the trick?
Did you try unchecking the fastboot option under the Power settings before using Vol Down and Power simultaneously?
Settings > Power > Fastboot (Uncheck this option) > Power Off
Hold Vol Down and press Power to get to the Bootloader screen.
Worked for me.

My phone can not boot normally, can not enter into recovery mode and just boot loop, so I can not disable fastboot from the system option of android.
Is there any way to disable fastboot? I heard pull battery will disable fastboot, is that true? If not, how to disable fastboot?

You can disable Fastboot in Settings > Power > Fastboot (it's at the bottom of the options).
If you have fastboot enabled, you cannot simply power down and power on again while holding volume down button to enter recovery, since the phone is not completely shut down.
By doing a battery pull, you shut the phone off completely, and thus you can enter recovery.
I'd suggest simply downloading a hot reboot app, from which you can simply press "Reboot into recovery" and that'll save you the hassle of disabling fastboot or doing a battery pull each time.
Thanks Moonbloom! I have got it, in my sitiation, pull battery is the only way to go into fastboot mode.
Well actually you can power down and reboot into your recovery with fastboot enabled. I do it all the time. Just power the phone down then hold volume down and tap the power button, then keep the volume button pressed down until you see the white screen that gives you your options.

now stuck in GERMAN. I have tried to hard reset by useing volume buttons and power key but this does not appear to work!

How to go in Recovery, Fastboot mode and Bootloader mode.

1) Bootloader Mode:
While your devices is powerd off, press "Power" + "Vol. Down"
until Bootloader screen appears.

Remove and then reinsert the battery.
Important: The Fast boot option in Settings > Power must not be selected.
Press and hold the VOLUME DOWN button, and then press and hold the POWER button.
Wait for the screen with the three Android images to appear, and then release the POWER and VOLUME DOWN buttons.
Press VOLUME DOWN to select FACTORY RESET, and then press the POWER button.

My fast boot option is selected, I forgot my password & I'm locked out of my one max. I can't hard reset it (because it just turns the phone on) & I can't adb in to reboot. Any ideas? *aargh*

I was factory reset > installing 21a but i forgot the 21a file wasn't in the sd card.
Now the phone is in the lg screen forever and i can't find a way to shut down the phone.
Connect via usb cable to PC,
then do a force reset (Power + Volume down), quickly keep only Volume Down pressed to get into fastboot/bootloader mode.
Once it's in fastboot mode ("bootloader"), tap volume down to switch between "Reboot" and "Shut down".
Once "Shut down" is displayed gently tap the power button, that should turn the phone off.

Guys, I'm in need of assistance, I've tried everything, even unlocked the OEM, and the USB debugging is on, but when I try to flash the TWRP recovery on Odin, the procedure fails and it shows a message on the phone saying, "CUSTOM BINARY (RECOVERY) BLOCKED BY OEM LOCK'.
Please, if anybody has a solution, it will really be appreciated.
1-Turn off the device.
2-press Volume Down + Volume UP + Connect the USB cable.
3-You will see a warning and under it three options (Volume up: Continue / Volume up long press: Device unlock mode / Volume down: Cancel restart phone.
4-Will choose Volume up long press: Device unlock mode.
5-Unlock bootloader page will appear, Under it are two options (Volume up: Yes / Volume down: No).
6-Press the volume up key.
The device will reboot and a message will appear telling you that the boot loader has been unlocked, Then press the power switch to continue.
7-Then press the power switch to continue.
8-The bootloader is unlocked, Done.
