Possible file corruption. Download again from another source, and if it keeps happening then extract the .img files and flash from fastboot in this order:
boot, system, userdata, recovery
and see if that fixes it.

i just did this 5 minutes ago. i ran the ruu.exe while my phone was in hboot and it didnt even check my current version, worked perfectly, you need to "fastboot oem lock" first though. i was getting errors when i tried to run the ruu while in the android system.
now im trying to figure out how to re-unlock it
edit... wow, that was so easy, i just typed "fastboot flash unlocktoken Unlock_code.bin"
unlocked again
i had the same problem but i got the ruu to run now im updated but now the unlocking is being a ***** (cannot load 'Unlock_code.bin') is what it keeps saying

so your unlock code is in your folder with fastboot, right. and your in hboot when you type fastboot flash unlocktoken Unlock_code.bin

Ok this might seem dumb but did you type it exactly as I have it in the post above yours, its case sensitive and it needs the _ between the unlock_code. And fastboot oem unlock isn't a valid fastboot command.
Also, which ruu did you use, I think this matters, I used the 2.17 from here http://www.filefactory.com/f/4ef0ef536face67a/
I think its the 7th one down.

Ok I finally got it rooted now .this is what was wrong I had accidentally downloadid the unlock code 3 times and when I pasted it to my folder I had the unlock code3 so it wasn't reading it because of the 3

i have hboot 1.50 i did the htc unlock method so i am good to go there. but here is where i have a problem when i try to flash recovery though either fastboot or bootloader nothing will flash the fastboot way it fails everytime when i do it through bootloader with the file being pg86img the phone pickes it up but then says that main version is older! update failed press power to reboot.anyone know what i need to do here?

If not check my post " unlocked and upset" and go to the bottom and click on the link the last person posted and follow their directions. When you are prompted to open a command window in the tools folder make sure the s.u folder and the twelve are in your platform tools folder and open cmd prompt in the platform tools folder.

On a MAC, ./adb devices works and returns the device but ./fastboot unlock oem returns waiting on phone.

You sure you're in fastboot and not just hboot? It should say Fastboot USB in red on the screen.
Make sure you have any antivirus programs or anything like that turned off.

Tried it again, but got an error: error: cannot load 'Unlock_code.bin': No such file or directory
That mean I have to unlock it again?
Make sure Unlock_code.bin is in the correct folder with the fastboot executabke, that sounds like the problem the unlock code file isn't in the fastboot folder.

Your name is very fitting, found the old email with the file. Placed the file in the same folder as fastboot.
Now appear to be in a hboot loop and can't get to fastboot usb
Okay so I used the SuperCID apk and it converted my CID to a bunch of Twos and then I used the HTC unlock method to unlock my phone. But after I select root my phone I'll void the warranty it boots into Android mode where I reboot it into recovery it says locked. It doesn't say relocked or unlocked it simply says locked like it never unlocked my phone even though it had just said it unlocked it maybe 2 minutes prior. Has anyone else had this problem? How do I fix it?

I then do "fastboot flash unlocktoken Unlock_code.bin" that I got from HTC and then select Yes on the prompt.
It then boots into normal android mode and then it goes through the normal setup. I then enable the debuging mode and then do
"adb reboot-bootloader" it then reboots into bootloder locked.

Is there a way for you to take a video of when you send the unlock token and click yes on your phone. Because I'm really not sure what's going wrong.

I've only heard of it taking multiple times if your trying to re unlock, not on the first unlock.

Just keep trying... some people get it first try other dont...
If you do a factory reset first from bootloader then unlock, it will work first time every time.......it'll wipe your SD tho. Once you have a custom recovery flashed don't use that method anymore tho

For me. I had to turn phone completely off. Then boot into HBOOT/Fastboot in order for the unlock to work. Make sure fast boot is unchecked in the power options before turning off.
In about three minutes this video should show a HTC DNA not unlocking even with SuperCID change and the correct file to unlock the phone with.

Just keep at the command. Keep redoing it until it sticks. Took me 5 tries. Now I'm S-OFF & unlocked...with the OTA. Just keep doing it. It'll stick...eventually.

Thanks a lot turning it completely off actually did the trick. Now my next question is since I had to put on a custom recovery to install Super User now do I install a stock recovery so that it can do over the air updates now that I have S-Off. If it updates my stock rom with OTA will I have to flash a custom rom to root it again or will SU still be there because its S-Off?

I'm not sure why'd you want to take the ota, but yes, you'll need the stock recovery if you want to take it. Then you'll need to install a custom recovery and install a superuser package.
Having s off has nothing to do with superuser or the ota.

So I bought an Evo 3D from a friend, he said he played with it quite a bit. Wifi (says error) and cell data is not working, haven't tried Bluetooth. Phone is supposedly from Rogers, so I'm trying to reflash the stock ICS rom that was linked from this site.
First off, the recovery wasn't doing anything, so I flashed cwm-4.0.1.4-shooteru. Now I've got a recovery at least.
Phone refuses to flash either
RUU_SHOOTER_U_ICS_35_S_Rogers_WWE_3.28.631.1_Radio_11.77.3504.00U_11.25.3504.06_M_release_262715_signed (obtained from the repository)
also tried to run the .exe version. The flash through CWM says Image Verification Failed or (bad), depends if I get it to actually check the signature. I have redownloaded and tried again with no success.
Hell, even the OTA update doesn't work, says the phone has been tampered with (duh).

I have no idea how he obtained S-OFF or did the unlock, and I have no idea of the phone has the stock radio/software/anything. All I want to do is use the stock ICS rom from Rogers, as I'm lending this phone from a friend. I'm hoping the wifi and cell data will be fixed with proper software.
if your using the zip file then you need to rename it to PG86IMG.zip and place it on the root of your sdcard. then boot into bootloader and accept the update.
if this doesnt work then relock the bootloader and then try running exe version from your pc.
btw what is your hboot?

Sooo, it started to update everything with the PG86IMG (i renamed the ICS SHOOTERU rom mentioned above) and about halfway through updating various things it powered off. It won't boot now, it gets stuck in a reboot loop, but I can still get into recovery and HBOOT. Going to try the exe while it's in HBOOT, see if that works (since I can't get the phone to fully power on.)
EDIT: So, since it's stuck in the bootloop now, I can't get the S-ON done. The RUU won't detect the phone now, either. (ERROR[170]: USB CONNECTION ERROR.
So I think I may have just bricked my phone.

Re-ran the PG86IMG.zip, CID is incorrect, the update failed. Do I have the incorrect ROM, or was the file I downloaded bad?
Not exactly sure. I am on a CDMA but most times I have to run the zip twice. Make sure you have downloaded the latest version. Also most sites post md5sums so you can check to make sure the download is correct.

I've MD5 checked both the Rogers Gingerbread and ICS ROMs from http://www.androidruu.com/index.php?developer=Shooter, they're good. Both get checked then tell me the CID incorrect!!! error. The radio currently installed matches the Gingerbread (WWE) ROM, but that's the first one I've tried (3 times each, just to be sure after reading above. Also, I am using the .zip files, the .exe files are the ones you run on your computer, right? (If I'm using the wrong files I'll be so pissed.)
Yes zip is for sdcard and exe for PC. Was the phone unlocked (htcdev or s-off) before?
Just reread your op. Got an idea but I have to find the files. You can still get into recovery right?

The zip locks bootloader so you can run the pg86img. I forgot that it has to be locked because I use custom hboots that are locked but don't show **locked** or **relocked**.

Just checked back now, lots of technical stuff that's beyond my knowledge scope, but I did find the zip you referenced. I can still get into cmw so I should be able to do this, ill edit this post when I have results (or not XD)
EDIT: Bootloader is now ***RELOCKED***, after running the query then the relock, now trying the PG86IMG.zip again.
EDIT2: CID is still incorrect. Going to see if I can hit the ***LOCKED*** status instead of ***RELOCKED***, see if that makes a difference.
EDIT3: CID is still incorrect. Must I have the wrong image?
maybe the phone isnt rogers. try an European RUU

and if you just aren't able to run a RUU then just flash a stock rom in recovery. you might also have to update your firmware. put on root of sdcard, boot into bootloader and accept update

Are you sure that the bootloader says "Fastboot USB"?
When you boot the phone into the bootlader, you have to choose the "fastboot" option (select by pressing the power button). It will go into fastboot mode (and display "Fastboot"). Then, when you connect your phone to your PC via USB cable, it should connect to your computer (assuming you have the HTC drivers installed), and "Fastboot" will turn into "Fastboot USB".
Then in the command prompt on your PC, type "fastboot devices". If your phone is detected, it'll show you it's serial number.
You can also verify it's connected by checking Device Manager on your computer.
in case you dont have the latest drivers installed, I suggest you do that first.

Bah, I wasn't going into fastboot menu in the bootloader. Go me. It says HTC__001, which means it should be taking anything I throw at it.
EDIT: Changed the CID to ROGER001, the CID for Rogers devices using this information:

I've searched and tried to relock the bootloader with fastboot oem lock on my MAC and windows through the command prompt and the command is not recognized. I need to reset this damn DNA tonight before I send it back in to Verizon. Please give me a little assistance here. I have the RUU on my desktop also, I just dont know wtf is wrong with the command fastboot oem lock...

There are many threads that I have read through...
when I fastbooted and so forth... I typed in the command box fastboot oem lock and that did nothing.
From what I read, you need to flash the lock boot loader zip to lock it.. Fastboot oem lock command won't work apparently if you have S-offed.

Are there any places around maryland or northern va who can assist...? I would love to learn the terminology but it took me 4 days to learn how to code on a MAC to perform the dang root lol and not I cant image how long it would take to reverse it.

No worries now guys, I suppose now I have to give Verizon a damn good reason as to why my phone was rooted as hell. I have a decent alibi created.
Flash the lock boot loader zip as you would if flashing a ROM.
After that, do the RUU instructions, then s-on (fastboot OEM write secure flag 3). It is pretty simple.

After that, do the RUU instructions, then s-on (fastboot OEM write secure flag 3). It is pretty simple.

Phone is gone now, however, what I did was...
restart phone, hold down volume blah...
boot loader fast boot and so forth, when fastboot USB popped up on phone on the windows computer open command up and type fastboot oem lock
some sort of error popped up.. so that is where I was. Pardon the english and grammar, had a few Jamesons'

You can't lock the boot loader by fastboot commands, you needed to lock it by flashing the lock zip in recovery.
