I did the OTA ICS update about a week ago and everything has been great so far. Today, my phone started acting up a bit, but it's an issue that has came and went all day.
Half of the time I tough the screen, it's like it registers it as 1 huge vertical click. For example, if I open the app page and try to flick through pages, every app in that column opens. If I try to type, it hits other keys in that column as well (IE backspace erases 1 character and puts a p in its place).
I tried wiping the cache & davlik cache, rebooting, checking running apps for anything that could be causing it, but nothing seems to be working. If it starts acting normal, it's only temporary. I'm not sure how well droid antiviruses work, but the avast one was supposed to have over 90% detection so I installed that but the scan came up clean. I used CWM to make backups before the jump to ICS, and after (after restoring apps with titanium backup). I made another backup and restored to right after I updated to ICS to see if the issue is still there, and it is.
I am currently restoring my pre-ICS backup. If the issue is there too, i'll try a factory reset. In the mean time, has anyone else had this issue or have any thoughts on how to fix it?

Should have known that was a terrible idea, trying to restore it to the pre-ICS backup bricked it, but I got it back up and running on a fresh install of ICS, but the problem is still there. Sad to say, but it's likely the phone if nobody else is having this issue. I'll get it restored to my latest backup and unroot it so I have a functional phone until Verizon can do something about it. I'll keep this thread updated so if anyone else has a similar issue, they have an idea of what to do.
Flash ics again. The problem you reported happens when you try to use ur razf while charging right?

Now that you mention it, yes. I thought it was doing it while not plugged in as well, but it hasnt done it all day. I plugged it in to test it, and it immediately started happening.
Are you using the original charger? We purchased an "oem" charger that makes all of our Androids go crazy. But nothing happens with other OS. I am aware it is a voltage issue though.

I am having that problem with the original OEM charger..but I don't care since I never use my phone while its charging

I keep the original charger at home and take the charger from a previous android phone (1A charger) to work. I put one in the top outlet and the other in the bottom just now to test it. The other charger acts up by the 3rd app page change, while the original went 30-40 without a single issue. This is definitely interesting!

Where I work now I have about an hour of work and 7 hours of doing nothing which is spent playing games/web browsing. My battery is usually down to 5-10% by the time I get off if it doesn't die. So, I started taking my charger and after its down to 10% I plug it in and continue what I was doing.
I can vouch for the wacky touchscreen issues while the phone is plugged in to a charger! It can be a huge pain in the ass at times.

Same problem here with the touch screen. Usually, it starts happening after the phone has been charging a little while, like 20-30 minutes.
I can unplug it, replug it, whatever, from that moment, the problem remains and it is really anoying, especially to type text messages or anything else...
I''m using the OEM charger and cable.
Interesting fact, I remember I had the same issue with my Nexus One.
However, it seems I have no problem at all with my car charger, which is plugged directly to the phone (no usb cable needed), with one one these system that allow you to change the part plugged to the phone. The brand is energizer. :good:

**Please note, i have already searched this forum and found some similar threads as: charging paused, voltage too high, SGS2 charged all the time, Charging suspended. Voltage too high and Phone keeps charging after plugin off the charger but none of them had exactly my problems so i'll try to describe mine better here.**
Some background and possible cause first
I have been using Omega ROM @ 2.3.5 (yes it's old, but it doesn't matter) for more than 6 months now and i haven't had any problems with it, but a few weeks ago i decided to upgrade to ICS so i did a full backup with CWM and also backed up sdcard to pc. Did a full wipe, formatted sdcard and installed the AOKP ICS, hated the camera app, wiped everything again, installed normal ICS, it was looking ok but i needed my usual apps the second day so i decided to go back to my 2.3.5, restored backup, restored sdcard. I don't remember exactly if i had the phone plugged in before installing ICS or after going back to 2.3.5 but when i disconnected it, it showed that it was still connected and charging...
Please note that everything was ok before updating to ICS (which is one reason why i believe it's software related), i used the original mains charger / micro usb cables at home and i also used a cheap car charger but long before this started happening, and the last time i used the car charger was 2+ moths ago.
The thing is, i don't care if it things it's charging all the time, my problem is that when charging it keeps the cpu active, and even when setting it to 100 MHz the battery only lasts about 5-10 hours with low-moderate usage (see the screenshots of cpu spy, it is clear that the phone does not enter deep sleep while it "thinks" it's charging).
Symptoms
At first, it kept showing it was charging all the time, sometimes when reaching ~10% it went back to normal (YES, normal aka reporting as no plugged in and no charging (=discharging), so i don't think there's any short-circuit/something wrong with the usb connector (which i tried to clean up, as suggested, but the problem is still there) :

Then (and currently) after 1-2 mins of disconnecting charger/usb cable it stops the charging animation but *#*#4636#*#* and some battery app stats show that it's still plugged in but discharging (also it reports over-voltage, which is weird because when it is still charging it shows health:Good).
Also, if it goes back to normal (aka not charging and not plugged) and i connect the mains charger then disconnect and connect usb cable it will still see/think that the mains charger is still connected and i can't connect the mass storage to pc (the dialog never shows, because it still thinks it's connected to mains) and vice-versa, but the battery is charged no matter where it thinks it's connected.
Now the weird part, and the reason why i don't think it's hardware (aka usb pin short circuit or anything else stated in the other threads) if i remove the battery for long enough (something around 10-12 hours) over night, in the morning the phone would be completely normal (i was able to do this 2 times so far and both times the phone clock&date were resetted which makes me think the "connected" state is stored somewhere in a temporary memory powered by a small battery which keeps track of the time and alarms?; also if the time isn't resetted, the phone would still think it's connected (yes, even after removing the battery for some 8+ hours)), but the problem reappears if i connect charger/usb cable (something to note here, if i connect the cable and it's not plugged in, the problem doesn't start so this excludes micro usb connector problem).

Also, something that i've wondered if all S2s do, when i remove the battery and plug in the charger (of any kind) the phone shows a no battery image:
One other thing that might worth mentioning is that sometimes, just a few seconds after i lock the phone, the screen just lights up for a second then turns back off for a few times (2-5 times), but this might be just an app problem and it's not really currently bothering me.
Also i just noticed when posting the screenshots that the battery is always pretty hot, is 32 - 37*C too hot or is it ok?
Conclusion
Well, it's been 3 weeks since this problem has started because i updated to ICS and i still didn't fix it. My plan is to order an Usb jig from ebay, get everything back to stock and send it to warranty, but this will take some 2-3 weeks until i get the jig and another 2-3 until the guys fix/replace it so i'm posting here hoping for some suggestions from more experienced members or people that also had this problem, because i ran out of ideas to try here.
It's a very nicely thorough post, very detailed. I like it.
However, I believe your conclusion that it isn't a hardware issue with the port is a faulty one. I think 100% of the times I've seen issues like this it's an issue with the port that is resolved either by cleaning it, (which will likely only be a temporary solution) or replacing it. It's VERY likely they will do just that (replace the port) if you send it to service. If you can get it covered under warranty, do it. Otherwise the part will run you probably $15 or so, USD.
But the fact that the problem comes and goes away without physical actions and also the fact that everyone started getting this after upgrading to ICS makes me a bit suspicious...

And the fact that anyone who experiences issues like this, when they replace the part, it fixes all these issues?
if it comes and goes, I'd check the port for something when the issue is present. There's likely something lodged in there when it's happening, which gets dislodged when it's NOT happening.
People tend not to protect the port. It can get damaged over time. Also, ICS versions will seem more sensitive to it, because of additional programming and error messages specific to charging and voltage errors added in.

I just wish everyone who had an issue that needed help was as thorough as you. Most often, posts in Q&A unfortunately look like: "Halp! My fone broked! Whut happen?!?!"
So while some may scoff at the length of your post, they need to understand; when troubleshooting issues, more than enough information is ALWAYS better than not enough.
Don't worry you're not alone.I'm having this issue for about a week and I don't think that it is a hardware issue because my problem appeared when I was doing a restore from backup from custom rom to stock rom and the only thing that I did is that I pulled off the charger while my phone was doing the restore process. The second thing that I think it is not a hardware issue is that my phone is new I bought it two month ago and the USB jig is so clean, I compared to my brothers phone he has sgs2 too and he's USB jig is full of dust and cockroaches and mushrooms and stuff like that but its working perfect. I'm so sorry that I don't have the answer for you but I'm so mad about this problem that I needed to say something and by the way good luck and pm to me if u find the fix

This is not a hardware problem. This is a Firmware/Backup related problem
Had this exact problem on a friends SGS2 and it kept on happening after he did a restore from Titanium. Not that Titanium is broken BUT your backup stored on your SD has a CRC somewhere. What can I say ... It happens.
Just do a complete wipe and install the Rom you want. DO NOT RESTORE YOUR BACKUP !!!
Install everything again as per normal and set it up and you should be good. If this then still occurs then you can be sure its the hardware but this resolved it for him,

I think it's a bit presumptuous to say it's DEFINITELY not a hardware issue. I've seen a TON of posts where people replace the part for the USB port and the problem disappears.
Maybe for someone its is a hardware problem but my problem appeared during restore from buckup.

Did you discover the source of the problem? I am having the same issue with my sgs2 (only 6 months old). I have all the same symptoms - it tells me the USB is connected and the phone is charging, and I get the same alternating symbols when the phone is off (battery charging/temperature warning). I have done nothing out of the ordinary - no system alterations, all is as it was when I got it, other than apps etc I have downloaded. The port doesn't look that dirty, although having read your posts I am inclined to think that this may be the source of the issue? Any help would be appreciated!

Same issue after upgrade, was searching for days ...
Factory restore, but didn't work.
Forced to stop all services, cleared all cache and cleared Ram and after restart everything back ok. Was not a hw issue.

And yes...back ups can be a BH.
My issue has been happening for awhile but its gotten a lot worse lately.
My screen, when on, will randomly start "touching" places. When this happens, it appears as if a phantom finger is pressing various things and it is causing a lot of issues. Also when this happens, the 4 buttons on the bottom stop responding. i have to use the power switch to turn the screen off and then back on again.
any thoughts on how I might be able to fix this?

Are you rooted/flashed a rom? If you are you should definitely wipe everything a few times and flash a new rom. If you aren't I would perform a factory reset.

It has happened across multiple roms. I will try again though.
If nothing else works, try SGS Screen Booster. My Rez has been having the same problem recently, and it's seemed to help quite a bit.
The right below the status bar, there seems to be a gap with what is and what isn't working properly, so I set everything to the max and lowered the response time to be as quick as possible.

Happens to me also. Do you have a screen protector maybe? That is what I always thought my problem was.
Certain chargers or USB cords with grounding issues can cause these problems. Does it only happen when it's plugged in?
I had one were it would cause the same issue when plugged into the laptop, but only while the laptop was plugged into the wall.

+1 for this answer. had it happen when i bought a cheap cable from HK. Went with OEM, and now i dont have this problem

Eh, mine does this as well. It's been happening more often in the colder weather I get into. So I just turn off the screen and let it sit in my pocket for a minute so the screen can warm up, and then it works. I swear, these Asurion replacements suck.
I'm having the same issues with my Rezound. I also have a lot of issues with the soft keys not working now ( Far more often than the random screen touch issue ). They won't respond at all or they will start randomly start clicking. I also notice that when they stop responding usually I can put the phone to sleep, then wake it back up right away and they will start working about 10-15 seconds later. Even when they are working I often have to hit them multiple times before it takes. This happened with the stock rom. It started happening shortly after I received the most recent OTA update, however I assume that is simply a coincidence and nothing to do with the update. I flashed the phone recently with CM10 hoping it was an issue with the ROM itself but the problem persists. This is a shame too because other than the screen issue it seems that CM10 would have given the phone a new life as it looks and runs so smooth on the hardware. I highly doubt I will ever buy an HTC again, we have had two of them and they both start having hardware issues much faster than they should for what they cost.
Im using the unofficial cm10.1 build with no mods. i went through and wiped everything twice. cache, system, boot, battery settings, rotation settings. i flashed the latest at the time (build from 4/3/13) and now my phone is working properly again. no more hanging up on people caue it preses by itself, no more emergency dialing.
hope that helps anyone who might have the same issue.

Wondering if anyone can help me...I'm at a total loss. Any ideas would be greatly appreciated.
About a month ago my i717 Note started having issues where I'd be using it and suddenly just power off without warning. When I try to reboot it without being connected to wall charger, it very rarely boot back up, usually dies again mid boot.
If it's actively being powered to the wall/USB the issue has never happened...Most of the time after this happens the battery will indicate that it is actually completely drained ie 0% batter life...however sometimes once it's plugged in it will show some other % that isn't even low battery.
This issue occurs even though I could have 90% battery life prior to the crash. My battery stats in settings shows normal discharge, then just a straight veritical line to 0% followed by the normal recharging...
I have 2 batteries, both Samsung OEM so I cannot think both my batteries happened to crap out at same time.
I had been on Black Jelly for many months without any sort of issue, then suddenly this. I have tried these things, to no effect at all:
- delete batterystats.bin in /system/data
- added several layers of tape on the bottom of battery to ensure has full/tight contact with pins
- loaded several different ROMs
- Completely wiped (Factory Reset, Cache, Data, System...and eventually even did Internal and External Memory wipes from TWRP)
- Odined back to Stock 4.1.2 image
The last application I loaded was Google Hangouts and was also the application I first noticed the issue. I video chat with my parents with it and first noticed it happen there which thought was odd since phone never warned me for low battery...however now it happens all the time despite all the steps I took above plus I don't have Hangouts loaded on the phone atm.
I am currently running Stock 4.1.2 and it's still happening there. As is I cannot use my phone for probably more than 5 minutes without this happening. If I can't solve this I'm going to have to go back to my Cappy which just makes me sad since my Note had been a good phone previously.
Since it's happening across ROMs and after total wipe (if there is any more wiping I should do that might help let me know), plus Odin back to Stock I feel as though it must be some sort of crazy hardware issue. I've only ever seen this happen when I'm actively using the device...it hasn't happened when in in sleep mode (ie screen is off).
Any ideas?

How old are the batteries? They are notorious to go south after about a year.
Are they swollen at all.

They both 2+ years old now. No bulging they would appear to be in good shape. I've never had excessive battery drain before, and currently when the issue is not happening the battery life is ok. IE if I don't use my phone at all, battery drain is about what I'd expect. I haven't seen the issue happen when it's sleeping, just if I'm actively using the phone.
Sometimes the issue doesn't happen, and it'll discharge all the way down to 0% normally...it's so random and frustrating...I do know that if I use my phone non-stop for probably about 10 minutes it's going to happen though, usually in less than 5 minutes. Again, I don't see the drain, it looks like normal discharge and I'll go from 75% -> phone off in the blink of an eye. Oddly if I then plug it in the battery charging icon you get (when is powered off) sometimes indicates it's actually not empty.
Also of note when it does happen the phone isn't hot, I can handle the battery fine. Also I have the OEM wall charger for the extra battery, so I know that the battery should be getting fully charged from that (so isn't a USB charge issue on the phone itself).
Is there a good battery stats app I can try? The battery stats in sys settings doesn't show anything hogging.

You are not the first to run into this issue with those symptoms. I believe more often than not, resolved with new battery. I just got one myself from Amazon for $9. Ravpower 2750 I think. There is a thread in accessories.

I was on vacation at the time, so I didn't have any time to try to figure it out. Then this battery thing started happening. Not sure how it could be related, but maybe my RAM is fubar.
I'm getting my Cappy back from a friend so I may try that first, I kinda miss that phone...

Yeah that's where I am right now...100% stock 4.1.2 and still doing it.
Anyone have anything to report yet?
Not a major issue yet!
I got a bad pixel, the I am waiting on to return for. No root, rom, or lollipop..... other than all that, so far so good.

Seems a little slow with cellular data at times.
Just random force reboots. I'm sure it's all because of the bloatware and hopefully Lollipop will fix it.
Some times mms message get stuck at unable to download, have to reboot to resolve

Few problems
Stamina mode causing texts to send late or freeze, SMS and MMS.
On stock messaging notification mark as read button doesn't respond, must be tapped twice.
With Hangouts dialer installed and u choose to call from Maps or other apps the default choice doesn't stick, it asks which dialer every time over and over, had to uninstall hangouts dialer very annoying that your choice isn't saved.
Battery takes forever to charge, from 0-100 it took 3 1/2 to 4 hours. That's horrible.
1 freeze where the screen stayed black and could not reboot, had to remove Sim card to fix.
Slow on data at times, not as good signal as Samsung or Motorola.
On YouTube no option for 1080p, 720p is highest, Z2 had 1080 option. If you disagree and before you comment try these out yourself and tell me if you're having the same problems.
I'm a little disappointed in the charging rate as well. I had read a handful of spec sheets that listed this as having something similar to quick charge. Having had the droid turbo and Note 4 I loved this setup. I know I didn't have the same feature verbatim but it did have some kind of fast charge feature mentioned. I thought it might be due to not having the correct cord similar issue with the one M8 but, a msg pops up at 90% on my wifes note 4 charger telling me its incompatible with that charger.

If it freezes again just push the small button in the SIM card area, just bellow the SIM card, will reboot.
When in navigation GPS Icon clogs notification area...Why Verizon why? All other variants put it next to 4g signal and not as a notification. Verizon sure knows how to mess up a good phone, there's no compatibility to Sony PC companion either...Verizon's doing...
Having an issue with a boot loop since Verizon update
I got the update from Verizon, it threw my phone into a boot loop. I let it run for like a half hour with no luck. During my entire ordeal, it would usually only make it to the Sony wave screen. It would make it to the Verizon screen on occasion, but would always loop back to the beginning. I would get two vibrations followed by a red notification light. I figured out the button combination for recovery mode. I tried to boot into Safe Mode. It once again went into a boot loop. I put it back into recovery mode and did a factory reset. It did the reset and went directly into a boot loop again. I ended up doing three factory resets in a row to see if it was an issue with the factory reset process. After three resets and a half hour boot loop, it did fully boot up. I went through the entire setup process, emails, wifi, bt, widgets, apps. During that process, it did a force reboot and went back into a boot loop. I went through the entire process listed above again. Safe Mode and three Factory Resets. I have once again gone through the setup process once again with no force reboot...yet. This could happen at any point any this is getting pretty aggravating. Any suggestions?
Bad unit
I think I had bad hardware on mine. I was experienceing randon screen freezes, that forced reboot. Got it replaced and new one is flawless so far.
Urgent: Stuck
Looks like I'm currently experiencing a problem. I was playing Asphalt 8, and as I was exiting the game, the phone froze. I hold the power button, and it just rings my text message ringtone. I hold the camera shutter, it plays the camera sound, but same screen. I call it, it will play my ringtone but I can't answer it.
With a day's worth of battery life, I can't just wait for it to die. What do I do??
EDIT: So what I had to do with a last resort is access my phone with Google Android Device Manager and erase my phone and factory reset. I hope to never experience this again. If only this was a Samsung, removing the battery and putting it back is the easiest hard reset to do. :\
I believe that there is a button inside the sim compartment that simulates a battery pull.

If you would have hit the reset button you probably wouldn't be complaining right now. Learn you device before comparing it to Samsung crap.

Yeah that's my only gripe with this phone. I have no issues. I came from a nexus 5 on ATT. Didn't get ATT in my new house. Glad I made the jump. I charge this phone with my old nexus wireless charger and the thing takes forever to charge.
You're complaining about a limitation of qi charging,not this phone. Compare the output limits of a qi charger and the power block to your phone. If you want the phone to charge faster, it will always be faster with a wire than wireless.

Trust me I knew that already. My nexus 5 did the same thing. I just like the wireless charger in my room. Just easier to grab when you grab the phone in mid sleep. Lol
I do agree with the convenience of qi charging. I just didn't want someone to think that qi would do rapid charging of some sort.
This is the 3rd time this has happened to me. If the tablet shuts itself off because of battery is 0% then it will get stuck @ Samsung screen on boot (after charging). I had to factory reset it the first time, the 2nd time got away with clearing the cache. This time looks like its another factory reset. Anyone else having/had this issue? This is a stock device with no mods at all. Any help would be appreciated.
Any reason you are running it down to 0% or is that part of the problem?

As a general rule, Lithium Ion batteries dont like to be completely discharged. I would.put it on the charger and just leave it there for a bit and it should be ok.
That aside, you can adjust some settings so that it doesn't run dead. Set your Wi-Fi not to stay connected when its off. Another very useful app is Greenify. I took it one step further and installed the app called Automate. Then I created a flow that turns off wifi and BY, and then triggers Greenify's hibernate mode, and then it turns the screen off. I put a shortcut to this flow on my home screen so I can trigger it when I'm not going to use the tablet for a while. Usually my tablet will loose 5% max over 24 hours of non-use.

This is again a system of the wifi power drain bug that samsung continues to ignore, the S2 would last for well over a week on standby just like Ipads.

I agree but at this point I am just trying to get my device to boot with out factory reset.
AQF5 build came out today for Korea, trying that now. The way they are increasing the build numbers, hopefully they are actually fixing something. Interestingly this had a modem file too though the SM-T820 is wifi only.

No dice still with AQF5 KOO firmware today, I officially give up. Likely a hw issue , only reason they have not addressed it across 6 firmware updates. Too bad since the tablet is very decent otherwise. Unlikely this will ever get fixed at this point since this is not a Note 7 type headline grabber. Time to find an used S2 to replace the one I sold or wait on something else.My S3 is going back tomorrow.
