You have to flash the SuperSU file from recovery to actually be rooted.

Download the "CWM flashable zip" and flash through Amon-Ra.
Indeed, you need two things for root.
The su binary, which is in one of your system folders.
And an app like SuperSu to manage root access for apps.

I was rooted on 2.08 and decided to relock and upgrade to 2.17 OTA. I was able to unlock again and install TWRP custom recovery. I flashed Superuser.zip (3.07) from recovery. But it seems I still don't have root access. Not of the rooted apps work. Tried installing busybox from app on the market but says don't have root. Trying to update binary from Superuser app fails. How do I get root access at this point. Should I flash an older Superuser. Thanks.
Did you flash a rooted ROM? If not, that is likely your problem.

I'm on stock ROM right now. I flashed a different version of superuser which worked. That was my problem. I have root access now.
I was rooted prior to the OTA. Relocked my phone and did a stock recovery. Acquired the update, now I'm trying to re-root and not having any luck. I've successfully unlocked again, flashed amon ra, and installed su in recovery. However when I check for root access or try and use titanium backup it's unable to acquire root access. I've updated to the newest SU and still no dice. Any ideas as to what I'm missing? Thanks for any input.

The su that Amon Ra installs is outdated and doesn't work. You could do one of two things:
1. Flash a rom(all roms have su) or
2. Go to the superuser website and find the latest binary.

Thanks very much that did the trick!
I just unlocked my DNA. I then installed CWM Recovery. I checked the All Things Root thread, but the files that are to be used in CWMR have been removed from the website they were hosted on? Where can I find these files now? I'm looking for the SU and busybox files that I flash in CWM. Thanks in advance.
Rebooting system in clock work should prompt you to install them
You can just flash a custom rom or download superuser from the market

Thanks. I just ended up installing a custom Rom that was rooted with su installed.
I did a factory reset from recovery on my EVO because it wouldn't boot. Now, I seemed to have lost root access. I can still boot into custom recovery and am S-Off.
I downloaded SuperSU from the play store, and get the message (There is no SU binary installed, and SuperSU cannot install it. This is a problem) when I try to open it.
What do I need to do to regain root access?
Thanks for the help.
Play store won't help you. You need to flash supersu zip from recovery.
Google "update supersu 1.94 zip".

Thanks, then do I need to download it from the Play Store? I flashed it successfully, booted up and the one that was in my app list (from the Play Store) is now gone. Rooter Checker still shows I don't have root access.
Yes you need to reinstall the app.

Flashing that zip should have worked. I question if you were using the right zip file. Do you see it in your apps on your phone?
Here's the URL I use:
http://download.chainfire.eu/396/SuperSU
Sometimes if you run root checker too soon after booting into the ROM, it will time out while waiting for permission from SuperSU and falsely conclude that you do not have root. If you're busy doing other stuff afterwards, you very well may have missed the popup where root checker asked for root permissions. If that's the case, when you go into SuperSU you should see root checker listed with red text, which would indicate that root checker was denied root permissions. If this is what you see, select that entry from SuperSU and change the setting from deny to grant. Rerun root checker.
If that doesn't work, please provide more information about your system?
HBoot
Recovery version

HTC Dev unlocked?
S-off method?
if you run or install TWRP custom recovery it will flash supersu for you when you run it.
Hey,
I just flashed the CM12.1 ROM by Temasek yesterday and tried to gain root access again.
Downloaded the SuperSu 1.94 and flashed it with the recovery.
Installed SuperSU from the Play Store but as I try to open the SuperSU option in the settings, I always get this message: There is no SU binary installed. SuperSU cannot install it - You need to manually re-root.
Does anybody know how to get root again?
Thanx very much.
