followed ur instruction, there's no theme called "sense 5 by reza" or ay sense5 related theme, and can't install it too. btw I'm using stock firmware.

Thanks. I managed to fix this by pushing the original SystemUI to the phone. Any way to get this modified EQS working? I definitely like it better than the stock.

Here is the Sense 5 skin for HTC Sense 4.1 Lite
It fully works and can be installed over any other skin, no need to remove older skins.
Note, this WILL NOT give you Sense 5 icons. For that you need to install a Sense 5 themed ROM.

You can. Just download ROM extract it, than extract every APKs and pull Sense 5 icons from every app
It's not difficult.

1. Extract ROM.zip
2. You will find some folders (META-INF, system, data, ecc.) go into system/app folder
3. Now take the SystemUI.apk and extract like a zip file (with 7-zip)
4. Go to res/drawable-hdpi and replace your pngs with Sense 5 ones
5. Now open SystemUI.apk and replace your images
NB: you can edit some pngs, but not all!! Be sure you do not modify .9.png files only replace them!

SystemUI.apk only has the icons for the statusbar. If you want to change icons in the appdrawer, you should change the icons in com.htc.resources.apk. But some of the icons are in the apps themselves, cause the htc resources only has icons for HTC apps.

The menu icon is default on the latest myONEdx RC4.0 by ckpv5.
I just installed them but I cant found them in personalize menu...this is after update my Desire X to 4.1, any ideas ?

Do you mean you update to JB 4.1.1 ? This skin is for ICS 4.0.4 only and particularly for myONEdx custom ROM.

What you need for best appearance:
- Flash a ROM with Leedroid Tweaks.
- Set Navbar and Rosie transparent.
- Set Status Bar @50%.
- Make a Backup of your ROM.
- Flash Zip from Recovery the Wipe cache and Dalvik..
- Go to Skins and pick the new one.
- Add a Fancy Widget with Sense 4 clock and configure it.
- Reboot and enjoy.

How to revert back to stock theme after flashing your theme?
A: boot into recovery
Wipe system only...(dont wipe data)
Flash the rom again on which you flashed these themes.
Reboot and let the android upgrade
Now you will be on completely stock theme.

Don't know but it should work because it is designed for all sense 5 roms but tested on insertcoin...
