Any thoughts on what might be causing the issue in the attached pic? I have replaced the lcd, the digitizer and the lcd ribbon, and I still have this display.

Looks like to me rolling boot screen.
Rolling recovery is a common issue with rooting and updating ClockworkMod Recovery (CWM) there is tons of faq guides etc on here if stuck see http://droidbasement.com/db-blog/

Unfortunately, i'm running stock-no root, so I don't think it's a software issue. You can almost see the home screen in portrait orientation.

Why did you have the need to replace all that hardware?? Everything seated correctly what happened to make the need for repairs
What does the boot screen look like is that readable. Why I ask if you have replaced hardware and the boot screen you can read it least you know the hardware is okay. Then you can look at the software. I'm no expert but then a master reset flash stock recovery and firmware.
The boot screen is unreadable also.
I swapped the digitizer and lcd after reading on a couple of other forums that this might be the cause of my problems. That was actually easier than I thought it would be. Because replacing the screen components didn't fix the issue, I figured I had overlooked the obvious and then replaced the lcd ribbon. The display is the same as it was prior to the hardware replacement.

Sounds weird, but this is the 2nd ATRIX I've received and within 4 months, a portion of the LCD screen just...dies. not dropped or water damaged either. Also with both phones it was the SAME part of the screen...and almost exactly 4 months apart...odd.

As you can see, that strip doesn't recognize contact, in either portrait or landscape. Anyone else experience this flaw?
Yup. But mine was at the top. I sent it in for warranty and got it fixed.

I've also got the same problem, except the dead portion of the screen is the exact part where the first notification in the notification bar is.

Digitizer. The weakest part of that phone. If its in warranty, you're lucky. If not, search around for a replacement.

I just bought my second digitizer. (first one was not oem and worked but it didnt have minor details like tint where the sensors are.) My original one went dead, the second one i dropped the phone and it started going bad. Here is a link to an oem one from amazon. It should get here by monday so I will update you on the condition of it. I would NOT recommend going for a cheaper non-oem version.

EDIT: I got the digitizer... and broke the ribbon cable lol. It was in perfect condition though and the company issued me an RMA so im getting another. I am sure that it worked fine. Seems like a great company (i got it from seller1on1) I have bought from them before, i cant say anything about how good the other companies are.
OK so long story short. Bought my s3 lastnight only to notice today my front sensors are off center. The camera, eye tracker and proximity sensor all work. But I'm guessing I'd be better off exchanging for a new one. Would u guys do the same? Just sucks cause this phone has been working better then my i9300. I'd hate to get a replacement that doesn't work as well

J/k but it all seriousness, If the offset of the lens has got you erred wait till you run LCD checker and observe the black screen in a dimly lit room, you're going to go apenuts.

what do I look out for on the S3, EG common issues, hardware and software?
Volume buttons can sometimes be sticky, screen defects such as scratches. Thats about it really.

if second hand make sure imei matches that on box
Check for broken pixels mate.. use something like LCD test and go through the colors looking for any pixels that may stand out

Check if there're any build issues or light bleed on the left edge of the bezel(some white units had this problem),check for dead pixels and last thing is to check the model number.Make sure it's I9300.

No issues on mine
Finally got my Mega and it's actually smaller that expected. The build quality is okay but there's a small ripple on the screen when you press too hard on it. I think its caused by the LCD and the glass when put under pressure. Anyone have this? It only appears when pressed hard and it's only one small point on the screen so I can live with it. I'm also afraid I'll get a worse one if I have it replaced.

So you have the same issue? I just called Samsung to have it repaired, they said it only takes 5 days. I'm a bit OCD so it will bug me to no end. If more people have this then I might just live with it.

It only appears on the same spot when pressed hard, I actually had a hard time taking that pic because it disappears after the tap.
I'm not sure if this is a real flaw or defect. Most LCD screens behave this way if you press really hard. However, pressing normally hard on the 3 GT-I9205 devices here, I can't reproduce the effect.

Maybe when I get it back the Xtralite ROM will be out I'm gonna use the phone all weekend and see on Monday if it gets better or worse. The warranty is 2 years anyway so I can have it repaired at another time if I get used to it. It doesn't appear on regular taps, just hard taps.
I have 4 spots on my screen that look like that when you press slightly hard on the screen. Same as what gx3k posted.
You can still see them as they stay brighter than the rest of the screen on white backgrounds. It's beginning to bother me because they are so visible on white backgrounds. I think I may have created these spots when I was installing the screen protector.
When you press too hard on the screen it sort of damages it and causes these areas.
Just took the phone out of my pocket, and the screen looks like the attached.
What's going on. Phone and touch screen still work fine.

Here's a photo. The entire display is messed up, particularly the Upper right. Tried powering down. No luck.

No. No drops, jarring, or anything. Screen is not cracked or damaged in any way, and touch works fine. I'm using it now.

Honestly, I can't recall anything I did that would have caused this, so I'm a little puzzled. Anyway, I'm shipping it back to HTC under warranty. Should only cost me $19.

FYI, replacement device is on the way back to me from HTC with no hassle. I returned my original device to stock, factory reset, and re-locked the bootloader before I shipped it back, but left it S-Off.
Well, I received the replacement device today and it doesn't vibrate. Looks like I'll be shipping this one back too. Don't they check these things before they send them out?? I'm beyond frustrated at this point.
