Just threw together a video showing how to manually update your Nexus 10 to 4.4 KitKat.
Hopefully this cuts down the questions. You must be on the 4.3 JWR66Y build to update.

Followed the guide but the upgrade fails.
ADB sends the file correct but recovery refuses to install it.
First i thought it was because of the unlocked bootloader but i get the same error even after i locked it
what to do?
EDIT: just tried to redownload the file still the same.
Full error is:

you have a modified stock rom. you will first need to flash pure stock and then try again.
yeah i figured that out - apparently it was xposed framework that gave the problems.

I have the same problem as you. When I push the .zip it fails at 100%.
I have twrp 2.6.3.1, stock 4.3, root access and unlocked bootloader.
Where to find a pure stock rom and how to flash it?
EDIT: Nevermind. Dit it via Toolkit.
I have KitKat installed now.

Strangely enough, I manually updated last week, apparently successfully (Android version, build changed, etc.). Eveything was working pretty well. This week, I got a OTA alert and figured why not? Performed the OTA, re-upgraded to 4.4, and things are working even better now. Not sure what the difference was, but performing the OTA cleared the minor issues I was seeing with 4.4 when manually pushed (i.e. Launcher selection wouldnt stick, home screen animations now smoother).

4.4 got a bug-fix version which happened the last week or so, from 4.4 KRT16O to 4.4 KRT16S. I got the same puzzlement on a Nexus 4, I flashed it to 4.4 via toolkit then to find it was offering a 4.4 upgrade.

I have a rooted Amaze with just some bloatware removed. Yesterday I received a notification in the status bar that a system update was available. When I try to run the update the phone reboots and then freezes with a big green X on the screen. Doesn't look like something official. Has anyone seen this before or know what it's about? I'm thinking maybe it's just failing to run the update because of being rooted or a related issue but can't find any info on Google about the green X.
I finally pulled the battery out because it wouldn't respond and then turned the phone on again. It booted fine and I still have the notice about the update. I'm reluctant to try again until I find out what's going on. Thanks.
I don't think you're able to install an update through a custom recovery..There should be a ROM with that update already in it.
If you have a Custom ROM you can't take the update that is intended for stock unrooted ROMS.

Thanks for the feedback. I don't have a custom rom - just the stock minus some bloat. I can see how the custom recovery, however, might interfere.
I can't find any info on the update and what it includes or why I might want/need it but assuming it would be good to install then is there an easy way to remove the custom recovery? Would running the stock update.zip do this or do I need to go through the full un-root process?
If you have CWM recovery already,
1. Go to /downloads folder inside the internal Storage. You can find the OTA.
2. Rename it to "update.zip" and copy to External SD.
3. Reboot the device to recovery.
4. Select update.zip and update manually.
5. After update & reboot, delete both original files to get rid of notification.

Seems its not possible as you have removed some programs
Think you removed friendstream also.
It seems to work only if the ROM is unmodded

Yes, I removed facebook and friendstream. Not sure why those would be necessary to run a system update but you may be right about not being able to update a modified phone. Maybe the update isn't anything I need anyway if it wants to update an app I don't want.

In case anyone has a similar issue, here is what I've figured out.
The update won't run on a modified system - even if it's a stock, but changed, rom. In the interest of exploration, I restored my backup using clockwork and then ran the update. It still didn't work, but following skr_xd's advice above I was able to run the update manually. The result was a more or less stock phone without root but with the most current software. I then re-rooted the phone and re-removed all the bloat with no apparent ill effects.

Btw, I didn't know what the green X was all about, but I think it's related to clockwork mod. Could be wrong though.

Thanks for the confirmation and awesome software. I noticed there is work on version 5.0.2.7. Assuming it would be wise to update once released.
Same issue here. I have stock rom without any bloatware removal.
I just unlocked the bootloader than installed CWM and rooted the phone.

I just spent abut 4 hours trying t figure this out...
This is my journey..
Relock bootloader
Flash one of the stock tmobile ruby rom in development section
Did the OTA update on stock
Unlocked my bootloader again
Then i tried to root using one click zerg method but it didnt work
So i flashed clockwork mod using fast boot
Then applied a su binary update i found in the development section.
In less than a minute i got root back.
Then i restored my data from nandroid backup.
My phone is pretty much rooted, bootloader unlocked, bloatware free alongside having the ota update that enabled wifi calling installed.
You don't need to relock your phone to do the update. If you do xboarder's recover to stock method just skip the relock part you can update your phone. So far going back to stock is the only way to get the radio updated as well which I am noticing way better 4g coverage than before!

NICE! Dude you must never sleep! Always the first to get something done!
I figured out what to do. You just need to rename this update file to update.zip. then reboot in recovery and choose install update from sd card. Next navigate to download folder and install update.zip file. You will be updated and stay rooted at the same time.
This worked for you because you didn't remove any of the t-mobile bloatware from your phone.. This will not work for people who messed with the pre-installed bloatware.

Maybe, but 8m I've got another problem after updating this way. Wifi doesn't work anymore. I receive an error while trying to switch on wifi.

Seems like you probably froze or deleted an app like Sprint Zone, I'm assuming. Either replace the apps moved/removed or skip the OTA which sucks anyway and manually install the firmware via Odin. This is the preferred method as it is cleaner and you won't need to worry about fixing errors in the system like missing apps. Here is the latest version:

Good call on the Sprint Zone. This was the first app I got rid of once i got the phone. Thanks for the quick respond. I will have to flash it and re-root it; no biggie.
I did notice that tar file is for these versions and I have L720VPUFNAE
L720VPUFNG2; L720SPTFNG2; L720VPUFNG2
Would be ok to flash it to this NG2 instead of AE?

Excellent; greatly appreciate it.
I have installed gohma 2.3 custom rom with twrp recovery and root and have taken gohma-gwatch-1.2 to get back 4.4 stock with stock recovery and unroot.
After that I clear all inside the recovery to have a fresh stock without old data. I got the setup and the paring to the phone and after I check for an update the lg told me that one is available and start the download.
After it was finished the watch boot into the recovery and start the flash but aborth with an error 7 message
Can anyone explain me why? After I start the watch again the update told me that 4.4 is latest and there is no update available.
The OTA is from 4.4 to 5.0.2 but now I dont get 5.0.1 like before. Another watch work fine with this method but why this one not? Andy ideas please
Same issue here. Returned to stock on 5.01 and when upgrading to 5.02 I get that error 7. Tried everything, wiped several times but no way to install.
thats strange because my own watch work fine with the downgrade and changing the recovery back to 4.4 stock. My dad can use the watch with 4.4 but I dont understand this :crying:
Solved. Used RootJunky v8 to go back to stock 4.4 and then flashed OTA 5.02. Worked.

Thank you sir :highfive: , you just saved my LG G Smartwatch!!! i wasn't able to update to smartwatch to the newest android wear 5.0.2 update without getting error 7 on recovery, i was using the LG G watch restore tool v9.5 to restore my watch and kept getting the Error 7 when trying to update, so i stumbled upon this forum and decided to use the v8 of the tool instead of the v9.5 and it worked perfect, i was able to update my watch to 5.0.2 without any errors :victory: i will share this news with other users that are in the same boat as i was and will give you the credit bro... awesome find really!!!

I am looking for a Thor and Apollo tester to confirm that they can indeed install Nexus ROM v4 with this.
I have confidence that this works, but if you agree to do this please understand as usual that you do so at your own risk.
So far the new version has been tested on unlocked devices to confirm compatibility and works fine.
The next stage is to test on locked devices, which as I have said have high confidence that it will work.
The prerequisites for test are:
- Must be running stock update-kindle-xx.4.5.2_user_452004120 firmware
- Must be rooted
Things to be aware of with the new version:
- Secondary slots boot, but the WiFi module does not load
- Since the secondary slots work but have no WiFi, you will need to flash Nexus v4 to the stock slot

so yeah had the problem of being on nexus rom for SS but i had no backup of 3.2.8 to return to so i did the following and it worked
downloaded 3.2.8 rollback file
made a backup of Nexus rom system partition
wiped system, data, cache, dalvik
flashed 3.2.8 rollback after i renamed it to .zip from .bin
bit nail with butterflies
success
poweroff
power on tablet
amazon logo
fire logo
lockscreen
breathe out
im gonna update to 4.5.2 just gotta get rid of safestrap then Im in for testing.

On Thor @zamtang confirmed he can get into Safestrap recovery
Now waiting for confirmation that HDX Nexus ROM v4 is running.
Still awaiting a Tester for Safestrap on Apollo...

Be mighty careful around the 4.5.2 update. Those who received it via OTA (may have been 4.5.3/4.5.4) were treated to a hard brick with no known resolution. Best do the update manually using the procedure/files outlined in this thread. Stay off wifi until 4.5.2 upgrade is complete.

my device had no safestrap but it was rooted decided to let it auto update so turned on wifi and checked for update. the update centre said no update found, so went and downloaded 4.5.2 from amazon that was taking a lot longer than it should. checked the update center again. a glitchy 4.1.1 was downloading with the download bar glitching from view. i then let it continue pausing 4.5.2 download it then promptly rebooted and attempted to install 4.1.1 the screen went black through the update. hard brick.. i have my warranty and i have another one coming.
here's a tip don't update to 4.1.1 if you are rooted.

Yeah I would say as a rule never let them auto-update.

@paul26rem has confirmed that Safestrap came up good on his Apollo.
Now awaiting confirmation that HDX Nexus ROM is running.
EDIT: Confirmed - Apollo Safestrap is good - Nexus v4 running on it.

For the benefit of others ...
NEVER take an OTA update on 3.2.8. Every reported incident has resulted in a hard brick. Keep WiFi off until OTA is blocked. The only effective method is via HDX toolkit. Renaming otacerts won't work on 3.2.8.
@ggow Any idea when this version will be released for Thor or if I could help test it? It would be awesome to have 4.5.2 with Safestrap.

I tried changing the bin to a zip and flashing didnt work said apollo_full instead of apollo as error. Anyone know how to fix it just downloads the update fully then says update failed...

Be happy 4.5.5.1 didn't take when SafeStrap recovery was installed; device would have bricked. Assume you used the Safestrap compatible variant of 4.5.2 (here). After flashing that (stock slot only; never use secondary slots in SS v4) reboot device and remove Safestrap recovery via SS app. Power down and boot into stock recovery via <power>+<vol-up> and perform a factory reset (you'll loose all apps/data). Reboot again and initialize FireOS. It should then accept OTA updates.
Be 100% sure Safestrap recovery is uninstalled prior to doing a factory reset.
Curious - why to you want 4.5.5.1? Offers no advantage over 4.5.2 if running custom ROMs. Nor will it automatically update to FireOS v5 as Amazon has no plans to release a v5 update for 3rd gen devices.

IDK - should work. You can try downloading the latest version of FireOS here; installation instructions here. Also verify OTA apk is installed and properly named (here).
It's likely 4.5.x is the terminal release of FireOS for 3rd gen devices; potential buyers will not be severely disadvantaged on 4.5.2.

Update 1: Tried 4.5.3 said update file not valid again. I'm beginning to think this restore file I found on here that was Safestrap 4.5.2 might be the wrong zip for this device even though it installs...
Update 2: Tried another TWRP 4.5.5, it installed, then I did the same thing again removed SafeStrap. Then factory reset...

As previously posted - IDK and I do not think anyone else does either. What I do know is every flash (some which appear questionable) presents another failure opportunity that could yield an unrecoverable device. Keep experimenting and posting results; others will learn from your efforts. Just recognize the risk and know when to say 'good enough'. Pushing from 4.5.2 to 4.5.5.1 buys you no additional capability/functionality/stability at this time.

Do you checked if the DeviceSoftwareOTA.apk is there? (which @Davey126 mentioned above).
I was in the same situation and had to rename it back, which I'd renamed before as described in the OTA blocking tutorial. http://forum.xda-developers.com/showpost.php?p=59160158&postcount=1

did you ever figure this out? i'm in the same boat right now, except i don't even know how to remove the recovery. i'm running safestrap 4.01 and fire rom 4.5.2. I saw another post here http://forum.xda-developers.com/showpost.php?p=49687774&postcount=9 but i'm not quite sure if it will work ( i just don't want to brick my kindle). I got it unrooted but that is about it

Ignore that post; dangerous to futz with build.prop on this device. Try the following:
- reroot (yes, this is necessary)
- browse to /system/priv-app/ and search for DeviceSoftwareOTA.???, where '???' is likely 'bak' but could be any series of characters
- rename above file to DeviceSoftwareOTA.apk (this is the app {likely one of several} that handles OTA updates on FireOS v4)
- reboot again
- verify device is registered with Amazon
- check every 10-15 min for availability of OTA update; it will almost certainly be 4.5.5.1; allow the update to proceed

Thanks for the steps but I've tried that already. The kindle does find the update but only to 4.5.3. It downloads it and then does nothing. just stays there. It might give an error after a while and go back to where it checks for updates. I tried to side load the newest rom which I have manually downloaded from amazon, the tablet recognized it at first but then gave me an error. Is it crucial to remove the recovery in order to obtain the update?

If you are referring to Safestrap v4 recovery then absolutely yes. The app itself can stay but recovery must be removed (uninstalled). Lucky you didn't score a brick trying to update FireOS with Safestrap recovery installed.

I got to the step where everything is ready to go. the device has been reset to factory default, safestrap recovery has been uninstalled. the device got registered in amazon store and found an update 4.5.3 but it still can't flash it after downloading. I copied the newest 4.5.5 bin file to the tablet, but when i tried to hit update I got a message: unable to complete update because the update file is not valid. please try downloading file again. Right now I'm on the 13.4.5.2 rom which I got here http://forum.xda-developers.com/kindle-fire-hdx/development/hdx-stock-images-pre-rooted-updates-t2911954. Is there any other ideas on how to make update to work?

Four choices:
- Modify build.prop, attempt a downgrade and brick your device. Go ahead, I'll wait ...
tip: relying on uncollaborated guidance in a three year old post is rarely best practice
- Root, reinstall Safestrap v4 + CM/Nexus and leave well enough alone. Your device works. Upgrading is not all it's cracked up to be. Kinda like that sweet looking honey/hunk in the bar on a foggy Saturday night.
- As above but reflash the Safestrap compatible (no other) version of FireOS 13.4.5.2. Possible something didn't go well the first time around which is preventing OTAs from being processed correctly. AFAIK - OTA is the only path to 4.5.5.1 as Amazon only offers 4.5.5 for download.
- Attempt a rollback to 3.2.8 followed by an immediate upgrade to 13.4.5.2 as documented here. This path is both the riskiest and the one most likely to clear whatever is blocking OTA updates. If you choose this option be sure to follow instructions exactly including linked files (not others you may have lying around). It is essential WiFi remain off (airplane/flight mode engaged) during the brief window your device is on 13.3.2.8. If an OTA update arrives on this version the device will hard brick with NO possible recovery.

thanks for the solutions. I figured since it is such a pain to get it to work I'll just stick with Nexus ROM now.

Yep - that would be my choice too. Take note there are two Nexus builds, one for devices that have a 4.5.5.1 kernel, the other for older kernels. Try the former first as it as actively maintained and has a broader feature set. Links below:
