I've been restraining myself from posting this for a few days, while browsing and looking through threads trying to figure out my answers. Something is wrong within my device, something is corrupted. Phone keeps rebooting on phone calls, and my UI is trashed, its all zoomed out and looking crazy. Flashing different ROMs has not helped. I have taken under advisement to RUU. However, I just want to make sure there is nothing I need to do before RUU'ing to keep from screwing up the device. Here is my current status: the only things I have done to this device since I got it last may are root and unlock using moonshine. So I have had no OTAs that I know of. This is the RUU I have:
RUU_DLX_WL_JB_50_S_VERIZON_WWE_3.06.605.4_Radio_1.01.01.1112_NV_VZW_4.46_002_release_340974_signed_2
I just want to make sure I only need to run that, and don't need to flash anything else (such as hboot, etc)

UPDATE: I tried running the RUU. I got Error 158. I was watching, and I had the phone in fastboot already, and when the screen said Rebooting Bootloader, then Waiting for Bootloader..... I never saw the phone actually boot to bootloader again, it just stayed black. then the program kept running like the phone was responding, and eventually shot out Error 158. should I follow this method http://forum.xda-developers.com/showthread.php?t=2293919 and relock the bootloader and then RUU? and does anyone know if I will be able to unlock again after I do this?
UPDATE: This is my main question if anyone can help. I have the above RUU, and I got error 158. Since I used moonshine's method, I am under the impression I need to re-lock bootloader, and THEN RUU. However, can someone please tell me if I need a different hboot version or if I'm just fine to go ahead like this? my hboot is below. Manually boot the phone to the bootloader before you start and it will go. For whatever reason it doesn't do it itself so manually doing it will give you success.

error 158= wrong Hboot ; Solution *You need to Update Hboot to 1.54 in order to Run RUU 3.06

I started the RUU with the phone in fastboot. I'm going to try to upgrade to 1.54 then RUU

Ok, i post it because i think i've read it all and still no fix.
I have 1.5 hboot S-ON HTCUnlocked, custom recovery.
putting the ruu in the SD does not work (unlocked or relocked)
when relocked there is a security warning and the phone doesn't boot, only bootloader.
Stock Rom and htc update just end in the red "!" symbol because i have custom recovery.
I flashed a "stock recovery" i downloaded using fastboot, same problem.
So, does anyone in the same circumstances have succided updating the firmware, antennas, etc?

extract all the .img files from the RUU zip and flash them one by one using fastboot. That should work.
Try to flash in this order (you must be unlocked again): boot, system & userdata. let it boot then access again fastboot and then flash recovery.
lets see what happens.

Well I just solve the problem, i expect it help some people in the future.
I had tu relock it again using fastboot oem lock.
download the RUU exe format.
I couldn'y run the OS because relocked and I think the custom roms previously installed by myself messed something so I couldn't start "relocked"
So start the phone in Bootloader (holding down button).
Relock using fastboot oem lock
Once relocked run the RUU app on your computer (It wont read your actual firmware, antennas, etc... just tell you the ones it will update).
Follow the instructions and Voila, your are updated...
So the bottom lines are:
being locked (relocked) no matter what, and run the RUU app because it won't ask for the recovery image installed.

I think WWE_2.17.651.5 is the lastest update, you could also apply an older version, just search for it.
Also follow my instruction, i couldn't get my phone updated with other method

I am about to try this because I have been having the exact same problem. I have been trying to update my phone for the past 4 days and I am just about through with trying.

Tampered is normal. Did you try flashing it with a ph98img, or as an .exe
It happens after you unlock and install a custom recovery I believe. Are you re-locked when you try to ruu?

Yeah make sure your bootloader is relocked
i was in the middle of doing s-off when the phone froze and corrupted the mem card...so i tried redoing the whole process over and i havent been able to get past the hboot...im still unlocked and have access to recovery and hboot but i cant flash a rom and boot into it...i cant run an ruu but the phone is not technically bricked...its an odd predicament...i just flashed a rom and i still cant get it to get past the hboot
Have you tried re-locking the bootloader and then run the ruu?

i was hesitant to do anything else aside from the things i mentioned incase i was unable to get those back...gonna let the phone recharge and try again later tonight...any ideas are welcome in the mean time...and ill go ahead and relock it and try later

just run the latest ruu in hboot if you are s-off it will make the phone like it was from factory.
if you are s-on but unlocked you will need to do the "fastboot oem lock" then runn the ruu.
in both cases the ruu will nuke the internal storage. first pass with install a new hboot and stock recovery along with 2 other items, then the phone will reboot into fastboot and install the rest. i recommend that you have a 60% or more battery life.

Yeah I already got it up n running...between being unlocked and having a slightly dead battery I wasn't even being given the option to run an ruu...its running now but I still need to do s-off again, and of course my sd card is corrupted so ill work on that tonight
The same thing happened to me, glad I found your thread! Were you trying to s-off on the official ICS? That's what I was trying. I ran the .12 RUU and had unlocked, rooted and was trying to get S-off. I did the wire trick I was getting an HTC logo instead of Jbear logo. Right now I just relocked and it's running the RUU so I hope I can get back up and running.
I guess I should have just S-offed before running the ICS RUU

Hey, I didn't have to do this, but if you noticed, controlbear makes backup copies of stuff like boot.img (forget what else) and puts them on your computer. If you crap out you ought to be able to fastboot flash boot [bootimgfilename.img] and be able to boot up your phone. I saw where one person did this to get going again after a failed soff.
Yeah well happened in my case is that I was already s-offed and then I flashed the ruu to go back to stock...then I relocked my phone cuz I'm having an issue with one of the pins on phone (the terminal that connects to battery).so I was gonna get it exchanged, I wanted it to look absolutely stock...so the "relocked" status wasn't good enough, so someone suggested doing s-off again so it will show locked in hboot...well that's where everything went wrong, the phone froze at the wire trick and then the entire memory card was corrupted which is normal...so when I went to redo the whole s-off procedure again I kept rebooting into the blank screen with the arrow...could not get past that, so I resorted to putting my real sd card in...I had no backups for that card cuz that one has my actual personal stuff so I figures it'd be ok since I used a spare to do s-off anyway...well upon powering on the phone it was also corrupted...so Ive actually given up on s-off and getting my phone restored and replaced for a while...I don't know why s-off has always given me problems I've watched and read every vid or post about s-off and am in no way tech illiterate but I've always had problems with s-off (i lost all my pics and documents the first time I did s-off even thought I backed everything up cuz the only way to get it right was to eliminate every file in the folder and rerun it again cuz it didn't back up my sd correctly If at all)...its beyond infuriating
well that didnt last long...i just unlocked and s-offed my phone again, only to be returned to s-on and fully locked (indistinguishable from brand new in box), as i originally intended...im gonna miss this particular phone but it has to be done
Gave my Evo 3d to my borother a few months back, but hes come back to me today saying it was stuck in bootloop.
He booted into the bootloader and did a factory reset, but it was still stuck in bootloop.
I tried to get into recovery to flash a new rom, but when i select recovery from the bootloader, that also goes into a bootloop.
Used adb to push the recovery onto the phone again but still just keeps bootlooping.
Not sure if its important but the phone is s-off. Any ideas on whats going on or how to fix?

You didn't say what version of the Evo 3D you have (CDMA or GSM) or what bootloader is installed, but if it's a Sprint CDMA model I would recommend flashing the Sprint ICS RUU.exe from Windows after putting the phone into fastboot mode in the bootloader. You may have to relock the bootloader with the "fastboot oem lock" command first if it shows "***UNLOCKED***" at the top of the screen.
That will fix a lot of problems, especially when the phone is in an indeterminate state. After flashing the RUU you can HTC unlock, flash a custom recovery and then root the stock ROM or flash a rooted ROM.

^^^^ He got it first with the short version.
No, you won't lose S-OFF and won't need to relock the bootloader first if that's your current status.
And if it's a GSM model everything still applies, you just need a different RUU, which you already have.

Error 170 means that the RUU utility can't find the phone. There's a readme.doc for running the RUU here, and the error codes are towards the end of that.
Are you putting it into "fastboot" mode in the bootloader, which should change to "fastboot usb" when it is connected to a computer? You also need to have the HTC USB drivers active, which can be accomplished by installing HTC Sync.

adb push into a bootlooping phone?
do you have the drivers for the phone? (if not download and install HTC Sync as they come with it)
did you try flashing a custom recovery img through fastboot? (go to bootloader select fastboot, connect USB, type "fastboot flash recovery nameOfYourRecoveryImg.img")
Yeah have tried installing custom recovery through fast boot again, and im in fastboot ok when trying to run the RUU. Also have HTC Sync installed from when i had the phone.. Drivers seemed to be ok as i was able to install the custom recovery with adb, alhough i still got bootloop after that.
Ive a few other things to try when i get home from work. Im sure it shouldnt be too difficult to resolve. Hopefully anyway.
Well this isnt going smoothly at all. Used another PC and was able to get the RUU to run.
However it now comes up with Error 131 Customer ID problem. The RUU was for HTC Europe (non branded). The phone was sim free. When i posted months back my cid was HTC_001. I checked it here: http://forum.xda-developers.com/showpost.php?p=22394863&postcount=172
However now when i get my CID it is showing as CID0202. Any way i can get an RUU to install? Any idea how a CID could change?
EDIT: noticed i used a different command last time.
fastboot getvar cid shows CID0202
adb shell getprop ro.cid now shows device not found (previously showed CID_001)
Thats typing both commands in same window, one after the other. One gets CID0202, the other device not found???
There's a change cid command in fastboot, fastboot oem writecid or something along those lines... however i believe the cid check is in the updater scripts of the ruu in which case a fastboot flash of all the img files should avoid it entirely... i don't remember the procedure for extracting the img files from the ruu but i believe ur supposed to run it and then dig through your temp files...

Need to bump this as im not getting anywhere. No RUU's will install, error 31 customer error. Have flashed recovery via ADB and through PG86IMG.ZIP, but still cant get in to recovery as it just continually reboots.

Got bigger problems than i thought. Finally got an RUU to install after 4 days of trying, by first using fastboot oem writecid 11111111. But when the phone rebooted it was back bootlooping again. Is there any hope for my phone?

You need to stop cross posting in the d3rp Q&A thread and your own thread if you want more help. Cross-posting is stongly discouraged on XDA and it's really confusing when you are asking for help in one thread while getting different advice in another.
It sounds to me like all the CID changes on your phone have left it in an indeterminate state and I'm not sure at this point what is the correct CID and RUU for your model. If you could detemine that, set the CID correctly and flash the right RUU the phone should work, but it might be too late if you have in fact been flashing firmware that doesn't match your phone because the right RUU may not back that out properly.
That's all I can offer at this point, but since I don't know the history of your phone and how it got the various CID's you have reported, you will probably be the one that needs to figure it out.

Hey guys. Yes I have searched, but have only found instructions so far for going back to stock gingerbread.
I need to send my amaze in to get fixed under warranty, and I'm assuming they won't like it if I send them the phone with an unlocked bootloader, rooted, and CM10 installed, and that's what I need help with. How can I reinstall stock ICS, unroot, and lock the bootloader of the amaze? And will it show as "relocked" or "locked"? Of course I plan on putting the root and unlocked bootloader and CM10 once I get it back, so I'm looking for a reversible way to do this. Thanks in advance!
Just to be clear. For your bootloader to say 'locked' you'll need to have S-Off and NOT unlock the bootloader again. But once that's done just flash the ICS ruu. Here's a link

No. That's the only way for it to say "locked" if you relock it it will say "relocked"
So if you really want it to say "locked" you'll need to S-Off first. Then flash the RUU.
Once you get it back you can do as you feel.

What makes you think you will be stuck with anything? Everything u need to do is reversible and then re-doable.
Im just not really sure which part u think would make it stuck. Let us know and im sure we can explain the cofusion

I've tried relocking the bootloader, and using the AllinOneToolkit by Hanson to install the Factory RUU but it keeps failing after the HTC boot logo appears and the zip RUU file gets transferred to the phone. Keeps saying Failed with 12 signatures or something.
Then I read with S-ON that you had to use Hanson's tool and perform Fix Mainver, but that doesnt do anything either. Keeps saying, error cannot find device.
So, how do I go back to Stock?
Would appreciate any help!
Boot into fastboot, type "fastboot OEM lock" in the command prompt. Put the appropriate RUU on your external SD card and rename it PH85ING.zip and boot into bootloader.
Edit: its PH85IMG.zip. sorry about my fatty fingers. Also, make sure to check the md5 sum on the RUU. I always had trouble with the download.

Fails after htc boot logo? If you are flashing an RUU, you should never see any boot screen until after its done flashing. What carrier are you on and what steps exactly r u fillowing to flash the ruu...?

its not the actual boot logo but that black screen with the HTC logo that I think is part of the stocky recovery maybe?
I have yet to try the PHS.IMG method in fastboot.

Lol what do you mean you havent tried it? Flashing the PH85IMG.zip though the bootloader is the ONLY way to flash the RUU.

noo it has nothing to do with what carrier you're using at the moment...
Alright, I hate having to post a new thread on this topic, but I cannot find any solution, or anything close to a solution for this. my problem is that i cant get back to stock from the newest firmware. I had unlocked, rooted, and gotten s-off all before the update. After that, i flashed a few things, and ended up updating to the newest radio and firmware number (2.04.605.2, and 1.01.01.0110). now i want to go back to stock and start fresh, but nothing seems to work. Trying the RUU method only gets me to a 158 Image error about 3 minute into the process, and i cant seem to get back to 1.15. any help is much appreciated. I've been at it, searching for 5 days. if i missed something, or missed a thread, please let me know, thank you in advance
Flash the ENG Hboot then you will be able to run the RUU. Making sure you have the bootloader drivers installed. I generally boot into the bootloader and verify the drivers are loaded BEFORE running the RUU. I can say I have flashed the RUU numerous times so I know it works with the ENG hboot.
Yeap zone is correct, flash the Eng RUU or relock your bootloader either way and then run the RUU. It won't run on a stock hboot that's unlocked.

ENG Hboot & unroot
I'm having the same problem as the op. I downloaded the updates, and i'm trying to unroot my device and put it back to stock (I broke my screen and I want Verizon to fix it, without giving me grief about my software). I have the RUU and Its giving me an 158 Image Error. Sounds like I need to flash eng Hboot and then run the ruu.
Two Questions:
1. Where can I get / download eng Hboot for my DNA?
2. Which version of Hboot do I need? I currently have hboot 1.33.0001.
