I read the wonderful "Don't Panic" sticky, but I think I need a more drastic fix.
SHORT VERSION:
TWRP keeps saying, "Unable to mount /sdcard", and "unable to mount internal storage". I can't boot past the HTC screen, and my attempts to flash a ROM have failed due to those errors.
LONG VERSION:
I tried using TWRP 2.2.1 and Cyanogenmod 10 today, my first time flashing a custom ROM.
My phone (HTC EVO 4G LTE) was already rooted using the plain ol' HTC bootloader unlocking method.
I booted into recovery and performed a backup first. That appeared to go okay, and verified a backup existed on my external SD card.
I booted back into the OS and used ROM Toolbox to download the ROM and gapps thingy, wiped the cache and dalvik's cache. The phone rebooted and started wiping the cache and stuff. I went away for a bit.
When I came back thirty or so minutes later, the phone was all black. No lights at all despite being plugged into the AC adapter.
I came back about thirty minutes after that, and the same thing. I rebooted. The phone went to the HTC screen and then blacked out again. I rebooted into recovery. I figured the ROM didn't get flashed right, so I went to flash it again. I used the file manager in TWRP and saw that the directories /sdcard /sdcard2 and /sd-ext were completely empty, except /sdcard had a TWRP directory, which was empty. I didn't see any of my storage!
I set up ADB on my Mac and tried mounting my sdcard. No dice. I tried using fastboot to flash a ROM. Everytime I tried to do that, I got an error in TWRP, "unable to mount /sdcard", "unable to mount internal storage". I tried using fastboot to flash the .zip on my Mac, and tried using "adb put" to move the .zip to the phone and install from there. Same result.
Any ideas?

this seem to becoming a more frequent problem maybe something TWRP is responsible for i came to this same obstacle when trying to flash a new rom like a week ago. i tried all the nifty moves i could but no luck even went to sprint looking for answers ultimately i was forced to format (with windows) both internal and external SD. end result. yep u got it i lost everything almost 26gigs of music and files i had backed up no where else. so i formatted (heart broken) got over it now i back up files daily on my laptop thats the only way ive come to see out of this one. but check around forums a solution might work for i saw a couple didnt work for me tho.
JB ROMs rewrites the addresses of your SD cards, good chance the internal is corrupted. Boot to recovery and go to mount/mount USB storage, if you have windows then as soon as you plug it in windows will tell you to reformat, if you have Mac or Linux manually reformat the card

Tried reformatting, but that didn't seem to help. On #twrp on freenode, they said it looked from my recovery.log that the internal and sdcard storage is hosed, and the maintainer of TWRP said to RUU back to stock. SIGH. . . .
So, since I don't have Windows at work, going to try these at home tonight:

Well that sucks, I'm lucky that I haven't hosed my emmc yet. I can't decide whether to stay on aosp or not, must have flashed back and forth a million times by now

If you're able to access bootloader, you can recover from this issue(been there myself). You'll end up losing all your ad card stuff but you'll be back up and running. Download the newly released 3.15.651.16 RUU, get into bootloader on phone, set it to fastboot, connect to computer, relock your bootloader (it can be unlocked after ruu app), run the RUU, and you should be back and can reunlock and install custom Roms etc.

I had backed up my sdcard so I didn't really care about not losing it. I think the problem was at least partially caused by having it encrypted.
I ended up flashing to stock and it let me format the sdcard from within the phone - that did it.

This just advice just saved my life. I thought for sure I bricked my phone, and right before memorial day weekend toooo! I did run into issue with the latest Lollipop RUU. I had to resort to using the old Kit Kat RUU I had saved to my computer from before. It worked like a charm.
So i rooted my phone using HTC amaze All in one tool kit by hasoon and everything was going smooth until i tried to change fonts using font installer, nothing happened so i got on aptoide and downloaded ROM toolbox pro and used its font changer, which i think use the same font changing program , and changed my fonts, again nothing happened. i started messing(by messing around i mean typing and looking for new keys on the keyboard) around trying to figure out what went wrong and my phone froze. i shut down my phone and when i rebooted i couldn't get past the android 4g t-mobile screen. can anyone help?
EDIT-i can still get to bootloader or that menu you get when you hold power and down volume. in my panic to fix my phone i restored to factory setting but that, i believe, only made it worse as now i couldn't mount my sdcard. when i saw i couldnt do that i looked it up and found a post that said to format my sdcard and so in a desperate attempt to save my phone i did, not stopping to think that it would erase everything on it. i do have a back up of everything that was on my phone prior to this.
Just restore a nandroid, like most people would do, and you'll be good. If all else fails then you'll need to flash a Rom and if that doesn't work for you then you'll need to flash an RUU. Read the Amaze Bible if you're stuck.

He formatted his SD card. Isn't that were the backup is placed?
if you can get to your bootloader check to see if you can get to your recovery, if you cannot get to ur recover go back to the bootloader to FASTBOOT and use the toolkit to flash 4EXT recovery then proceed to try and factory reset your phone, or check your system file settings or repartition your sd card, you can use 4EXT's mount usb feature to connect your phone to the PC while your phone is in recovery to place a ROM on your SD card

Trying to install MeanBean...I updated the Firmware to the latest, have the phone unlocked and rooted (it says TAMPERED and UNLOCKED in the recovery screen) and did a factory reset. I have TWRP installed and can still get to the TEAMWIN RECOVERY project interface...
I was installing MeanBean and it just stuck on about 75% and stayed there. I waited for about ten minutes and figured something must have gone wrong so I didn't really have a choice but to shut it down. After starting back up when I tried to install again it said "FAILED" in red letters. I decided to re-download the ROM but now when I mount the SD CARD from within the recovery menu (to transfer the new file) my computer says that I must format the drive before I can use it and won't let me see the contents of the sd-card. I didn't want to do that for fear of bricking...so I don't really know what to do from here. I should add that I don't have a backup made in TWRP (I got the phone yesterday and hadn't installed or done anything to the OS)
Anyone know what should be done at this point?

I'm updating this for anyone else who runs into issues like this: I went ahead and formatted the SD card from within windows, then relocked the phone, reinstalled the RUU and I'm back in a workable mode

I had the exact same problem. I just reformatted the internal storage and that fixed a lot of my issues. I can't however get the RUU to work. Can't seem to find my phone, neither can HTC Sync. Is this just a problem caused by MeanBean? I use to run MeanBean but then switched to CM10 for a few months and was going to switch back. Never had an issue like this before.
Hello. About a month ago I rooted my phone using this guide http://forum.xda-developers.com/showthread.php?t=2189646. Everything went great until today. Few hours ago I received a system update (about 10MB) and installed it. And since then every time I reboot my phone I get ClockworkMod recovery menu showing up on my screen. I tried pulling out the batery, I even made a factory reset, but it still gives the the CMW menu every time I reboot my device. Maybe anyone of you could tell me what to do? Cause I'm kinda panicking right now
P.S. I also made such thing as "format /system" through "mounts and storage" menu. I don't know why, but I think it should be mentioned (just in case)
Well the best way now will be to download CWM backup for your phone from this thread, send it to sdcard and just recover using CWM. This way you won't lose root and CWM. If it won't work just you use KDZ to flash ROM from scratch. Check this thread too: http://forum.xda-developers.com/showpost.php?p=36352031&postcount=3

If you share which L9 variant you have, which country rom you flashes or stay, then people will be able to help/tell you, which cwm backup is suitable to your L9.
Update your profile with your country name (if u r not criminal? lolz kidding), operator name and add L9 variant in your signature. at the moment, your profile is completely naked.
Ok, I managed to deal with my problem. Firstly I tried using cmahendra's guide on installing KDZ offline, but after aproximately 2 hours of complete failage I began searching for other ways. Luckily user edriskus answered to the message I sent him and suggested me doing the same as I tried to do before. He didn't mention the offline version so I thought that I should try the "online" one. And it worked! I don't really know how it sounds in numbers, but now my phone says that it has 4.1.2 version of android (hopefully the latest one available for L9).
There are few questions left:
1. Does my phone remain rooted or I have to root it once again (since I have reinstalled the software)
2. This one confuses me the most. Now my phone feels very laggy (which is strange, because it was factory reset previously and there aren't any apps here). I feel it from sliding menu/photos, surfing the settings and other similar actions. Maybe there is a way to fix it?
Nevertheless, my phone is working again and I really appreciate your help, fellows!

I tryed to flash after this method http://forum.xda-developers.com/showthread.php?t=2085344 but i stuck at step 6 at the Post-JB issue, since every time I reboot my phone I get ClockworkMod recovery menu. I finally succeded! Thx Feero21 for helping me, i followed your advice by trying it in "online" method. But there is one more problem, after i flashed the Rom i saw that i have only 1 gb free on my internal storege and another one occupyed. In total i have 2 gb instead of 4. I belive that the old Rom wasnt deleted and its still their and its occupying my also too small space. what can i do? Thanks!
OK. So. I've been encountering nothing but problems since I received my Huawei Ascend P2 phone.
To begin with -
I bought the phone for the sole purpose of it being 4G/LTE. When I got it, I couldn't figure out how to get it, so I contacted Three and was told that my phone had to be 4.3 - it was running 4.1.2.
When I tried to get 4.3 through whatever means, it stopped working.
I tried to do a complete factory reset and now I'm getting many more problems -
I keep getting errors that anything Google related has stopped working (especially Google keyboard).
My contacts won't auto-sync, despite me having my Google account synced up with it.
The icons at the bottom of the screen (back, home, settings/whatever else) won't light up and the home button isn't working.
It's a different loading screen than normal - before it was a Huawei Ascend splash screen, now it just says "Android".
It is not being detected by my laptop whatsoever.
When I try and download anything using a browser, it says "starting download" but nothing works.
So, anyone have a solution? It is driving me demented. And all because I wanted bloody 4G.

I think we need some more info on the attempt to upgrade via whatever means. What steps / guide were you following? Did you install a custom rom? If so, what rom and what recovery are you using? From a few minutes of searching around, it seems that there is no official 4.3 update for the phone, and just being ON 4.3 itself will not automagically enable LTE service as this has much to do with the radio/baseband used as much as the actual operating system and that will not get updated via a custom rom (99.999% of the time unless SPECIFICALLY stated). Seems like they are working on an LTE capable update/patch for 4.1.2 soon according to this: http://forum.xda-developers.com/showpost.php?p=50495570&postcount=162 (that post is part of a thread about your phone on a UK carrier so should be pertinent).

I'll talk you through the steps I took as much as I can.
Firstly I rooted the phone using eRoot, which did work and allowed root access.
I then downloaded P2Tools, which was obtained from this thread and it allowed me to put CWM on it.
Once this was done, I attempted to put CM 10.2 on it, but kept getting a Status 7 error. Some quick Googling later, I discovered this video, which showed how to solve it.
When I followed the steps on there, I kept getting a Status 0 error.
That was when I started to encounter those problems, so I wiped everything I could, even system, but that part failed.
And that's about it.

K. So you are still on the stock rom, rooted, with CWM recovery (which version?), and after you attempted to wipe /system, it failed to wipe (what was the error message there?), and now it boots and has all kinds of weird glitches? First thing to try would be to go back to CWM and wipe Cache and Dalvik Cache and then Fix Permissions. This MIGHT help. If it doesn't then you will just probably want to throw a stock rom back on there to get back to running right again.

I've wiped the Caches and Dalvik Cache and can't seem to find "Fix Permissions". I did some searching and it seems this feature was removed from CWM? I'm running v6.0.3.7 of CMW.
When I try to format the system, it says
E:format_volume failed to unmount "/system"
Error formatting /system!
Weird, they did take it out. Koush said it was a placebo. Which normally I would take as law, if it weren't for many times I have ran fix permissions and it caused apps that were force closing to not force close anymore. But whatever, if you wanna try, flash this: http://db.tt/9aWdF8Rp. It'll either help fix something, or do nothing. Won't make it any worse. If it can't mount to format though, it's either or a bug with CWM for your device, or after factory reset, reboot recovery and try again. But you really don't want to wipe /system unless you have a nandroid or another rom on your storage that is accessible.
Thanks so much for your help so far. Took me ages to figure out how to get that file on my phone, but I managed to do it. I then installed zip from recovery mode and chose the fix_permissions. Then reboot and it asked me to root the phone again. It's now upgrading and hopefully it will work now.
It is still saying "Unfortunately, Android keyboard has stopped" and other things. What do you reckon I can do now? You're right, it seems like I can't make it any worse.

Ok. So looks like first thing to check is to see WHICH version of the P2 you have because it seems like there are at least 2 different sub versions of it. The P2-6070 and the P2-6011. Please go to Settings / About Phone/Device and let me know all the software info there.
Also, have you unlocked the bootloader (I'm guessing so since you have recovery, but just checking)?
I found some more info on how to get you back up and running, but want to confirm the system version so everything goes as smoothly as possible. There's a new update that just came out not too long ago that people are saying LTE works on, so hopefully we can get you to that point.

Yes, the bootloader is unlocked.

Need to be on stock recovery. Unpack the rar file and put the whole folder /dload on the root of the SD card. This has the update.app that should take care of everything for you. Turn off the phone and turn it on by holding Vol + and - and Power together. This is supposed to boot it into the mode to take care of the update for you.
If you don't wanna go back to complete stock, I have another way that involves switching to TWRP recovery and restoring a rom via nandroid restore. If you'd prefer that, lemme know and I'll shoot you the links.

Need to be on stock recovery. Unpack the rar file and put the whole folder /dload on the root of the SD card. This has the update.app that should take care of everything for you. Turn off the phone and turn it on by holding Vol + and - and Power together. This is supposed to boot it into the mode to take care of the update for you.

If I ever meet you I definitely owe you a drink. Did exactly as you said and everything (from what I can tell) is working.
It's one of the things I love about Android phones - unless you do some serious damage to it physically, for the most part it is fixable.

I'll talk you through the steps I took as much as I can.
Firstly I rooted the phone using eRoot, which did work and allowed root access.
I then downloaded P2Tools, which was obtained from this thread and it allowed me to put CWM on it.
Once this was done, I attempted to put CM 10.2 on it, but kept getting a Status 7 error. Some quick Googling later, I discovered this video, which showed how to solve it.
When I followed the steps on there, I kept getting a Status 0 error.
That was when I started to encounter those problems, so I wiped everything I could, even system, but that part failed.
And that's about it.
