Hey,
I bring you the Galaxy S6 S Health app for Lollipop.
Link
Put it in priv-app/S Health, But first back up and then erase the old one.
Rename to SHealth3_5.apk and change permissions to rw--r--r
Done!

Bugs:
Tell me, I didn't see any on SM-N910F BOC3. Even pedometer works!
NO HEART RATE SENSOR or UV SENSOR on some devices.
Reported:
Lock Screen is not right size.
S cover isn't displaying steps.
Reported on Note 3:
FC when launched but it doesn't close.
Doesn't let to make an account.

In a hour I will upload my launcher with no black status bar.
not sure about S5.

Put the apk and lib folder into priv-app/TouchwizLauncher directory.
The other folder into lib folder in system, not into the lib folder in the zip.
And remember to fix permissions.

I don't know. On my note 4 it works great!
Maybe Albe can convert some things to png.
Maybe delete data but first back up the data.
Soon I'll upload my Health service. Maybe that's the solution.

App is work well but all sensor is not work.. iam on note 4

Thanks Buddy!!! Its awesome. Just one suggestion, can you change the 3 dot menu icon to black or blue like music player had it blue. Also, does your launcher have transparent statusbar even when a wallpaper from SecWallpaperPicker is applied?

There is no heart rate monitor
