This is my use case that probably occurs 25% of the time.
I'm using an unrooted fully stock device and have the Google Nexus 9 Case - MoKo Ultra Slim Lightweight Smart-shell Stand Cover Case.
On a typical night I turn on the flashlight through the quick pull down menu, close the cover and use the flashlight to find my way to bed after turning off all the lights.
About 25% of the time, when I goto turn off the flashlight (again from the quick pull down menu), it doesn't work and I cannot get the flashlight to turn off unless I power down the device. Also, the flashlight can take anywhere from 2 - 10 seconds to turn off AFTER the screen goes black from shutting down.
Has anyone else experienced this issue or does anyone know of a root cause or work around ?
Did you ever find a solution here? I just started experiencing this on both of my stock, not rooted WiFi N9s. Exact same scenario. Flashlight on to navigate to bed and then it refuses to turn off and stays on a few seconds after the screen goes black during shutdown.
I thought I caused my own problem because it only appeared after I dirty flashed the factory image to go from 6.0.0 to 6.0.1 but from your post it sounds like it has existed a lot longer.
I found that someone has already opened this as an issue on the AOSP tracker a few hours before I first started digging into this. Will be submitting the requested debug info on both my devices after I get home from work today.

This might prove trivial, but I have not had success yet.
When I go to navigation mode, my display will dim after ~15 seconds (not turn off). Periodically, it will go back to the set brightness after ~30 seconds. This is while a charger is connected.
What setting do I change to cancel this behavior? I find it a feature when not connected to a charger or when not in certain applications.
....One thing to note and will likely work for me...
Testing in "Car Home" appears to resolve the problem.

I am rooted, and running Stock GB.
I'm sure I'm missing something obvious (or did something stupid). I've installed a few of the 'Flashlight' or 'Torch' application onto my Rezound (Nexus Torch, TeslaLED, etc). I do like using my phone as a flashlight in many situations.
However I've noticed that when the screen itself turns off (either by timing out, or manually), the flashlight turns off too. This is very bothersome as I then need to keep turning it back on and/or I accidentally keep hitting icons on my home page(s).
Now with my old D1 this didn't happen. I don't know if maybe I've installed something, or changed a setting that's causing this to happen, or if its a Rezound issue or not.
I could have sworn this didn't happen when I first got my phone (about two months ago), but since then I've wiped it twice (I had that silly reboot loop/4G issue until I learned to remove the back or change the connect to 4G on boot setting).
I would appreciate your thoughts/suggestions

I use the stock flashlight app on Nils' Business ICS and it doesn't do that to me. Just tested Tesla and that works fine too.
Maybe you should try a wipe/reflash.

When I got my Rezound, the HTC Flashlight didn't exist for it.... so I looked and ended up using

When used as a widget, I use the round icon and if I click it the light goes on and I can turn the screen off and it will stay on.
I use it a lot, especially at night. Putting the phone into the dock at night can be hard in the dark, so it turn the light on, turn off the screen so I can hold it by the screen as I put it in the dock. turn the screen back on, and turn the light off. Note that i am using the widget, not the app itself which is a squarish looking icon with a round icon on it. if you use the app, it will turn off when you turn off the screen
It has other modes, but I don't use them. I kind of like it better than the HTC flashlight now as it is just on and off without it pulling the app up.
I use https://play.google.com/store/apps/details?id=com.katecca.screenofflock to turn the screen off when I set the phone down. 1) to conserve power and 2) prevents me activating something as I pickup and set down the phone.
I am GB stock rooted S-Off, so I would think it would work for you.

Thanks everyone for their help and advice on this minor (yet annoying) issue.
I'm not sure how exactly I fixed it but I did. I think I may have had a 'battery saving' program installed that was inhibiting the flashlight function when the screen was off.
I removed a few programs and checked my settings in the the one(s) I had left. I also tried tiny flashlight +LED (per RoryHe suggestion), and found it works perfectly.
I did find like most of you mentioned that it does turn off for a second when you turn the screen off, but then comes back on (much better than before). I can live with this, and I love the 'shake to turn on in Lock mode' that tiny flashlight has. . . except when I'm playing with my 2 1/2 yearold twin boys and the light randomly turns on. But it doesn't happen often.
Anyway, I'm all good, and thanks again for helping me resolve this issue.
What would I do with out my Rezouding Friends.

Ok, so I've found the section in 'Display' and switched Smart Stay on, but it doesn't seem to do anything. The small eye icon never appears on the notification bar, and the function doesn't work. I've noticed that with auto-brightness the screen is pretty much always really dim, and I noticed that smart stay won't work when the room is dim - does the phone think the light's too low and never switches the function on?

Right i too thought smart stay wasnt working. I too never saw the eye.
BUT... Having played with it some more heres how it works well in my eyes anyway
The eye icon only appears when the light is at the end of its duration setting.
For example i set mine to 15 seconds beacuse i wasnt sure it was working.
So every 15 seconds if your looking at your phone the eye will flicker on.
If its set at a minute you rarely see it as 60 seconds is a pretty long time to look at the screen with out pressing ought.
I hope this makes sense. Just set light time out to 15 secs to see what i mean.

I thought it wasnt working properly since every now and again id spot this icon flashing up and going just as fast as it came on, but i figure, its a battery saving method, so it wouldnt permanetly be tracking your eyes with the camera like the xbox kinect for instance, it comes on for a split second, if it recognises eyes are open and looking it stays on if not it turns off... i tested this, changed screen dim to 15 seconds for test purposes, staired at it whilst on web browser and every 10 seconds the icon flashed up and screen stayed on, closed my eyes counted to 10 and the screen was off
I am unable to wake the phone during a call. As soon as I answer a call, or press the call button my screen turns off. I can not wake it with proximity sensor, double tap or power button. There is literally no way to turn on the screen once a call has started. It was random when it started. I had been using Vanir for a while and then one day it started on my phone, and never left. I have even flashed my OG nandroid stock and the issue was present there after flashing to that.
I have swapped ROMs, AOSP x4, BarRin, Stock, you name it, but to no avail. I have cleared everything off my internal, wiped the internal memory, sideloaded a ROM back from scratch and booted up plain jane... same issue.
I am attaching a link to a logcat incase anybody has any ideas. I'm simply lost with this, and it is a major issue considering every time I make a call I have to be ready to take the case off so that I have faster access to pop the back off and battery pull. This is not a way to live.
PLEASE if someone has any ideas on how/where to go about fixing this, I will be grateful.
Not sure if it matters, but after a call ends, I need to wait about 5 or secs before the power button will be recognized. After it's first press it seems to first power off the screen, as though it is awake the whole time, but black and inoperable. After the screen thinks it's off, the second press turns it back on. I have also tried other dialers too, but it totally seems like an integrated OS/Firmware issue at this point.
Are you by chance running Greenify? I was having some issues when sleeping system apps. What Xposed modules are you currently running?
No battery apps installed whatsoever. No battery saving modes enabled.

Sorry a bit tired. I just noticed you stated it still happens after you nandroid back to your previous backup. Do you have a screen protector or anything on it?
Try going to Settings/Call and disable the proximity sensor and see if you have the same results
It's not the sensor though b/c it has 0 results with the double tap to wake and also the power hardware button itself. Holding the button, pressing it quickly, tap it multiple times in a row. Additionally, I do not have a screen protector.
I've been documenting my struggles with my lock screen in another thread. But I just had a major breakthrough and didn't want this to get buried.
To recap, my lockscreen has been a buggy mess from day one. The fingerprint sensor barely works, the fingerprint icon randomly disappears, and the gray border that's supposed to appear around the fingerprint icon simply doesn't show up. The lockscreen doesn't light up when I pick up the phone either (it stays dimmed). Also, wallpapers don't show up on the lock screen, and the big ugly clock sometimes disappears or moves itself to the top-left.
I've tried just about everything to fix this. As a last-ditch effort, I added a new user profile only to find that the lockscreen works perfectly well for this new profile (but still not for my main one).
Here's my main profile with the unusable lockscreen:

So there's clearly some obscure bug that most people aren't experiencing here. I'd love to just figure it out so I can move on.
When I didn't have the "always on display" turned on, the phone was very consistant about turning on properly when I picked it up. I ran it this way for 3-4 days without any issues.
After reading some articles on the P6, I realized that it has an AOD and that it was turned off. So I turned the AOD on while leaving all the other settings set the same. At first, the finger print icon wouldn't appear consistantly, but that is because I had adjusted the animation scale settings in the Developer settings. Changing that back to stock fixed the issue with the finger print icon not showing up.
That beings said, I have experienced the same "unresponsive" screen many times over the last couple of days that I have had the AOD turned on. The screen stays dimmed (or even turned off if I pull the phone out of my pocket) and the finger print reader is inactive (even though the icon is displayed). The screen is suppose to turn on when I lift the phone up, but it doesn't always do it. When this happens, I have to either manually turn the screen on using the power button, or double tap the screen to wake it. This is clearly a bug in the system somewhere. I probably need to report it to Google, but hopefully it is fixed soon.
After spending way too long on this, I've determined that the bug occurs consistently with these specific settings (although it may also occur for similar ones):
Lock after screen timeout: 30 minutes (or anything high enough to prevent it from locking between tests)
Power button instantly locks: disabled
Lift to check phone: disabled
Tap to check phone: disabled
Always show time and info: enabled
Replication steps:
1) Enter the above settings
2) Press the Power button to go to the lockscreen
3) Set your phone down for a couple seconds
4) Pick your phone back up and try to use the fingerprint sensor
Result: The fingerprint sensor won't work the first time.
I think all the other issues I had were actually stemming from this one issue.
