Really I'm just looking to get my phone back to a "stock" condition. I'm not sure I understand which RUU is right for my situation. I haven't updated anything in about a year so my software is a bit outdated.
Here's what I'm working with:
Bootloader unlocked with HTC Dev
S-ON
HBOOT 2.09
TWRP 2.7.0.8
Two ROMs currently saved
(stock) 3.17.651.5 <-- just goes black after a long HTC screen
Cyanogenmod 10 (I think) <-- boots fine
It appears the latest on HTC's site is 3.17.651.9. Will that do the trick? Other guides here point the same page but specifically refer to 3.17.651.4. Is there a reason I would go with .4 over .9? Is there a some other option?

If you're looking to get it back to "stock", any Jewel RUU ought to get you there. Though, I do recommend using the most up-to-date version you can find. Newer ones have updates included. The newest one is 4.13.651.4.

I get to step 12 on the htcdev website and everything looks normal yet nothing on my phone changes (no screen asking me if i want to unlock the bootloader). I just got this refurb today and am desperately in need of titanium backup to restore some important info. Has anyone seen or heard of this problem? I would love to get suggestions on a possible solution.

Short Answer: off the top of my head, is the device updated to the latest hboot version? Probably will need the latest hboot version to complete the htc unlock process.
Long Answer: I don't have the HTC steps for their specific unlock method memorized. In order to help you, I logged into their web site, created an account and went as far through the steps as possible without completing their method as I'm already unlocked through the revolutionary method.
Step 10 is the step where the hash code from the device is submitted to HTC. I'm not able to see any step beyond Step 10.
The more information you're able to share, the wider group of individuals you'll allow to provide solutions. If you're able to share with us the specifics of Step 11 and Step 12, we might be able to provide more detailed help.

Im having the same issue unlocking my white evo 3d. came with the lastest firmware out of the box and now step 12 says successful and nothing happens on the phone, very frustrating ive tried getting new unlock bins and nothing im running windows 7 and am very familar with rooting other devices. Any advice would be wonderful

Your white EVO came with 2.3.4 out of the box? Mine didn't. Shut down the phone, remove battery, then boot into bootloader by holding volume down and power. Once in bootloader tell me the bootloader version. If it is 1.40 like mine was you can use revolutionary. You guys are probably stuck at that step because you don't have the 1.50 hboot and probably have a lower hboot. If it is lower than 1.50 do the revolutionary method.

I just wanted to chime in and say for anyone having the issue of the HTC Dev root method not working for them on steps 12 or 13:
flash error due to too many links
flash failing
flash hanging on sending Unlock_code.bin
flash appearing to send successfully, but still no disclaimer screen appearing
All of these problems were solved for me after by updating my phone to 2.08.651.3 via HTC software update in the Android OS.

I just got a new 3d yesterday, it came with 2.08.651.2 and I got hung up on step 12 as mentioned, it looked like it flashed but I got no unlock screen on the phone. I updated my phone to .3 as mentioned by bleaktrotter and I'm having the same problem...currently downloading the .2 RUU....?

Did u check your hboot version? If u have 1.30 or 1.40 its been suggested to run the "revolutionary" method to get s-off.

Nope. You need to I.D. your phones Hboot version first.
- Hboot 1.30 or 1.40 : use the revolutionary.io Full S-Off Method (Official RUU updates will kill the exploit).
- Hboot 1.50 : Your stuck using the HTCdev.com unlock method (S-On, Unlocked bootloader, mostly rooted). I used Zedomax's Guide from HtcEvoHacks.com but had some issues that weren't due to Max's guide and had to run the RUU (Rom Update Utility).

i hav same issue with my GSM version , anyone can fix that , it stuck on step 12
i'll found how to fix this problem and feedback as fast as i can
Ok after 5 hours of trying to unlock my bootloader, all you have to do is install the 2.17 RUU.exe and then go into fastboot and unlock using the htc method.

My phone is s-off on HBOOT 1.15.2222, Radio 1.12.11.1210 which I believe means that I've flashes captain throwback's latest full firmware but truthfully I don't really remember Also have TWRP installed. I'm trading this phone in through my work for a new HTC ONE and want to get it back to totally stock. I'm wondering what RUU I can flash or should flash to prevent bricking this phone. I'm just worried because my firmware and HBOOT don't currently match up and I don't know if that'll cause issues, especially since I couldn't find an RUU that contains the radio/firmware that I'm currently on.

I apologize if this is dumb, but I have been away a while, and am fairly new to this scene as is.
A few months ago, I flashed my EVO 4G LTE to 1.13.651.1 710 RD with root s-off and HTC sense. Since that time, Sprint has released OTA updates that flash the phone to the most recent HTC rom (3.16.651.3 I believe).
I would like to upgrade, even if I have to lose root, but naturally a full featured solution is better.
I can't OTA update, and that just puts me into TWRP, not to surprising. I would like to know how to get the latest update without bricking anything. As it's been almost 8 months since 3.16.651.3, you can sort of tell how cautious I am.
I wouldn't say no to a non Sense (pun not intended, but kept) rom, but I have heard those cause issues with various parts of the EVO 4G LTE's hardware, and I would like the phone to remain fully functional.
What is the best option. I'm leaving this fairly open for everyone's superior expertise.
IF you are indeed S-OFF and just HTC-Dev unlocked you can update your firmware manually then flash whatever rom you wish.

I'm still looking for the safest way to get to the latest HTC sense, or any android version that won't reduce functionality of my phone hardware. My brother tried Cyanogen on his phone a while back, but he went back to sense after issues with his voicemail.
If you want the latest firmware you'll have to go back to stock using RUU or get S-OFF and manually flash the latest firmware. With your setup I would go for S-OFF and manually flash the latest firmware. You can find three different ways to get S-OFF in the HTC EVO 4G LTE Original Android Development section (they're stickied at the top). A little reading should tell you which version will work best for your setup between Moonshine, Facepalm, and DirtyRacun.
Once you're S-OFF you can update your firmware on your own. Captain_Throwback has a link to the zip file in his signature (It's the link for the Evo 4G LTE Baseband: 1.12.11.1210).
You'll be S-OFF with the latest firmware and you'll be ready to flash any of the latest ROMs.
Best thing to do is get S-off, as stated above. You can use Facepalm or Dirty Racun to accomplish that. Facepalm is the easier of the two and takes about five minutes or so from start to finish. After that you can update your firmware and flash a JB ROM. If you click on the link in my sig you'll find instructions on how to update your firmware, as well as a link to Captain Throwback's firmware thread.
A few months ago I, posted a thread with a similar idea in mind however, the only answer I got was (I'm paraphrasing) "Why go back to stock,the custom ROMS are much better anyways". Well the time has come where I figure out the "why?". Yesterday I brought the phone into Wind for a check of why it's not sending MMS messages when the data is on (strangely enough it turns data on while sending the mms but, that's a story for another time). The people at Wind refused to look at my phone and told me to come back when it has stock firmware. (because, obviously I haven't returned to stock yet). Now from what I understand, from doing some research today on the forums, that the HTC Amaze (Wind Variant) has some weird software issues upon flashing a T-Mobile based ROM which I can link back to Speed Rom. So, I do the standard "Relock the bootloader and run the RUU.exe" and it just comes up with an error saying my hboot is the wrong version. The forums are pretty blank on ways to get around this (well besides going S-OFF which is a problem considering I need to bring my phone in to get the MMS issue checked. Also, I don't have that level of confidence in myself to insure I don't destroy my phone when going s-off). So, I was wondering if anyone has a fix for the issue. I'm confident that someone could possibly help me with my issue. (PS my current ROM is the latest Cyanogenmod 10 nightly, and I'm using 4ext recovery on the Amaze/Ruby).
Hope, I can get this issue resolved and thank you for helping me out with what ever problems I've had on my journey with this phone.
P.S: I didn't mean to double post the same question it's just I really needed this problem solved. I hope you can forgive me. (I fully understand if I can't return to stock under these circumstances.)
It's funny, I'm kind of in the same boat, except for 2 "minor" differences:
1) The "Smart Flash" on 4EXT never worked for me (and has consistently corrupted my internal SD card, but been able to fix it each time), so I have S-ON yet
2) I've rooted the phone itself and unlocked the bootloader, but the HBOOT is 1.93.0002, and from what I've seen, the latest firmware patch is .2222 although that really shouldn't matter
This is my 2nd HTC Amaze, and the first time I've gone through the rooting and that, took me at most an hour to complete, including time spent choosing a CMOD (I ended up with 7). This time around...I'm practically pulling my hair out because I cannot perform the S-OFF via Linux (LiveCD of course) nor starting from "square 1". I seem to be in limbo with 4EXT.
I installed Viper Amaze 1.7.2, but continually got stuck on the boot screen. Further investigation revealed that I needed to superCID the phone, which in turn would be to S-OFF first.
Additionally, I've downloaded and flashed the "stock ICS" ROM (since that was my stock ROM to begin with, naturally upgraded ICS), renamed to PH85IMG.zip yadda yadda. It recognizes and loads, but never "updates", more or less does anything after the loading bar on the bootloader.
So, just to try how much I've dug myself in with this, I've downloaded and tried 2 other ROMS, both saying "bad" (no status 7 error like the RUU flash).
I want to try out the Viper Amaze ROM since so much work and effort went into it, and it seriously looks awesome, but because my original tool that I used (HTC Super Tool) didn't require me all of these extra steps (and I didn't remember the program until now), I want to return back to stock so I can start over in a sense and complete those couple of steps that I inadvertently missed. I really feel like I'm missing something stupid.
Out of 7 Android phones that I've completed the custom ROM setup, it has to be this phone...
Anyways, tl;dr here's what I got:

I actually have the same HBOOT version as you! I have never met a device which had more difficulties going back to stock than the Wind variant of the Amaze. Anyway best of luck on your issue, and just try to remember me if you find a way back to stock. I guess we are on the same boat and the Wind isn't helping us out *Budum Tssss* . (BTW if you manage to make it back to stock I suggest you use hasoon2000's toolkit for the Amaze. It's a good toolkit for the Amaze.)

I managed to install the Viper Amaze. I went back, relocked the bootloader, got a new
ID token unlock key, and reflashed (the hell out of) everything. It actually worked.
As for the Wind Rom push back, try the EXE found here on XDA:

I tried, that EXE seems to only be for update from Gingerbread purposes or, was broke after I flashed a T-Mobile ROM on the device.
I have a rooted and soff EVO LTE and in wanting to up to 4.3 ROM I understand I need to update my bootloader. When I try loading a couple 4.3 roms my phone starts ups but reboots after displaying the HTC white logo screen.
I currently have hboot 1.19.0000 and if I understand correctly I need to update to 2.10 It also has Team Win Recovery 2.2.1 which I'm not sure if that needs updating or not.
It has been awhile since I rooted or updated anything and when I did in the past I followed instructions for the entire root process so I'm a little stuck in getting things patched up.
Can anyone offer me the path to proceed? I'm only messing with my old phone as I plan to reactivate it as I'm having terrible issues with my new HTX M8 in not having any data nor able to make calls (due to the Spark support in my local towers throwing chucks since Sprint doesn't have their towers upgraded yet.
Perhaps I should ask, am I on the right path in needing to update my hboot to update to 4.3?

I think never mind, after more reading I believe my problem is I don't have s off which appears to be required, let me get that done and I will see were I'm at......
You can run the RUU for the latest update (Android 4.3). That will get you the latest bootloader and put you on the new partition layout, also. You will only be able to flash ROM's based on the new layout. You'll need to update your recovery version, also. For the RUU click the top link in my sig and scroll down to the bottom of the first post. You'll need to relock your bootloader to run the RUU. Running the RUU does not require S-off and there are instructions in the post I mentioned that detail how to run the RUU.
