I lost TWRP not sure how but I did. I was going to flash my old OC1 to to tmobile so I could have a backup. I loaded all the roms and apks. to the phone and realized i was missing TWRP.
I downloaded TWRP in the ap store and tried to install it. It gave me root access and said it installed it. I turned on the phone and it failed to find he recovery. I tried to install over and over and kept trying previous versions. I think at around 2.8.7 it started booting only into down load mode. I can no longer get into recovery only into download mode. how can I get back to an OS?
Uhhh.. you are only able to flash/install twrp or any other custom boot with MDK and that was several revisions back.. There are dozens and dozens of thread stating this. your are going to have to use odin to reinstall your bootloader/system there are tutorials in the "Help & Troubleshooting" where this probably should have been posted for you to get a the best chance of help.

I was lucky enough that I could get the phone into download mode and I was able to ODIN a previous version of TWRP...specifically 2.4.4.0 and then the phone booted again and recovery worked. I've also seen issues with TWRP 2.4.3.0 and some ROM installs which go to black screen during install....forced to hold power to reboot phone and then rom boots...
So if you get the bootloop after installing the latest TWRP, simply use ODIN 3.7 and grab a tar file of either CWM or older TWRP and install to fix.

Well, I'm not seeing a recovery.log so they can have actual information on what's wrong. I'll be doing that today at work.
I cant get anything above 2.4.1.0 to work for me....g

Same issue here. had to revert to 2.4.4.0
So I'd been having some problems lately with my phone hanging on the Samsung screen on boot. Been running CM11 1/1/2014 nightly most recently. Decided to try using TWRP instead of CWM. That didn't seem to help my issue, so I tried installing CWM again. I used an app called Recovery Tools to download the touch version of CWM. After it said it installed, I tried to boot into recovery and got the error "Unauthorized software found. Please contact Verizon." (paraphrased a little).
Doing some research it looks like that error typically comes up when the bootloader is locked. Now, obviously I had my bootloader unlocked (well, Loki'd) because I was running CM11. Any help fixing this? I'm able to bypass the warning screen on boot if I hold down vol up + power like I'm going into download mode, but then hit vol down to cancel to boot up the phone. Once booted everything is ok, but any reboot just brings that up again. Using ROM manager to flash CWM recovery seems to work ok and it says its installed, but same issue. So does trying to flash TWRP from Goo.
If I were to run this tool that I used to root and loki before (http://forum.xda-developers.com/showthread.php?t=2301720) to install a recovery, would I be able to get everything going again? I think that somehow the recovery I installed wasn't Loki'd, although I picked the right one for my device.

The fix I found was by pure dumb luck. I ended up trying to flash a recovery with Odin. Odin failed but reboot back into the os. I then used the twrp manager app from the playstore (mainly because goomanager wasn't downloading the recovery at the time) to flash the latest twrp recovery and all was good. Even though Odin failed at flashing the recovery I still think it helped somehow. I haven't thought much about why but I might have to now.

You could probably try using ODIN to get back to stock MDK and then Re-Root and install a recovery after.
Edit: And then flash CM11 again.

I was actually able to reflash TWRP w/ their app from the Play Store. Still get a lot of boot hangs on the Samsung logo though. Seemed to happen less with CWM, but now I'm too scared to try to install a different recovery. I appreciate all your help guys.

My sister updated her MEGA OTA last month. She cant remember which version she was on. I am assuming it was 4.3. shes doesnt know much,
Her phone got stuck in bootloop.
Initially a got a hard time even getting into restore mode. however after a lot of goggling and trials and errors here is where i am today :
1- I can install TWRP and/or Philz recovery and boot into them.
2- I can enter download mode
3- I have tried installing many ROMS and different versions. I was only partially successful with 4.4 versions because I get a passed message in Odin however upon restart the phone enters boot loop again.
4- I have tried multiple wipe data/dalvik cache/cache and restoring factory settings but i still get the same results.
5- Only in Philz recovery i can mount the external SD card and copy zip files. I was able to root the phone that was (but im not sure if it still is). However all ROMs zip files I tried to install (both from PC and from extSD card) wouldnt open in recovery mode.
Because I get a passed message when I install a ROM from Odin , I dont know what the issue could be. But there is something wrong that happens as soon as this phone boots up.
I seriously do not know what to do anymore. I just need this phone to be working again. At this point I dont care about warranties/ROM versions/stock or custom...

It sounds like you need to download the stock rom and flash it in Odin (you gotta put the phone in download mode to flash in Odin). The stock rom can be found on the sammobile website in the firmware section. That should get you back to square one (stock rom, stock recovery, no root). Then you start over from scratch.

I did and I still get the same problem.
In fact after installing stock rom, i could not enter "stock" so I installed philz recovery, logged in philz and wiped cache a couple of time, did a factory reset.
and yet when i restart , the phone is stuck in bootloop.
Maybe she has a Galaxy Mega with a repaired screen. Starting with 4.4 Samsung only supports a digitizer with Synaptics brand chip. It's a big mess and there are countless screens and digitizers made worthless. I think unofficial 4.4 by Cyanogen and others might work, but not official Samsung. With some Galaxy Megas you get a bootloop. With others it boots however the digitizer and touch functions no longer work. I don't know how to go back after you are on official 4.4 because last I saw, there was no root for it. If I'm wrong, someone else please clarify and continue to help.
IMHO only one way out from such situation is try to flash MegaFire MJ2 which is derived from stock 4.2. It's only one 4.2 ROM which can be safely flashed onto KK.

So I'm having a bit of an issue flashing a recovery on my NH7 4.4.4 HLTESPR Note 3. I've had it rooted/ROM'd for a few months now without any issues. I used my upgrade to buy an LG G3 for $1 and gave it to someone else, and when they did this, they had to deactivate my Note and activate the G3 for the upgrade to apply. After that, they reactivated my Note 3. I was getting data connection at Best Buy immediately after, but later that night I noticed I had completely lost data connectivity so I went into TWRP, made a backup, and reflashed to a stock/rooted recovery I had made prior so that I could access the profile and PRL updates.
I applied the updates and reestablished data connection. Then I tried to boot into recovery from TWRP to revert to my ROM. No go. I try it with the button method - no go. Then I went to Root Checker and it told me my device no longer had root access. Super User still installed but root was gone. So then I ran it through CF-auto-root again, and tried to flash TWRP from the TWRP manager a few times but it crashed each time. Then I tried to use Flashify to flash TWRP from my SD card. Seemed to work, so I used TWRP manager to reboot into recovery and it went into a boot loop. Odin'd back to NH7 and retried the whole process. Now even when I flash TWRP from odin, I go into TWRP Manager and it doesn't detect a recovery. I've tried a few different versions of TWRP and used Odin, TWRP manager, and Flashify to try and flash a recovery but it just doesn't seem to work. I have no idea what's going on. Have you tried a full revertin? Stock everything including recovery and starting over with the root process?

i didnt realize the OC1 locked the bootloader and i could not longer install twrp.
I downloaded TWRP in the app store and tried to install it. It gave me root access and said it installed it. I turned on the phone and it failed to find he recovery. I can get to the Verizon help screen but can't get it to autoboot verizon assistant to set to factory reset. It is a spare phone I use to stream xfinity via HDMI so i don't need root or anything special

click ap slot
load tar (can take a min just wait it will load)
click start
wait for flash to complete should take 5 to 15m
device should reboot on its own
if it doesn't you can reboot it manually after the flash is complete first boot can take 5M or so
