Ok, here we go.
(Was ) Running on SC2.8 voodoo, no problems with the ROM so far. However, just since yesterday my 3g data connection has...um...went south.
Being the curious type, I followed GizmoDroid's instructions in one of the other recent threads on how to turn your hidden menu on (*#22745927) and other dial codes (##data# and **debug). Now, also reading his warning about what had happened to him, I changed NOTHING.
However, soon (I think after pressing the Advanced menu button under ##data#), the Phone process forced closed. Ever since that, my 3G connection has come in and out randomly, being gone about half the time. That's not all though: even when the service is present, it's sloooow. I've run tests in the Speedtest app many times; only once have I achieved a ping better than 999 (the highest it'll show), and downstream and upstream data seldom exceed 100 kbps.
Now, I've tried a variety of things: *228 (of course), restoring a nandroid (of EB01 I think), flashing an entirely different ROM, Odin-ing an old package for DL09, and most recently, Odin-ing Adrylnalyne's "whole enchilada" EB01 package, including PIT. None have fixed the problem.
I've read where GizmoDroid activated an old phone then re-activated his Fascinate, and this solved his issue. I can't really do that right now, as I don't have an old smartphone laying around.
Anyone have any suggstions?

I did the same thing with the ##data##. Changed nothing, was just curious about the menu. My 3g did exactly the same thing as yours, spotty and slow- icon would completely disappear at random. Searched everywhere for an answer.
I used Odin to go back to 100% stock, factory reset, *228 , dl09 modem- the works. No difference at all.
I called Verizon out of sheer desperation. Went through the usual battery pull, etc.
Much to my surprise, they said my phone was defective and sent out a new one overnight as a warranty replacement. Just flashed everything back to SC2.9 and did a nandroid restore - works perfect.

Yeah, that's what I may end up doing...I'm pretty sure they'll give me a new one; they have no idea what's actually going on, thus they can't no I might've screwed it up myself.
I do think re-activating an old phone then this one again would fix it, but I have the promotion for $9.99 off per month for upgrading to a smartphone during that time. It goes away if I remove the smartphone data plan from my plan, which would happen automatically if I'd activate an old non-smartphone, and I don't have an old smartphone laying around.
So perhaps into Verizon I go...

Yeah, apparently you can't stress enough not to go into the Verizon specific menus, at least not in SC.
I'm going to end up doing that here pretty shortly. About the screen protector: I've never used one as, in my opinion, the gorilla glass does a pretty decent job of resisting scratches. After ~3 months and having dropped it more that once, stored it with keys, etc., I only have one fairly tiny scratch.
Nothing worked for me either. Flashed every modem through odin/heimdall, restored nandroid, and put another phone on the account. It seems unfixable to me. :-/

Is my fascinate defective? Why does my back button randomly press itself? In fact, it seems like it's doing it as a long press sometimes since pressing the power button takes a random screen shot!
I've posted about this before, but this issue came from EB01. Surely others are experiencing such a thing!
This happened to me on stock DI01. I don't know why, but all of a sudden one day, it started doing those phantom presses. For me it was the back button and menu button. I also had a hard time long-pressing the home button to pull up the recent app list.
I have insurance through Verizon, so I just took it into the store, and they ordered a refurbished replacement for me. I didn't have to turn in my phone until the new one came.
It's a different story if you go directly to Samsung. Through them, you are without your phone for 6 days (2 days shipping there, 2 days repair, and 2 days shipping back), and no temporary phone is provided. I was only going to do that as a last resort, but Verizon actually came through.
The end result is that the new phone is definitely better, and I can tell there's a difference in the button response - not in a bad way, though. It's a more "solid" experience as opposed to the previous phone being more finicky.
I have this same issue as well, and have had it pop up on me well before EB01. It's not all the time, but it happens often enough that it's annoying as hell.
Anyone happen to know of a software based solution? Considering all of the rooting/etc. I've done to my phone, I don't know how feasible it is to get replacement hardware..

I believe that if you were to odin back to stock (or any official OTA update) that they wouldn't know you'd ever been rooted.

That's what I did. I was rooted and removed a bunch of the VZW bloat when I first started seeing it. I ended up flashing an Odin package for stock/unrooted DI01 onto the phone to make sure that rooting didn't cause the problem. It still ended up happening on stock. I took it in to Verizon and had them look at it, and they were none the wiser. It's been a couple of months now, and I've heard nothing about my returned phone.
Both the DI01 and DL09 Odin packages should be available in the development forum. Be sure to disable voodoo (if you are using it) before you use Odin.

I just hate the idea of getting a refurbished phone. I sure hope this is a software issue.

Verizon should be quite familiar with this problem at this point. This problem ran wild on the OG Moto Droid. I've had family members return their phones and within the first year get replacements per warranty (factory defect).
The problem stems from either using a charger that doesn't have a USB module (aka doesn't require you to plug the USB data cable into a charger brick) or from improperly plugging in your phone (for instance, not putting the phone in a dock completely or having the USB connection slip partially). The unusual changes in voltage kill these smartphones and cause them to almost short-circuit, resulting in the "ghost" phenomenon. Many people have had this problem within the last couple of years and have found this to be the culprit. Just google "Ghost Droid" for more information.

Verizon's insurance does go through Asurion. That's not what I did, however. I told Verizon I didn't want to make an Asurion claim (which comes with a $89 deductible). They said I could go through them for the Samsung warranty. They sent me the replacement and then I sent back my phone (like you do when you make an Asurion claim), but there was do deductible or charge for it.
Now, they might have only said this because I have the extra protection through them. I can't guarantee they will do this for everybody.

Your phone comes with a 1 year warranty from Samsung for hardware issues. Assurion picks up if the user damages the phone, like dropping it or water damage.
Update for anyone following this thread..
I got a replacement phone and restored via nandroid, ensuring identical software. The phantom button presses are gone! So, it seems the button presses are indeed a hardware failure, not software related. If you have phantom button presses, get a replacement phone and save yourself some grief.

Dammit. I assumed it was software since this only started happening when I switched to froyo. The kernel is buggy and I figured this would go away once the devs bring us ones from proper source.

The only thing I've noticed on my phone is my power/sleep button sometimes randomly takes screen shots instead of going to sleep, which I thought I read was the back key sticking, is this similar to what everyone is experiencing with these phantom button presses? Mine isn't backing out of programs or anything?
I've had these phantom button presses on mine on and off since I got the phone. Even before I rooted it. I always assumed it was because the soft keys were dirty/oily and were incorrectly registering touches. Wiping off my phone has always taken care of the problem.
My fascinate has infrequently done this over the last several months. Today, the stupid thing started pressing the back button every few seconds. Rebooted several times. No improvement. Switched kernels. No improvement.
I decided to try to see if this is software or hardware. Rebooted to CWR and left it setting on one of the options. Right now it has been setting for about 10+ min and no random back buttons have been pressed. My thinking is, if this is a hardware bug it should have selected the highlighted option since the back button is used as the select in CWR.
Anyone have any ideas?
I think I might try a clean reload starting from the stock ODIN when I get home.
Phone is rooted and running SC 2.9 Voodoo Kernel, voodoo disabled.
I have this issue really bad ever since upgrading to Gingerbread...
If I use Odin to flash back to the Stock Verizon 2.2 the issue disappears, but if I flash any gingerbread, doesn't matter if its VGB or one of the CM7 nightlies, the phone presses the back and menu button pretty consistently and also if you remove the battery for any reason at all, it loses or forgets a lot of apps that you may have installed (Live Wallpapers for instance) and also takes 3-5 reboots or power down cycles to finally boot up NOT in Safe Mode.
Not sure what the hell is going on, but I may have to just Odin back to Verizon stock 2.2, root it and live with the bloatware
Oh, and this issue is persistent from my original Fascinate to the refurbished replacement that Verizon just sent me. So either they sent me a refurbished phone that has the exact same symptoms or its an issue with Gingerbread and the Fascinate.
Any suggestions?

Could my stock Samsung 16gb memory card be causing this? That's the only thing that was carried over from my old phone that had the button press issues to the new refurbished one.

Googling about this problem, it seems this is a wide spread problem with Fascinate and few other Samsung phones. No one knows of the real problem nor a fix.
It is unacceptable for Samsung not handle this problem which sounds like it's hardware related for this long.
Any more input guys?
I was about to buy the Samsung Fascinate myself but this sounds like a major problem to me.
So after rebooting about a week ago on BAMF Remix 1.7 (I think the ROM is irrelevant but just putting it in for information's sake), my 3g/mobile data stopped working. When I go into network settings, the Mobile network option is checked to connect to the internet and under APNs, there is a vzwinternet profile with a green (I'm guessing turned on?) marker.
I've tried various ROMS: BAMF, BAMF Remix, CM7, and even the Gingerbread variations of BAMF as well.
Also I've experimented with all of the radios (every one listed under the obligatory radio thread) and different kernels.
And lastly, I've wiped multiple times every time I've flashed a new ROM and have even formatted my SD card hoping it was some rogue application that disabled it.
So what can I do to bring back 3g/mobile data?
Note: Wifi, calls, and sms will works for me. MMS does not however.
I have the same problem but mine shows after rebooting, than say error when I try browser or anything that has to do with data, But works at times.

How you checked with your Carrier? I'm with Orange UK and my 3g was down for 4 days once due to cell tower maintenance...

This happened to me, exactly as you've stated. I posted here but got no remedy. I tried all the things you've mentioned, reflashing the ROM, raeflashing the radio, etc. Nothing helped.
Finally, I had the idea to restore a recent backup from before I had the problem. THAT worked. I hope it helps you too.
I didn't have a chance to call during the day but I did head over to my friend's house who lives in a different area. She has VZW just as me and she was getting 3G on her blackberry but 3G/mobile data was still unavailable for me.
It's not a coverage issue but on my end.
While lurking around the mobile data settings, there was a profile for VZW under APN settings. Could someone post theirs so I could see if they match with mine? Maybe that's the solution to my problem?
I don't think it's rom dependent. I lost 4g the other day and I have yet to get it back. I have my own theories but the jerk at the VZW store wouldn't let me try my theory. I'm waiting for my usual rep to comeback, she lets me try just about anything I want. The rest are just dicks. I have tried swapping roms, radios, and all but going back stock to fix it. And nothing has brought it back. BTW, have you tried CDMA prl to see if you can get data at all? I can get 3g that way, but when I go into evdo/lte or lte only I lost all data. Also, do you have it in rev A mode under your modem settings?

Go into settings and do a check for new software.
This will force 3g to cut on. (Note: it may take a few times for the 3g to come on and STAY ON. It took me almost a day doing it about every hour.

I have to go to work right now but yes it is turning on 3G at the moment but it's only using the up arrow (eg. uploading data but not downloading data). I"m guessing with a couple of more tries it'll eventually lock in.
If this doesn't work on gingerbread, I'll try reverting back to froyo and try it then as well.

Glad I could help.. I spent nearly two hours on the phone with Tier II support and they guy said.. well this is a long shot but try this. Didnt work the first time I tried it so he sent me a new phone but in the time it took to get the new phone, I kept trying and it finally locked in.
Ok, well I thought it worked. Every time I try it, it will give me the 4gLTE indicator, but if I open up anything that uses data it just says no mobile conectivity then after about 5-6 min it stops showing any data. Also, I can still go back to CDMA(PRL) and get 3g. It's just under 4GLTE/evdo and lte only.
BTW, for any other future reference, I started experiencing these issues right after I debloated (uninstalled) the blockbuster app.

When you guys went to the software update option, during that duration when it was enabled were both 3G arrows animated? I'm only getting an up arrow (uploading).

i had the same problem as you OP, but after pressing check new enough times and jumping straight into the browser, i got it to stick. took at least 12 tries, however and i'll see if it lasts.

Yes, I had both arrows. But, for some reason when I would open the browser to google it wouldn't work. I tried about 4 times, each time it would show data, but I wouldn't have a connection. Also, it would show data being up and down stream, but I couldn't manage to get it to work when I tried it.

Sometime today I lost my phone signal on my SG2. I'm not sure exactly at what moment as I first noticed that I had a couple of bars but no 3G when I tried to browse the web. So I restarted the phone but to no avail. Since then I've lost all bars and can't make any calls either.
The only thing I have done today "out of the ordinary" was to connect to the phone using Wifi File Explorer to transfer a file, which I tried to connect to without being on a wifi network by first trying the "Wifi Direct settings" in Settings, which didn't work, and then by tethering by Wifi which worked. Maybe some setting got stuck in the process, but I'm not sure since never checked if it worked right before and after.
If anyone has any idea what could be wrong please let me know, as I am literally without a phone!
I recommend downloading http://www.androidzoom.com/android_applications/communication/no-signal-alert-pro_fyyo.html this will definitely be of benefit to you if your suffering from signal problems, it has been of good use to me.
Check your IMEI is the correct one for your phone .If not restore your backed up EFS folder .
Or try backup data and factory reset as a first step.

The IMEI does not correspond to the one I have written down, though I'm not 100% I what have on my note is the latest IMEI; but how could that change though by itself, if my note is correct??
Also, I just saw something very strange: I turned on my phone and had full bars (no 3G though), and right in front of my eyes all bars dropped, one by one, during the course of 5-6 seconds. Something is VERY off...
Regarding factory reset: my phone is rooted (thanks to XDA!) and I have Tit.backup: can I will my phone still be rooted if I do a factory reset?
Something went probably wrong when rooting it (not neccesarily any errors) and effed up tge EFS folder.
Try to flash back to stock ROM, it might work to restore your.folder.

I should have been more clear on that, but I rooted the phone about a month ago and it has worked flawlessly since then, so it couldn't be that.
Anyway, I looked around a little and found this post:

He seemed to have had the exact same problem that happened to me, so I decided to wait, and sure enough the connection reestablished itself about an hour ago and while I was watching for it, with the signal slowly creeping back up during a couple of minutes, and several hours after I lost it. Very odd.
This surely has to be an Android bug but that only shows up under certain and rare conditions.
To be sure I made a speed test and got 4.84 Mbps/up, 1.19 down, which I guess is fine; I never did one prior to this so I have nothing to compare to.
Again, appreciate the advice!
EDIT: For posterity, I should add that during these hours, even when I briefly apparently had a (non-3G) signal, I had a notification during all this time saying that I could only make emergency call.
I'm having the same issue. No Bars at all most of the time and when I have some, it never connects to 3G, G at most, and can't load any thing at all.
Very frustrating. I have been using Lite'ning rom 6.1 for a few months and had no problems at all until about a week ago.
I flashed back to a stock rom and still having the same issue.
I've also seen the bars drop down one by one in a 5 second window, bizarre!
Anyone find a fix for this? Or could the phone be faulty?

I was on Lite'ning about 3 weeks ago and actually completely lost signal several times. Reboots and hard resets solved the problem temporarily. I then flashed Batista FH 2.4 with ICS and things were great. I'm now on GingerMod. No issues.
I don't know how it could be kernel related though, especially if you are 100% stock again.
I RUU'd to Jan2012 OTA update, unlocked with HTCDev.com, installed AmonRa, rooted using AmonRa and everything has been fine....for about four days. Then yesterday, after using IM+ Free for a few days and liking it, I decided to buy the Pro version. Coincidentally, my extended battery was running out and I didn't have time to charge it at work, so I powered down, removed the battery, put my charged stock battery in and voila...boot loop. No signal at unlock screen, sits for 10 seconds, and reboots. I can do a factory wipe and it will boot, but if I restore anything...data only, system only, secure only...all 3, 2 of 3...doesn't matter...issue returns. Anyone know why this might be happening?

My concern is that it was running fine, and then suddenly wasn't when nothing but an app install (IM+) was done. I don't want to go through setting everything up again only to lost it all again four days from now.
I am having similar issues as you are. Tried to charge a 3.8v stock battery overnight and it bootlooped all night long. It seemed like it settled down after putting back in my 3.7v extended. So I called HTC and created a rma for the battery, no big deal.
Then today listening to music while the phone is in the dock i had a few reboots/bootloops, noticed some odd stuff in logcat so I did a reset permissions in recovery. Seems better so far compared to before where it would only take a few minutes to bootloop.

Yes, sounds similar...however, I ran my stock battery for about two weeks with no issues, and ran the extended battery for a good month with no issues, and even ran fine for a few weeks after htcdev.com unlock, AmonRA flash and rooting took place. This all came about out of the blue.
Right now, I am attempting to use the guide specified concerning connecting to the wrong type of network. I'll update when finished.

UPDATE: That worked. It was a communication issue, the phone had changed it's connection preferences. The sad part is that I've been without a phone for almost a day over this, and flashed all kinds of kernels and recoveries, and even considered going back to Verizon for a replacement. The even sadder part is that I already knew of this fix and completely forgot about it. Thanks a MILLION to MrSmith317, not only for directing me to the fix, but also for not bashing me for accidentally posting in the wrong area of the forum.
So this morning when I took my phone off the charger, it shut off immediately. Pulled batter and put it back in multiple times, and it still wouldnt boot. Battery was fine cause it worked in another Note 2. I wiped the contacts that connect to the battery, and then it powered on. Thought it was very strange since it was working just fine the night before. Upon power up, I couldnt make or receive phone calls, couldnt make or receive texts. Really have no idea whats happening!! I've tried flashing several different roms and all react the same. The signal just bounces around and sometimes even goes out (circle with line through it). My IMEI and number still show up just fine in "About Phone".
I'm very confused, because this seems to have happened randomly! I didnt do anything but charge it overnight. I've tried flashing the modem that scott has posted. Any ideas? Should I just try and ODIN back to stock?

Try to put it back to bone stock using the full wipe Odin image from here. Of this doesn't fix it then is probably hardware so take it to Verizon.

Well then move or delete it. I have tried to flash the L4 modem, but to no avail.

No problem. Wasn't really sure, now I know. I'm in the process of restoring to stock. Hopefully it works!
Well turns out it may have been a hardware problem! Restored to stock, and I still had the same problem. No 3G, couldnt call or text. But I did have a 4G signal that worked, plus I could receive texts. So strange! Took it to Verizon and between the store rep and tech support, they couldnt figure it out. So they're sending me another tomorrow. In case anyone was curious...
Well, just an update to my saga, in case anyone cares!! My second replacement came this morning. Upon putting my battery and sim card in, it booted up. Was going through the setup process when i just shut off. Took me about 10 min or so to get it to power on again. When it finally booted up, I had the same problem I had originally, 3G and talk/text doesnt work. Also had the faint smell of electronics burning up. My first though was my battery is frying the radios. But why wouldnt it do the same in my original phone that had the proximity sensor problem?!!
I'm ready to move on from the Note 2, Ive had nothing by problems with Samsung hardware being faulty for me.
