This problem appeard on other devices as well. You should report this to Verizon support, ive seen 3 threads having the same issue with Verizon...

Guessing you're rooted? If so you can try and use a root file manager (amaze is a good one) and delete your apn.config.xml in system/etc and replace it with a different one and set permissions and reboot and then see if you can manually add your Verizon apns. For me this did the trick on a few roms and allowed me to add my carrier apns where as before it would not allow me to due to them being greyed out.. I have a few if you'd like to try them let me know and I'll post them here. I had no data on a few gsi roms on a few devices that I own and apns were greyed out as you say yours is. I don't have Verizon I currently use Boost but it's worth a shot and this will not break anything. Trust me I've had my fair share at messing with apns.

Here's the deal, I bought my Rezound in 2012 and flashed it to PagePlus Cellular using CDMA WS.
I've used it and it's been fine, running CM10 most of the time. I just bought a Evo 4G LTE and switched to that. So I RUU'd my Rezound, and it says a SIM card is required, etc.. but there are no signal bars.
I go into About Phone > Network >
and Operater name is "Unknown," signal strength is "0", service state is "out of service."
Now, the question is, is that normal? Does the phone need a sim card to connect to verizon towers?
I don't want to sell it while it's not working. I did call up Verizon and they said the ESN was clear for activation though.
So if I sell it and the new user puts in a SIM card, will it clear up all the signal issues? Or have I done the RUU incorrectly? [I did it a couple times because it didn't work the first time (the SD card method, then I did fastboot method)]

That is not normal behavior and you may need to do some more work before you sell it.
Make sure all related network settings are correct for Verizon instead of GSM under Settings > Mobile Network menu.
Try entering on the stock dialer *#*#4636#*#* to access the hidden testing menu as well for the phone radio and verify it is on CDMA. If that code doesn't work on the dialer try it via an app like "Hidden Menus" or even "Battery Monitor Widget" by 3C (click "Tests" icon > Phone information from main page of app.)
Background: I broke off of my Verizon contract six months in and used my phone as a WiFi-only device (VoIP and internet texting apps) for the longest time. While I typically hid my signal icon and ran the phone in Airplane mode with WiFi toggled back on, I would occasionally turn my radio on to resync time and date to the cellular network. With no SIM in it clearly was syncing off Verizon CDMA signal still; I simply wasn't fully activated on the network any longer.
I just made the StraightTalk AT&T SIM card plunge the past couple of weeks and did an NV edit in CDMA WS as well, all alright on GSM. I am on the stock global ICS ROM at the moment, one I know could sync time on CDMA before. However, I cannot get the phone to switch off of GSM even in the hidden testing menu, even without SIM in... Likely has to do with the NV edits I made, and perhaps that may be your case too.
You *could* repurpose the sale as a GSM enabled unit... If it was even set up and working well as such. But due to the current technical issues of getting GSM mobile data above 2G I'd suggest to get it fixed and working back on stock CDMA for the consumer's sake if you can. Again AFAIK It should pick up a CDMA signal even without active service or the Verizon LTE SIM in, did so for over a year lol.

in the mobile network, I have it set to "CDMA / LTE" but I tried "CDMA only" and "Global" but none worked.
*#*#4636#*#* didn't work in stock dialer, it just disappeared when I finished typing it.
then I downloaded Hidden Menu, found *#*#4636#*#* in the list, tapped it, and it did nothing. I did some other ones and they did do something. I found htc diag mode, the same one I used to program it originally. I clicked read, and the phone number and min were blank. so I went to the prl and tried flashing a verizon prl file, then rebooted, and still nothing.
I'm going to try to RUU again and hope that solves it.

Can anyone confirm whether or not, say you got a Rezound out of the box never activated, would you need a SIM card to get the signal initiated? I'm thinking either that's the case and my phone just needs a sim card to jump start it, or my rezound is toast.

I've noticed that my wife and I will sometimes be in the same place and she with her un-rooted LG phone gets good 4G/LTE signal while I'm on 2G. I went to the T-Mobile store (did not tell them I was rooted, they didn't ask) and they gave me a new sim. That seemed to work ... for about 30 minutes. Then I got booted off 4G/LTE again. So I went back. They messed around with APN settings and stuff, still no permanent fix. But what I have noticed since doing that and learning a little bit about the network settings menu is I can manually get myself back on 4G/LTE by going into network settings>mobile networks>network mode and disabling, then re-enabling, GSM/WCDMA/LTE auto. (There is no "LTE only" mode for me to select.)
I printed out a logcat dump shortly after doing that successfully, but I know nothing about reading those logs.
Just for kicks I've tried adding an APN using epc.tmobile.com instead of fast.t-mobile.com but that one won't even connect me to the internet; perhaps I am doing something wrong in setting it up.
Anyone have any ideas here? I'd prefer not to have to go through several menus every time I want decent data service.

Just a piece of additional info: I've now noticed that I seem to get kicked off 4G when I make a phone call. So it seems like the phone is having some problem reinitiating its connection with the 4G data network ... ?
I am attempting to switch my Verizon GS4 handset to Straight Talk Wireless, but I can't do anything to change the APN settings. I restarted my phone multiple times with the ST SIM in the slot, but it continued to connect to Verizon's network. Some research later, I found that I needed to change the APN to Straight Talk, but I can't.
Only Verizon's LTE access shows up in the menu. There's a + button at the bottom, but it's greyed out, with either SIM (tried both as per this HTC One thread). Pressing the menu softbutton gives me the "reset to default" option, also greyed out and inoperable.
The phone is rooted, still running Jellybean 4.2 MK2 because once I rooted, the OTA updates would fail, and I was never brave enough to try Safestrap. I'm worried this might be part of the same break.

In my experience, IT DOESN'T MATTER!!!
I do NOT mean that in any disrespectful way to you, but I DO mean it in a way against Verizon and Tmobile.
I have the same phone AND the same problem with service; however, I CAN add an APN. The catch is IT STILL CONNECTS TO VERIZON!!!
Now, my phone updated to the utter crap that is Lollipop, and I can no longer get my call-Verizon-voicemail-and-disconnect workaround to work as the connection is a lot more seamless and does not drop/fully disconnect after the call disconnects. Before, I'd goto my notifications and see "NO SERVICE" in the bottom of the slide down box with my VZW voicemail notification with it's 10 voicemails. Then, I'd do my workaround and everything would work (and, by everything, I mean text messaging as voice and data WERE working pre-workaround). But, like I said, now I can no longer do that (Lollipop SAYS it's connected to Tmobile off the bat (right after booting up); however, I CANNOT send text messages NO MATTER WHAT I DO!!!
It's completely aggravating and how Verizon can say they sell a phone "UNLOCKED" when their APN is in there with other APNs and GREYED OUT so you CANNOT REMOVE IT (which, as I've shown above, causes massive problems) and how Tmobile can say ON THEIR SWITCH WEBSITE that my VERIZON SAMSUNG GALAXY S4 *WILL* WORK on their network... it's all beyond me. It's deceptive/straight up lying marketing and the way Tblowbile always tells me they cannot support my phone and to buy a new phone WHEN THEIR OWN WEBSITE SAYS IT WILL WORK is 100% bait and switch and up for a class action lawsuit!
Update: it does appear my service with the ST SIM really is through my Straight Talk account now, despite what the APN settings show. I got my device activation text from ST. This makes sense, since Straight Talk is a virtual carrier buying airtime from the real networks like Verizon.
However, I don't have an LTE connection. I don't have any data connection, just voice and SMS. I'm not sure if that's a problem on my end or the carrier's, but getting the data I'm paying for is less of an emergency than not having any mobile communication.
Has anyone found a work around for this. On straight talk, I can get calls and even internet when set to global network. I cannot text either sms or mms. My APN setting completely grayed out. I can not add or change existing and tried an apn manager with no luck.
I suspect I'll have to flash a completely Verizon-less build to fix it, but I did have an ST representative call me to ask if everything was working and offer to try to help me fix it. I just didn't have time right then and they didn't call back when I asked them to.

Hey not sure if this will help anyone out but I'll add what I've experienced. I have a gs4 running hyperdrive and my data service from straight talk has cut out a couple times before. The first time I just waited, and it fixed itself after a day or two. Second time I called ST and they told me to restart my phone (like I hadn't done that 10 times already). Later that day, data randomly comes back. I noticed that the fix for me was to switch apn settings. Somehow my phone did it automatically. But now my data is MUCH slower than before. Straight talk seems to be a tricky company. You never really know what you'll get with them. Just other carriers' sloppy seconds. Cutouts also seem more common at the end of usage cycles. Seems a lot of people fix it by calling ST
Also, I can edit my APN. Not sure if it's because I'm on an aftermarket ROM or what. But now I'm on the hunt for an APN with better data speeds if possible.

It took me 3 months of calling Straight Talk and telling them they needed to send the right APN settings to my phone, after they finally listened it took 2 minutes until I received a text saying they updated my APN settings. I had the exact same problem as you. I had voice and SMS but not data. And my APN settings were greyed also. Now it says

You just have to get them to update the APN. Getting them to listen is the hardest part! I hope this helps!
So, I just flashed my old s4 to Straight Talk. Voice, and SMS worked fine right from the jump, only thing I had an issue with was Data. When I went to look everything suggested to change the APN settings. Upon trying, I wasn't able to change anything in the APN setting, everything was greyed out. I spent countless hours trying to figure out solutions.. Here are some things I tried:
Entering Developer Mode
Editing something to "yes" in the customer.xsl file
Adding ro.code_source or something like that in the build.prop file
None of that worked. I'm running Hyperdrive 20, so I would think it would be easier with an aftermarket Rom, but I'm stumped. Any tips?

I'm having the same problem. I have had several conversations with Straight Talk reps and so far their answers haven't helped. They are sending me a new Sim card and that is supposed to fix the problem. I doubt it, but it can't get worse!

edit - misread question
I FINALLY have LTE on my Verizon S4.
I now have LTE on Straight Talk!
In settings, under more networks, mobile networks, access point names, tap on Verizon Internet, if you change where it says APN to
tracfone.vzwentp
Tap the back button and make sure it still says tracfone.vzwentp under APN. Then hit the back button and make sure there's a green dot next to Verizon Internet. Then reboot and you should have LTE!
My wife's s4 and my g3 have been on straight talk vzw Sims for a few months now. Every month when the auto refill goes through our data shuts off and the 4g symbol is greyed out next to the signal bars. So I call and they ask me the same dumb questions and then they reset something and I reboot and I have data again.
I saw a thread on Howard forums where a guy detailed out some Apn settings that were different and mentioned something about old and new systems for connecting.
Since verizon is such a bastard about not letting us edit the apns our self fully, I'm going to try this on my wife's s4 next month and try the other apn and this fix on my g3 when the data shuts off. I can edit and create new apns from a hidden menu with shortcut master on my g3.

I love this phone....there, I said it. But it's driving me crazy.
I bought it from a Chinese retailer, who shipped it with X821 firmware, but the box states that it's an X820. I doubt there is any difference in the hardware.
I would have left everything as it was, but I can't seem to make phone calls (don't have a data plan yet...yeah, I know....) with anything other than 2G, and that is about to go extinct. When I set it to WCDMA, it shows a UMTS connection, with no carrier designation, but the tower is the same one I connect to with 2G. The phone does sometimes show an AT&T 3G connection in the status bar, but I can't make calls, even though I have full signal. When I place the same SIM in my old HTC Inspire, the same monitoring app shows "UMTS - AT&T HSDPA+" and works just fine.
I'm at my wits end. I have rooted, installed TeamSpain recovery, replaced the firmware with the X829 and even tinkered with the frequency configurations through DFS Studio (which did a lot of complaining, and I'm still not sure if the changes stuck 100%). I also played with APN settings and tried the SIM in different slots, both alone, and with another SIM. And after all of that, still no progress in getting this phone to use a 3G or 4G connection to make calls. If I don't set it to "GSM only", no phone calls in or out. I'm essentially right under a tower, so signal shouldn't be an issue.
If anyone is using the same provider in the US, and is not having these kinds of issues, I'd really appreciate hearing what firmware you are using, and/or what you did to get things working.
I am using it on x820 with official stock 0.17s firmware, and did a dirty flash to cuoco92's 17s firmware. Everything works including HD voice calls, and full LTE.

I tried this, as well as the Remix ROM. In fact, I think I've tried just about everything, without success. At one point, I had one of the apps indicating a 4G connection, but the phone diagnostics showed GSM (2G). Even the apps aren't all sure what to make of what's going on with the connections reported by this phone.
Currently on the cuoco92 v35 ROM. When WCDMA is enabled, I see simply a good UMTS connection , sans carrier name, in the "Network Cell Info Lite" app, and no ability to make calls. I also often (but not always) see the "AT&T 3G" sign. 4G or LTE never shows at all, even though the diagnostics show that the phone is communicating with an LTE transmitter, when preferred network is set to "unknown". It won't even let me set things like "LTE only".
Any chance you could set your phone to WCDMA and see what the "Network Cell Info Lite" app shows you? Anyone else with this phone on AT&T?
BTW, I am now using a data plan, which I thought might make a difference. It doesn't. EDGE data only for me . New SIM card is on the way, but my hopes aren't high.
Is it possible for a Chinese seller to have borked the phone so thoroughly with their x821 ROM (it's an x820 marked phone), that it keeps any subsequent radios from flashing properly? I don't know how/why it would work for some and not others.
Starting to really dislike this "phone"......it's more of tablet at this point.

Thanks, but I wish it were so simple. I have tried this, and other APN settings. None work to get this phone to be usable on anything other than 2G.
I just received and activated the replacement SIM. Still no improvement. While typing this, I have Network Cell Info Lite app running, and with the Set Preferred Network Type showing "Unknown" in the phone's diagnostic screen, It will switch randomly between "EDGE AT&T" (which works fine) and "UMTS -" (which is useless). Both have strong signals. For a split second before it switched to "UMTS-", the app showed "GPRS AT&T", but only for a split second before it kicks over to "UMTS-".
I wish someone using this "phone" on AT&T in the US, would run this app to compare notes. It may be that that the phone works fine on a full implementation of LTE (i.e. in locations where AT&T have already deprecated 2G) but not in other areas. Still, that doesn't seem to explain why this phone seems unable to register on the same 3G network, used easily by my other phone, with the very same APN settings and SIM.
You would have to call att and check if they turned our phone on to use lte. If your phone is not "registered" as an LTE capable phone it wouldn't be able to pick it up. This phone has all the bands needed for AT&T because I am using it now. All LTE work straight out the box, and i'm on the latest 19s cn rom

I'm a bit confused. Is this what you did to achieve proper function of this device? I.e. calling AT&T and getting them to do this for you? My conversation with their tech person didn't seem to bear fruit in this regard.
Also, which slot do you have the SIM card in? I've used the stock 17s and now on cuoco92 v35 ROM. There definitely seems to be a difference in how the network settings behave, based on the location of the SIM. At one point, after selecting BAND 6 (when it would let me), the phone showed a constant "AT&T 3G" connection, but it was useless. No calls and no data.
